Objective To investigate the expression and significance of platelet membrane glyco- proteins and antiplatelet antibody in patients with anaphylactoid purpura. Methods Forty-five patients with anaphylactoid purpura were divided into three groups according to their clinical manifestations,15 in dermal purpura group,18 in mixed group and 12 in nephritis group. The expression of CD62P,CD63 and CD41 in these 45 patients was detected by flow cytometry. The level of antiplatetlet antibody was analysed by ELISA with monoclonal antibody as the probe. Results The percentage of CD62P,CD63 and the level of an- tiplatelet antibody in patients in acute phase were significantly higher than those in regressive phase and those in normal control.Neither of the expression of CD62P,CD63 and CD41,nor the level of antiplatetlet antibody in nephritis group was significantly different from that in the mixed group.Compared with dermal purpura group, the percentage of CD62P was significantly higher in the nephritis group and mixed group. There was no significant difference in CD41 expression among all groups. Conclusion Abnormal humoral immunity exists in anaphylactoid purpura. The activation of platelet could be closely related to the progress of anaphylactoid purpura.CD62P and CD63 could be considered as an index for monitoring of symptoms, determination of prognosis and predicting the outcome in anaphylactoid purpura.

Objective To compare anatomically the potential incidences of injury to the nerve roots of ventral and dorsal rami caused by the transarticular screws and Magerl lateral mass screws in the fixation of subaxial cervical spine. Methods Fixations with the transarticular screws and Magerl lateral mass screws were performed in this study. For each kind of fixation,20 screws were inserted from C3 to C7 in two specimens.A 20-ram long screw was used to over-penetrate the ventral cortex. The anterolateral aspect of the cervical spine was carefully dissected to allow ob- servation of the screw-ramus relationship. Results The overall percentage of nerve violation was significantly lower in the transarticular screw fixation(45%)than in the Magerl lateral mass screw fixation(90%).The difference was sta- tistically significant between the two fixation methods(P<0.05).The greatest percentages of nerve violation caused by the transarticular screws and lateral mass screws were found at the dorsal ramus,25% and 45%,respectively. Conclusion The potential risk of nerve root violation caused by the transarticular screw technique is lower than that by the Magerl lateral mass screw technique in the lower cervical spine.

OBJECTIVETo study the clinical effect of arthroscopic excision of popliteal cysts through a combined anterior and posterior approach.
METHODSFrom January 2010 to December 2012,20 patients with popliteal cysts were treated with arthroscopic excision. There were 14 males and 6 females, with an average age of 49.5 years old, ranging from 45 to 65 years old. The lump has been found for 4 to 18 months, with a mean time of 12 months. Their mean sagittal diameter was 4.5 cm (ranged from 3 to 7 cm). There were 12 popliteal cysts in the left and 8 popliteal cysts in the right. The main clinical manifestation included lump at popliteal fossa, swelling and pain at knee joint and some extent of dysfunction. All diagnoses were determined according to MRI, which clearly showed the communication of cyst and articular cavity. The cyst was removed under arthroscopy, through the posterior approach and then the intra-articular lesion was treated via the anterior approach. According to Rauschning and Lindgren classification, 2 patients were grade I, 6 patients were grade II, and 12 patients were grade III. The guidelines of Rauschning and Lindgren were used to evaluate the clinical effects.
RESULTSNo complications such as the injury of blood vessel and nerve, or incision infection occurred in all patients. All the patients were followed up, and the duration ranged from 8 to 24 months, with a mean time of 16 months. According to the criteria of Rauschning and Lindgren, there were 14 cases of grade 0, and 6 cases of grade I after operation, which was improved obviously compared with that pre-operation. No cyst reoccurred and the knee joint pain was relieved.
CONCLUSIONTreatment of popliteal cysts with arthroscopic excision through a combined anterior and posterior approach is effective to remove the cyst sac and treat intra-articular diseases simultaneously, resulting in the decrease recurrence rate of cyst.

6.Clinical application progress of hip arthroscopy.
China Journal of Orthopaedics and Traumatology 2011;24(9):794-797
The hip arthroscopy develops quickly in the past 20 years. Its use in the diagnosis and treatment of the hip injury get more notably. Many of pathologic conditions previously unrecognized through X-ray, CT or MRI have been diagnosed correctly under hip arthroscopy. The technology has become gradually a golden standard to confirm the hip discords. In the present, the examining path of the hip arthroscopy contains mainly the outside path, the anterior path, the outside-posterior path. The application range of the hip arthroscopy includes the management of labral tears, the femoroacetabular impingement, the ligamentum tear injuries, the chondral lesions, the synovical abnormalities, the intra-articular infection, the loose bodies in the joint etc. The hip arthroscopy is a comprehensive technology. Its superiority can be reflect well if the doctors are familiar with the indication, the contraindication and the operation procedure. And also the thorough physical examination and imaging examination should be made before operation, as well as reasonable and effective postoperative function training should be conducted.

Plasmodium vivax merozoite surface protein-1 (PvMSP1) gene codes for a major malaria vaccine candidate antigen. However, its polymorphic nature represents an obstacle to the design of a protective vaccine. In this study, we analyzed the genetic polymorphism and natural selection of the C-terminal 42 kDa fragment within PvMSP1 gene (Pv MSP142) from 77 P. vivax isolates, collected from imported cases of China-Myanmar border (CMB) areas in Yunnan province and the inland cases from Anhui, Yunnan, and Zhejiang province in China during 2009–2012. Totally, 41 haplotypes were identified and 30 of them were new haplotypes. The differences between the rates of non-synonymous and synonymous mutations suggest that PvMSP142 has evolved under natural selection, and a high selective pressure preferentially acted on regions identified of PvMSP133. Our results also demonstrated that PvMSP142 of P. vivax isolates collected on China-Myanmar border areas display higher genetic polymorphisms than those collected from inland of China. Such results have significant implications for understanding the dynamic of the P. vivax population and may be useful information towards China malaria elimination campaign strategies.
