1.Protective effect of gynostemma on myocardium in diabetes associated with hyperlipemia and its mechanism

Objective To investigate the protective effect of gynostemma on myocardium and its mechanism in patients with diabetes associated with hyperlipemia.Methods 178 patients with type 2 diabetes mellitus with hyperlipidemia from the department of endocrinology in our hospital were selected and divided into two groups, 89 cases in the control group treated with basic treatment of diabetes+atorvastatin, 89 cases in the experiment group received gynostemma on the basis of the control group, myocardial injury index, blood glucose level, blood lipid level, serum cytokine levels and serum oxidative stress response indexes were detected and compared after the treatment.Results Compared with the control group, serum levels of aspartate aminotransferase(AST), creatine kinase(CK), CK-MB, lactate dehydrogenase(LDH)were lower in the experiment group after treatment, fasting plasma glucose(FPG), 2h postprandial blood glucose(2hPG), HbA1c level were lower after treatment, serum levels of total cholesterol(TC), triglycerides( TG ) , low-density lipoprotein cholesterol ( LDL-C ) were lower, high-density lipoprotein cholesterol ( HDL-C ) level was higher after treatment, serum level of adiponectin(ADPN) was higher, tumor necrosis factor-alpha(TNF-α), interleukin-6(IL-6), interleukin-18(IL-18) levels were lower after treatment, serum levels of malondialdehyde(MDA), reactive oxygen species(ROS) were lower, glutathione peroxidase(GSH-Px), superoxide dismutase(SOD) levels were higher after treatment, with significant difference (P<0.05).Conclusion Gynostemma can significantly reduce the cardiac muscle damage of patients with diabetes associated with hyperlipemia , plays a role in myocardial protection, the mechanism may be to reduce blood glucose and blood lipid levels, inhibit the inflammatory reaction caused by inflammatory cytokines, improve the antioxidant capacity of the body and reduce oxidative stress damage.

4.Exploration on new role of ancient perilous but key acupoint of renying (ST 9).

The specific functions, stimulating methods and ranges of clinical application of Renying (ST 9) are explored. Because Renying (ST 9) is located in the neck which is a dangerous area, it is perilous if the acupoint is stimulated, so it is mostly forbidden in the past dynasties. In recent years, the main stimulating methods for Renying (ST 9) include acupuncture and press. The keys of acupuncture are to apply correct technique of needle insertion and control the depth and direction of needle. The keys of press are focused on strength and time of press. Renying (ST 9) is effective for vertigo, palpitation, asthma, hemiplegia, aphasia, hysteria, acute attack of pain, hyperplasia of mammary, continuous hiccup, disorder of throat and so on. As long as the acupoint location is accurate, manipulation is careful and stimulating method is correct, not only the safety could be guaranteed, but also the function can be well-played in clinical application.

5.Antineoplastic effect of laser immunotherapy in mice with multiple cutaneous squamous cell carcinoma

Objective To evaluate the antineoplastic effect of laser immunotherapy in mice with multiple cutaneous squamous cell carcinoma (CSCC).Methods Normal immunocompetent hairless SKH-1 mice were consecutively irradiated by ultraviolet rays to establish mouse models of multiple CSCC.Then,20 CSCC-bearing mice were randomly and equally divided into 4 groups:laser group irradiated with an 808-nm near-infrared laser (1 W/cm2,10 minutes) once every two weeks for a total of 3 sessions,laser immunotherapy group irradiated with an 808-nm nearinfrared laser as above and topically treated with imiquimod cream once a day for 3 days before and after each session of irradiation,imiquimod group treated with imiquimod cream as in the laser immunotherapy group,and control group receiving no treatment.The volume of tumors was calculated,and their morphology as well as the survival of mice were observed after the treatment.The t test and Mantel-Cox log-rank test were used to compare the volume of tumors on the dorsum and survival rates of mice,respectively,on days 27 and 60 after the start of treatment.Another 12 CSCC-bearing mice were randomly and equally divided into the 4 groups mentioned above,and tumor tissues were resected from these mice for histopathological examination on day 5 after the first treatment.Results On day 27,the volume of tumors significantly increased in the control group and imiquimod group (both P < 0.05),but experienced no significant increase in either of the other two groups compared with that before the start of treatment (both P > 0.05).Moreover,the volume of tumors in the control group was slightly larger than that in the imiquimod group (P > 0.05),but significantly larger than that in the laser group and laser immunotherapy group (both P < 0.05).The volume of tumors in the laser group increased,and was significantly larger than that in the laser immunotherapy group on day 60 (P < 0.05).Mice all died within 40 days in the control group,and within 50 days in the imiquimod group;one mouse died on days 52 and 53 separately,and the other mice all survived longer than 60 days in the laser group;no mice died within 60 days in the laser immunotherapy group.By day 60,the survival time of mice was slightly longer in the imiquimod group (P > 0.05),significantly longer in the laser group and laser immunotherapy group compared with the control group (both P < 0.05),but similar between the laser group and laser immunotherapy group (P > 0.05).Histopathological examination showed that 5-day topical application of imiquimod cream did not cause death of tumor cells,but numerous tumor cells died in the laser group and laser immunotherapy group.Multiple inflammatory cells were observed around the tumors,and the number of the inflammatory cells was obviously larger in both the laser group and laser immunotherapy group,especially in the latter group,compared with the control group.Conclusion Laser immunotherapy is a new effective and safe treatment for CSCC in mice,and has potential value especially in the treatment of multiple or metastatic CSCC.
6.The relationship between phosphorylation at Ser3 of cofilin1 and Taxol resistance of ovarian cancer in elderly female patients

Objectives To analyze the relationship between phosphorylation of serine-3 of cofilin1 and Taxol resistance of ovarian cancer and to evaluate the prognostic value of cofilin1 phosphorylation in estimating ovarian cancer survival using clinical samples.Methods Wild type (WT) plasmids with over-expression of wild cofilin1,S3L plasmids with over-expression of cofilin1 and inhibited phosphorylation at serine-3,and siRNA-C plasmids with down-regulation of cofilin1 were constructed using molecular biology techniques.After the SKOV3 and SK-TR30 cell-lines were transfected with these plasmids,changes in biological characteristics and drug resistance after instant transfection were compared.Fifty-one elderly female patients with microscopically confirmed ovarian cancer,who had received chemotherapy with Taxol and cis-platinum (TC)after surgery,including 30 chemo-sensitive and 21 chemo-resistant cases,were recruited in this study.Immunohistochemical methods were used to detect the expression of cofilinl and phosphorylated cofilin in tissue sections from the two groups.Progression free survival(PFS)was analyzed.Results Compared with the control group,the proportion of cells increased at the G0/G1 phase(P=0.034)and decreased at the S phase (P=0.031),and the apoptosis rate decreased(P=0.020),in SKOV3 cells with high expression of cofilin1.However,these characteristics disappeared when the wild type was replaced with the inactive mutant S3L.When cofilin1 expression was down-regulated in SK-TR30 cells,the proportion of cells at the G0/G1 phase decreased(P=0.020).The expression of cofilin1 was detected in 56.7 % (17/30)and 57.1% (12/21),respectively,sections of ovarian tumor tissues of the chemo-sensitive and chemoresistant groups,and there was no statistic difference (x2=0.332,P =0.943)between the two groups.The expression of phosphorylated cofilin was much higher in the chemo-resistant group (85.7% or 18/21)than in the chemo-sensitive group (53.3% or 16/30),and the difference was statistically significant(x2=5.829,P=0.016).The higher expression of phosphorylated cofilin was also correlated with shorter PFS(x2 =21.440,P<0.01,95% CI:0.068-0.883),especially in the chemo-sensitive group.Conclusions Serine-3 phosphorylation status of cofilin 1 is associated with paclitaxel resistance in ovarian cancer,but the underlying mechanisms of regulation need further investigation.
7.Renal complications of hepatic glycogen storage disease

Objective To study renal involvement in hepatic glycogen storage disease(GSD) in childhood. Methods One hundred and eight patients aged less than 21 years old with type Ⅰa GSD (54 cases), type Ⅲ (29 cases) and uncertain type hepatic GSD (25 cases). Urine analysis, urine albumin, urine protein of 24 h, urine ?_2-MG, BUN, creatinine, Ccr were evaluated. Results Of 108 patients with hepatic GSD, 16 patients (20.8%) had proteinuria proven by urine albumin or urine protein of 24 h, their ages first found proteinuria were 8~15 years. Two 15-year-old patients had proteinuria over 1.0g/24h. Among 72 patients, urine ?_2-MG of 51 cases (70.8%) increased (175~10 623mg/L), and the mean urine ?_2-MG of type Ⅰ a GSD was much higher than that of type Ⅲ GSD, 4138.2 and 1790.1mg/L respectively. Of 91 patients, 10 had renal insufficiency, 1/10 (15-year-old girl) had heavy proteinuria (3.5g/24h), elevated BUN (9.3mmol/L) and Scr(1061 ?mol/L). Five elder patients (11~21 years old) had hematuria with renal colic caused by renal calculus. Conclusions Persistent protenuria, increased urine ?_2-MG, decreased Ccr, and renal stones are common complications of hepatic GSD in childhood. Renal function should be thoroughly evaluated during follow-up.
8.Expression of HBME-1,CK19 and CD117 in papillary thyroid carcinoma and its significance

Objective To explore the value of three antibodies in the differential diagnosis of papillary thyroid carcinoma ,by de‐tecting the expression of HBME‐1 ,CK19 and CD117 in papillary thyroid cacinoma ,thyroid follicular adenoma and Hashimoto′s thy‐roiditis tissues .Methods Totally 85 cases were collected from January 2013 to December 2015 ,including papillary thyroid cacino‐ma ,thyroid follicular adenoma and Hashimoto′s thyroiditis .They were immunohistochemical stained by HBME‐1 ,CK19 and CD117 .SPSS16 .0 software was used to analyze the relationship between the staining results with different pathological changes . Results The positive rates of HBME‐1 ,CK19 and CD117 were 87 .3% ,98 .2% ,and 7 .3% ,respectively .The positive expression of them in benign and malignant groups had significant difference (P< 0 .05) and their consistency checking Kappa were 0 .582 , 0 .551 ,and 0 .874 ,respectively .Conclusion In the differential diagnosis of papillary thyroid carcinoma and benign lesions ,CD117 is better than HBM E‐1 and CK19 .It′s possible to use a combination of them in practice .
9.Surgical Treatment of Chronic Pancreatitis with Mass in Head

Objective To summarize and discuss the diagnostic and treating experiences of chronic pancreatitis with mass in the head. Methods Eight patients of chronic pancreatitis with mass in the head who were misdiagnosised as carcinoma of head of pancreas were analyzed retrospectively in the past 10 years. Results All the patients exhibited abdominal pain,5 of whom were with jaundice and 3 with anorexia. All the patients were misdiagnosised as carcinoma of head of pancreas before the operation,but the pathology after operation indicated chronic pancreatitis. The pancreaticoduodenectomy was performed in 5 patients,the choledochojejunostomy in 2 patients,while the exploratory laparotomy in 1 patient. After the operations,the abdominal pain was relieved in 7 patients, while 2 patients who accepted pancreatoduodenectomy suffered from pancreatic fistula,1 of whom died in the end. Conclusion It’s hard to differentiate the chronic pancreatitis with mass in the head from the carcinoma of head of pancreas before operation. If the carcinoma of head of pancreas can’t be excluded during the operation,the pancreatoduodenectomy should be performed,while the duodenum-preserving total resection of the head of the pancreas or any intra-drainage operations should be done if chronic inflammation is found in the whole pancreas with a negative result of the biopsy of the pancreas through the needle aspiration.
10.Analysis of characteristics of ultrasound and fine needle aspiration cytology in patients with papillary thyroid carcinoma

Objective To analysis the diagnostic values and characteristics of ultrasound and ultrasound guided fine needle aspiration cytology (FNAC) in patients with papillary thyroid carcinoma (PTC) before operation. Methods The data of ultrasound and ultrasound guided FNAC in 129 patients (including 148 PTC nodules) with PTC were collected from January 2014 to February 2017, and the diagnostic reports and feature descriptions of ultrasound and ultrasound guided FNAC were analyzed retrospectively. Results Among the 148 PTC nodules, the ultrasonographic imaging showed that 84.5% (125/148) with low echo-level solid nodules, 61.5% (91/148) with echo heterogenicity, 77.7% (115/148) with a ratio ≥ 1 in longitudinal/breadth, 69.6 % (103/148) with fuzzy boundary, 75.0% (111/148) with microcalcification in nodules, 97.3% (144/148) without or with incomplete aureole, 64.9% (96/148) with rich blood flow and 7.0% (9/129) with enlargement of cervical lymph nodes. The FNAC diagnosis showed that 78.4% (116/148) was diagnosed with suspected papillary carcinoma, 1.4% (2/148) was diagnosed with malignent tumor, 11.5% (17/148) was diagnosed with atypia of undetermined significance (AUS), 1.4% (2/148) was diagnosed with benign lesion, 0.7% (1/148) was diagnosed with follicular neoplasm and 6.7% (10/148) could not be diagnosed. If the suspected papillary carcinoma and malignent tumor were defined as cytodiagnosis, the diagnostic accordance rate with intraoperative pathology was 79.7%. Conclusion The preoperative accuracy rates of ultrasound diagnosis and ultrasound guided FNAC diagnosis for patients with PTC are high, and the characteristics of the both are also typical. The two examinations before operation are helpful for early diagnosis and treatment formulation for patients with PTC.
