[Objective]To investigate the treatment of fractuees of distal end of humerus with open reduction and internal fixation with anatomical plate.[Method]From February 2001 to April 2006,open reduction and anatomical plate internal fixation were performed in 22 cases with distal end of humerus fracture,of which olecranon osteotomy approach was carried out in 11 while musculus triceps brachii linguiform flap approach in other 11.All the patients received combined therapy including fumigation and rinsing with Chinese herbs,CPM(continuous passive motion) and manipulation.[Result]All the cases were followed up for 13 to 22 months.All the fractures got union.The postoperative elbow functions were excellent in 9 cases,good in 7 cases according to Mayo standards.The satisfactory rate was 72.71%.[Conclusion]Open reduction and internal fixation with anatomical plate can provide stable fixation and allow early postoperative elbow functional training in treatment of distal humerua fracture.Good clinical results can be achieved using fumigation and rinsing with Chinese herbs,CPM and manipulation after operation.

OBJECTIVE:To study the preventive effect and mechanism of butylphthalide on myocardial ischemia-reperfusion injury in rats. METHODS:SD rats were randomly divided into sham operation group,model group,butylphthalide low-dose,me-dium-dose,high-dose groups (50,75,100 mg/lg),10 in each group. All rats were intragastrically administrated related drugs once a day,sham operation group and model group were intragastrically administrated equal amount of normal saline,for 3 d. Af-ter last administration,except for sham operation group,120 min of repertusion was conducted. Ratio of myocardial infarction ar-ea, apoptosis rate of myocardial cells, expressions of apoptosis-related protein [proapoptotic proteins (Caspase-3, Fas, Cas-pase-9),apoptotic protein (Bcl-2)],left ventricular ejection fraction (LVEF),and content of vascular endothelial growth factor (VEGF)in serum of rats in other groups were detected. RESULTS:Compared with sham operation group,myocardial infarction ar-ea and apoptosis rate of myocardial cells in model group were obviously increased;expressions of Caspase-3,Fas,Caspase-9 pro-tein in myocardial cells were obviously enhanced,Bcl-2 protein expression was obviously weakened;LVEF and serum VEGF con-tent were obviously reduced (P<0.01). Compared with model group,except that LVEF in butylphthalide low-dose group didn't changed obviously (P>0.05),above indexes in other administration groups improved obviously (P<0.05). Compared with bu-tylphthalide low-dose group,myocardial infarction area and apoptosis rate of myocardial cells in butylphthalide high-dose group were obviously decreased(P<0.05);expressions of Caspase-3,Fas,Caspase-9 protein in myocardial cells in butylphthalide medi-um-dose,high-dose groups were obviously weakened,and Bcl-2 protein expression was obviously enhanced(P<0.05). CONCLU-SIONS:Butylphthalide can effectively reduce myocardial ischemia-reperfusion injury in rats,protect cardiac functions,and its mechanism may be related with up-regulating VEGF.

Objective To compare the leukocyte-reduced red blood cells and plasmas respectively prepared by automated blood components separator and manual method so as to provide theoretical basis for automatic and standardized flow of blood components preparation.MethodsThe prepared red blood cells and plasma were measured on the hemoglobin concentration,mean corpuscular volume (MCV),red blood cell count (RBC),haematocrit (HCT),mean corpuscular hemoglobin (MCH),mean corpuscular hemoglobin concentration (MCHC),average red blood cell volume distribution width,plasma weight and protein content after the first separation,preparation time and etc.These results obtained from different methods were compared and analyzed.Results The plasma weight after the first separation which was one-off prepared were (267.57 ±16.31) g (by automatic method) and (246.06±22.48) g (by manual method) (P＜0.05) respectively.The preparation time were (66.66±9.01) s (by automatic method) and (162.33±22.00) s (by manual method) (P＜0.05) respectively.Other results had no significant difference (P＞0.05).Conclusion All blood components products in this research meet the criterion of GB 18469-2012.However,the quality and separation efficiency of products made by automatic blood component separator are obviously higher than those by manual method.

BACKGROUND:The age, complication, injury to operation time, and pain level are important factors that affect patients with postoperative mortality, but many scholars disagree. Factors affecting death in elderly patients after femoral head replacement stil need further study. OBJECTIVE:To observe factors related to the survival status of elderly patients within 1 year after receiving femoral head replacement. METHODS:A total of 96 elderly patients receiving femoral head replacement at Department of Orthopedics, the First Affiliated Hospital of Xi’an Jiaotong University from January 2011 to January 2014 were selected. On admission, patients’ age, sex, time from injury to operation, injury to admission time, pain classification, admission hemoglobin, serum albumin content of admission, admission lymphocyte count, and the amount of blood transfusion and preoperative complications were recorded. Fisher test was used to analyze the factors affecting survival status within 1 year after replacement in patients with femoral head replacement using one-way analysis of variance. Multivariate Cox test was utilized to perform multi-factor analysis. RESULTS AND CONCLUSION: The injury to hospital time was > 5 days. Pain classification was grades III and IV. The hemoglobin content was < 120 g/L on admission. The volume of blood transfusion was > 1 000 mL. Serum albumin content was < 35 g/L, which was associated with the increased mortality (P < 0.05). The mortality was significantly higher in patients with more than 86 years old than in patients with 70-85 years old (P < 0.05). The mortality of patients with the time from injury to operation > 7 days was significantly higher than patients with the time from injury to operation < 7 days (P < 0.05). The serum albumin content < 35 g/L and grades III and IV of ASA classification were factors related to patient’s death (P < 0.05). Results confirm that the risk factors for survival status within 1 year of femoral head replacement contain intraoperative volume of blood transfusion, admission to operation time, hemoglobin content, and grading of pain.

BACKGROUND:Recent studies have proved the presence of adjacent vertebral fractures after percutaneous kyphoplasty, suggesting that cement leakage may be an important reason. OBJECTIVE:To investigate the effect of bone cement amount during percutaneous kyphoplasty on adjacent vertebral endplate using finite element analysis. METHODS: Three-dimensional finite element pathological model of Osteoporotic vertebral compression fracture was established using finite element analysis, and 10%, 20% and 30% volume of bone cement were filed, respectively. Then the adjacent vertebral endplate stress under the axial compression, anteflexion and rear protraction were analyzed. RESULTS AND CONCLUSION: Compared with that before operation, the adjacent vertebral endplate stress increased with the increase in filed bone cement volume, suggesting that the increased amount of bone cement wil lead to the increasing of adjacent vertebral re-fractures.

Objective To study the imaging distribution feature and diagnostic value of high resolution computed tomography(HRCT)in peripheral lung cancer(PLC).Methods The feature of imaging distribution was analysed in 37 patients with PLC by pathological proved,which compared with those in 23 cases with lung benign nodules by selected randomly.A double blind method was taken on the manifestations of HRCT about lung nodules tumor-lung interface in near heart side and far heart side.①cloudy or/and shaggy②spiculate③smooth.To search and define the correlation between its distributing feature;manifestations of 3 kinds HRCT;alteration of segment level bronchus and lung benign malignancy nodules.Results Cloudy,or shaggy,spiculalte departing from heart side in lung-tumor interface by HRCT were observed in peripheral lung cancer(79%) and benign nodules(22%);smooth was observed in peripheral lung cancer(14%) and benign nodules(74%).Some cases possed simultaneously two or more than two kinds HRCT's signs.Incidence rate of emphraxis and stenosis signs of segment level bronchus in PLC was higher than that in benign nodules.Conclusion Asymmetry apo-tip dominant position distribution of cloudy or shaggy,and spiculate change of tumor-lung interface by HRCT played an important role in qualitative diagnosis of peripheral lung cancer(≤3.5 cm).The appearance reason relates with the bronchial ventilation that the lesion results in occlusion.

Objective To explore the effects of perfluorocarbon (PFC) on the damaged type Ⅱ alveolar epithelial cells (AEC Ⅱ) induced by lipopolysaccharide (LPS),and the apoptosis and inflammatory reaction of AEC Ⅱ induced by LPS.Methods Primary AEC Ⅱ was divided into control group according to the random number table method,LPS group,PFC group and PFC + LPS group.LPS group:LPS (1 μg/ml) was added to cells.PFC group:PFC (20％) was added to cells.PFC + LPS group:PFC (20％) and LPS (1 μg/ml) were added to cells.The apoptotic rate of AEC Ⅱ was detected by flow cytometry.Morphologic change was observed by electron microscope.Concentrations of intedeukin (IL)-6 and IL-10 of supernatant were detected by ELISA.Results Apoptotic rate of AEC Ⅱ remarkably increased in LPS group than in control grouop [(10.89 ± 1.04) ％ vs (14.29 ± 1.93) ％] (P ＜ 0.05).Compared with LPS group,the apoptotic rate of AEC Ⅱ decreased remarkably in the PFC + LPS group [(12.22 ± 1.47) ％],(P ＜ 0.05).IL-6 production of AEC Ⅱ significantly increased in LPS group than in control group [(482.58 ± 26.84) vs (229.40 ± 7.61) pg/ml pg/ml] (P ＜ 0.05),while decreased in PFC + LPS group [(265.44 ± 29.95) pg/ml].IL-10 production of AEC Ⅱ significantly increased in LPS group than in control group [(1 497.29 ±191.89) pg/ml vs (725.87 ±51.83) pg/ml] (P ＜0.05),while there was no difference between LPS group and PFC + LPS group (P ＞ 0.05).Conclusion PFC can protect AEC Ⅱ against the injury induced by LPS.PFC can also release the level of inflammatory response.

Objective:To analyze normal pure tone hearing thresholds patients with aural fullness using evoked otoacoustic emission in order to detect the early cochlear impairment. Method: Forty-three normal pure tone hearing thresholds patients(72 ears,aural fullness group)with aural fullness were served as subjects and 30 normal volunteers(60 ears,control group)as controls. Transiently evoked otoacoustic emission (TEOAE) and distortion product evoked otoacoustic emission (DPOAE) were tested with Capella otoacoustic emission machine.The DPOAE detection rate and amplitudes at all frequencies,the passing rate and wave signal noise ratio (SNR), wave reproducibility,band SNR and band reproducibility of TEOAE were recorded and analyzed.Result:①Only on the frequency points of 0. 50 kHz and 0. 75 kHz,the detection rate of DPOAE in aural fullness group was lower than that in control group(P＜0. 05). There was no significant difference in the rate among other frequency points(P＞0. 05). ②The passing rate of TEOAE was 100% in control group and 90.28% in aural fullness group.There were statistical differences between two groups(X~2=6. 16, P＜0. 05).③Compared with the control group,the DPOAE amplitudes at all frequencies,the wave signal noise ratio(SNR), wave reproducibility,band SNR and band reproducibility of TEOAE in patients with aural fullness were significantly decreased.There were statistical differences(P＜0.05 or P＜0.01). Conclusion:Some patients of normal hearing thresholds with aural fullness have had early harm of outer hair cell in cochlear.TEOAE and DPOAE may be used to detect these lesions early before the hearing impairment occurred.
