1.Clinical Comparison of Incidence of Diarrhea between Cerebral Hemorrhage and Cerebral Infarction

Objective To analyze and compare the incidence of diarrhea between cerebral hemorrhage and infarction. Method We observed and compared the time when diarrhea occurred in both 147 cases of cerebral hemorrhage patients and 142 cases of cerebral infarction patients by using statistical methods. Result The incidence of diarrhea in former group was 36.05%, and that of latter was 16.91%. Conclusions The incidence in patients with diarrhea of cerebral hemorrhage was markedly higher than that of patients with cerebral infarction. This may attribute to the higher intracranial pressure in cerebral hemorrhage patients than that of cerebral infarction ones. The complications of reduction of immune function,intestinal infection and hypoproteinemia may be the other factors resulting in the higher incidence of diarrhea in cerebral hemorrhage patients than that of cerebral infarction ones.
2.Primary central nervous system lymphoma:report of 43 cases

Objective To explore the clinical characteristics,therapy and prognosis of primary central nervous system lymphoma (PCNSL).Methods Forty-three PCNSL patients were retrospectively analyzed.Results The most frequent PCNSL clinical presentations were an increased intracranial pressure, focal neurologic deficits.The median survival time of patients was 18.0 months (14.2—21.8 months,95% CI).Cox regression model analysis revealed that ependymal and leptomeningeal involvement was the only independent factor for overall survival (P=0.002).Conclusions PCNSL is a aggressive disease with a poor prognosis and difficult to diagnose before operation.The optimal treatment modality for newly diagnosed PCNSL is still under investigation.
3.Inhibitory effects of PTEN gene transfection combined with L-OHP on proliferation of human cholangiocarcinoma cells

Objective: To investigate the inhibitory effects of PTEN gene transfection combined with L-OHP on human cholangiocarcinoma cell line, QBC939, providing a new method for gene therapy of human biliary duct carcinoma. Methods: A eukaryotic expression vector containing PTEN gene was transfected into human QBC939 cells under mediation of lipofectamine and positive cell clones were selected and amplified. Expression of PTEN gene was detected by immunohistochemistry. MTT test was used to determine the in vitro activity of cells, electron microscope was applied to observe cell ultrastructure, and flow cytometry was used for determining the cell cycle and apoptosis. 4.Ear endoscope applied in otosclerosis artiifcial stapes surgery

Objective To investigate the application of ear endoscopic in artificial stapes surgery of otosclerosis.Methods Artificial stapes surgery was performed by two amplification device, microscope or ear endoscope, in 64 cases of patients with otosclerosi. Then compare the operation time and analyze the effect of surgery.Results Patients postoperative hearing of the two groups were signiifcantly improved, the difference of air bone conduction was statistically signiifcant (P < 0.05). There has no postoperative complications of all the patients. While the operation time was slightly longer in ear endoscope group than that in microscope group, and the difference has statistical significance (P > 0.05).Conclusion Compared with microscopic surgery, ear endoscope artificial stapes surgery has its’ unique advantages with clear image and small trauma.
5.Risk factor analysis of idiopathic epiretinal membranes in a routine health check-up group

BackgroundIdiopathic epiretinal membranes(ERMs) is a common eye disease condition that leads to progressive decline of visual acuity. Studying the risk factors relating to this disease will shed light on its pathogenesis and allow opthalmologists to screen the affected individuals among the high-risk population and prepare for prevention and management strategies. ObjectiveThis survey was to investigate the risk factors of idiopathic ERMs in the population undergoing routine health check-ups. MethodsThe clinical data of idiopathic ERMs was obtained from the population of routine health check-ups in Peking Union Medical College Hospital from November 2009 to October 2010. The examination outcomes were compared between the individuals with and without idiopathic ERMs. The demographic and clinical factors associated with idiopathic ERMs were analyzed and assessed using univariate and multivariate logistic regression analyses. Result A total of 27 400 people were included in the survey and idiopathic ERMs were diagnosed in 76 cases. No obvious eye complaint was obtained from the idiopathic ERMs. The number of people affected with idiopathic ERMs was 12 ( 12/11 659 ) in the below 40 years group, 21 (21/4595) in the 51-60 years group and 32 (32/2544) in the over 60 years group. Hypertension, diabetes, diedyslipidemia, renal function insufficiency ,and cataract were found in 42% ,5% ,66% ,6% and 8% of the patients, respectively. The univariate logistic regression analyses revealed that significant correlations were found between age,hypertension,hyperlipidemia and history of cataract( P<0. 01 ). Multivariate regression models showed that the risk of idiopathic ERMs increased in age of 51-60( OR=2. 5,95% CI:1. 2-5.4,P=0.02) and over 60 years( OR =7.3,95% CI:3.4-15.6 ,P<0.01 ) and patients suffering from hyperlipidemia ( OR--2. 1,95% CI:1. 3-3.5, P<0. 01 ). ConclusionsOver the age of 50 years and hyperlipidemia are primary risk factors of idiopathic ERM.

Background Granulocyte colony-stimulating factor(G-CSF)has been reported to have beneficial effect on cardiac dysfunction in post infarction and doxorubicin-induced cardiomyopathy.Objective To investigate the effects of G-CSF on cardiac remodeling in cardiac hypertrophy induced by angiotensin Ⅱ(Ang Ⅱ).Methods Thirty-six male wild type mice(WT)were allocated randomly to receive subcutaneously G-CSF(10 ?g/kg per day, n=9),or Ang Ⅱ(2.88 mg/kg per day,n=9),or Ang Ⅱ plus G-CSF(Ang Ⅱ 2.88 mg/kg+G-CSF 10 ?g/kg,n =9)for 4 weeks with untreated WT(n=9)as controls.Blood pressure and cardiac function were measured. 8.Semiquantitative evaluation of salivary gland function after 131I therapy in patients post thyroidectomy

Objective To semiquantify salivary gland damage after 131I treatment in patients post thyroidectomy using salivary gland scintigraphy. Methods Fifty-six patients underwent salivary gland scintigraphy 6 months after 131I ablation therapy following thyroidectomy, including 21 with both baseline (before 131I treatment) and follow-up (6 months after the 1st 131I treatment) imaging. Salivary gland function was quantified by uptake ratios at 4 minutes (UR4) and 15 minutes (UR15), and excretory index at maximum secretion (MS), time duration from stimulation to minimum count (Tmin ). Paired t test was used for the 21 patients with both baseline and follow-up imaging. All the studies were divided into four groups: before 131I therapy, after 1st therapy, after 2nd therapy, and after 3rd or more times of therapy. Group differences were evaluated by the one-way analysis of variance (ANOVA)/Kruskal-Wallis test. Spearman test was used to analyze the correlation between the parameters and times of therapy. Results After the 1st 131I therapy, UR15 for the left and right parotid glands were 16% and 14% lower than the baseline, respectively (t=2.188, 3.322, both P<0.05). All the other parameters were not significantly different from those of baseline (t: -0.952 to 2.039, all P>0.05). Among the four groups, significantly different parameters for both parotid glands were found: UR4, UR15, MS for the left parotid of the four groups were 1.76±0.29, 2.60±0.38, (72.8±24.2)%; 1.55±0.34, 2.15±0.51, (64.4±21.6)%; 1.55±0.40, 2.02±0.68, (57.2±34.2)%; 1.45±0.33, 1.69±0.46, (30.6±36.9)%; respectively (F values for UR4 and UR15 were 7.018, 3.112 and H value for MS was 12.240, all P<0.05). UR4, UR15, MS for the right parotid were 1.81±0.33, 2.57±0.51, (69.1±18.5)%; 1.61±0.38, 2.19±0.59, (64.2±25.0)%; 1.60±0.42, 2.00±0.62, (53.2±41.7)%; 1.48±0.38, 1.63±0.29, (26.1±45.9)%, respectively (F=13.393,10.194,H=26.569,all P<0.05). However, no statistical differences were P>0.05). According to pair-pair comparison, only the degree of reduction of UR15 for parotid glands was significantly different between the 1st and 2nd therapy (P<0.05). UR4, UR15, MS for bilateral parotid glands reduced significantly after 3 or more times of therapy (all P<0.05).The parametres UR4, UR15, MS were correlated with times of 131I therapy (r:-0.296 to -0.566, all P<0.05). Conclusions Salivary uptake function is impaired slightly after the 1st radioiodine therapy. After several times of therapy, both parotid uptake and excretion functions are impaired. Submandibular functions are not affected even after repeated 131I therapy.

BACKGROUNDAppropriate planning and staffing for medical services at large-scale athletic events is essential to provide for a safe and successful competition. There are few well-documented accounts describing the demand for such services. The present study provided the data from the Beijing 2008 Olympics and Paralympics, with a view to provide the guidance for planning future events.
METHODSA total of 22 029 and 8046 patients, who received medical care from a physician at an Olympic or Paralympic medical station, were included. The patient proportion among different personnel, various disease proportions at different kinds of venues, and the disease spectrum at specified venues at the Olympics and Paralympics were analyzed.
RESULTSAt both games, the patient proportion varied by accreditation status. The staff accounted for the largest number of visits at the Olympics (44.83%) and Paralympics (36.95%), with respiratory diseases the most common. Various disease spectrums were discovered at the different kinds of venues. Surgical diseases were the most frequently listed reason for visits, both at competition and non-competition venues, especially during the Paralympics. The sport-related injuries accounted for a majority of the surgical cases during both games. At training venues, ear nose and throat diseases accounted for the greatest number of visits during both games.
CONCLUSIONSDuring both games, people contracted different diseases at different venues. Adequate surgeons should be designated to offer assistance mostly in trauma situations. Appropriate numbers of physicians in respiratory diseases and otorhinolaryngology is of great importance.
