Objective To investigate the predictive value of trauma index on the prognosis in pa-tients with traumatic spinal cord injury.Methods Enrolled 168 cases of patients with traumatic spinal cord injury,the traumatic index score and the American Spinal Injury Association (ASIA)grade were as-sessed on admission,then followed up for 6 months,the Spearman correlation analysis was performed to a-nalysis the relationship between trauma index and ASIA grade,and Receiver operating curve (ROC)anal-ysis was used to evaluate the relationship between trauma index and prognosis.Results There was a sig-nificant negative correlation between trauma index score and ASIA grade(r =-0.68,P <0.01).A total of 157 cases (93.5%)completed follow-up,the trauma index score had a good predictive value on the traumatic death in these patients,it's sensitivity was 71.4%,specificity was 76.1%,the best cut-off point was 25 points(P <0.01 );Compared with the low score group (<25 points),the high group (≥25 points)showed a lower ASIA grade and higher mortality,the differences were statistically significant (P <0.01).Conclusion The trauma index score was related with the spinal cord injury,and it is a predictive factor of poor prognosis in patients with traumatic spinal cord injury.

Objective To explore the differences of H .pylori infection between residents in different humidity environment. Methods 137 persons from Sanjiang of Wenchuan and 320 persons from Chengdu were examined.Blood samples were ob-tained for serological analysis by dot immunochromatographic assay.Results The infection rate of H .pylori in Wenchuan of Sanjiang residents was 26.28%,and the Chengdu rate was 16.88% (P <0.025),with the significant difference among which,and removed the effection of nation constitution.Conclusion The differences of H .pylori infection rate between res-idents in different humidity environment were existent.High humidity environment maybe one of factors that lead to high H.pylori infection rate.

Objective To compare the serum pepsinogen (PG)in different altitude population.Methods By testing serum PG of people from different altitudes,detected the concentration of serum PG.Results The serum PGI,serum PGII and PGR were respectively 136.89±22.64 μg/L,10.05 ± 5.14 μg/L and 13.07 ± 7.41 in 1 000 m group,and they were 30.87 ± 48.66 μg/L,11.55±2.78 μg/L and 12.89±4.76 in 1 300 m group.There were significant difference in PGI and PGII(P <0.05)except PGR (P >0.05)between the two groups.The corresponding indices in 1500m group (126.64±29.84 μg/L, 12.18±5.99 μg/L and 12.2±14.84,respectively),the serum PGI was decreased obviously compared with those groups (P<0.05),the serum PGII was increased obviously compared with those groups (P < 0.05),No significant difference was found when compared with those groups (P >0.05).The serum PGI was negative correlated with higher altitudes,the ser-um PGII was positively correlated with higher altitudes.Conclusion The study found that the serum PGI content would re-duce with the increase of altitude,and on the contrary the serum PGII would rise with the increase of altitude.The main rea-son that the serum PG change may be influenced by oxygen partial pressure.

Objective To explore the clinical value of transvaginal three-dimensional color Doppler ultrasound in assessing effects of Chinese herbal drugs on improving endometrial receptivity in patients with thin endometrium.Methods A total of 76 cases of patients diagnosed with thin endometrium in Jiangbei District Hospital of Traditional Chinese Medicine from June 2015 to October 2016 were selected and treated with Chinese herbal prescription for three months.The endometrial thickness,endometrial volume and blood flow indexes before and after treatment were measured by using transvaginal three-dimensional color Doppler ultrasound.Results After 3 months treatment,the endometrial thickness and endometrial volume were increased,and the pulse index (PI) and resistance index (RI) of endometrial blood flow were decreased,compared with those before treatment,there were statistically significant differences(P＜0.05).The results of three-dimensional color Doppler ultrasound indicates that after 3 months treatment the vascularization index (VI),flow index (FI) and vascularization flow index (VFI) were significantly higher than those before treatment,there were statistically significant differences (P＜0.05).Conclusion The Chinese herbal prescription could increase the thickness and volume of endometrium,augment blood supply,and improve endometrial receptivity significantly.Transvaginal color three-dimensional Doppler ultrasound is a convenient and objective method for evaluating clinical efficacy of Chinese herbal prescription in patients with thin endometrium,which has high practical value.

OBJECTIVEThe device of a bone age film of analysis and process was designed, can calculate the expected patient's height and identify growth level and development.

METHODSSelect the DR bone age of 100 children of 6-13 years old, used semi Lagrange algorithm of target region of interest on bone age piece (ROI) for image analysis, compared with 2 pediatric endocrinologists (A, B) who used TW3 artificial to judge bone age (two methods were detected 2 times), and report the results.

RESULTSBone age assessment process, forecast error of bone age reduced to 0.12 years.

CONCLUSIONSThis device can quickly calculate the expected patient's height and identify his growth level, improve the speed and accuracy of bone age assessment, especially in the poor medical conditions in rural and remote areas.

Objective To identify whether the helper T lymphocyte 1 (Th1)/helper T lymphocyte 2 (Th2) of patients' serum and bronchoalveolar lavage fluid (BALF) at admission could represent the severity of idiopathic pulmonary fibrosis (IPF) and whether its change at six months could predict the progression of the disease.Methods Eighty-three patients with IPF were subjected to pulmonary function tests (PFFs),dyspnea scores,arterial blood gas analysis,six-minute walk test (6MWT) and high-resolution computed tomography (HRCT).The serum and BALF specimen of these patients were obtained as well as 20 control serum and 10 control BALF specimen.A total of 55 patients were followed up,and their BALF and serum levels of interferon γ (IFNγ) and IL-4 were detected by enzyme-linked immunoadsorbent assay (ELISA).The correlation between the IFNγ/IL-4 levels (at admission and the change of that at six months follow-up) and the clinical,physiological and image features in the IPF patients were analyzed.Results The baseline serum and BALF level of IFNγ/IL-4 (0.8 ± 0.3) in the IPF patients was lower than that in the control group (1.4 ± 0.2),which showed significant correlation with the course of disease,dyspnea scores,FEV1 ％,FVC％,TLC％,maximum desaturation,6MWD and CT-fib (all P values ＜ 0.05).The serum level of IFNγ/IL-4 showed positive correlation with CT-alv (r =0.340,P ＜ 0.01).During follow-up,no statistic difference was found in the serum levels of IFNγ,IL-4 and IFNγ/IL-4 between the IPF patients with or without glucocorticoids treatment.There were significant aggravation in the dyspnea scores,FEV1％,FVC％,CT-alv,CT-fib,IFNγ and IL-4 at six months follow-up.Significant correlation had been showed between the change of the serum IFNγ/IL-4 level with the changes of the dyspnea scores,FVC％,TLC％,DLCO％,6MWD and CT-fib in the IPF patients (P ＜0.05).Conclusions There are disequilibrium of the Th1/Th2 in the serum and the BALF of the IPF patients.The Th1/Th2 level could represent severity of the disease,and the serum level change of Th1/Th2 in the follow-up could predict the progression of the diseases in the IPF patients.

Objective To study the effect of modified air bag deflation lubrication by Sengstaken-Blakemore tube on voluntary dysphagia induced by esophageal variceal bleeding.Method Twenty seven patients with voluntary dysphagia induced by esophageal and gastric variceal bleeding were managed with modified air bag deflation lubrication with Sengstaken-Blakemore tube.Results The indwelling time for the Sengstaken-Blakemore tube was 3~6 d.There was no complication of gastrointestinal mucosal injury and aspiration complications.Conclusion The modified air bag deflation lubrication by Sengstaken-Blakemore tube may effectively avoid the complications by hemostasis with Sengstaken-Blakemore tube.

97 cases of jaw bone fractures were treated with rigid internal fixation.16 cases had malocclusion postoperation.The etiology was analyzed and the prevention methods were proposed.Intermaxillary elastic traction or fixation and craniomaxillofacial fixation or craniomentum elastic traction were performed to treat the malocclusion.By this way,14 cases were cured.1 case was operated again to resume the normal occlusion,1 case received occlusal adjustment.Incomplete reduction,incorrect operative performance,deficiency of postoperative intermaxillary fixation and incorrect intermaxillary fixation were the main causes to malocclusion.So anatomic in time reduction,correct operative performance,intermaxillary fixation,application of craniomaxillofacial fixation or craniomentum elastic traction instead of intermaxillary fixation for some special cases are effective methods to correct postoperative malocclusion.

Objective To identify the independent prognostic factors for femoral head avascular necrosis after internal fixation of femoral neck fractures.Methods A retrospective analysis was made on 246 patients (246 hips) with femoral neck fractures treated by internal fixation between January 2002 and January 2010.There were 111 male and 135 female patients aged 19-59 years.A total of 101 patients were injured in traffic accidents,79 patients in falls from the height and 66 patients in ground-level falls.Eight parameters were identified as the prognostic factors of femoral head avascular necrosis,including gender,fracture type,preoperative traction,interval from injury to surgery,reduction methods,quality of fracture reduction,time of weight bearing,and fixation removal.Multivariate Logistic regression analysis was used to explore the risk factors of postoperative femoral head avascular necrosis.Results Period of follow-up was 1-13 years (mean,7.5 years).There were 38 patients (15.4％) with avascular necrosis of the femoral head.Multivariate analysis identified fracture type (95％ CI-3.41--2.43,P ＜ 0.01),preoperative traction (95％ CI-3.77--0.6,P ＜ 0.05) and quality of fracture reduction (95％ CI -3.33--0.94,P ＜ 0.01) as the independent predictors of avascular necrosis of the femoral head.Conclusion Fracture type,preoperative traction and quality of fracture reduction are the independent risk factors for avascular necrosis of the femoral head after internal fixation of femoral neck fractures.

Objective To study the identification of rare Nocardiaotitidiscaviarum and its drug sensitivity .Methods Morpho-logical ,physiological and biochemical phenotype identification methods combined with 16S rRNA sequence analysis were employed to identified the bacteria strains .Broth method was used to detect their drug sensitivity .Results The clinically isolated Nocardia strain was identified as Nocardiaotitidiscaviarum which was resistant to piperacillin ,erythromycin ,ceftazidime ,cefoperazone ,gen-tamicin ,cefoxitin ,piperacillin/tazobactam ,and was sensitive to vancomycin ,levofloxacin ,imipenem ,etimicin ,ciprofloxacin ,amika-cin ,sulfamethoxazole .Conclusion 16S rRNA gene sequence analysis can be used to identify Nocardia otitidiscaviarum ,and sulfa drugs shows good therapeutic effect .
