Objective To investigate the functional status of adrenal cortex status in newborn with septic shock, and explore the diagnostic criteria of relative adrenal insufficiency (RAI) and catecholamine resistance in shock. Methods Fifty newborns with septic shock, 28 males and 22 females, admitted into neonatal intensive care unit (NICU) of Hangzhou Second People's Hospital from Jan. 2005 to Jun. 2009, were involved in the present study. Serum cortisol levels of the newborn patients were measured with radio immunoassay at baseline and 30 and 60 minutes after-administration of a low-dose (1?g) of adrenocorticotrophic hormone (ACTH). The patients were divided into two groups by maximal cortisol increment levels: normal adrenal function (NAF) group and RAI group. Some clinical factors, for example, the fluid demands on 1st day, shock duration, mortality, neonatal critical illness score (NCIS) and the incidence of catecholamine resistance shock, were observed. According to prognosis these newborn patients were then divided into two groups: nonsurvivors and survivors. The serum cortisol levels were compared after ACTH test between the two groups. Objective To discuss the clinical features and treatment of hepatogenous diabetes. Method Retrospective analysis was conducted in January 2002-April 2009 170 cases of clinical data in patients with liver cir-rhosis. Result 170 cases of liver cirrhosis 40 cases of diabetes, the incidence was 23.5% ,various types of cirrhosis may occur;typical "over three" obvious symptoms; 10 cases admitted tohospital when fasting blood glucose≥7.0 mmol/L, postprandial 2 hours blood glucose were ≥1.1 mmol/L;high level of total bilirubin,serum albumin,cholin-esterase,total cholesterol low. Given to diabetic diet and small doses of ct-glucosidase, short-acting insulin treatment; 26 cases of normal blood glucose, improvement in 8 cases (blood glucose 6. 1～11.1 mmol/L) ;6 deaths. Conclusion Prone to occur in patients with liver cirrhosis glucose metabolism disorders, its clinical features, treatment of liver damage and the need to take into account two aspects of diabetes, the use of insulin as the main.

AIM: To evaluate the clinical effects of modified hydroxyapatite implant in scleral shell.
METHODS: Thirty - four cases were performed eye evisceration, autogenous sclera shell anterior and posterior petaloid shape with posterior sclera fenestration, hydroxyapatite artificial eyeballs were implanted at stage I, and conjunctival wound, orbital activity and other complications were observed after surgery.
RESULTS:The follow-up was 6 ~12mo. There was no patients with implant exposure, sclera dissolution, conjunctival wound dehiscence, conjunctival sac constriction noted. All the patients got good activity of artificial globe, and the active range of side motion of the HA was 10o ~15o. The artificial eyes looked symmetrical, and the eyes socket were full.
CONCLUSION: The modified hydroxyapatite implant in scleral shell can maintain the normal anatomy of the orbital tissue, and also can get full eyes socket and good activity, It was an easy and simple surgery which could obtain satisfactory clinical effect and less complications.

Objective To explore more effective treatments for irritable bowel syndrome(IBS)in old people.Methods 80 patients with irritable bowel syndrome were selected.adverse drug reaction during treatment,and therapy effect of a symptom score after treatment observation group and control group were compared.Results Total improvement rate of symptoms from observation group was 91.7％,and total improvement rate of symptoms in control group was 65.6％.Difference of proportions of significant improvement and improvement was significant,according to the situation that was controlled between observation group and control group.The difference of symptom score after treatment was significant,according to the situation that was controlled between observation group and control group.Conclusion Trimebutine maleate,oryzanol and anisodamine combined application could remarkably improve irritable bowel syndrome treatment effect,and was more safe.

ObjectiveTo explore curative effect of eradication of Helicobacter pylori by sequential therapy and traditional triple therapy.MethodsTo retrospective analyze clinical data of Helicobacter pylori masccline patients,which were divided into the sequential therapy group and the traditional triple therapy group.ResultsThe results of ITT analyze and PP analyze of the sequential therapy group were higher than those of the traditional triple therapy group (x2 =19.30,11.18,all P ＜ 0.05) ;The total effective rate of clinical curing of the sequential therapy group was higher than that of the traditional triple therapy group( x2 =14.25,P ＜0.05 ) ;The untoward reaction rate of the sequential therapy group was lower than that of the traditional triple therapy group( x2 =10.53,6.93,6.93,all P ＜0.05 ).ConclusionThe curative effect of eradication of Helicobacter pylori by the sequential therapy were obviously and little untoward reaction rate,which was to be worthy used.

Objective To summarize the experience of surgical treatment of venous drug-induced infective endocarditis(IE). Methods The 17 cases of drug-induced IE were found to have valvular vegetations,16 patients had involvement of tricuspid valve, 1 mitral valve associated with ventrieular septal defect. Preoperative blood culture was positive in 8 patients. The operations included tricuspid valve repair (6 cases),tricuspid valve replacement(8 cases), mitral valve replacement and VSD closure(1 cases). Results All patients were alive and discharges. After follow-up of 6 months to 6 years, 1 case had severd tricuspid alve regurgitation, 1 had haemoptysis because of incorrect anticoagulation. Conclusions This study indicated surgical intervention is effective treatment of venous drug-induced infective endocarditis.

Objective With the development of minimally invasion and the popularization of arthroscopy the technology of ar-throscopy has been widely applied for the treatment of acute and the safety of this has been validated.To analyze the clinical effect of ar-throscopically assisted double Endobutton plate reconstruction of coracoclavicular ligament in the treatment of acute acromioclavicular joint dislocation. Methods From November 2013 to December 2014, a total of 23 patients with Rockwood type III,Ⅳ,Ⅴshoulder joint dis-location were included, all cases was treated by arthroscopic double Endobutton fixaton technique.Patients were followed up for 6 to 18 months.The changes of shoulder joint forward elevation and external rotation, Japanese Orthopaedic Association score(JOA), and Visual Analogue score( VAS) of acromioclavicular joint in both perioperative and postoperative were recorded. Results The mean follow-up time was (9.3 ±2.4) months ( range from 6 to 18 months),and mean time from injury to surgery was(10.1 ±3.2)days.The therapeutic effects were achieved in all patients after the arthroscopically assisted double Endobutton plate reconstruction.All the measurements, in-cluding the mean forward elevation JOA scores before and after fixation (71.0 ±5.4 vs 93.5 ±3.6), and mean VAS scores (4.6 ±1.0 vs 1.9 ±0.9), mean forward elevation [(102.5 ±8.6) degrees vs (144.3 ±6.3) degrees], and mean external rotation[(107.1 ±6.2) de-grees vs (149.6 ±7.5) degrees] improved significantly (P<0.05) after the treatment. Conclusion Arthroscopic double Endobutton fixaton technique could provide an effective immobilization of shoulder and acromioclavicular joint, thus improving the function of the joint and achieving an excellent effect in the treatment of acute acromiocla-vicular joint dislocation.
