2.Delayed Brain Myelination in Children with Phenylketonuria Complicated with Epilepsia

Objective To observe the delayed brain myelination of children with phenylketonuria(PKU)combined with epilepsia,and explore effectiveness of the treatment and provide an objective criteria for patient recovering evaluation.Methods There were 42 PKU patients,aged 3 to 72 months were selected.The concentration of phenylalanine tested by high pressure liquid chromatography was greater than 1.2 mmol/L in blood,diagnosed as PKU.According to electroencephalogram and clinical symptom,21 cases were diagnosed as epilepsy,the other 21 cases were used as control group.All patients were taken MRI before treatment.Myelination in 10 sections(cerebellum,pons,mesencephalon,internal capsule posterior limb,corpus callosum,internal capsule anterior limb,occipital lobe,parietal lobe,temporal lobe,frontal lobe)were evaluated.Results Delayed myelinations were located mainly in the cerebral lobes and corpus callosum,average delayed incidence of the 10 region was 44.8% in epilepsy group and 30.9% in control group.The incidence of the corpus callsum was 80.9% in epilepsy group,52.4% in control group,the number of sections of delayed myelination showed statistically significant between 2 groups(P

Objective To discuss the frequent etiology of spontaneous sub-cortical hemorrhage and its diag- nosis.Methods The clinical materials of 79 cases of spontaneous sub-cortical hemorrhage were analyzed.Results 56% of the hemorrhage was caused by arterial-venous malformation.48% of the hemorrhage was caused by occult AVM.Conclusion AVM is the most frequent etiology of spontaneous sub-cortical hemorrhage.CTA plays a special role in its diagnosis.
5.Effect of estrogen receptor alpha gene polymorphism on the variations of T lymphocyte subsets and its related cell factors in female patients with primary cholestasis cirrhosis.

OBJECTIVETo study the effects of estrogen receptor (ER) alpha gene polymorphism on the migration of T lymphocyte subsets and related cytokines in the female patients with primary biliary cirrhosis(PBC).
METHODSThis study was conducted with sixty female PBC patients without treatment as the study group and fifty-two healthy people wtih sex and age met the requirements of the study as the control group. The polymorphism of restriction enzyme cutting site of Xba I and Pvu II in intron 1 of ERa gene was detected by polymerase chain reaction-restriction fragment length polymorphism(PCR-RFLP). CD4+, CD8+, CD4+CD25+ and CD4+CD28- T lymphocytes in peripheral blood were quantitatively detected by flow cytometry. RT-PCR method was used to detect the expression of TNFa, IL-2, IFNgamma, IL-4, IL-6 and IL-10 in peripheral mononuclear cells.
RESULTSThe positive rate of Pp in ERa gene Pvu II enzyme gene subtypes of female PBC patients was significantly greater than that of the control group, and the positive rate of pp gene subtype was significantly smaller than that of the control group (X2 = 7.2880, P = 0.0261). The difference of Xba I genotype and allele frequency between the female PBC patients group and the control group was not of statistical significance (X2 = 6.5382, P = 0.5833). The proportion of CD4+ T in T lymphocytes of PBC patients was increased to 45.31%+/-5.26%, compared with 33.81%+/-3.87% in the control group; and the proportion of CD8+ T lymphocytes was decreased to 27.78%+/-1.43 % from 31.83%+/-1.73% in the control group. In comparison with the control group, the proportion of CD4+CD25+ T lymphocytes decreased significantly, while that of CD4+CD28- T lymphocytes rose significantly. The expression levels of TNFa, IL-2 and IFNgamma mRNA were 0.59+/-0.19, 0.71+/-0.29 and 0.67+/-0.21 respectively, which were significantly higher than that in the control group (0.22+/-0.13, 0.31+/-0.14, 0.27+/-0.13) (t = 6.93, 5.07, 7.01, P value less than 0.01); the expression level of IL-6 mRNA was increased to 0.45+/-0.21 from 0.34+/-0.16 in the control group (t = 1.84, P value less than 0.05); and the difference of the expression levels of IL-4 and IL-10 mRNA between two groups was not of statistical significance.
CONCLUSIONPp of gene Pvu II was a genetically susceptible genotype in female PBC patients, and the allele p was a susceptible gene. Th1 cell subsets and related cytokines were dominant in peripheral blood of PBC patients. As a background of genetic susceptibility, ERa gene polymorphism could affect the shift of T lymphocyte subsets and the expression of the related cytokines in PBC patients.

6.Vertebral bodies implanted with biodegradable reticulated balloon and calcium phosphate cement:Changes in the vertebral biomechanics

BACKGROUND:It has become a focus to look for new vertebral body fil ing materials which have the biomechanical property, biological activity and low cement leakage rate.
OBJECTIVE:To investigate the biomechanical characters and cement leakage rate of the vertebral bodies implanted with biodegradable reticulated bal oon and calcium phosphate.
METHODS:Thirty-two vertebral bodies from pigs were randomly divided to four groups. For A group, 2.5-3.0 mL polymethacrylate cement was injected into the body through a unilateral thoracic pedicle pathway;for B group, 2.5-3.0 mL calcium phosphate cement were injected by the same protocol;for C group, the biodegradable reticulated bal oons fil ed with 2.5-3.0 mL calcium phosphate cement were implanted;D group, including normal vertebral bodies, was designed as controls. Leakage of bone cement was observed in each group. The load-shift curves were recorded by an electronic universal testing machine (SCHENCK RSA-250).
RESULTS AND CONCLUSION:The stiffness and strength of A group were significantly higher than those of D group (P<0.05), and the stiffness and strength of B group were statistical y lower than those of D group (P<0.05). The stiffness and strength of C group, otherwise, were similar with those of D group (P>0.05). Cement leakage rate of C group was lower than that of A or B group (P<0.05). Vertebral bodies implanted with biodegradable reticulated bal oons may lead to similar biomechanical characters as the normal vertebral bodies and reduce the cement leakage rate.
7.Correlation research on contents of podophyllotoxin and total lignans in Sinopodophyllum hexandrum and ecological factors.

To explore the correlation between the ecological factors and the contents of podophyllotoxin and total lignans in root and rhizome of Sinopodophyllum hexandrum, podophyllotoxin in 87 samples (from 5 provinces) was determined by HPLC and total lignans by UV. A correlation and regression analysis was made by software SPSS 16.0 in combination with ecological factors (terrain, soil and climate). The content determination results showed a great difference between podophyllotoxin and total lignans, attaining 1.001%-6.230% and 5.350%-16.34%, respective. The correlation and regression analysis by SPSS showed a positive linear correlation between their contents, strong positive correlation between their contents, latitude and annual average rainfall within the sampling area, weak negative correlation with pH value and organic material in soil, weaker and stronger positive correlations with soil potassium, weak negative correlation with slope and annual average temperature and weaker positive correlation between the podophyllotoxin content and soil potassium.

8.Clinical effect of arthroscopic debridement and infusion-drainage on septic arthritis after arthroscopic anterior cruciate ligament reconstruction.

OBJECTIVETo investigate therapeutic strategy on septic arthritis after arthroscopic anterior cruciate ligament reconstruction.
METHODSThe clinical data of 6 cases of septic arthritis after arthroscopic anterior cruciate ligament reconstruction in our department from March 2005 to February 2014 were analyzed. All the patients were male,ranging in age from 18 to 36 years old. After operation, the knee joint became painful and swollen, and ESR and CRP were both increased. Culture of joint fluid allowed the recovery of staphylococcus epidermidis. The patients were dealt with arthroscopic debridement and infusion-drainage. The clinical results were evaluated by Lysholm rating system and range of motion.
RESULTSThe infection of all the patients was controlled. The ESR and CRP both recovered to normal level. The score of Lysholm rating system ranged from 85 to 95,and the range of motion was 120 to 135 degree.
CONCLUSIONArthroscopic debridement combined with infusion-drainage is effective in septic arthritis after arthroscopic anterior cruciate ligament reconstruction.

10.Drilling decompression for femoral head necrosis at collapse stage.

OBJECTIVETo explore clinical effects of drilling decompression for femoral head necrosis at collapse stage.
METHODSTotally 22 patients with femoral head necrosis on both side were analyzed from March 2006 to February 2009. Among them, there were 20 males and 2 females aged from 24 to 52 years old with an average of 38.4 years old. All patients were Ficat I on one side and Ficat I or II on another side. Drilling decompression were performed on all patients. The average followed-up period was 5.4 years (ranged, 1 to 8 years). The endpoint of severe pain(VAS ≥7), severe dysfunction (Harris ≤ 70), radiographic progression on Ficat IV and total hip arhtroplasty (THA) was used for survivorship analysis.
RESULTSSevere pain as an endpoint,13 cases were survived on Ficat III and 18 cases on Ficat I or II (P>0.05). Severe dysfunction as an endpoint, 6 cases were survived on Ficat III and 18 on Ficat I or II (P<0.05). Radiographic progression on Ficat IV as an endpoint, 2 cases were survived on Ficat III and 18 on Ficat I or II (P<0.05). THA as an endpoint, 13 cases were survived on Ficat I and 18 cases on Ficat I or II (P>0.05).
CONCLUSIONFemoral head necrosis with Ficat III treated with drilling decompression could prolong progression of disease , decrease percentage of THA.
