OBJECTIVE To investigate the changes in and clinical significance of C-reactive protein(CRP) and endotoxin in patients who are firstly examined in fever clinic.METHODS To review and study the results of CRP and endotoxin from 81 patients(including 48 patients who were diagnosed as bacterial pneumonia and 33 patients who were diagnosed as a cold) in fever clinic of our hospital in 2005.RESULTS In 81 bacterial infection patients, the analysis indicated the levels of endotoxin between the pneumonia patients and the patients with a cold had significant difference.And the levels in the first group were significantly higher than in the latter one.The levels of CRP had no difference in two groups.CONCLUSIONS The result indicates plasma endotoxin detection helps to determine the early diagnosis of serious infection.It is also of significance to select antibacterial rationally and handle syndrome in time.In fever clinic,the levels of endotoxin may be a method to help diagnosis and treatment.CRP could be help to estimate the bacterial infection or virus infection,but it could not adjudge the seriousness in bacterial infection.

Objective To explore the relationship betw een injury age and expressions of erythropoietin (EPO ) and its receptor EPO R in the brain tissue of rats after cerebral injury. Methods Seventy-tw o rats w ere random ly divided into control group (36 rats) and cerebral injury group (36 rats). The rats w ere sac-rificed at 1, 2, 4, 8, 12, 24 h after cerebral injury (6 rats at each tim e point) and the brain tissues w ere extracted. The expressions of m RNA and protein of EPO and EPO R at different tim e points w ere de-tected by real-tim e fluorescent quantitative PC R and W estern bloting. Results The expressions of EPO and EPO R increased w ithin 24 h after injury. The expressions of m RNA and protein of EPO w ere relat-ed to the injury age, and the correlations w ere 0.875, 0.911, respectively (P<0.05). OBJECTIVE To study the best timing of tonsillectomy for peritonsillar abscess (PTA).METHODS The clinical data of 326 cases with PTA were reviewed.Among them,167 cases (group 1) underwent tonsillectomy in the period of abscesses,the other 159 cases (group 2) underwent elective tonsillectomy.RESULTS In group 1,the mean unilateral bleeding was (10?2.3)ml during operation,postoperative bleeding rate was 3.0%, mean operation time was (10?1.2)min and mean hospitalization period was (8?1.1)d.While in group 2,the mean unilateral bleeding was (21?2.1)ml during operation,postoperative bleeding rate was 6.9%,mean operation time was (20?1.5)min and mean hospitalization period was (16?1.3)d.There were significant differences between the two groups in the bleeding amount,operation time,postoperative bleeding rate and hospitalization period (P0.05). CONCLUSION For patients with good general conditions and no contradiction for operation, tonsillectomy implemented 3-4 days after drainage of abscess shows obvious advantages than elective operation.

Objective To observe the effects of repetitive transcranial magnetic stimulation (rTMS) on the expression of glial fibrillary acidic protein (GFAP) and OX-42 in hippocampus and dentate gyrus in rats.Methods The rats were treated with 1 Hz, 100 mT TMS 10 min once a day for 14 days, and then the expression of GFAP and OX-42 in hippocampus and dentate gyrus were investigated by ABC technique of immunohistochemistry.Results Compared with the control group, there were no significant difference in the expression of GFAP and OX-42 between the two groups.Conclusion rTMS using our parameters does not cause brain injury in rats.

OBJECTIVE: To investigate the efficacy and safety of nasal septal suture in septoplasty. METHOD: Sixty patients who had undergone septoplasty were allocated into two groups: packing group and suturing group. Post-operative signs and symptoms (visual analogue scale) were compared between the two groups. RESULT: The patients of packing group experienced significantly more postoperative pain, epiphora, tinnitus, ear fullness, sleep disturbance and nasal congestion(P<0. 05). No statistically significant difference between two groups in hemorrhage (P>0. 05). CONCLUSION Nasal septum continuous suture is a feasible method to replace nasal packing after septoplasty.

Objective To investigate the training effect and application development of the health industry standard of Nursing Grading, and to explore it'sclinical applicability. Methods An electronic questionnaire survey was conducted among nurses in 30 three general hospitals of Shandong Province from November 10 to December 10 in 2015. The questionnaire mainly included four parts: basic information, training status, application status and the clinical applicability. All the questionnaires were automatically submitted by the nurse′s cell phone or computer, 3386 valid questionnaires were recovered. Results The status of standard training:93.92%(3180/3386) nurses received a comprehensive training about the standard. The answer rates of 4 theoretical examination questions was 88.95%(3012/3386), 84.49%(2861/3386), 85.17%(2884/3386), 65.71%(2225/3386)respectively. 77.26%of the nurses needed to continue training. Application status: 97.67% (3307/3386) nurses in the hospital has begun to apply Nursing Grading standard, and 97.01% (3208/3307) and 97.76% (3231/3307) of the hospital revised the standard of care systems and work processes; 78.23% (2649/3386) nurses divide the level of care based on both the patient's condition and self-care ability presently. 74.45%(2521/3386) nurses did it as follows,the doctor determine the patient′s condition level, the nurse determine the level of patient′s self-care ability, then the nursing grading was determined according to the both. In the clinical applicability survey, the choice proportion of very agree and agreeofHelping nurses to determine the patient′s level of care and It is beneficial to improve the efficiency of nursing management was 86.30% (2922/3386) and 82.66%(2799/3386);The choice proportion of very agree and agree ofHelp nurses to improve the quality of nursing services, reflecting the value of the nursing work, to meet the needs of patients was 79.36% (2687/3386), 76.56% (2592/3386), 80.98% (2742/3386) respectively. Conclusions The hospital attaches great importance to the standard of Nursing Grading, but the training effect needs to be improved;standard has been widely used in Shandong Province Physician-nurse collaboration has become the decision-making main body of nursing grading;the standard application can not only help the nurses to do decision-making correctly and improve the efficiency. It also can help nurses to improve the quality of nursing service, and enhance patients′satisfaction. So it is very suitable for clinical practice.

Objective: To investigate the effect of irrational use of traditional Chinese patent medicines (CPM) on medication safety.Methods: The adverse drug reaction (ADR) reports on traditional CPM (including traditional Chinese medicine for injection) in our hospital during January 2013 to October 2016 were collected and analyzed retrospectively.After finding the corresponding prescriptions, the situation of irrational drug use in the reports including indications, usage and dosage, combination of traditional CPM, combination of Chinese and western medicines and course were analyzed, and the rate of irrational drug use was calculated.Results: A total of 217 reports were collected including 217 patients.The primary diseases ranked top three were cardio-cerebral vascular system diseases, digestive system diseases and respiratory system diseases.The cases involved 81 kinds of drugs, and among them, 37 cases were induced by Shenmai injection.The rate of irrational drug use associated with indications, usage and dosage, combination of traditional Chinese patent medicines, combination of Chinese and western medicines and course was 6.45% , 5.99% , 2.76% , 0.92% and 1.84% , respectively, and the total rate was 17.97% (39/217).Conclusion: The irrational drug use maybe one of the important factors related to ADR of CPM.According to syndrome differentiation, CPM should be used rationally, and pharmaceutical care should be strengthened to reduce the incidence of ADR.

Objective To compare the body fluid count results detected by Sysmex XE-5000 automatic blood cell analyzer and manual method .Methods A total of 300 cases of body fluid specimens (including cerebrospinal fluid and fluid of serous cavity ) were analyzed .RBC ,WBC counting and classification were respectively detected by XE-5000 and manual method of improvement Neubauer counting plate .Results The fresh specimens without contain a large number of cell clusters ,which RBC counts(RBC-BF)(100-10 000)× 106/L ,and WBC counts(WBC-BF) (9-50)× 106/L ,showed there were a linear relationship between the in-strument method(r=0 .998 5 ,0 .986 3) .In the range ,there was no significant difference between XE-5000 and manual method(t=9 .96 ,P>0 .05) .Also in this range the results of instrument correlated with those of manual method(r= 0 .989 3 ,0 .971 7 , 0 .924 9) .For those specimens which contain a large number of cell clusters ,RBC-BF and WBC-BF were a badly linear relationship between the instrument method(r=0 .564 8 ,0 .456 1) .Conclusion Body fluid specimens which are fresh and do not contain a large number of cell clusters ,in the range of RBC-BF (100 -10 000) × 106/L ,WBC-BF (9 -50) × 106/L ,Sysmex XE-5000 automatic blood cell analyzer could ensure the results have good accuracy .

38 patients with dentition defect in our hospital for magnetic attachment dental treatment were analyzed. At the same time,34 cases of patients with dentition defects were ,being treated with conventional prosthodontics, selected to compare the stability of overdentures and masticatory function with magnetic attachment. The stability of overdentures and masticatory function after restored with magnetic attachment was significantly higher than before(P< 0. 01),while there was no difference after 4 weeks. Cementation language,comfort and appearance satisfaction after 4 weeks were significantly higher than before(P < 0. 05). The stability of overdentures,masticatory function, cementation language,comfort and appearance satisfaction after restored with magnetic attachment was significantly higher than conventional prosthodontics(P < 0. 05). It finally indicated that magnetic attachment had the advanta-ges of comfort,convenience and good effect. The stability of overdentures,masticatory function and application sat-isfaction was superior to conventional prosthodontics,and it had an application prospect.
