2.Applied value of muitislice CT in selecting living donor kidneys and excision methods

Objective To evaluate the applied value of multislice CT (MSCT) in the selection of living donor kidneys and excision methods.Methods Ninety living renal donors underwent MSCT assessment.The nonenhanced,arterial,venous and excretory phase examinations were performed.Using maximum intensity projection and volume rendering techniques for vascular imaging,two blinded radiologists independently analyzed and evaluated all MSCT images.According to the CT reconstructive images,radiologists and physicians selected the left renal or the right renal donors,and chose laparoscopic or open live donor nephrectomy.Results On the 90 cases of donors,78 donors underwent nephrectomy in the left kidney.Seventy-one left kidney donors having no significant variation received the routine laparoscopic live donor nephrectomy.Seven left kidney donors on both sides had relatively obvious anatomical variations such as accessory renal artery,multi-branch renal vein and renal vein in the back of the abdominal aorta,and they were subjected to the left kidney open donor nephre.ctomy.Other 12 donors having significant variation in the left kidney were given nephrectomy in the right kidney,and all of them received hand-assisted laparoscopic live donor nephrectomy.All intraoperative records of urine collection system and renal vascular anatomy were consistent with the preoperative evaluation of MSCT,and the accuracy was 100 %.Two imaging experts in the evaluation of renal artery,renal vein and urine collection system showed good consistency.Nephrectomy was successfully performed on 90 cases of donors,and.postoperative recipients had no renal vein thrombosis and other vascular complications.Conclusion MSCT can provide accurate and valuable information for the selection of living donor kidneys and excision methods as a “one-stop” technique for the preoperative evaluation of living renal donors.
3.Changes of plasma biomolecules in patients with acute cerebral infarction before and after treatment

Objective To study the changes of plasma biomolecules in patients with acute cerebral infarction(ACI) and the potential influences from lumbrukinase and aspirin(ASA). Methods Seventy patients with ACI were randomly divided into three groups according to the ways of administration: lumbrukinase group(n=26),ASA group(n=24)and lumbrukinase + ASA group(n=20).Plasma levels of P-selectin,D-dimer(D-D),tissue plasminogen activator(t-PA),plasminogen activator inhibitor-1(PAI-1),plasmin-antiplasmin complex(PAP),thrombin-antithrombin complex(TAT),homocysteine(Hcy) and thrombin-activatable fibrinolysis inhibitor(TAFI) were measured in these patients with ACI and normal individuals(n=20) by ELISA method.Changes of these parameters were detected before and after the treatment,and the diagnostic efficiencies for ACI were compared by receiver characteristic curve(ROC).Results Before the treatment,all the parameters in patients with ACI were significantly higher than those in the healthy controls(P0.05).However,there were significant changes of PAP in the lumbrukinase group(P
4.Establishment of a hypertension risk assessment model among middle-aged and elderly populations based on the basic publichealth service program

Objective:
To establish a hypertension risk assessment model among the middle-aged and elderly populations based on residents' electronic healthcare records of the basic public health service program, so as to provide insights into prevention of hypertension.
Methods:
Demographic features and physical examinations were collected among residents at ages of 40 years and older from residents' electronic healthcare records of the basic public health service program in a county of Zhejiang Province from 2019 to 2020. The risk factors of hypertension were identified using a multivariable logistic regression model, and the odds ratio (OR) for each risk factor was transformed into approximate relative risk (RR), which was included in the formula for calculation of the disease risk proposed by Harvard School of Public Health to create a hypertension risk assessment model. The predictive value of the model was evaluated using a receiver operator characteristic (ROC) curve.
Results:
Totally 7 275 subjects were enrolled, with a mean age of (66.15±7.91) years, and the participants included 3 189 males and 4 086 females, with a male-to-female ratio of 0.78∶1. There were 190 cases with new-onset hypertension (2.61%). Multivariable logistic regression analysis revealed that overweight, obesity, central obesity, borderline high triacylglycerol (TG), elevated TG, abnormal fasting plasma glucose (FPG), prehypertension and family history of hypertension were included in the hypertension risk assessment model, with approximate RR values of 1.66, 1.96, 1.54, 1.17, 1.64, 1.45, 1.69 and 1.11. The area under the ROC curve (AUC) of the model was 0.678 (95%CI: 0.641-0.715, P<0.001), and the optimal positive cut-off was 0.899. The model predicted 139 subjects with RR>0.899 for hypertension, with a sensitivity of 73.16% and specificity of 55.79%.
Conclusions
The hypertension risk assessment model created in this study is feasible to predict the RR for developing hypertension among the middle-aged and elderly populations, which has a predictive value in healthcare management.
5.Large-scale expansion of clinical-grade human adipose-derived stem cells using the extracellular matrix

BACKGROUND:Large-scale expansion of undifferentiated and multipotential adipose-derived stem cells using serum-free culture system is a difficult issue to be resolved. OBJECTIVE:To establish an in vitro culture system combined with the extracellular matrix in order to investigate the efficiency, effectiveness and security of extracellular matrix on expanding adipose-derived stem cells. METHODS:In vitro isolated adipose-derived stem cells were seeded in traditional two-dimensional plastic plates and extracellular matrix-coated plates supplemented with serum-free medium respectively. After in vitro expansion, total cellnumber, expression of cellsurface markers, cellsenescence degree and multipotent differentiation ability (adipogenic, osteoblastic and chondrogenic differentiation) of adipose-derived stem cells cultured under both conditions were detected and compared. Moreover, the clinical safety of adipose-derived stem cells expanded in extracellular matrix-coated plates was investigated. RESULTS AND CONCLUSION:Total cellnumber of passage 5 adipose-derived stem cells cultured in extracellular matrix-coated plates was 10 times more than that in traditional two-dimensional plastic plates. Flow-cytometric analysis showed that adipose-derived stem cells cultured with extracellular matrix expressed stem cellsurface markers. cellular senescence examination showed that almost al of passage 15 adipose-derived stem cells cultured with extracellular matrix showed no aging, while most passage 5 adipose-derived stem cells cultured by the two-dimensional system aged and lost their proliferation ability. Multidirectional induction of adipose-derived stem cells showed that passage 15 adipose-derived stem cells cultured with extracellular matrix could stil differentiate into adipocytes, osteoblasts and chondrocytes as passage 5 adipose-derived stem cells did, which performed much better than the induced differentiations of passage 5 adipose-derived stem cells cultured by the two-dimensional system. Karyotype analysis and in vivo invasion experiment insured the clinical safety of adipose-derived stem cells expanded with extracellular matrix. Al above results suggest a safe and more efficient expansion system of extracellular matrix for clinical application using the serum-free culture system combined with extracellular matrix.

7.Clinical study of Botulinum toxin type A injection in the treament of upper limb spasticity in stroke patients

Objective To investigate the effect of local injection of Botulinum toxin type A(BTXA) on spasticity and function of the affected upper limb in stroke patients.Methods A total of 32 stroke patients were re- cruited and randomly divided into two groups:a BTXA group and a control group.All the patients had spasticity of upper limb muscles,which scored grade 2 to 3 with the Modified Ashworth Scale(MAS) ,and decreased elbow joint range of motion.The 16 patients in the BTXA group received BTXA injection in the biceps brachii muscles and flexor muscles of forearm on 10~15 points,while those in the control group did not.All the patients in both groups were treated with rehabilitation training techniques.The MAS,Fugl-Meyer upper limb function assessment and Barthel In- dex were employed to evaluate the changes of muscle tone,upper limb function and activity of living (ADL)perform- ance of the patients before injection and at 1st,2nd,6th 12th weeks after injection.Results The therapeutic effect between the BTXA group anti control group was significantly different in terms of biceps muscle tone,the scores of Fugl-Meyer upper limb function assessment and Barthel Index.Compared with preinjection,muscle tone was de- creased significantly and ADL performance was improved after injection in BTXA group.The effects of BTXA lasted more than 12 weeks.Conclusion Intramuscular muhipoint injection of BTXA was useful in reducing muscle spas- ticity,and was helpful for increasing motor ability of the affected upper limb and ADL performance of the stroke pa- tients.

Objective:To study the influence of Yizhiping Capsules on hemorrheology of experimental rats with hyperlipoidemia and its embryonic toxicity, teratogenicity. Methods:① The rat experimental hyperlipoidemia models were established by feeding with high lipid diet for 10 days. Meanwhile Yizhiping was taken orally at dosages of 75、150mg?kg -1 , some indices of hemorrheology markers were observed. ②According to the procedure of toxicology, the rat in the experimental groups were administered with Yizhiping capsules by gavage at a dose of 250、500、1000mg?kg -1 respectively, the embryonic toxicity and teratogenicity were observed. 9.Effects of cysteinyl receptor agonist and antagonists on rat primary cortical neurons.

OBJECTIVETo determine the effect of cysteinyl receptor agonist leukotriene D(4) (LTD(4)) and its antagonists on rat primary neurons.
METHODSIn the primarily cultured rat cortical neurons, the neuron injury was evaluated by measuring intracellular calcium, 3-(4, 5-dimethylthiazol-2yl)-2, 5-diphenyl tetrazolium bromide (MTT) reduction, and propidium iodide (PI) and Hoechst 33258 staining. The in vitro ischemic injury was induced by oxygen-glucose deprivation (OGD) for 1.5 h and reperfusion for 24 h.
RESULTLTD(4) (0.01-1 micromol/L) did not induce the elevation of intracellular calcium, neuron viability changes and neuron death. OGD-induced injury was not significantly ameliorated by the CysLT(1) receptor antagonists, pranlukast (0.2-10 micromol/L) and montelukast (0.2-10 micromol/L), as well as by the CysLT(1)/CysLT(2) receptor non-selective antagonist, BAY u9773 (0.02-1 micromol/L).
CONCLUSIONNeither agonist nor antagonists of cysteinyl receptors have the effects on the viability and ischemic-like injury in rat primary neurons.

10.Mediation of abdominal obesity on the association between cigarette smoking and β-cell function

Objective To confirm the effect of cigarette smoking on β-cell function, and further investigate the mediation effect of abdominal obesity. Methods Participants would include 1440 Chinese smokers who had participated in a community-based chronic disease screening project in Guangzhou and Zhuhai from 2006 to 2007. They were interviewed with structured questionnaire on their socio-demographic status and smoking behaviors. Waist-to-hip ratio (WHR) and fasting serum C-peptide concentration were also measured. Results After adjustment for the potential confounding factors, when compared with smokers with consumption 1-10 cigarettes/day, smokers with consumption of 11-20 cigarettes/day (adjusted OR=1.53, 95%CI: 1.22-1.90) or >20 cigarettes/day (adjusted OR=1.92, 95%CI: 1.32-2.79) had significant higher risks to get C-peptide concentration larger than its median. Furthermore, 37.54% of the effect of cigarette smoking on C-peptide concentration was partially mediated by abdominal obesity. Conclusion Cigarette smoking might be a risk factor for β-cell dysfunction and abdominal obesity.
