1.Respiratory muscle training on the rehabilitation of patients undergoing cardiothoracic and abdominal surgery: a Meta-analysis

Objective To systematically evaluate the effects of respiratory muscle training by using respiratory muscle training device on rehabilitation of people undergoing cardiothoracic and abdominal surgery. Methods The relevant randomized controlled trials were searched in the databases of the CINAHL, Medline, PubMed, EBSCO, China National Knowledge Infrastructure (CNKI), Wanfang Data, VIP. The quality of studies was critically appraised and data were extracted by two reviewers independently, a meta-analysis was conducted for the included studies. Results Finally 12 randomized controlled trial involving 1060 patients were included. Meta-analysis showed that respiratory muscle training device could improve respiratory muscle strength [SMD =0.70, 95%CI (0.15-1.24)]; improved lung function [forced vital capacity:SMD=0.48, 95%CI (0.10-0.87);inspiratory capacity:SMD=0.55, 95%CI (0.07-1.04)]; improve; effectively reduced postoperative pulmonary complications [OR=0.21, 95%CI:(0.15-0.31)]; shorten the length of hospital stays [SMD=-0.52, 95%CI (-0.87--0.16)], all P< 0.05. Conclusions The application of respiratory muscle training with respiratory muscle device could promote the rehabilitation of patients undergoing cardiothoracic and abdominal surgery.
2.The effect of enhanced external counterpulsation on the sleep of some coronary heart disease patients who suffer from insomnia

Objective To investigate the effects of enhanced external counterpulsation on coronary heart diseas epatients who suffer from insomnia .Methods Ninety-two cases of coronary heart disease patients with insomnia were divided into observation group and control group randomly ,46 cases in each group .The cases in control group were given routine drug treatment and nursing for coronary heart disease and insomnia .On this base ,the cases in observation group were given EECP .Use the pittsburgh sleep quality index (PSQI) to evaluate the sleep quality of two groups on day of admission and 30d after treatment .The dimensions score and total score of two groups in each time were compared and analyzed .Results After the intervention of 30 d ,PSQI scores in the observation group were lower than those before intervention ,and lower than that of control group ,and there was statistical signifi-cance(P<0 .05) .Conclusion The EECP can relieve the insomnia of coronary heart disease patients ,and improve there sleep quali-ty .
3.Risk Factors of Perioperative Complications in Patients Undergoing Radical Retropubic Prostatectomy: A Ten-year Experience

Radical retropubic prostatectomy (RRP) has been one of the most effective treatments for prostate cancer.This study is designed to identify the related predictive risk factors for complications in patients following RRP.Between 2000 and 2012 in Department of Urology,Fudan University Shanghai Cancer Center,421 cases undergoing RRP for localized prostate cancer by one surgeon were included in this retrospective analysis.We reviewed various risk factors that were correlated with perioperative complications,including patient characteristics [age,body mass index (BMI),co-morbidities],clinical findings (preoperative PSA level,Gleason score,clinical stage,pathological grade),and surgeon's own clinical practice.Charlson comorbidity index (CCI) was used to explain comorbidities.The total rate of perioperative complications was 23.2% (98/421).There were 45/421 (10.7%),28/421 (6.6%),24/421 (5.7%) and 1/421 (0.2%) in grade Ⅰ,Ⅱ,Ⅲ,Ⅳ respectively,and 323/421 (76.8%) cases had none of these complications.Statistical analysis of multiple potential risk factors revealed that BMI >30 (P=0.014),Charlson score ≥1 (P<0.001) and surgical experience (P=0.0252) were predictors of perioperative complications.Age,PSA level,Gleason score,TNM stage,operation time,blood loss,and blood transfusion were not correlated with perioperative complications (P>0.05).It was concluded that patients' own factors and surgeons' technical factors are related with an increased risk of development of perioperative complications following radical prostatectomy.Knowing these predictors can both favor risk stratification of patients undergoing RRP and help surgeons make treatment decisions.
4.Clinical application of the compound flap of distally based of saphenous nerve-great saphenous vein nutritional vessels

Objective To explore the method of clinical application and the final result of the com- pound flap of distally based of saphenous nerve-great saphenous vein nutritional vessels.Methods Basing on the anatomic basis that the nutrient vessels of saphenous nerve-great saphenous vein have the same deriva- tion as the branches of the muscle,bone and skin.Eight cases of the tissue defects of ankles and feet were re- paired by the flap.Results Seven cases were survived completely except the distal partly necrosis in one case.The calcaneal defects of one case were repaired by the iliac graft after two weeks of the operation.Follow up from 3 monthes to one year,the texture,appearance and function of the multiflap were good.Conclusion The compound flap of distally based of saphenous nerve-great saphenous vein nutritional vessels has the ad- vantages as follow:the reliable blood supply,avoidance of sacrificing the major arteries,reconstruction of sen- sation and simply procedure.Besides,the defects of soft tissue and bone can be repaired on one operation.
5.Three biflavonoids from ethanol extract of the root of Daphne genkwa.

AIMTo investigate the chemical constituents of the secondary metabolites of the roots of Daphne genkwa.
METHODSThe roots of D. genkwa were extracted with 95% ethanol at 60-70 degrees C for 7 days to obtain the crude extract. The crude extract was purified by silica gel and Sephadex LH-20 column chromatography as well as the HPLC techniques. The structures of the isolates were elucidated by combined spectroscopic methods including 1D and 2D NMR, MS, UV, IR and CD.
RESULTSThree new biflavonoids were isolated from the ethanol extract of the roots of D. genkwa and their structures were identified as daphnodorin H-3-methyl ether (1), daphnodorin H-3"-methyl ether (2) and daphnodorin G-3"-methyl ether (3).
CONCLUSIONCompounds 1, 2 and 3 are three new biflavonoids.

6.Development of a Forensic Multiplex Amplification STR Kit for 15 Autosomal STR Loci and 10 Y-STR Loci.

OBJECTIVE:
To establish a multiplex STR genotyping method for autosomal STR and Y-STR loci in forensic biological practice.
METHODS:
Widely used autosomal STR loci and Y-STR loci were selected. A set of PCR primers was designed, and a 5-dye fluorescent labeled STR multiplex PCR reagent kit was developed.
RESULTS:
A kit was developed which can simultaneously detect 15 autosomal STR loci, 10 Y-STR loci, and an Amelogenin.
CONCLUSION
The 15 autosomal STR plus 10 Y-STR kit in combination with capillary electrophoresis method was used to STR genotyping with accurate and reliable results. The new one-step testing kit can potentially be widely used in forensic cases and DNA databank in the future.

8.Quantitative evaluation of Colles' fracture by Multislice CT multiplanner reconstruction: a feasibility study.

OBJECTIVETo investigate the feasibility and reliability on the quantitative evaluation of Colles' fracture by multislice CT (MSCT) multiplanner reconstruction (MPR).
METHODSA total of 36 patients with Colles' fracture from July 2011 to July 2014 were investigated in this study. There were 11 males and 25 females with a mean age of (42.5 ± 5.4) years old (ranged 35 to 72 years). All the patients underwent anteroposterior and lateral X-ray films and MSCT scans on wrist joints within 2 days after trauma. Images were sent to the workstation through picture archiving and conserving system (PACS). One associate chief physician independently and respectively measured the dorsal intercalation depth of distal fracture block, palmar angle and dislocation degree of wrist articular surface collapse on anteroposterior and lateral X-ray film and MSCT-MPR. The time interval between the two measurements was 2 weeks. All the data between the first and second measurement on X-ray and MPR and the mean value between the X-ray and MPR was examined with paired t-test. The pearson analyzed their correlation.
RESULTSAmong the 35 cases, 35 cases of palmar angle, 21 cases of intercalation depth and 16 cases of dislocation of wrist articular surface collapse could be measured on both X-ray and MPR. For the above parameters, the first measurement results were (12.5 ± 3.6)°, (4.5 ± 2.1) mm, (3.7 ± 1.6) mm and the second measurement results were (4.8 ± 2.2)°, (6.4 ± 3.6) mm, (2.5 ± 1.2) mm on X-ray films respectively. The first measurement results on MPR were (14.5 ± 5.3)°, (4.2 ± 1.2) mm, (5.7 ± 2.3) mm, and the results were (13.2 ± 2.6)°, (4.7 ± 2.2) mm, (4.6 ± 2.1) mm for the second measurement respectively. The three parameters between the first and second measurement on plain film had statistical difference and low correlation (r = 0.681, 0.640, 0.345, P < 0.05). The data between the first and second measurement on MPR showed that the dislocation degree of wrist articular surface collapse had statistical difference (P < 0.05) and no statistical significance was found for the other two parameters (P > 0.05), with the moderate correlation (r = 0.954, 0.854, 0.642). The three parameters had low or moderate correlation with each other on X-ray (r = 0.454, 0.532, 0.378, P < 0.05), compared with the mean value on MPR.
CONCLUSIONUsing MSCT MPR images may carry on the multiple parameter measurement of Colles fracture, to make quantitative evaluation, and repeated measurement is better reliability.

10.The study of combining high-risk human papillomavirus types checking and cytologic test in the screening of cervical lesions.

OBJECTIVETo study the relationship between thinprep cytologic test and the types of human papilloma virus (HPV) infection in cervical precancerous lesion screening.
METHODSTo perform high-risk HPV types test in 1375 samples. Choose 256 positive samples to take thinprep cytologic test (TCT) and directed biopsies under colposcopy. Adopting two-channels real time PCR to genotype and quantify eight high risk HPV DNA (high risk types: HPV 16, 18, 45, 31; intermediate risk types: HPV 33, 52, 58, 67).
RESULTSThere are 256 positive samples in High risk HPV DNA test (18.62%). WNL rate for TCT is 16.41% (42/256), ASCUS and above rate for TCT is 83.59% (214/256). There is no statistically significant difference in the viral loads of HPV infection rate between the TCT negative patients and positive patients (P > 0.5). Positive correspondence rate for TCT and biopsy are 92.86% (39/42), 81.36% (48/59), 85.19% (23/27) and 9/10.
CONCLUSIONHigh-risk HPV types checking combined with TCT and biopsy can raise positive rate significantly. It should be used as a reliable method for early diagnosis in cervical cancer and CIN screening.
