2.Determination of aconitine and tetrandrin in separate and compatible decoction of Radix Aconiti and Radix Stephaniae Tetrandrae

Object To determine the variety of aconitine and tetrandrin in decocting Radix Aconiti and Radix Stephaniae Tetrandrae separately and compatibly, and explore the mechanism of synergy between above two decocted herbs. Methods TLCS was used to determine the contents of aconitine and tetrandrin in separate decoctions and compatible decoction, respectively. Results The contents of both aconitine and tetrandrin were less in compatible decoction than in separate ones. Conclusion A scientific basis is provided by the compatible decoction of the two herbs for more effective and attenuate use.
3.Postoperative improvement of diplopia and eyeball movement disorder in patients with reconstructive surgeries for orbital fracture

Objective To observe the postoperative changes of diplopia and eyeball movement disorder in patients with orbital fracture. Methods The clinical data of 62 patients with reconstructive surgeries for orbital fracture were retrorespectively analyzed.The position,range of fracture and incarceration of extraocular muscles were determined by CT scanning.The eyeball movement and diplopia were examined and recorded pre-operation,3,6 and 12 months after operation,respectively. Results Among the 62 cases,23 were simple orbital wall fracture and the other 39 were complex orbital fracture.Eighteen of the patients with simple orbital wall fracture had diplopia and 19 had eyeball movement disorder of Ⅰ degree or Ⅱ degree before operation.Twelve months after operation,5 had diplopia ofⅠ degree or Ⅱ degree,and 5 had eyeball movement disorder of Ⅰ degree.For those with complex orbital fracture,36 had diplopia and 38 had eyeball movement disorder.Twelve months after operation,29 had diplopia(III degree diplopia in 6) and 22 had eye movement disorder.Among the 12 patients with complex orbital fracture who received operation within 3 weeks after trauma, only one had III degree diplopia,while 5 of the 23 with complex orbital fractures repaired 3 months after trauma had III degree diplopia. Conclusion Surgical management can effectively improve diplopia and eyeball movement disorder resulted from orbital fracture.Earlier treatment is superior to later treatment in regard with the effect of surgical intervention.
4.Practice and System Construction of Telemedicine for Coronavirus Disease 2019 Epidemic Prevention and Control.

Telemedicine is one of the five key components of the "Internet Plus Healthcare".Due to its high speed,real-timeness,low cost,and wide spread,telemedicine is highly feasible in the prevention and control of major infectious diseases.This article introduces the practiceof telemedicine in Peking Union Medical College Hospital during the cornavirus disease 2019(COVID-19)epidemic,during which the network resources were applied to break geographical restrictions and resolve communication barriers between hospitals and departments.This article summarizes the telemedicine application before,during and after COVID-19 control and elucidates how to build a telemedicine prevention and control system for infectious diseases,with an attempt to further improve telemedicine and is application in the public health emergency system in China.

6.Effectiveness of Tai Chi on movement, emotion and quality of life in patients with stroke:a Meta-analysis

BACKGROUND:Tai Chi exercise can relax the affected muscle of patients, increase muscle flexibility and strength, promote normal movement patterns of stroke patients, inhibit abnormal posture and spasm patterns, improve patient motion control and balance function. OBJECTIVE:To systematicaly assess the effectiveness of Tai Chi on the movement, emotional disorder and quality of life in patients with stroke. METHODS: Al extracted data from databases of PubMed, EMbase, Web of Science, EBSCO, Google Scholar, CNKI, VIP and Wangfang were obtained, which were the randomized controled trials addressing the effects of Tai Chi on the movement, emotional disorder and quality of life in patients with stroke. The retrieval time was from database establishment to July 1st, 2015. According to the inclusion and exclusion criteria, two reviewers independently screened, extracted data and assessed the methodological quality of the included literatures. Then the meta-analysis was conducted using RevMan 5.3 software. RESULTS AND CONCLUSION:A total of 15 randomized controled trials involving 1016 patients were included. The results of Meta-analysis suggested that: Tai Chi was superior to the conventional rehabilitation in improvement of balance function [mean difference (MD)=7.87, 95% confidence interval (CI) (4.56, 11.18),P < 0.000 01], gait speed [MD=0.27, 95%CI (0.04, 3.94),P=0.02], anxiety [standardized mean difference (SMD)=-0.47, 95%CI(-0.89,-0.04),P=0.03] and quality of life [SMD=0.65, 95%CI(0.10, 1.19),P=0.02], and there were statistical differences. But there was no statisticaly significant difference in the improvement of depression and functional walking ability. These findings indicate that Tai Chi is superior to the conventional rehabilitation in the improvement of balance function, gait speed, anxiety and quality of life. However, large-sample, high-quality randomized controled trials are needed to provide more reliable evidence for the effect of Tai Chi in depression and functional walking ability.
7.27-gauge microincision vitrectomy surgery system for clinical application

Objective To observe the preliminary clinical results and safety of 27-gauge microincision vitrectomy surgery for partial vitreoretinal diseases.Methods A total of 13 patients (13 eyes) who underwent 27-gauge microincision vitrectomy surgery were enrolled.The follow-up period was 6 to 12 months.Preoperative and postoperative visual acuity and intraocular,total operative time,cutting time for removing vitreous,wound healing status,intraoperative and postoperative complications were observed.Results Mean best corrected visual acuity improved from preoperative (1.26±0.66) logMAR (0.10±0.09) to postoperative (0.63±0.52) logMAR (0.35±0.24),and the difference was statistically significant (t=2.743,P=0.018).The difference of mean preoperative intraocular pressure (IOP),IOP of postoperative day 1,day 5,one month and final postoperative visit were not statistically significant (F=0.593,P>0.05).The mean total operative and cutting times were (36.38±14.97) min and (10.12±3.54) min respectively.Postoperative scleral incision showed linear closure,no cases of postoperative sclerotomyrelated complications such as wound dehiscence,vitreous incarceration and subcoujunctival fluid were observed.No intraoperative and postoperative complications of iatrogenic retinal breaks,endophthalmitis,choroidal detachment,retinal detachment and vitrous hemorrhage were observed.Conclusions The 27-gauge microincision vitrectomy surgery can improve postoperative visual acuity for treatment of vitreoretinal diseases and induce fewer sclerotomyrelated complications,which maybe a safer surgical approach.
8.Impacts of the elbow in different positions on the displacement of median nerve and radial nerve:an X-ray evaluation

BACKGROUND:The position of median and radial nerves corresponding to elbow is usual y estimated by macroscopical measurements, but there are big data errors when the elbow is bent. OBJECTIVE:To measure and analyze the impacts of elbows in different positions on median and radial nerves under X-ray. METHODS:Ten fresh frozen upper limb specimens were selected. The distance of median and radial nerves to the osseous marks inside the elbow was measured when the elbow was in extension, extension-half pronation, bent at flexion 90 degrees and bent at 90 degrees with the injection of normal saline into the joint capsule, respectively. RESULTS AND CONCLUSION:The positions of median and radial nerves were not influenced when the elbow was in extension or in extension-half pronation (P>0.05). The distance of median and radial nerves to the osseous mark of the elbow was significantly lengthened when the elbow was moved from from extension to was bent at 90 degrees (P<0.05). The intra-articular injection of normal saline when the elbow flexion 90 degrees significantly elongated the distance of median and radial nerves to the osseous mark of elbow (P<0.05). These results suggest that the elbow in different positions can induce the displacement of median and radial nerves. In addition, the distance between median, radial nerves and the osseous marks of the elbow is obviously lengthened when the elbow is in flexion with intra-articular injection of normal saline.

10.The role of TLR4 acetylization in its inflammatory pathway in the pathogenesis of gestational diabetes mellitus

Objective To probe into the pathogenesis of gestational diabetes mellitus (GDM) by studying expression of TLR4 acetylation in peripheral blood mononuclear cells of gravida with GDM as well as its role in TLR4 inflammatory pathway. Methods 30 normal gravidas and 30 gravidas with GDM were enrolled in the study, 15 mL peripheral blood from every participant was collected for extraction of mononuclear cells via density gradient centrifugation and culturing in vitro by LPS. The expression of TLR4 acetylation in the mononuclear cells was detected using immunoprecipitation and western blot and the levels of inflammatory cytokines, TNF-α, IL-1 and IL-10 in the supernatant were detected by ELISA. Results TLR4 acetylation appeared positive in GDM group, and negative in the control group. After the LPS intervention, the degree of TLR4 acetylation was enhanced significantly and the latter was significantly higher than those of the GDM group and the normal + Lps group (P < 0.05). The differences in the levels of inflammatory cytokines, TNF-α, IL-1, IL-10, were statistically significant (P < 0.05). Conclusion TLR4 acetylation exists in mononuclear cells of gravida with GDM, and mediates the release of inflammatory cytokines by influencing the activation of TLR4 inflammatory pathobenesis, which contributes to the promoted antiinflammatory - proinflammatory imbalance in gravidas. In this way, it is involved in the growth of GDM.
