1.Intervention Effect of Xiaotan Hezhong Recipe on Pancreatic Fibrosis and Expression of Related Proteins in Chronic Pancreatitis Rats

Objective To observe the effect of Xiaotan Hezhong Recipe on pancreatic fibrosis forming and the expression of related proteins in model rats of chronic pancreatitis. Methods Twenty-four Wistar rats were randomly divided into normal group, model group, pancreatin group and Chinese medicine group (n=6). Rats model were made by DDC solution intraperitoneal injection. Pancreatin group and Chinese medicine group were treated by intragastric administration with pancreatin and Xiaotan Hezhong Recipe, respectively. Body weight of rats was measured regularly. The rats were killed after 4 weeks, with pancreatic pathological detection and laminin, matrix metalloproteinase-1 detection. Results After intraperitoneal injection of DDC solution, pancreatic tissue fibrosis in different degree and inflammatory cell infiltration were found. Chinese medicine group was better than the model group in general conditions, body weight change, pancreatic pathological score and expression of laminin, matrix metalloproteinase-1, the differences were statistically significant (P<0.05). Conclusion Xiaotan Hezhong Recipe can improve chronic pancreatitis rats generally and inhibit pancreatic fibrosis, its mechanism may be related with decreasing laminin expression and increasing the expression of matrix metalloproteinase-1.
2.Effects of Huoxue huayu decoction on expression of retinal glutamate transporter in acute ocular hypertension of rats

Background Hypertension,ischemia and hypoxia induce the increase of glutamate level in eye tissue and furthermore produce excitatory damage and apoptosis of retinal ganglion cells(RGCs).At present,the study on the protection of traditional Chinese medicine on glutamate-induced retinal excitatory damage is lack.objective Present study was to explore the protective effects of Huoxue huayu decoction,a Chinese herbal recipe,on RGCs in acute ocular hypertension model. Methods Fony-five healthy clean Wistar rats were divided into three groups randomly.The acute ocular hypertension models were established in 40 Wistar rats by injecting the normal saline solution into the anterior chamber to elevate the intraocular pressure for 60 minutes.Huoxue huayu decoction of 4 ml was administered intragastrically once per day in 20 model rats.Other matched 5 normal Wistar rats served as normal control group.The rats were sacrificed on 1,3,7 and 14 days after modeling and the retinas were isolated for the histopathologieal and uhrastructure examination.Expression of glutamate transporter in the retina of acute ocular hypertension and normal rat retina were detected using quantitative real-time polymerase chain reaction.The utilization of animals followed the Association for Research in vision and Ophthalmology. Results After acute ocular hypertension.the retina thickness attenuated and the numbers of RGCs decreased under the light microscope in 1,3,7 and 14 days after modeling in comparison with normal control rats.The degradation of the organelle and edema as well as changes of cell nuclei were seen in model rats under the transmission electron microscopy.The expression of glutamate transporter mRNA in model group and glutamate transporter group was rapidly elevated in the first day (P<0.05),descended at the third days(P<0.01)and returned to the normal level 7 days later(P>0.05).Conclusion Huoxue huayu decoction can protect the retina against RGCs damage in the acute ocular hypertension by elevating the expression of glutamate transporters in retina.
3.Study progress of treatment for children with tuberous sclerosis complex

The tuberous sclerosis complex (TSC) in children is also named Bourneville disease,the 3 main symptoms namely epilepsy,mental retardation and facial angiofibromas.It is an autosomal dominant disease.It is an important cause of epilepsy,skin disease,and renal and pulmonary disease in children and adults.The appropriate therapy and prognosis for TSC patients are often different than that for individuals with epilepsy,renal tumors,or interstitial lung disease from other causes.In recent years,certain progress has been made in management of tuberous sclerosis,inhibitors of the mammalian target of rapamycin (mTOR) have demonstrated regression of astrocytomas,angiofibromas,and angiomyoliomas,as well as improved pulmonary function in persons with TSC.This article reviews the current therapeutic recommendations for medical and surgical management of neurologic,renal,and pulmonary manifestations of TSC.
5.Different methods treatment for intertrochanter fracture of femur

Objective Probe into different method safe and effective fixation method,clinical result and cliniical to indications of different treatments in intertrochanter fracture of femur in the old elderly.Methods Adopting the femur near end dissecting type to lock the stencil plate fixation 14 of 58 cases of old intertrochanter fracture of femur separately.the hip screw of motive force(DHS) fixation was used in 24,femur near end stencil plate fixation in 20,and the blood loss,operation time,ossicle split off situation and fracture heal time were compared among the three groups.Sanders grade results and complications were followed up.Results Postoperative follow up Was done in all patients,from 8 months to 3 years,averaged 1.5 years.three groups of skill type fine rate of the three groups being 92.9%,92%,90%respectively.Conclusions Different methods showed good clinical results in treating the old intertrochanter fracture of femur,each having its indications.Operation should be done should follow based on fracture type,age,physique factors and patient's osteoporosis intensity,choosing the best healing solution by characteristics of intermal flxation while each combining.The hook board(ALHP)is suitable for old patients with serious osteoporosis,DHS for Evans Ⅰ,Ⅱ types and some Evans Ⅲ type,and femur near end dissecting type stencil plate for Ⅳ and Ⅴ types fractures.
7.Safety analysis of new type antidepressants in treatment of depression in patients with coronary heart disease

Objective To evaluate the safety of new-generation of antidepressants and amitriptyline in the treatment of depression in patients with coronary heart disease (CHD). Methods A total of 194 patients with first-episode depression with CHD were divided into amitriptyline group(n=40), venlafaxine group(n=40), mirtazapine group(n=48)and escitalo?pram group(n=66). The blood routine test, liver function, blood lipids and blood glucose (GLU) were monitored after treat?ment for six weeks, and which were compared before and after treatment. Results The levels of white blood cells (WBC) and neutrophil count (NE) were significantly lower in amitriptyline group after 6-week treatment (P<0.05), but the levels of acid alanine aminotransferase (ALT), aspartate aminotransferase (AST), total cholesterol (T-CHO) and GLU were significant?ly increased after treatment than those before treatment (P<0.05). The levels of WBC, NE and GLU were significantly de?creased in venlafaxine group after 6-week treatment (P<0.05). The levels of ALT, AST, low density lipoprotein (LDL) were significantly increased in mirtazapine group after six-week treatment (P<0.05). In escitalopram group, the level of three ac?yl glycerin (TG) was significantly increased after six-week treatment than before treatment ( P<0.05). There was a signifi?cant difference in AST change after treatment between venlafaxine group and mirtazapine group (P<0.05). There was a sig?nificant decrease in WBC in amitriptyline group than that of mirtazapine group after six-week treatment ( P<0.05). There was a significant decrease in NE in amitriptyline group than that of mirtazapine group and escitalopram group ( P<0.05). The increase level of AST was significant higher in amitriptyline group than that of venlafaxine group (P < 0.05). Conclusion Three different kinds of new-generation of antidepressants have fewer influence in routine blood test, liver function, blood lipids and blood glucose than those of amitriptyline in the treatment of depression in patients with CHD.
8.Interaction of calcitonin receptor gene polymorphisms and environmental factors in the population lived in coal-burning-borne endemic fluorosis areas in Chongqing

Objective To explore the interaction of calcitonin receptor (CTR) gene polymorphisms and environmental factors in the population lived in coal-burning-borne endemic fluorosis areas in Chongqing.Methods A 1 ∶ 1 case-control study was carried out and Duping Township of Wushan County and Xinglong Township of Fengjie County of Chongqing were chosen as the endemic fluorosis areas.The observation subjects were divided into case group 121 cases and internal control group 130 cases.The Alu I polymorphism in the CTR gene was genotyped using the PCR-RFLP procedure.Logistic regression model was used to analyze the environment and genetic factors,and the interaction between genes and environment was determined according to interaction indicators.Results The rate of CC genotype in case group was lower than that of the control group [60.33% (73/121) vs.74.62% (97/130)],while the TT genotype was higher than that of the control group[9.09% (11/21) vs.3.85%(5/130)].Significant differences in Alu I genotypes were observed between groups(x2 =6.57,P =0.037 < 0.05; 95%CI:0.029-0.036).Allele frequencies of CTR genotypes differed significantly between groups(x2 =7.67,P =0.006 < 0.01 ; OR =0.53,95 % CI:0.338-0.834).Urinary fluoride level (≥ 1 mg/L) was demonstrated to be a risk factor of fluorosis(OR =1.814,P =0.041 < 0.05).There was a positive interaction(OR =5.530,γ =2.457) between CT + TT genotypes in CTR and the fluorosis environment of the people (urinary fluoride level ≥ 1 mg/L).Conclusions There is a certain type of interaction between CTR gene C/T polymorphism and environmental fluorine content (urinary fluoride ≥ 1 mg/L) in Chongqing population lived in coal-burning-borne fluorosis areas,and the onset of fluorosis is the result of interaction between heredity and environment.
9.Effect of Social Support, Coping Styles and Self-efficacy on Depression of Adults with Epilepsy

ObjectiveTo explore the effect of the social support, coping styles and self-efficacy on depression of adults with epilepsy. Methods84 patients with epilepsy were evaluated with Zung's self rating depression scale (SDS), general conditions questionnaire, Social Support Scale, Coping Styles Questionnaire, General Self-efficacy Scale. Based on the SDS, they were divided into the depression group and the epileptic group.Results52 patients suffered from depression (61.2%). There were significant differences in general self-efficacy, coping styles and social support between the depression group and the epileptic group (P<0.05). The score of SDS was positively correlated with the frequency of seizure, duration of epilepsy, fantasy, self-accusation and avoiding (P<0.05), and negatively correlated with the economic status, total support, objective support, subjective support, utilization of support, problem solving, help seeking, rationalizing and self-efficacy (P<0.05). Multiple regression analysis showed that the score of SDS was correlated with the self-efficacy, help seeking, self-accusation, economic status and frequency of seizure.ConclusionThe depression of adults with epilepsy are affected by the social support,coping styles and self-efficacy.
10. Analysis on the prevention of osteoporosis in middle-aged and elderly residents of Shanghai community

Objective : To investigate the correlation between knowledge, self-efficacy and prevention management behaviour of osteoporosis among middle-aged and elderly community residents in Shanghai, based on the knowledge, attitude, and practice (KAP) theory. Methods ¡¤ By using convenient sampling, a cross-sectional study was conducted to obtain information of 2 058 residents from 20 communities in Shanghai, concerning socio-demographics, osteoporosis knowledge scale, osteoporosis self-efficacy scale and osteoporosis prevention management be-haviour scale questionnaires. The structural equation model was applied to correlation analysis and path analysis. Results ¡¤ A total of 2 001 valid questionnaires were collected, the average age being (62.12±12.08) years; female osteoporosis prevention self-management behaviour (17.25±5.35) was shown to be better men's (t=3.78, P<0.001). A higher score of prevention management behaviour in osteoporosis was obtained from residents of college education or higher (18.25±5.18), compared with those of primary school or lower (15.34±4.74), and high school (17.09±5.31) (F=26.93, P<0.001), respectively. Higher scores were revealed in groups with family history (18.13±4.73) compared with those without (16.89±5.33) (t=3.89, P<0.001). Higher income groups achieved higher scores (17.90± 4.52), compared with relatively lower income groups (F=8.66, P<0.001). Multiple factor analysis demonstrated that knowledge and self-efficacy were significantly correlated with prevention management behaviour (β=0.27, 95% CI 0.22-0.32; β=1.01, 95% CI 0.92-1.11). Path analysis showed that relevant knowledge (β=0.169, P<0.001) had direct effect on the prevention management behaviour, and could also be mediated by self-efficacy (β=0.410, P<0.001). Conclu-sion ¡¤ Male, low education, no family history of osteoporosis and low income are the risk factors for the decrease of osteoporosis prevention and management behaviour.
