1.Clinical Characteristics and Immune Function Change of Cytomegalovirus Infection in Newborn Infants and Little Infants

Objective To explore the clinical manifestations,diagnosis and treatment,restoration of disease and immune function change of pediatric patients infected by cytomegalovirus (CMV). Methods The clinical manifestations of 52 CMV patients:15 cases (29%) of common infections,12 cases (23%) of hepatitis,11cases (21%) of inflammatory reactions,6 cases (12%) of pneumonia,4 cases (7%) of rash,2 cases (4%) of CMV encephalitis and 2 cases of (4%) intrauterine growth restriction. The examination of peripheral blood,including leucocyte counts and classification and platelet counts,were performed twice,6 ?L/time. C-reaction protein (CRP) using femoral vein were tested right after admission and during treatment period. The molecular biology technique was used to detect pathogens. Liver function examination was also performed. Moreover,the examination of cell immune function was done using the flow cytometer to test lymphocyte counts.Results 1.Blood test:white blood counts of 2 cases (4%) exceeded 20?109 L-1,and 18 cases (35%) in the area of (10-20)?109 L-1,the other patients were all normal.The lymphocyte counts in 46 cases (88%) increased and the thrombocyte counts in 12 cases (23%) were less than 100?109 L-1,the lymphocyte and thrombocyte counts of other patients were all normal.2.CRP:11 cases (21%) exceeded 20 mg/L,others were
2.Shape variation of optic nerve head by mechanical analysis using three-dimensional finite element model

BACKGROUND:The measure and prediction of the shape of optic nerve head are the key issues for early diagnosis and duration prediction of glaucoma. Therefore, it is significant to research the shape variation of optic nerve head under high intraocular pressure.
OBJECTIVE:To establish three-dimensional finite element model of optic nerve head and to analyze the deformation of optic nerve head and the change in retina thickness after acute high intraocular pressure.
METHODS:Animal model of acute ocular hypertension was established by methods of anterior chamber perfusion. The tomographic images of the optic nerve head of a cat were obtained at the normal intraocular pressure and 5 320 Pa, 7 980 Pa, 10 640 Pa, 13 300 Pa, 15 960 Pa intraocular pressures using optical coherence tomography. Then, we measured the variation of retinal thickness at typical location. Basing on the tomographic images of optic nerve head of a cat at normal intraocular pressure, we obtained three-dimensional structure of retina and choroid using software MIMICS. Then three-dimensional model of optic nerve head was established by assembling the retina and choroid. The deformation of optic nerve head and the change in retinal thickness under high intraocular pressure were observed using software ABAQUS. The effectiveness of the model was verified by the experimental result.
RESULTS AND CONCLUSION:With the increase of intraocular pressure, the retinal thickness is thinner, the optic nerve head depth, width and the ratio of the depth to width are gradual y increased. It suggests that the acute high intraocular pressure causes retinal thinning, optic nerve head widening and deepening. It is feasible to establish optic nerve head modeling in vivo by using optical coherence tomography, mechanical analysis can be applied to predict the shape variation of optic nerve head, which is significant to further deduce the pathological process of glaucoma.
3.Application of evidence-based nursing in patients with acute myocardial infarction undergoing PTCA and intracoronary stenting

Objective To validate the clinical effect of evidence-based nursing on early rehablilitation of patients with acute myocardial infarction undergoing PTCA and intracoronary stenting. Methods 47 patients with acute myocardial infarction undergoing PTCA and intracoronary stenting were divided into the nursing group(24 cases)and the control group(23 cases).The nursing group received nursing with evidence-based nursing pattern while the control adopted routine nursing method. The nursing effect in the two groups was compared in the following aspects such as the incidence rate of adverse effect,average hospitalization days, athletic ability and psychological status. Results The incidenece rate of adverse effect Was lower(P<0.05)and the average hospitalization days were shorter(P<0.01)than those of the control group with addition of cardiovascular complications(P>0.05).The athletic ability and psychological status were also superior to those of the control group(P<0.05).Conclusion Application of evidence-based nursing could hasten the rehabilitative process of patients undergoing PTCA and intracoronary stenting.
5.Identification of Epmedii Folium and its counterfeit leaf of Quercus variabilis.

Epmedii Folium is a commonly used traditional Chinese drug, and is beneficial for the "liver" and "kidney" s function in Chinese medicine. Recently, the origin of this drug is more complex. Most of the identification studies are emphasized on the species certified by the pharmacopoeia and other related species from the same genus of Epimedium, but few was emphasized on the counterfeit. In this paper, one counterfeit of Epmedii Folium, identified as the dried leaf of Quercus variabilis (Fam. Fagaceae), has been reported based on field investigation, comparing specimen of Epmedii Folium and Q. variabilis,using the macroscopic, microscopic and TmC methods. It is resulted that they could be identified clearly not only by the macroscopic features, such as the vein character and the tooth apex, but also by the microscopic features, such as the vascular bundles of the midrib, the non-glandular hair, the anticlinal wall of the epidermis cell and the calcium oxalate crystal. Furthermore their TLC chromatograms showed also difference. This study will give reference for the identification of Epmedii Folium and the related supervision and inspection work.

6.Effect of total glucosides of paeony on Wnt/β-catenin signal transduction pathway expression in kidney of diabetic rats.

The study is to explore the effect of total glucosides of paeony (TGP)on Wnt/β-catenin signal transduction pathway expression in kidney of diabetic rats, and discuss the protection of TGP in diabetic nephropathy and possible mechanism. Ninety male SD rats of 8 weeks age were randomly divided into normal control group (n = 10) and model group (n = 80). Rats of the normal control group were fed with regular diet, while rats of the model group were fed with high-fat high-sugar diet and 4 weeks later were given an intraperitoneal injection of 35 mg x kg(-1) streptozotocin (STZ). The successfully induced type 2 diabetic rat models were then randomly divided into DM group, three TGP (50, 100, 200 mg x kg(-1) x d(-1)) treatment group and tripterygium wilfordii glycosides (8 mg x kg(-1) x d(-1)) control group. Rats of DM group and each treatment group were given high-fat high-sugar diet. At week 14, the levels of blood sugar, 24 hour urine protein, serum creatinine and blood urea nitrogen were tested. The rats were then sacrificed. Renal pathological changes were examined. Renal tissue Wnt-1 and β-catenin expressions were detected by immunohistochemical assay. Wnt-1 mRNA and β-catenin mRNA expression was semi-quantified by RT-PCR. Wnt-1 protein and β-catenin protein expression was semi-quantified by Western blot. The Result show that Wnt-1 and β-catenin expression increased in kidney of high-fat high-sugar induced type 2 diabetic rats. Compared with diabetic group, the level of serum creatinine, blood urea nitrogen, 24 h urine protein, mean glomerular area and mean glomerular volume were decreased, renal histopathology were improved, expression of Wnt-1 and β-catenin mRNA and protein was reduced in TGP group. Tripterygium wilfordii glycosides had the similar effect. In conclusion, these results showed that Wnt/β-catenin abnormal activation in kidney of type 2 diabetic rats, TGP can improve kidney damage in diabetic rats and delay the development of diabetic nephropathy by inhibit the Wnt/β-catenin signaling pathway.

7.Discussion on the clinical indications for blood transfusion in anemia of prematurity.

OBJECTIVETo compare the effect of different indications of blood transfusion on the treatment and complications in anemia of prematurity, and to investigate the suitable and reasonable indication of blood transfusion for our country.
METHODSNinety six hospitalized premature infants with birth weight less than 1500g and stay in hospital equal or more than 2 weeks were enrolled in the retrospective study and divided into two groups according to different indications of blood transfusion. There are 54 cases in observation group 1 (with strict indications for transfusion) and 42 cases in observation group 2 (with loose indications for transfusion).
RESULTSIn the observation group 1, the total amount of blood transfusion \[M(P25, P75):71.0(43.25, 107.25) ml\] and the times of blood transfusion \[3.00 (2.00, 4.00) times\] are both higher than those of the observation group 2 \[36.0 (29.50, 56.25) ml and 1.50 (1.00, 2.25) times, respectively, P = 0.00 1\]. The days of mechanical ventilation and the days of continuous positive airway pressure were more in the observation group 1, but oxygen supplement in the two groups had no significant difference. In observation group 1, the incidence of intracranial hemorrhage, fungal infection rate and mortality were higher than those of the observation group 2 (P < 0.05).
CONCLUSIONSAnemia of prematurity easily led to the complications, and had obvious influence on the survival of premature infants. Clinician should consider concrete conditions of every patient before blood transfusion. It has positive significance on the survival of premature infants to loosen the indication properly and make the use of blood scientifically and rationally.

8.Comparative study on different methods for quantitative analysis of oral common microorganisms

Conclusion These three non-culture methods can be used in the quantitative analysis of oral microorganisms.Real-time PCR and laser scanning confocal microscopy are better than the traditional culture-based CFU count,and real-time PCR is the most sensitive method.
9.Differential gene expression between Streptococcus sobrinus 6715 and its fluoride-resistant strains

Objective To detect the differential gene expression between Streptococcus sobrinus(S.sobrinus) 6715 and its fluoride-resistant strains. Methods The fluoride-resistant strains of S.sobrinus 6715 was induced by increasing the concentration of fluoride step by step.Total RNA of both S.sobrinus 6715 and its fluoride-resistant strains was extracted,mRNA was separated and purificated,and then cDNA was obtained by reversed transcription.Suppression subtractive hybridization(SSH) technology was used to detect the differential gene expression between them.The differential gene expression fragments were cloned and compared with the GenBank by BLAST.Results After comparing with the GenBank by BLAST,it was identified that there were two differential gene expression fragments,fruA and SMU.438c. Conclusion The cDNA subtractive lib of differential gene expression between S.sobrinus 6715 and its fluoride-resistant strains was successfully constructed through SSH,which paves a way for the further study of fluoride-resistant mechanism.
