Objective: To study the attenuating effect of angiotensin I type 1 receptor antagonist losartan and an-giotensin converting enzyme inhibitor captopril on aortic atherosclerosis in rabbits. Methods: Thirty-one male New Zealand rabbits were randomly divided into 5 groups:control group, high cholesterol diet group, losartan group, captopril group and combined drug administration groupdosartan+captopril). The animals were killed after 16 weeks and the serum total cholesterol ,triglyceride, high and low density cholesterol .atherosclertic ratio, endothelin,NO,plaque area percentage, aortic cholesterol content and vascular smooth muscle cell (VSMC) apoptosis were determined. Results:The plaque area percentage, aortic cholesterol contents and endothelin levels of 3 drug treatment groups were significantly lower than that of high cholesterol group,NO contents and VSMC apoptosis were significantly higher. Conclusion:Losartan and captopril can attenuate aortic atherosclerosis induced by high cholesterol diet .combined administration of the 2 drugs at low doses are more effective. The mechanism may be related to the protection of endothelial function and the effect on apoptosis of VSMC.

Objective To integrate urine strip chemistry analysis with urine sedimental analysis and set up the criteria for urine microscopy review following automated urine analysis. Methods A total of 1 714 urine samples were collected from Peking Union Medical College Hospital from November 2008 to October 2010.Out of 1 714 samples, 1 300 samples were used for the establishment of review criteria, and 214 samples were used for criteria verification. The other 200 samples from healthy donors were used to set up the normal reference range of fully automated urine sedimental analyzer UF-1000i.RBC,WBC,PRO and CAST in all the samples were measured by Siemens Bayer Clinitek 500 urine strip chemistry analyzer, Sysmex UF-1000i urine sedimental analyzer and microscopic examination.Based on the different laboratory automation in urine analysis, four microscopic review protocols were defined: (1) Protocol 1: based on chemistry results only, microscopy review was performed when any of WBC, RBC and PRO was positive; (2) Protocol 2: based on fully automated sedimental analyzer only, microscopy review was performed when any of WBC, RBC and CAST was over the upper limit of the reference range; (3) Protocol 3: All the results of urine chemistry analyzer and sedimental analyzer were integrated. If two WBC results were different between two systems (in one system WBC was positive or over the upper limit of the reference range but in another system WBC was negative or within the reference range), and any of RBC, PRO/CAST was positive or over the upper limit, microscopic review was performed; (4) Protocol 4: if any of WBC, RBC, PRO/CAST was different between two systems, microscopic review was performed.Review criteria were performed with Sysmex Laboman UriAccess 3.0 software. Results The reference ranges of UF-1000i parameters were RBC 0-7.5/μl (male), 0-15.9/μl (female); WBC 0-11.6/μl (male), 0-12.7/μl (female); Epithelial cell were 0-6.5/μl (male), 0-21.4/μl (female); CAST 0-1.3/μl.The results of microscopic examination revealed that positive samples were 47.46% (617/1 300) and negative samples were 52.54% (683/1 300). Among positive samples, majority showed the presence of RBC (60.13%,371/617), followed by CAST (8.43%,52/617).The false negative rates of four protocols were 8.38% (109/1 300), 4.69% (61/1 300), 0.62% (8/1 300) and 0.54% (7/1 300), respectively. The review rates were 47.85% (622/1 300), 59.38% (772/1 300), 72.85% (947/1 300) and 52.23% (679/1 300), respectively.Although there were false negative cases in protocol 4, all the patients had normal serum creatine level. In those 214 patients for verification, the false negative rate using protocol 4 was zero, the review rates were 53.74% (115/214).Conclusions Protocol 4 shows lest false negative rate and lower review rate.Importantly, there was no patients with serious renal function abnormality missed using protocol 4.Therefore, protocol 4 is an ideal criteria for microscopy review following automated urine analysis.

Objective To examine the interrelations among vascular calcification, bone denaity, osteoprotegerin (OPG) and soluble receptor activator of NF-kB ligand (sRANKL) in maintenance hemodialysis (MHD) patients. Methods The levels of serum OPG and sRANKL from 39 MHD patients were measured by ELISA. The vascular calcification was detected by plain radiographs. Bone mineral density (BMD) was measured with dual-energy X-ray absorptionmeter.Interrelations among above parameters were examined statistically. Results (1) Among 39 MHD patients, 25 cases were identified as vascular calcification by radiographic film. The proportion of patients with mild vascular calcification was 41.0% (16 cases), moderate and severe vascular calcification was 23.1% (9 cases). Compared to patients with mild vascular calcification, serum OPG level [(342.50±171.53) ng/L vs (206.21±137.88) ng/L,t=-2.253, P=0.0251 and OPG/sRANKL ratio (454.65±455.63 vs 135.31±136.81, t=59, P=0.035)were significantly higher in patients with moderate and severe vascular calcification, while serum sRANKL level [(0.10±0.08) pmol/L vs (0.12±0.08) pmol/L, t=0.534, P>0.05] was not significantly different. Multiple linear regression analysis showed that OPG/sRANKL ratio was independent factor of vascular calcification score. (2)Compared to patients with normal bone volume, the patients with abnormal bone volume had higher serum OPG level [(249.05±137.66) ng/L vs (226.67±170.12) ng/L], lowerserum sRANKL level [(0.11±0.08) pmol/L vs (0.12±0.02) pmol/L], and higher OPG/sRANKL ratio(202.31±219.24 vs 148.08±210.10), but these parameters were not significantly different betweenthese two groups. Multiple linear regression analysis showed that OPG/sRANKL ratio was anindependent factor of T score of lumbar vertebra. (3)Muhiple linear regression analysis revealed that vascular calcification score was an independent factor of T score of lumbar vertebra and hip. Conclusions Vascular calcification score is an independent factor of BMD of lumbar vertebra and hip in MHD patients. Serum OPG/sRANKL ratio may play an important role in the association between vascular calcification and BMD in MHD patients.

Objective To investigate the possible mechanism that hepatocyte growth factor (HGF) inhibits renal tubular epithelial-mesenchymal transition (EMT),and to determine whether Smurf2 expression induced by TGF-β1 can be reversed by HGF in normal rat kidney epithelial cells (NRK-52E).Methods Using rat NRK-52E cell line as an in vitro system,NRK-52E cells were incubated with 5 μg/L TGF-β1 for 0-24 h.Part of cells were pretreated with 20 μg/L HGF for 30 min or not, then incubated with or without 5 μg/L TGF-β1 for 1 h or 48 h.The other cells were transfected with pFlag-Smurf2 or Smurf2 siRNA for 24 h,then treated with or without 20 μg/L HGF for 24 h.The expressions of Smurf2,SnoN,E-cadherin,alpha-smooth muscle actin (α-SMA) and fibronectin (FN) were detected by Western blotting and indirect immunofluorescence staining assays. Results Compared to normal control,TGF-β1 could rapidly induce Smurf2 protein expression in a short time (P<0.01).Meanwhile,the expressions of FN and α-SMA were significantly induced, and the expression of E-cadherin was reduced in NRK-52E cells by TGF-β1.In contrast,in the NRK-52E cells pretreated with HGF,HGF could obviously inhibit Smurf2 expression induced by TGF-β1,and reversed the down-regulation of SnoN (P<0.01) and E-cadherin (P<0.05),the up-regulation of α-SMA (P<0.01) and FN (P<0.01) induced by TGF-β1.Moreover,overexpression of Smurf2 in NRK-52E cells could partly inhibit the up-regulation of SnoN protein by HGF,while down-regulation of Smurf2 could up-regulate the expression of SnoN induced by HGF.Conclusions HGF can abolish EMT induced by TGF-β1 in renal tubular epithelial cells through down-regulating Smurf2 expression and suppressing ubiquitin-proteasome dependent degradation of SnoN.

Objective To investigate the effect of creatine phosphate sodium on postoperative fatigue syndrome (POFS) in gastric carcinoma patients. Methods Nighty cases of radical resection in gastric cancer were randomly divided into control group (n=45) and treatment group (n=45).Normal saline (100 mL) and phosphocreatine sodium (2 g) was admin-istrated intravenously after anaesthesia induction in treatment group while only normal saline was given in control group . After operation, the two groups of patients were all treated with enteral and parenteral nutrition. General performance was ob-served in both groups. Fatigue score (VAS score) was measured on the day before the operation, the first, third and fifth day after operation. Body weight, triceps skinfold thickness (TSF), mid upper arm circumference (AMC), plasma total protein (TP), plasma albumin (ALB) and prealbumin (PA) were examined at the same time. Results The total effective rate of the control group and the study group were 73.3%and 95.6%respectively and the difference was of statistically significant. Fa-tigue scores (VAS score) was significantly difference in both groups pre- and post-operation time points, also between 2 groups. Body mass, TSF and AMC in time points postoperation were all lower than those in the time point of preoperation. TP, ALB and PA of two groups in postoperative time points were significantly different compared with those in preoperative time point. The above indexes were all decreased with postoperation time, and these indexes in treatment group were signifi-cantly better than those in control group. Conclusion Creatine phosphate sodium can alleviate postoperative fatigue, im-prove nutritious status and promote early rehabilitation of patients.
