1.Primary malignant pericardial mesothelioma:clinicopathological and immunohistochemical analysis

Objective To explore the clinicopathological and immunohistochemical features of primary malignant pericardial mesothe- lioma.Methods Seven cases of primary malignant pericardial mesothelioma,in 4 females and 3 males,with the age ranged from 22 to 51 and a mean of 40 years old,were studied with clinicopathological,immunohistochemical and histochemical techniques,The pertinent litera- ture was also comprehensively reviewed.Results 3 biopsy specimens and 4 surgical specimens,including 2 autopsy specimens,were stud- led.The main clinical symptoms were palpitation and short breath,which were aggravated after movement.The longest course was 4 years,and the mean disease course was 12 months.The chest films demonstrated an obviously increase in heart shadow,and the pericardi- um was thickened in some cases.CT clearly showed tumorous shadow in 2 cases.The histopathologic picture showed tubular type in 2 ca- ses,papillary in 2,signet ring cell type in 1 and myofibroblast types in 2.In 2 autopsy specimens it was found that pericardium was dif- fusely thickened.In one case the tumor was metastasized to the right lung,mediastinum,and pulmonary hilar lymph node.In another case there was metastasis to right kidney.Immunohistochemical staining showed that the neoplastic cells of all cases were positive for CK5/6, calretinin,CK.vimentin and EMA,but negative for CEA.In the histochemical staining seven cases,the malignancy was positive for CI to different extent,but negative for HCL Conclusion Immunohistochemistry and histochemistry play an important role in diagnosis and dif- ferential diagnosis for primary malignant mesothelioma and other tumors in pericardium.
2.Retrospective study on the treatment of severe asthma

Objective To evaluate the effect for the treatment of severe asthma. Methods The data of 47 patients with severe asthma who were admitted to emergency department were retrospectively anayzed. Results Of total 47 patients ,45 were rescued, with the survival rate of 95.7%. Arterial blood gas was improved after treatment (P < 0.01). Conclusion Appropriate commencement, mode, strategy, and early weaning of mechanical ventilation, combined with the administration of bronchodilators and eorticosteroids are the important way to rescue patients with severe asthma.
3.Influence of Reproduction Methods on Volatile Oil of Pogostemon cablin Benth.

Objective To compare the volatile oil and constituent content in Pog ostemon cablin Benth. propagated by cuttage and tissue culture. Methods The vola tile oil assay of Chinese pharmacopoeia and GC-MS method were used. Results The volatile oil and constituent content of Pogostemon cablin Benth. reproduced by cuttage was 14.2 mL/kg and 81.31 %,respectively. The volatile oil and constituent content of Pogostemon cablin reproduced by tissue culture was 12.2 mL/kg and 82.98 %,respectively. Conclusion The volatile oil content and constituent content of Pogostemon cablin Benth. reproduced by tissue culture are similar to which reproduced by cuttage.

5.Observation on therapeutic effect of acupuncture combined with medicine on mild cognition disorders in patients with post-stroke.

OBJECTIVETo explore the curative effect and safety of acupuncture for mild cognitive disorders after stroke.
METHODSRandomized controlled trial was adopted and one hundred cases conforming to criteria were divided into a combined acupuncture and medication group (group A) and a medication group (group B), 50 cases in each group. The basic treatment of cerebrovascular disease was applied in two groups. The group A treated was with acupuncture at Baihui (GV 20), Shenting (GV 24) and Qucha (BL 4) etc. and oral administration of Nimodipine. In group B, Nimodipine was taken orally. The treatment lasted for 3 months, the scores of Mini-Mental State Examination (MMSE), modified Hasegawa Dementia Scale (HDS-R) and Barthel Index (BI) were observed before and after treatment in two groups.
RESULTSThe total effective rate in group A was 66.7% (32/48), which was superior to 30.4% (14/46) in group B (P < 0.05). The scores of MMSE, HDS-R and BI after treatment were apparently improved as compared with those before treatment in two groups (all P < 0.01). The improvements in group A were superior to those in group B (all P < 0.05). There was no adverse event reported during the treatment in two groups.
CONCLUSIONAcupuncture can improve activity of daily living and has significant effect in treating mild cognition disorders after stroke without adverse reactions.

6.Efficacy and safety of dapoxetine in the treatment of premature ejaculation.

OBJECTIVETo evaluate the clinical effect and safety of dapoxetine in the treatment of premature ejaculation (PE).
METHODSWe randomly assigned 116 PE patients to receive dapoxetine on demand at 30 mg qd (dapoxetine group, n = 60, aged 23-49 years) or oral tamsulosin at 20 mg qd (control group, n = 56, aged 24-46 years). After 4 weeks of medication, we compared the clinical global impression of change (CGIC) , PE profile (PEP) scores, intravaginal ejaculation latency time (IELT) , and adverse reactions between the two groups of patients.
RESULTSCompared with the baseline, the IELT was remarkably prolonged after treatment both in the dapoxetine group ([0.86 ± 0.17] vs [4.32 ± 2.23] min, P < 0.05) and the control ([0.88 ± 0.15] vs [4.17 ± 2.26] min, P < 0.05), with no statistically significant difference between the two groups (P > 0. 05). The post-treatment rate of CGIC in the dapoxetine group had no statistically significant difference from that in the control (85.00% vs 82.14%, P > 0.05). In comparison with pre-treatment, the patients of both the dapoxetine and control groups showed dramatically improved scores after medication in perceived control over ejaculation (0.85 ± 0.23 vs 2.13 ± 0.97 and 0.88 ± 0.21 vs 2.06 ± 0.34, both P < 0.05), ejaculation-related personal distress (1.15 ± 0.64 vs 2.89 ± 0.26 and 1.19 ± 0.53 vs 2.82 ± 0.69, both P < 0.05), satisfaction with sexual intercourse (0.81 ± 0.33 vs 2.58 ± 0.37 and 0.79 ± 0.28 vs 2.45 ± 0.32, both P < 0.05), and ejaculation-related interpersonal difficulty (2.05 ± 0.61 vs 3.24 ± 0.35 and 2.03 ± 0.65 vs 3.18 ± 0.76, both P < 0.05), with no significant differences between the two groups (P > 0.05). The incidence of adverse reactions was significantly lower in the dapoxetine than in the control group (3.33% vs 30.36%, P < 0.05).
CONCLUSIONDapoxetine is effective for the treatment of PE, with its advantages of prolonging the intravaginal ejaculation latency time, improving the quality of sexual life, and low incidence of adverse reactions.

8.Gender differences in patients with primary hyperparathyroidism concurrent with upper urinary tract calculus

Objective To explore the gender differences in patients with primary hyperparathyroidism (PHPT) concurrent with upper urinary tract stone.Methods The clinical data of 40 PHPT concurrent with upper urinary tract stone cases treated from January 1997 to December 2013 were analyzed retrospectively.There were 21 males and 19 females,with an mean age of 51 years (26-77 years).All patients underwent operation for PHPT,with preoperative parathyroid hormone (PTH) level of (38.42± 30.11)pmol/L.The PTH level in female patients was significantly higher than that in male patients (29.37± 17.64pmol/L versus 48.47±40.55 pmol/L,P=0.04).Serum biochemical variables were monitored,and 21 cases had 24 h-urinalysis.Results Postoperative pathology confirmed 36 cases of parathyroid adenoma and 4cases of parathyroid hyperplasia.Hypercalcemia and hypophosphatemia were improved with no significant gender difference (both P>0.05).But in 21 cases of 24 h-urinalysis,urinary calcium and phosphorus in males were significantly higher than those in females (8.26±2.60 mmol/24 h versus 6.08± 1.59 mmol/24 h,P=0.03; 7.03±1.44 mmol/24 h versus 5.42±1.25 mmol/24 h,P=0.01).Conclusion In patients with PHPT concurrent with urolithiasis,male patients may be more associated with hypercalciuria.
9.Relationship between ERK1∕2 and STAT3 signaling pathways involving in cardioprotection induced by diazoxide postconditioning in rats

Objective To evaluate the relationship between extracelluar signal?regulated protein kinase 1∕2 (ERK1∕2) and signal transducer and activator of transcription 3 (STAT3) signaling pathways involving in cardioprotection induced by diazoxide postconditioning in rats. Methods Sixty adult male Sprague?Dawley rats, aged 3 months, weighing 240-260 g, were randomly divided into 5 groups ( n=12 each) using a random number table: sham operation group ( SH group ) , ischemia?reperfusion ( I∕R ) group, diazoxide postconditioning group ( D group ) , ERK1∕2 inhibitor U0126 group ( U group ) , and STAT3 inhibitor Stattic group ( St group) . Myocardial I∕R was produced by occlusion of anterior descending branch of the coronary artery followed by 120 min reperfusionIn I∕R and D groups, 0?4% dimethyl sulfoxide 1 ml and 7 mg∕kg diazoxide ( in 1 ml of 0?4% dimethyl sulfoxide) was injected through the femoral vein at the onset of reperfusionIn U and St groups, U0126 100 μg∕kg and Stattic 500 μg∕kg were injected through the femoral vein at 10 min before reperfusion, and the other procedures were similar to those previously described in group DAt 120 min of reperfusion, the rats were sacrificed, and myocardial specimens were obtained from the left ventricle for determination of myocardial infarct size, cell apoptosis, and ERK1, ERK2 and STAT3 mRNA expression ( real?time PCR), and phosphorylated ERK1∕2 ( p?ERK1∕2) and phosphorylated STAT3 (p?STAT3) (using Western blot). Apoptosis index (AI) was calculated. Results Compared with group S, the myocardial infarct size and AI were significantly increased, and the expression of ERK1, ERK2 and STAT3 mRNA, p?ERK1∕2 and p?STAT3 was down?regulated in group I∕R. Compared with group I∕R, the myocardial infarct size and AI were significantly decreased, and the expression of ERK1, ERK2 and STAT3 mRNA, p?ERK1∕2 and p?STAT3 was up?regulated in group D. Compared with group D, the myocardial infarct size and AI were significantly increased in U and S groups, the expression of ERK1, ERK2 and STAT3 mRNA, p?ERK1∕2 and p?STAT3 was down?regulated in group U, and the expression of STAT3 mRNA and p?STAT3 was down?regulated, and no significant change was found in ERK1 and ERK2 mRNA and p?ERK1∕2 expression in group S. Conclusion STAT3 signaling pathway is located downstream of ERK1∕2 signaling pathway in the mechanism by which diazoxide postconditioning reduces myocardial I∕R injury in rats.
