1.Clinical research of 1 . 8 mm microincision phacoemulsification and intraocular lens implantation combined with trabeculectomy

?AlM:To evaluate the effect and safety of 1. 8mm coaxial microincision phacoemulsification - trabeculetomy with ultra-thin intraocular lens ( lOL ) implants for treating glaucoma complicated with cataract, and to compare with the traditional 3. 0mm small phacoemulsification-trabeculetomy with foldable lOL implantion.
?METHODS: ln this prospective study, 36 patients ( 36 eyes) with glaucoma and cataract in lnner Mongolia Autonomous Regian People’s Hospital were collected and randomly divided into 2 groups. For the small incision group: 18 cases ( 18 eyes ) underwent 3. 0mm coaxial incision phacoemulsification-trabeculetomy with foldable lOL implantion; ln th microincision group: 18 cases ( 18 eyes ) underwent 1. 8mm microincision phacoemulsification-trabeculetomy with ultra-thin lOL implantion. The two groups were recorded for 1wk, 1 and 3mo of visual acuity, corneal endothelial cell density, surgically induced astigmatism, intraocular pressure ( lOP ) , filtering bleb and complications. Pearson’s Chi-square test ande t-test were used to determine differences between the two groups.
?RESULTS:At 1wk postoperatively, visual acuity in the microincision group was better than that of small incision group, the difference was statistically significant ( P<0. 05). At 1 and 3mo, the difference in corrected visual acuity between the 2 groups had no significant difference (P>0. 05). At 1wk, 1 and 3mo, there was a significant different between the 2 groups in surgically induced astigmetism (P<0. 05). At 1wk postoperatively, there was a significant difference in corneal endothelial cells density between 2 groups (P<0. 05). But there were no significant difference at 1 and 3mo ( P > 0. 05 ). lOP reduced after surgy ( microincision group: 15. 26 ±3. 12mmHg, small incision group: 14. 57±2. 86mmHg), there was no significant difference between the 2 groups (P>0.05). There was no significant different between the 2 groups in blebs (P>0. 05). Neither iris injury, posterior capsule rupture nor anterior chamber bleeding complications was found in any groups.
?CONCLUSlON: TBy compared with traditional 3. 0mm coaxial small incision phacoemulsification -trabeculectomy with foldable lOL implantion, 1. 8mm microincision phacoemulsification-trabeculectomy withe ultra- thin lOL implantion can effectively reduce the astigmatism operation. This operation is safe, effective, convenient surgy for treating cataract and glaucoma.
2.Study of Quality Control for Tianmatoufengling Capsule

Objective To establish a quality control for Tianmatoufengling Capsule. Methods HPLC method was taken for the determination of Gastrodine in Tianmatoufengling Capsule,A Kromasil C18 column was used,with methanol 0.05% Calcium phosphace(2.5∶97.5) as the mobile phase,and its flow rate was 1.0 mL/min. The detection wavelength was 220 nm. TLC method was used for the identification of Tianmatoufengling Capsule. Results Under the used chromatographic condition,Tianmatoufengling had fine linear responses(r =0.999 9) in the concentration range of 6.016~96.256 ?g/mL,the average recovery was 100.02%,RSD was 2.24%. The method used in TLC-qualitative analysis was simple and sensitive with high specificity,four ingredients identified in Tianmatoufengling Capsule could be detected and all showed specific spots. Conclusion The method employed in this study was convenient and reliable with good repetition. It can be used for quality control in production of Tianmatoufengling Capsule.
3.Phloroglucinol for Injection:Preparation,Quality Control and Stability Study

OBJECTIVE:To prepare phloroglucinol for injection,establish its quality control and determine its stability.METHODS:The type and dosage of the adjuvant in the formula were optimized and the phloroglucinol for injection was prepared,with its content determined by HPLC.Meanwhile its stability was tested.RESULTS:Mannitol 100.0g was determined as the adjuvant in the formula.The linear range of phloroglucinol was 0.13~ 1.04mg.mL-1(r=1.000 0).No obvious changes were observed at 6 month of an accelerated test or 12 month of long-term storage test.CONCLUSIONS:The preparation is reasonable and feasible in techniques,controllable and stable in quality;and the established method is recurrent,specific and the determination results are within defined margin.
4.Clinical study of a novel platelet dumps count in differentiating true thrombocytopenia from pseudothrombocytopenia

Objective To study the clinical significance of a novel marker of platelet clumps count provided by hematology analyzer in differentiating true thrombocytopenia from EDTA-dependent pseudothrombocytopenia (EDTA-PTCP). Methods Samples from 65 cases of thrombocytopenia (including 15 EDTA-PCTP samples and 50 random samples of true thrombocytopenia) and 50 healthy controls were analyzed using hematology analyzers, and samples with low platelet counts were checked by replacing citric acid and using manual microscope observation to identify true thrombocytopenia from EDTA-PTCP. A novel marker of platelet clumps count was used to differentiate the two diseases for samples anficoagulated with EDTA or citric acid. Results In 65 patients with thrombocytopenia, platelet counts were (48±11)×109/L detected by automatic hematology analyzers. Fifty of 65 cases were true thrombocytopenia which showed low platelet counts [(48±10)×109/L by automated analyzer and (46±11)×109/L by manual assay]. No significance was observed between them (t=-1.26, P0.05). Platelet clumps counts were 86±15. No platelet clamps were detected under microscope. The other 15 cases were EDTA-PTCP [platelet counts were (48±12)×109/L and platelet clumps counts (840±184) were increased significantly by automated analyzer and using EDTA anticoagulant] which showed obviously platelet clumps and no less platelet counts under microscope. After replacing citric acid, platelet counts [(141±13)×109/L by automated analyzer and (134±17)×109/L by manual microscope assay] were increased significantly. No significance was observed between them (t=-1.29, P0.05). Platelet clumps counts (75±12) were decreased obviously compared with EDTA anticoagulant method (t=-6.82, P<0.001). No platelet clumps were detected under microscope. Conclusion Platelet clumps counts may be a useful clinical indicator for monitoring of platelet aggregates, especially for EDTA-PTCP caused by platelet clumping.
5.Influence of gender on enhancement of neuromuscular blocking effects of cisatracurium or rocuronium by sevoflurane

Objective To investigate the influence of gender on the enmcement of neuromuscular blocking effects of cisatracurium or rocuronium by sevoflurane. Methods Two hundred and forty ASA Ⅰ or Ⅱ patients, aged 20-60 yr, weighing 45-85 kg, scheduled for elective surgery under general anesthesia were assigned into 2 groups ( n = 120 each): cisatracurium group and rocuronium group. Each group were divided into 4 subgroups according to gender and anesthetics ( n = 30 each): female propofol group, male propofol group, female sevoflurane group and male sevoflurane group. Anesthesia was induced with midazolam, propofol and fentanyl. After loss of consciousness, the laryngeal mask was inserted and the patients were mechanically ventilated. Propofol groups received target-controlled infusion (TCI) of propofol (target plasma concentration 2-6 μg/ml), and sevoflurane groups inhalation of sevoflurane. Cisatracurium 0.15 mg/kg or rocuronium 0.6 mg/kg was injected intravenously 5 min after propofol infusion or after the end-tidal concentrion of sevoflurane was maintained at 1.71% (1 MAC) for 5 min. Neuromuscular block was assessed with accelerograph F (TOF-watch SX). Train-of-four (TOF) stimulation of ulnar nerve was used. The onset time, duration of peak effect and times for recovery of T1 to 25% and TOF ratio (TOFR) to 25% were recorded.Results Compared with propofol groups, the time for recovery of TOFR to 25 % for rocuronium, and the duration of peak effect and times for recovery of T1 to 25 % and TOFR to 25% for cisatracurium were significantly prolonged in female sevoflurane group, the onset time of rocuronium was significantly shortened, while the duration of peak effect and times for recovery of T1 to 25% and TOFR to 25% for rocuronium and cisatracurium were significantly prolonged in male sevoflurane group ( P < 0.05 or 0.01 ). The times for recovery of T1 to 25% and TOFR to 25% for rocuronium and onset time of cisatracurium were significantly shorter in female than in male during sevoflurane anesthesia (P < 0. 05 or 0.01 ). Conclusion The enhancement of rocuronium-induced neuromuscular block by sevoflurane is stronger in male than in female,but there is no gender variation in the enhancement of cisatracurium-induced neuromuscular block by sevoflurane.
6.Des-γ-carboxy prothrombin in hepatocellular carcinoma

Des-γ-carboxy prothrombin (DCP)is produced abnormally in hepatocellular carcinoma cells. Vitamin K utilizing deficiency and the decreasing activity of vitamin K-dependent γ-glutamyl carboxylate and coenzyme may involve in production of DCP. The enzymes in bepatocellular carcinoma cells are failed in carboxylating all the ten glutamic acid residues in amino- terminus of prothrombin precursor to Gla and make the difference between DCP and prothrombin. DCP is considered to be the marker for diagnosis of hepatocellular carcinoma. DCP might also be one of the growth-promoting factors to carcinoma cells. Therefore, inhibition of DCP in hepatocellular carcinoma cells is considered to be the therapeutic method of blocking hepatocellular car-cinoma growth. Vitamin K and its analogs are found to have the functions of inhibiting hepatocellular carcinoma cell producing DCP. The mechanism may relate to the increase of γ-glutamyl carboxylase activity.
7.Quality Control Measures in Dryness Process of Traditional Chinese Medicine Based on the Angle of Government Functions

This article began with the duality of traditional Chinese medicine, expounded the government functions in the dryness process and quality control of traditional Chinese medicine from macroscopic and micro levels, and proposed the measures of improving dryness process based on government functions.
8.Advances of TGF-β-induced long non-coding RNAs in tumors

The TGF-βsignaling pathway plays a crucial role in regulating cell proliferation, differentiation, and apoptosis. This pathway exerts either tumor-suppressing or tumor-promoting effects, which are cell-or context-dependent, making it simultaneously advanta-geous and disadvantageous in the process of carcinogenesis and tumor progression. Long non-coding RNAs (lncRNAs) do not encode proteins, but they are involved in the regulation of various signaling pathways and biological functions. Moreover, lncRNAs can induce tumor angiogenesis, as well as affect tumor proliferation, invasion, and metastasis by acting as oncogenes or tumor-suppressor genes. Recent studies revealed that some lncRNAs may be induced and regulated by TGF-βto form a complicated crosslinked regulatory net-work. This review focuses on the crosstalk between the TGF-βsignaling pathway and TGF-β-induced lncRNAs.
9.Relationship between augmentation index and severity of coronary artery disease

Objective To explore whether augmentation index is an independent risk factor for coronary heart disease or not,and the relationship between augmentation index and coronary artery disease severity.Methods In the study,245 subjects who were hospitalized between Dec.2003 and Dec.2004,were classified into 4 groups and their clinical and biological characteristics and the summary of the scores in each of the eight segment and aortic pressure waveform in ascending aorta were recorded respectively.Results The larger the augmentation index was,the more the number of coronary stenosis vessels was.Linear regression analysis indicated that augmentation index was significantly correlated with grade of coronary stenosis.When augmentation index was more than 45%,the sensibility and specificity was 91.5%and 92.6%respectively.In the logistic regression model,augmentation index was the independent risk factor for coronary heart disease,and the odds ratio of coronary heart disease was 1.893.The 95%CI was 1.421~2.521.Conclusion Augmentation index is an independent risk factor for coronary heart disease and a predictor of angiographic coronary artery disease severity.
10.Experimental studies on bone defect reconstruction of dog radius using injectable osteoinductive material with fibrin sealant as vehicles

Objective To study the effect of injectable osteoinductive materials with fibrin sealant as the vehicle on the reconstruction of segmental radius defect in dog, and to provide an experimental foundation for its clinical application in the future. Methods 20mm bone defects were created in the radius 18 dogs, and in the experiment group b [FS+bFGF+bBMP (bovine BMP)] composite, and in experiment group r (FS+bFGF+rhBMP-2) were injected percutaneously into the defects. A control group (FS) with bone defect only was also included. The reconstruction effect on the segmental dog radius defects was evaluated by radiography, histology, bone mineral density (BMD) at 4, 8, 12, 16, 20 and 24 weeks after operation.
