1.Discussion on construction of medical equipment in grass-roots unit

This paper aims to find out the use status of medical and sanitary equipment in primary unit. By means of investigating grass-roots health institutions and summarizing practical works, such problems in equipment were detected as standard vacancy, datedness and laggardness, partly absence, nonstandard management, untimely maintenance and low rate of utilization, etc. This paper puts forward some suggestions corresponding to the above disadvantages.
2.Construction of lentivirus vector containing human ?-catenin-EGFP and its expression in human hair follicle stem cells

Objective To construct the lentivirus carrying human ?-catenin-EGFP(enhanced green fluorescent protein)and observe its expression in human follicle stem cells.Methods The ?-catenin gene sequence was amplified by RT-PCR from extraction of total RNA of human vascular endothelial cells.TA cloning technique was utilized to acquire gene subcloned pUCm-T-?-catenin.After transformation reaction,candidate clone was further analyzed by PCR and gene sequencing.Then the plasmid was transfected into FT293 cells.After identification by Western blotting,the plasmid was transfected into FT293 cells again for packaging.Infection titer was monitored by green EGFP expression.The expression of ?-catenin-lentivirus in human follicle stem cells were observed under inverted fluorescence microscope.Results The ?-catenin gene was cloned into the lentivirus successfully.The high expression of green fluorescence protein in FT293 cell line was found under fluorescent microscope.Viral titer checked by real-time PCR was about 2.0?108 TU/mL.When the multiplicity of infection(MOI)was 10,the infection efficiency of ?-catenin-lentivirus in human follicle stem cells was nearly 80% after infection 48 h around.After 3 weeks of continuous observation,we found the infection efficiency still keeping in the range of 80%-90%.Conclusion The lentivirus expression vector for ?-catenin was successfully constructed.It can steadily infect human follicle stem cells and the infection efficiency is considerable high.
3.Distribution and drug resistance of pathogens isolated from wounds in orthopedic patients

Objective To determine the distribution and drug resistance of pathogens isolated from wounds in orthopedic patients.Methods Wound samples were collected from 999 orthopedic patients in the Third Hospital of Hebei Medical University during July 2010 and June 2012.Pathogens were isolated and identified.The antimicrobial susceptibility test was performed by Kirby-Bauer method.Extended spectrum β-1actamases (ESBLs) were detected by double-disk diffusion test,and methicillin-resistant Staphylococcus aureus (MRSA) was detected by cefoxitin disk diffusion test.WHONET 5.4 was used for drug resistance analysis.Results A total of 1056 strains of pathogens were isolated,of which gram-positive cocci,gramnegative organisms and fungi accounted for 69.98% (739/1056),28.79% (304/1056),and 1.23%(13/1056),respectively.The top 5 pathogens were Staphylococcus aureus (265.25.09%).Pseudomonas aeruginosa (245,23.20%),Acinetobacter baumannii (199,18.84%) and Escherichia coli (86,8.14%).The resistant rates of Pseudomonas aeruginosa to cefoperazone/sulbactam,ciprofloxacin,levofloxacin and piperacillin/tazobactam were 13.58%,21.25%,21.67% and 22.45%,while the highest resistance rate was to compound sulfamethoxazole (98.04%).Acinetobacter baumannii was highly resistant to most antibacterial agents except cefoperazone/sulbactam (14.29%),and 11 out of 199 strains were pandrug resistant.Enterobacteriaceae were completely susceptible to imipenem and meropenem,and their resistance rates to cefoperazone/sulbactam,piperacillin/tazobactam and amikacin were 1.16%-28.12%.ESBLs positive rates of Escherichia coli and Klebsiella pneumoniae were 48.84% (42/86) and 34.38%(11/32),respectively.Staphylococcus aureus were susceptible to vancomycin,teicoplanin and linezolid,but were highly resistant to ampicillin (100.00%),penicillin G (99.25%) and erythromycin (80.06%).The rate of MRSA was 41.51% (110/265).Enterococcus were highly resistant to erythromycin,rifampin,levofloxacin,high-concentration gentamicin,minocycline and nitrofurantoin,but were susceptible to teicoplanin and linezolid,and the rate of vancomycin intermediate strain was 4.55% (1/22).Conclusions The pathogens of wound infections in orthopedic patients were of wide variety.Staphylococcus aureus,Pseudomonas aeruginosa,Acinetobacter baumannii,Escherichia coli and Enterobacter cloacae were the most prevalent pathogens,and most strains were multi-drug resistant.
4.Value of serum procalcitonin in diagnosis of bacterial infection in burn patients

Objective To explore the clinical significance of procalcitonin(PCT) in the diagnosis of bacterial infection in burn pa‐tients .Methods Among 169 burn patients ,96 cases were bacterial infection confirmed by blood ,secretion and puncture fluid culture and 73 cases were non‐bacterial infection .PCT ,WBC and hyper sensitive C reactive protein (hs‐CRP) were detected .The diagnostic values in bacterial infection were compared among these 3 indexes and the relationship between PCT level and burn degree was fur‐ther studied .Results Serum PCT ,WBC and hs‐CRP levels in the bacteria infection group were obviously higher than those in the non‐bacterial infection group(P<0 .05);the sensitivity ,specificity ,positive predictive value and negative predictive value of PCT were 90 .63% ,89 .04% ,91 .58% and 87 .84% respectively ,which indicated that PCT had higher diagnostic value than WBC and hs‐CRP .The PCT level was positively correlated with the degree of burn degree .Conclusion Serum PCT ,WBC and hs‐CRP in the burn patients with bacteria infection are greatly increased .PCT as the marker of bacterial infection has the higher sensitivity and specificity than WBC and hs‐CRP in the diagnosis of bacteria infection .

6.Etiology and prevention of malocclusion after internal fixation in jaw bone fractures

97 cases of jaw bone fractures were treated with rigid internal fixation.16 cases had malocclusion postoperation.The etiology was analyzed and the prevention methods were proposed.Intermaxillary elastic traction or fixation and craniomaxillofacial fixation or craniomentum elastic traction were performed to treat the malocclusion.By this way,14 cases were cured.1 case was operated again to resume the normal occlusion,1 case received occlusal adjustment.Incomplete reduction,incorrect operative performance,deficiency of postoperative intermaxillary fixation and incorrect intermaxillary fixation were the main causes to malocclusion.So anatomic in time reduction,correct operative performance,intermaxillary fixation,application of craniomaxillofacial fixation or craniomentum elastic traction instead of intermaxillary fixation for some special cases are effective methods to correct postoperative malocclusion.
7.Distribution and drug resistance of bacterial pathogens isolated from orthopedic wounds during 2008 and 2012

Objective To investigate the distribution and drug resistance of bacterial pathogens isolated from orthopedic wounds.Methods Data of bacterial strains isolated trom orthopedic wounds in the Third Hospital of Hebei Medical University from January 2008 to December 2012 were retrospectively analyzed.Strains were identified by using French bioMérieux Vitek32 identification system,and the drug susceptibility was tested by Kirby-Bauer method.Chi-square test for linear trend was performed to reveal the changes of distribution and drug resistance of the strains.Results A total of 2 456 bacterial strains were isolated,vith 1 652 (67.26%) gram-negative bacilli,777 (31.64%) gram-positive cocci,26 (1.06%) fungi,and 1 (0.04%) gram-positive bacillus.The top five pathogens were Staphylococcus aureus (666 strains,27.12%),Pseudomonas aeruginosa (606 strains,24.67%),Acinetobacter baumannii (355 strains,14.45%),Escherichia coli(188 strains,7.65%) and Enterobacter cloacae (187 strains,7.61%).The positive rate of Acinetobacter baumannii was on the rise during 2008 and 2012 (x2 =35.266,P < 0.0l).The rates of pan-drug resistant strains in Acinetobacter baumannii and Pseudomonas aeruginosa were 6.20% (22/355) and 0.17% (1/606),respectively.The rates of extended-spectrum β-lactamases positive strains in Escherichia coli and Klebsiella pneumoniae were 39.89% (75/188) and 29.23% (19/65),respectively.The rates of methicillin-resistant strains in Staphylococcus aureus and coagulasenegative Staphylococcus were 40.69% (271/666) and 52.38% (22/42),respectively.The rate of vancomycin-intermediate strains in Enterococci was 3.70% (2/54).The positive rate of methicillin-resistant &aphylococcus aureus was on the rise during 2008 and 2012 (x2 =18.317,P < 0.01).Staphylococcus aureus were sensitive to teicoplanin,vancomycin and linezolid; Resistance rates to rifampicin and amikacin were 11.29%-33.33%; Resistance rates to penicillins and erythromycin were 76.80%-100.00%; Resistance rates to cefazolin,cefuroxime,cefoxitin,amikacin and levofloxacin were on the rise (P < 0.05) ; And resistance rates to sulfamethoxazole (28.11%-48.35%) were on the decline in the same period (P < 0.01).Resistance rates of Pseudomonas aeruginosa to imipenem,meropenem and sulfamethoxazole were on the rise (P < 0.05) ; Resistance rates to ciprofloxacin,levofloxacin,amikacin,gentamicin and piperacillin/ tazobactam were on the decline (P < 0.05) ; Resistance rates to cefoperazone/sulbactam were the lowest (9.15%-20.51%).Resistance rates of Acinetobacter baumannii to imipenem,meropenem,levofloxacin,piperacillin/tazobactam,sulfamethoxazole were on the rise (P < 0.01); Resistance rates to cefoperazone/ sulbactam were the lowest (11.86%-19.70%).Escherichia coli and Enterobacter cloacae were sensitive to imipenem and meropenem,and the resistance rates to cefoperazone/sulbactam and piperacillin/tazobactam were low (0-14.29%); Resistance rates of Escherichia coli to piperacillin,cefepime,amikacin,levofloxacin,cefoperazone/sulbactam were on the decline (P < 0.05) ; Resistance rates of Enterobacter cloacae to cefoxitin were on the rise (P < 0.01),while the resistance rates to piperacillin,ceftazidime,cefoperazone,ceftriaxone,levofloxacin were on the decline (P < 0.05).Conclusion During 2008 and 2012,the predominant bacterial pathogens of orthopedic wound in patients of the Third Hospital of Hebei Medical University are Staphylococcus aureus,Pseudomonas aeruginosa,Acinetobacter baumannii,Escherichia coli and Enterobacter cloacae,and most strains are multiple drug resistant.

In the field of artificial muscle(AM) research,it is very difficult for humans to make the biological muscles,but there are experiments trying to put the function of muscles into practice of technology.With the development of materials and models of mathematics theories,people have made the AM of different materials and models true,however,as far as the characteristics of them and the widely used are concerned,they need deeply scientific research.And,in the near future,AM will play an important role in medical equipments,rehabilitation medicine,bionics,intelligent robots,military weapons,aerospace,etc.The authors review the types,characteristics,application in rehabilitation,as well as the function evaluation of AM.
9.Expression of myelin basic protein in rat optic nerve with experimental allergic encephalomyelitis

Background Optic neuritis is closely associated with multiple sclerosis (MS).Its pathogenesis is uncompletely clear,and less basic researches are carried out at home and abroad. Objective This study was to reveal the expression of myelin basic protein (MBP) in the optic nerve of rat with experimental allergic encephalomyelitis (EAE) and to provide a theoretical evidence for the research of the relationship of optic neuritis with MS. Methods Fifty clean Wistar rats were randomized into the control group and immune 8,12,18 and 25 days groups.Myelencephalon was collected from 5 guinea pigs to prepare the homogenate and mixed with the isovolumetric complete Freud' s adjuvant (CFA).The 0.5 ml mixed antigen emulsifier was subcutancously injected into the 4 maps together with Bordetella pertussis 0.2 ml under the cutancous of dorsalis pedis at 0 and 48 hours to induce the EAE.Behavior of the rats was evaluated to score the neurological function.The optical nerve sections were prepared 8,12,18 and 25 days after immunology for the histopathological examination,and immunochemistry and Western blot were used to detect the expression of MBP in optic nerve.The use of the animals complied with the Regulation for the Administration of Affairs Concerning Experimental Animals by State Science and Technology Commission. Results The disorder of motor nerve was seen 12 days following the immune,and the clinical neural functional scores were significantly higher 12 day and peaked on 18 days myelination and then gradually reduced.The histopathological examination showed that the irregular alignment of neural fiber was found at 12 days,and changes of cellular structure,edema of neural shaft bunch were observed at 18 days.However,the abnormal cells were significantly less 25 days following the immune.Immunochemistry showed that the MBP was expressed mainly in the myelination of optic nerve fibers.The numbers of positive cells for MBP were (115.75±26.49)cells/5 fields at the 12th day,showing a significant lowing in comparison with (167.44±22.49)cells/5 fields of control group (t=4.537,P<0.05 ).The positive cells were lest at the 18th day with the values ( 75.57 ± 34.54) cells/5 fields ( t =6.362,P<0.01 ).At the 25th day,positive cells increased to ( 117.63 ± 13.78) cells/5 fields,which still was lower than those of the control group ( t =4.068,P<0.05 ).Western blot assay illuminated that with the prolong of immune time,MBP/β-actin ratio in optic nerve was gradually reduced and followed the same pattern at the 12th day( t =4.639,P<0.05 ).At 18 days after immune in comparison with the control group,the expression of MBP/β-actin ratio in optic nerve was the least (t=8.427,P<0.01). Conclusions MBP can be degraded in rat optic nerve.This is a further evidence that optic neuritis is a severe demyelination disease.It is clearly related to MS.
10.Titanium alloy steel artificial rib reconstruction of chest wall after tumor resection

Objective To evaluate the effects of chest wall reconstruction after tumor resection patients with chest wall defect. Methods From March 1999 to August 2008,28 patients with chest wall tumor underwent chest wall reconstruction.The number of rib resected was 3-6. Twenty-one huge chest wall defects underwent chest wall reconstruction using titanium alloy steel artificial rib. The materials for the soft tissue reconstruction include latissimus dorsi myocutaneous flap. Results No died of operation, postoperative respiratory function was good, no abnormal breathing exercises. Concurrent osseous tissue repair and rebuilt in 3 cases, the fluid recovered after conservative therapy. Followed-up 8-76 months,2 cases were lost, follow-up rate was 92.9% ,4 cases with benign tumor existed 7.8,7.2,5.4 and 3.6 years, 22 cases with malignant tumor after 1, 3, 5 years survival rate were 77.3% (17/22) ,63.2% (12/19),41.2% (7/17) respectively. Conclusion A favorable clinical outcome can be achieved by titanium alloy steel artificial rib reconstruction of chest wall.
