1.Influence of diabetic self-management education on blood glucose control and inflammatory factor levels in inpatients with diabetes mellitus

Objective:To explore the influence of diabetic self-management education on blood glucose control and inflammatory factor levels in inpatients with diabetes mellitus (DM).Methods:A total of 168 DM patients,who hospitalized in department of endocrinology in 163rd hospital of Chinese PLA from Jun 2012 to Jun 2013,were ran-domly divided into self-management education group (self-management group,n=88)and routine health education group (routine management group,n = 80).Self-management group received self-management education course namely AADE7TM ,provided by full-time DM health education staff,including healthy eating,being active,monito-ring,taking medication,problem solving,healthy coping and reducing risks,while routine management group re-ceived routine DM health education.Blood glucose control,changes of levels of myeloperoxidase (MPO),tumor necrosis factor-α(TNF-α)and intercellular adhesion molecule-1 (ICAM-1)were examined before and after interven-tion.Results:Compared with before intervention,there were significant reductions in levels of fasting blood glucose (FBG),postprandial blood glucose (PBG)and glycosylated hemoglobin (HbA1c)in both groups after interventions (P <0.05 all).Compared with routine management group,there were significant reductions in levels of FBG [(7.1 ±1.3)mmol/L vs.(6.0±1.1)mmol/L],PBG [(9.5±1.6)mmol/L vs.(8.8±1.4)mmol/L]and HbA1c [(7.9 ±1.5)% vs.(7.2±1.1)%]in self-management group,P <0.05 all.Before discharge,there were significant de-crease in inflammatory factor levels in both groups compared with before intervention (P < 0.05 all).Compared with routine management group after intervention,there were significant decrease in inflammatory factor levels [TNF-α:(201.0±57.3)pg/ml vs.(100.7±49.2)pg/ml,ICAM-1:(25.2±3.6)ng/ml vs.(18.2±2.9)ng/ml] in self-management group,P < 0.05 both.Conclusion:Self-management education provided by full-time diabetic health education staff is superior to routine diabetic health education method in controlling blood glucose and decrea-sing inflammatory factor levels,indicating it can improveβcell function and relieve insulin resistance.

4.Observation on clinical therapeutic effect of Alprostadil combined with Magnesium Isoglycyrrhizinate on patients with Hepatocirrhosis at active phase

Objective To evaluate the effect of Alprostsdil combined with Magnesium Isoglycyrrhizinate on hepatocirrhosis at active phase. Method 74 inpatients collected from our hospital were randomly divided into control group(37 cases)and treatment group(37 cases). The patients in control group were given conventional liver protecting treatment. In addition to routine therapy of the control group, the patients in treatment group received intravenous Alprostadil and Magnesium Isoglycyrrhizinate injection once a day for 4 weeks. Results The total effective rate of treatment group was 67. 6%, and that of control group was 40. 5%, and the difference was significant (P < 0. 05).Conclusion Alprostadil combined with Magnesium Isoglycyrrhizinate has good clinical therapeutic effect on hepatocirrhosis at active phase.
5.Cryopreservation study on seeds and embryos in Dalbergia odorifera.

The mature seeds and excised embryos of Dalbergia odorifera were used as materials to study the effect of moisture content on their survival, as well as the effect of rapid freezing and vitrification freezing method on seeds and in vitro embryos cryopreservation. The results showed that the germination rate and vigor decreased from 82.67%, 85% to 18.35%, 25% respectively, when the seed moisture content decreased from 15.04% to 8.14%; and the germination rate decreased from 82.67% to 37.50%, 25.37% respectively by vitrification freezing method and rapid freezing method, when the seed moisture content decreased from 15.04% to 9.37%. Among all the moisture content gradient, 12.35% moisture reached the maximal germination rate, which were 63.58% and 50.45% respectively by vitrification freezing and rapid freezing; and when the embryo moisture content was 26.32%, the germination rate decreased from 95.67% to 58.31% and 33.82% respectively by vitrification freezing and rapid freezing. And when the moisture content was in the range of 14.17% -21.34%, the germination rate was a bit of decrease. The experiment results showed that the optimum conditions of seed cryopreservation were: moisture content 12.35%, vitrification freezing; and the optimum conditions of in vitro embryo cryopreservation were: moisture 15.04%, vitrification freezing. In conclusion, the effects of moisture content on germination rate after cryopreservation in D. odorifera seeds and embryo were significant, and vitrification freezing method is much better than rapid freezing method.

6.Establishment and optimization of two-dimensional electrophoresis for myocardial proteomes of mice

BACKGROUND: Two-dimensional electrophoresis (2-DE) is precise and effective for the isolation and analysis of complex protein mixtures, has become one of valuable key techniques for proteome. Mice models were as research subjects to model human disease status, one of important methods for cardiovascular disease. The establishment and optimization of 2-DE for proteomes of mice myocardium will provide basis for heart disease.OBJECTIVE: To establish and optimize a stable technique of 2-DE for proteomic analysis of mice myocardium.DESIGN: Mice were used as research subjects, observational comparative study.SETTING: Department of Pathology, Nanfang Hospital Affiliated to Southern Medical University.PARTICIPANTS: The experiment was performed at the Department of Pathology, Nanfang Hospital Affiliated to Southern Medical University between February and September 2004. Totally 12 male BABL/c mice of 4-5 weeks without special-pathogen free with the body mass of 12-15 g were selected, which was provided by the Experimental Animal Center of Southern Medical University.METHODS: The mice were put to death by dislocation of cervical vertebra under drugged state to obtain heart. Various protein extraction method,loading sampling buffer, strip range of IPG, sampling volume of protein,staining method and so on were compared, analyzed, scanned and digilized,and then performing image analysis. Each link in the study was established and optimized.MAIN OUTCOME MEASURES: Resolution of protein and reproducibility of the trial.RESULTS: A method that was optimal for cardiac tissue can isolate about a total of 910 protein spots on pH 4-7 gel strips. 200 μg was fit for silver stain while 1000 μg was for Coomassie stains. It was found that in the myocardium of normal mice about (920±30) protein spots were obtained in 17 em pH 3-10 L gel strip, while around (880±30) were gained in the pH 4-7 L gel strip. The mean matching rate achieves to 86. 9%.CONCLUSION: The findings of the trial showed that 2-DE technique can be effectively applied for proteomics of myocardium of mice to regulate and optimize, and relatively stable separation method of myocardial protein was established.
7.The value of serum pro-adrenomedullin for assessing prognosis of patients with closed rupture of small intestine

Objective To explore the relationship between preoperative serum pro-adrenomedullin (pro-ADM) in the patients with closed rupture of small intestine,and postoperative secondary abdominal infection,and to study the value of postoperative serum pro-ADM in assessment of severity of postoperative secondary abdominal infection of this patients.Methods Eighty-five patients with closed rupture of small intestine treated in the emergency surgery from June 2014 to May 2015 were selected.According to the presence of postoperative abdominal infection or not,these patients were divided into infection group and non-infection group.The infection group was further divided into SIRS,sepsis,severe sepsis,septic shock subgroups as per the severity of infection.The levels of serum pro-ADM,interleukin-6 (IL-6),C-reactive protein (CRP) were determined and the acute physiology and chronic health evaluation Ⅱ (APACHE Ⅱ) score was calculated at admission and the first day、the second day and the third day after operation.Comparisons of these biomarkers were carried out to find out the factors associated with postoperative abdominal infection.In addition,ROC curve was used to verify the factors for predicting the abdominal infection of these patients after operation.The relationship between serum levels of pro-ADM and APACHE Ⅱ score after operation was analyzed.The severity of abdominal infection after operation was assessed with laboratory findings.Results Compared with non-infection group,preoperative and postoperative serum pro-ADM (P =0.03,P < 0.01),IL-6 (P =0.02,P <0.01) levels and APACHE Ⅱ scores (P < 0.01,P < 0.01) were significantly higher in infection group (P < 0.05).In the infection groups,the postoperative levels of serum pro-ADM and APACHE Ⅱ scores were increased with the severity of infection increased (r =0.924),and the difference between the groups was statistically significant (P < 0.05),but there were no significant differences in IL-6 and CRP levels between two groups.Conclusion (1) The serum levels of pro-ADM before operation has value in predicting the genesis of abdominal infection in these patients after operation.(2) The serum levels of pro-ADM after operation has value in severity assessment of abdominal infection in these patients after operation.
8.Comparison of the therapeutic effects between penetrating resin and fluoride on early enamel caries

Objective To investigate the therapeutic effects of penetrating resin and fluoride on early enamel caries. Methods Sixty intact bovine incisors were immersed in demineralized solution for 24 hours to make bovine incisor enamel caries.The specimens were divided into four groups(n=15 for each group)according to the treatment methods:control group (CON)-immersion in artificial saliva, DF group-immersion in 0.05% fluoride solution daily, WF group-2% fluoride gel weekly and IC group-resin infiltration.After processing for four and eighe weeks,the microhardness of the surface of each group was measured.After the treatment for four weeks,the depth of penetration and the microhardness of the samples were measured. After 8-week treatment, all samples were reintroduced into the demineralized solution for 24 hours and the microhardness of the samples was measured again.Results Results of microhardness assessment showed that there were no significant differences in baseline values(after white spots)between four groups(P>0.05).After treatment for four weeks the microhardness value reached the peak in IC group. After treatment for eight weeks the microhardness values reached the peak in DF group and WF group.The values of microhardness were significantly higher at different time points in IC group than those of other groups(P<0.05).After 4-week treatment,the percentages of penetration depth were significantly higher in DF,WF and IC groups than those of control group(P<0.01).The penetration depth was significantly higher in IC group than that of DF group and WF group (P<0.05). There was no significant difference in the penetration depth between DF group and WF group(P>0.05).Conclusion For the early enamel caries penetration resin,the penetration percentage is significantly higher than the fluoride treatment.
9.Research in effect of comprehensive case-style training methods for improving nursing skills of low age and service seniority nurses

Objective To improve the nursing skills of low age and service seniority nurses and meet the needs of clinical nurses,in order to serve patients better and improve the quality of cave.Methods According to knowledge tests 50 points for nurse job-related skill issued by Chinese Ministry of Health,our hospital selected 12 commonly used basic operation skills and designed comprehensive cases through integration and reformation.We trained and examined nurses with undergraduate degree came to our hospistal in 2008.Results After training,the score of clinical nursing skill reached 91.81,86 nurses reached the standard and the effective compliance rate was 95%.Conclusions It proved to be an effective method to improve the nursing skills of low-age and service seniority nurses by carrying out the comprehensive casestyle training methods.
10.Increased expression of the 20S proteasome in peripheral blood mononuclear cells of type 2 diabetic patients

Objective To investigate the dynamic expression of the 20S proteasome in peripheral blood mononuclear cells (PBMCs) of type 2 diabetic patients without vascular complications. Methods PBMCs were prepared from 30 type 2 diabetic patients and 30 nondiabetic controls. The general indexes including weight, height and blood pressure were recorded. Fasting plasma glucose, fasting plasma insulin and glycosylated hemoglobin were measured. The protein level of the 20S proteasome was measured by Western blotting. The mRNA expression levels of the 20S proteasome β1, β2 and β5 subunits were detected by real-time PCR. Results Compared with that in the nondiabetic controls, the protein level of the 20S proteasome was significantly increased in the diabetic patients and was positively associated with glycosylated hemoglobin. Conclusion Type 2 diabetic patients without vascular complications have an increased 20S proteasome expression, the significance of which needs to be explored by further study.
