1.Application of vacuum sealing drainage in the clinical treatment of destroyed hand injuries

Objective To evaluate the effect of vacuum sealing drainage(VSD)ON the destroyed hand injuries.Methods From January 2006 to December 2008,forty pafients with destroyed hand injuries (surface of wound)were randomly divided into VSD group(20 cases)and control group(chlorhexidine wet compress)(20 cases)after debridement. The outcomes were retrospectively analysed by pain score,swelling score,bacterial positive rate and necrotic tissue.Results Three days after treatment,the pain score of VSD group was(2.62±1.54)degree,and that of control group was(3.39±1.64)degree.There wag significant difference between the two groups(P<0.01).The swelling score of VSD group was(1.51±0.25)degree which was different from control group(2.29±1.26)degree(P<0.05).The bacterial positive rate of control group was 20%(4/20),but that of VSD group was 5%(1/20).There was significant difference between the two groups(P<0.05).The VSD group had no necrotic tissue occurrence,but the control group had 2 cases,one of which had serious necrosis and had escapology.Conclusion VSD Can inhibit bacterial infection,ease pain,exempt changing dressings and stimulate growing of granulation tissue also can maintain a good environment for tissue transplantation later.So VSD combining with tissue transplantation is an ideal therapy in destroyed hand injuries.

4.Regulating effect of diltiazem on serum microRNA expression in aged patients with dilated cardiomyop-athy

Objective:To explore influence of diltiazem hydrochloride on expression of serum microRNA in aged pa‐tients with dilated cardiomyopathy (DCM ) . Methods :A total of 120 aged DCM patients (DCM group ) and 60 healthy aged people (normal control group) were enrolled .DCM group was further randomly and equally divided in‐to routine treatment group (n=60) and diltiazem group (n=60 ,received additional diltiazem hydrochloride based on routine treatment) .Left ventricular end -diastolic dimension (LVEDd) ,left ventricular posterior wall thickness (LVPWT) and left ventricular ejection fraction (LVEF) were observed and compared between routine treatment group and diltiazem group before and after treatment ,cardiac function improvement was compared between these two groups after treatment .Changes of expression amount of serum miR -1 ,-133 ,-499 and -208 were com‐pared among three groups before ,six and 12 weeks after treatment .Results:Compared with before treatment ,after treatment ,there were no significant changes in LVEDd ,LVPWT and LVEF in routine treatment group , P>0.05 all;there were significant reductions in LVEDd [(69.41 ± 6.42) mm vs .(61.32 ± 5.31) mm] and LVPWT [(20.98 ± 1.98) mm vs .(12.05 ± 2.42) mm] and significant rise in LVEF [(32.81 ± 4.71)% vs .(50.52 ± 6.88)% ] in dilt‐iazem group , and LVEDd , LVPWT were significantly lower than those of routine treatment group [LVEDd:(67.25 ± 4.21) mm ,LVPWT :(18.49 ± 2.86) mm] ,and LVEF was significantly higher than that of routine treat‐ment group [ (47.77 ± 3.92)% ] , P<0.05 all .For cardiac function improvement ,markedly effective rate (71.7%vs .56.7% ) and total effective rate (98.3% vs .85.0% ) of diltiazem group were significantly higher than those of routine treatment group , P<0.05 both . Compared with normal control group , the 4 kinds of miR expression sig‐nificantly reduced in routine treatment group , and compared with routine treatment group , those of diltiazem group were significantly rose , P<0.01 all , but compared with 6‐week , miR expression had downtrend during 12‐week , the miR‐208 expression was most significant ( P< 0.01 ) .Conclusion:Diltiazem could effectively modulate serum microRNA expression in aged CDM patients ,which provided molecular biological basis for studying diltiazem mech‐anism .

7.Clinical observation on therapeutic effect of bloodletting therapy for treatment of patients with piriformis injury syndrome

Objective To observe the curative effect of pricking bloodletting combined with acupuncture on patients with piriformis injury syndrome.Methods A prospective randomized controlled clinical trial was conducted, 84 patients with piriformis injury syndrome admitted to the Department of Acupuncture and Massage in the First Affiliated Hospital of Xinjiang Medical University from May 2011 to May 2014 were enrolled, and they were divided into observation group and control group by random number table, each group being 42 cases. The patients in the observation group were treated with bloodletting once a week (after bloodletting no other treatment was given), and then once a day of acupuncture was performed for 4 days in a week, completing one therapeutic course; while in the control group, the patient was treated with routine acupuncture once a day for 5 days in a week, finishing one therapeutic course. The interval between two therapeutic courses was 2 days, two courses were carried out, and afterwards the clinical therapeutic effects were evaluated in the two groups. The changes of visual analogue scale (VAS) of two groups before and after treatment and the therapeutic effects were observed.Results The VAS score of the two groups after treatment was lower than that before treatment, and the degree of decrease of VAS score in the observation group was significantly lower than that of thecontrol group (1.50±1.35 vs. 2.69±1.68,P < 0.01); the total effective rate in the observation group was obviously higher than that in the control group [100.00% (42/42) vs. 69.05% (29/42),P < 0.01].Conclusion Pricking bloodletting combined with acupuncture therapy for treatment of piriformis injury syndrome can shorten the course of treatment.
8.The expression research of osteopontin in nasopharyngeal carcinoma tissues

ObjectiveTo study the expression and clinical significance of osteopontin in nasopharyngeal carcinoma tissues.MethodsOsteopontin expression was determined in nasopharyngeal carcinoma tissues in 44 cases and normal mucosa tissues in 25 cases by immunohistochemical staining.The relationship between osteopontin expression and pathological features of nasopharyngeal car(e)inoma was analyzed.ResultsThe positive expression rate of osteopontin in nasopharyngeal carcinoma tissues was significantly higher than that in normal tissues and the positive staining area was larger [77.27%(34/44) vs.8.00%(2/25),(60.24 ± 17.51 )% vs.(1.32 ± 0.48)% ](P<0.05).The positive expression of osteopontin was correlated with clinical staging and lymph node metastasis(P < 0.05),while had no relationship with pathological staging(P> 0.05).ConclusionsThere is a close correlation between expression of osteopontin and tumor cell invasion and metastasis.Over-expression of osteopontin may be one of the important factors contributing to the invasion and migration of nasopharyngeal carcinoma.
9.Expression of microRNA146a in ultraviolet B (UVB)-damaged mouse skin

Objective To investigate the expression changes of microRNA 146a (miRNA146a) in UVBdamaged mouse skin. Methods C57/BL6 mice were divided into dose groups to be irradiated on the back with UVB of 30, 60, 90, 180, 270 mJ/cm2 respectively for 24 hours, and time groups irradiated with UVB of 180 mJ/cm2 for 1, 12, 24 and 48 hours respectively. The mice received no irradiation served as the control. Skin samples were subsequently obtained from the irradiated sites of mice and subjected to real-time fluorescent PCR for the detection of miRNA146a expression as well as immunohistochemical staining (IHC) for the detection of STAT3. Results The expression levels of miRNA146a were 0.01158 ± 0.00098, 0.01083 ± 0.00104,0.00872 ± 0.00031, 0.00851 ± 0.00033, 0.00810 ± 0.00057 and 0.00770 ± 0.00031 in the unirradiated control mice and mice irradiated with UVB of 30, 60, 90, 180, 270 mJ/cm2 for 24 hours, respectively, 0.00730 ±0.00036, 0.00805 ± 0.00035, 0.00810 ± 0.00057 and 0.00837 ± 0.00039 in mice irradiated with UVB of 180mJ/cm2 for 1, 12, 24 and 48 hours, respectively. IHC suggested an intensive expression of phosphorylated STAT3 in mice irradiated with a high dose UVB. Conclusions miRNA146a may play an essential role in the mechanism of UVB-induced photodamge, which is mainly correlated with the negative regulation of inflammatory reaction likely mediated by the JAK/STAT3 signal transduction pathway.
10.Time difference attack therapy in multiple Acinetobacter bauamnnii.

Objective To explore time difference attack therapy in Acinetobacter bauamnnii infection. Methods 67 patients with Acinetobacter bauamnnii were divided into two groups. The experimental group were first given Fosfomycin 4 g 5% GS 100 ml iv girt to finish within 60 min and then given Cefperazone/Sulbactam 4 g 0.9% NS 250 ml iv gitt immediately bid. The control group were given: Ampicillin/Sulbactam 3 g 0.9% NS 250 ml iv gitt (tid) + Ciprofloxacin 0.2 g iv girt (bid). The treatment course all was 11 days. Results The overall effective rate of experimental group methods was superior to that of control group(X2 =9.56 ,P =0. 023). Conclusion The Fos-fomycin Cefperazone/Sulbactam time difference attack therapy for the treatment of Acinetobacter the bauamnnii in-fection is a new way.
