1.Multi-factor analysis of influencing factors of the self-care efficacy for PHC patients by transcatheter arterial chemoembolization of different age groups

Objective The objective of this study was to explore the difference factors for PHC patients by transcatheter arterial chemoembolization of different age groups of senior people in China and support reference for individualized intervention by clinical medical workers.Methods 153 cases of PHC patients by TACE were selected by convenience sampling method from October 2013 to June 2014.The data were collected using general data questionnaire,Health Promotion Strategy Scale,QOL-LC,Memorial Symptom Assessment Scales and Social Support Scale to evaluate the physical,psychological and social aspects of the PHC patients.We divided the patients into young,middle-aged and elderly groups.Single factor and multiple factors regression analysis were used to compare the status and influencing factors.Results Psychological function,value of blood AFP,marital status entered the model and were self-care efficacy and correlated factors of young-aged patients,which could explain 50.6% of the total varience.Physical symptoms,exploitation degree of support and intervention treatment times entered the model and were self-care efficacy and correlated factors of middle-aged patients,which could explain 47.8% of the total varience.Symptom distress,degree of cuhural and psychological function entered the model and were self-care efficacy and correlated factors of elderly-aged patients,which could explain 42.4% of the total varience.Conclusions The self-care efficacy were low in all ages of PHC patients by TACE,it is associated with a variety of factors.Doctors and nurses should pay more attention to the self-care efficacy for PHC patients by transcatheter arterial chemoembolization with different age groups.Also we should evaluate and make well-targeted policies and personalized inter-ventions to promote self-management self-efficacy.
2.Outbreaks of acute schistosomiasis in Anhui province in 2003.

OBJECTIVETo analyze the outbreak patterns of acute schistosomiasis in Anhui province in 2003.
METHODSThe criteria of emergency event for schistosomiasis issued by the Ministry of Health were applled in this study. Epidemiological data of acute schistosomiasis outbreak in Anhui province in 2003 were collected, checked and analyzed.
RESULTSIn 2003, there were 6 episodes of acute schistosomiasis outbreaks in Anhui which had met the criteria of being emergency events, issued by the Ministry of Health. The preplanning was initiated in 5 outbreaks to respond to the outbreaks of acute schistosomiasis. All of the acute schistosomiasis cases received timing treatment, except those misdiagnosed cases coming from the non-endemic areas. There were no deaths or follow-up cases during the outbreaks. Health education, preventive treatment for people living in high risk villages were conducted in order to timely control the epidemics of schistosomiasis.
CONCLUSIONThe implementation of preplanning has played an important role in early cases-finding, timing report and response to the outbreaks of acute schistosomiasis.

3.Gastrointestinal stromal tumor with synchronous isolated parenchymal splenic metastasis of ovarian cancer.

Gastrointestinal stromal tumor (GIST) represents the most common intramural mesenchymal tumor of the gastrointestinal tract, but the synchronous occurrence of GIST in the stomach and gynecological cancer is rare. We present a unique case of a 56-year-old female patient who was diagnosed with the synchronous development of GIST and an isolated parenchymal splenic metastasis of ovarian cancer. She underwent a wide local excision of gastric lesions with splenectomy. A morphological (histological and immunohistochemical) study established a spindle-cell type of gastrointestinal tumor that expressed CD117, and a parenchymal recurrence of ovarian papillary serous adenocarcinoma. The patient has remained alive and disease-free for 30 months since the last operation. A small GIST concomitant with an isolated parenchymal splenic metastasis of ovarian cancer is rarely encountered. The coexistence of GIST with other malignancies constitutes an intriguing oncologic model. Surgeons are advised to be alert against possible primary GIST accompanying other neoplasms.

4.Analysis of surveillance results of drinking water type of endemic fluorosis in Qinghai province in 2009

ObjectiveTo investigate the development trend of drinking water type of endemic fluorosis in Qinghai province, and to provide the basis for the prevention and treatment of the disease. MethodsIn 2009, six monitoring counties were chosen by using simple random sampling methods, all diseased villages of the six monitoring counties were classified into light, moderate and severe disease types according to water fluorine content on the historical data, and 1 village was respectively chosen from each type. In monitoring villages with improved water, 3 tap water and one source water samples were collected, respectively. Five water samples were collected randomly in water unimproved monitoring villages according to water well locations of east, west, south, north and center. The fluorine content in water and urine was determined according to the Standard Testing Methods for Drinking Water (GB/T 5750-2006). Children aged 8 to 12 were examined for dental fluorosis by Dean method.Clinical osteofluorosis of all the resident over the age of 16 was examined, 2 village of these counties were randomly selected, and clinically diagnosed patients with skeletal fluorosis were examined again by X-ray using Diagnostic Criteria of Endemic Skeletal Fluorosis (WS 192-2008). Urine sample of 30 children aged 8 to 12 and of 20 adults over the age of 16 were randomly collected and urinary fluoride was determined by F-ion selective electrode method (WS/T 89-2006). ResultsImproving water projects had been implemented in 14 monitoring villages of the 18 villages in 6 counties, the rate of improved-water was 77.78%(14/18). Among the 14 projects, 5 improved-water projects ran normally, and 9 projects ran with intermittently water supply. Seventy-five water samples were tested, themean of water fluoride was 0.48 mg/L. The prevalence of dental fluorosis was 31.95% (285/892), that of clinical skeletal fluorosis was 36.55%(1570/4295) and the X-ray detection rate of skeletal fluorosis was 25.64% (20/78).Five hundred and seventy-one urine samples of children were determined, and geometric mean of urinary fluorine was 1.04 mg/L; 370 adult urine samples were determined, and geometric mean of urinary fluorine was 1.52 mg/L Conclusion Epidemic of drinking water type of endemic fluorosis is still serious in Qinghai province, and drinking water defluoride measures should be further strengthened and improved.
5.Endemic fluorosis in Huangyuan county Qinghai province in 2009: an analysis of surveillance results

Objective To investigate the prevalence change of drinking water type of endemic fluorosis and the effect of control measures implemented in Huangyuan county of Qinghai province. Methods In 2009, all the endemic fluorosis villages in Huangyuan county were divided into two degrees, light and medium, according to the water fluorosis content before implementing the improving water project, 1 to 2 villages were selected from each degree village, respectively,as monitoring sites, and a total of 3 villages were selected. Source water and tap water samples were collected from each village and water fluoride concentration was determined. Dental fluorosis of all children aged 8 to 12 of monitoring villages was examined, and urine samples were collected by age group of children for determination of urinary fluoride. Clinical skeletal fluorosis of adults over 16 years of age was examined, and 20 copies of adults urine samples were collected to determine urinary fluoride. One village was selected in the 3 villages monitored to conduct X-rays examination of skeletal fluorosis. Water fluoride was tested in accordance with the "Non-metallic Targets Test Methods for Drinking Water" (GB/T 5750.6-2006); urinary fluoride was tested by fluoride ion-selective electrode method (WS/T 89-1996); dental fluorosis was diagnosed using Dean method;adult skeletal fluorosis was diagnosed by "Clinical Diagnostic Criteria for Endemic Skeletal Fluorosis"(WS 192-2008). Results Twelve water samples were assayed, water fluoride was (0.35 ± 0.43) mg/L. The detectable rate of dental fluorosis of 122 children aged 8-12 was 34.43%(42/122) and the geometric mean urinary fluoride was 0.89 mg/L of the 96 children. Of the 834 adults aged 16 and over, clinical detection of skeletal fluorosis was 47.72% (398/836) and geometric mean urinary fluoride was 1.10 mg/L of the 65 cases of adult urine samples assayed, detection rate of X-rays was 31.4% (11/35) in Gangou village of the 35 adults examined.Conclusions In Huangyuan county, water fluoride of the 3 surveyed villages are normal but the endemic fluorosis is still serious. It should strengthen monitoring and analyze the causes and improve prevention measures.
6.Shenfu Injection suppresses inflammation by targeting haptoglobin and pentraxin 3 in rats with chronic ischemic heart failure.

OBJECTIVETo investigate the regulatory effects of Shenfu Injection (SFI, ) on hemodynamic parameters and serum proteins in rats with post-infarction chronic heart failure (CHF).
METHODSForty-five healthy Wistar rats were randomized into three groups: sham, heart failure (model) and SFI group. The CHF was induced by left coronary artery ligation. Seven days after the surgical operation, animals in the sham group and the model group received saline (6.2 mL/kg/d), while animals in the SFI group received SFI (6.2 mL/kg d) intraperitoneally. Four weeks later, cardiac hemodynamic parameters were measured via the carotid route. The expression of serum proteins was analyzed by two-dimensional electrophoresis and matrix-assisted laser desorption/ionization time-of-flight mass spectrometer (MALDI-TOF MS).
RESULTSRecording of hemodynamic parameters showed that left ventricular systolic pressure (LVSP), maximum rate of left ventricular pressure (+dp/dtmax) rise, and maximum rate of left ventricular pressure (-dp/dtmax) decrease, while the left ventricular end diastolic pressure (LVEDP) rose in the model group compared to those in the sham group (P <0.05). The results of the MALDI-TOF MS indicated that haptoglobin (HP), pentraxin 3 (PTX3) and alpha-1-antitrypsin were up-regulated, while serum albumin and 40S ribosomal protein were down-regulated in the model group (P <0.05). Compared with the model group, LVSP, +dp/dtmax and -dp/dtmax were higher, while LVEDP was lower in the SFI group (P<0.05). Expression levels of HP and PTX3 were lower than in the model group (P<0.05).
CONCLUSIONSFI could improve hemodynamic function and decrease inflammatory reactions in the pathophysiology of CHF. The serum proteins HP and PTX3 could be potential biomarkers for chronic ischemic heart failure, and they could also be the serum protein targets of SFI.

7.Association between depression and G72 gene polymorphism.

OBJECTIVETo investigate the association between G72 gene polymorphisms and depression,and to probe the difference of G72 gene polymorphisms between depression with and without mixed family history.
METHODSThe polymorphisms of G72 gene (rs947267 and rs2181953) were detected by PCR technique in 100 depressive patients without mixed family history, 50 depressive patients with mixed family history and 86 normal controls.
RESULTS(1) The frequencies of rs947267 genotypes and alleles in female depressive patients without mixed family history were significant different to the controls (P=0.017 and P=0.008), the OR scores were 0.300 (A/A, P=0.010), 0.456(A, P=0.008) and 2.195(C, P=0.008) respectively; but in male patients there were no significant differences to the controls (P>0.05). (2) The frequencies of rs2181953 genotypes and alleles in the depressive patients without mixed family history were not significantly different to the controls regardless of sex (P>0.05). (3) The frequencies of rs947267 and rs2181953 genotypes and alleles in the depressive patients with mixed family history were not significantly different to the controls regardless of sex (P>0.05).
CONCLUSIONThe G72 gene polymorphism may be associated with female depressive patients without mixed family history,C allele of rs947267 may be the risk factor.

8.Infectious status of infection sources in the epidemic regions of Schistosomiasis japonica in China.

OBJECTIVETo understand the infectious status of infection sources in some epidemic regions of Schistosomiasis japonica, and provide a scientific basis for further controlling infection sources in a comprehensive way.
METHODSLongshang village which lies in the area of hills and mountains and Yuye village which lies in the area of lakes and marshlands were chosen for field investigation. The study was targeted at snails and 1512 residents, while 197 samples of livestock were randomized (80 cattle, 46 pigs, 45 dogs, 18 cats, 8 sheep) and 32 wild animals (field rats) were screened in Anhui province between October to November in 2007. The infection rate and intensity of infection were calculated after pathogenic examination on the populations, livestock and wild animals (field rats).
RESULTS(1) The infection rate of snails in Longshang and Yuye village were 2.26%, 1.06% in 2007, and 0.55%, 0.72% in 2006 respectively. (2) Of the infection rate and geometric mean of egg per gram (EPG) of population, 3.8% and 0.14 (EPG) were in Longshang village and 3.4% and 0.13 (EPG) were in Yuye village. The positive rate of blood examination of the different sex in Longshang village was 17.5% (44/252) of the males, higher than that of the females 11.0% (25/227) (chi(2) = 4.026, P = 0.045), whereas, in Yuye village was 21.4% (66/309) of males and 19.4% (25/129) for the females, without significant differences (chi(2) = 0.217, P > 0.05). The positive rate of fecal examination of the different sex in Longshang village was 5.2% (14/268) of the men and 2.1% (5/236) of the women showing no statistical significance (chi(2) = 3.336, P > 0.05); whereas, in Yuye village was 5.7% (14/245) of the men and 1.2% (3/250) of the women (chi(2) = 7.603, P = 0.006). (3) The infection rate and the arithmetic mean of EPG of the cattle, 10.8% (8/74) and 135.00 (EPG) were in Longshang village, meanwhile, the infection rate of the bull was 9.1% (6/66) and 25.0% (2/8) for cow without statistical significance (chi(2) = 0.586, P = 0.444), whereas, the total number of cattle was 6 that had been examined and there was no positive case in Yuye village. Of 8 sheep examined in Yuye village, 6 was positive, with arithmetic mean of 254.82 (EPG), while there was no sheep in Longshang village. (4) Of the positive rate of the incubation for dog's feces and intensity of infection, 55.6% (24/36) and 20.00 (EPG) were in Yuye village whereas 23.81% (5/21) and 1.21 (EPG) were in Longshang village. (5) Of the infection rate of the field rats, 13.64% (3/22) in Longshang village whereas a total number of mouse was 10 that had been examined and there was no positive case in Yuye village.
CONCLUSIONThe farm cattle should be still the chief sources of infection in the regions of lakes and marshlands for schistosomiasis. Whereas, with the implementing in-depth of the strategy on controlling source of infection in a integrated way, high priorities should be given to the epidemiological factors of the animals such as sheep, dogs, field mouse and so on which are spreading schistosomiasis.

9.Safety of ticagrelor and clopidogrel for acute coronary syndromes: A systemic analysis

Objective To systemically evaluate the different on the incidence of cardiovascular events of platelet aggregation inhibitors ticagrelor and clopidogrel for acute coronary syndrome (ACS),so provide cardiovascular event reference for the selection ofACS platelet inhibitors.Methods Articles were collected according to the inclusion criteria from the database CNKI,Chongqing VIP,Taylor & Francis Open Access Journals,Wanfang,Cochrane Library,SinoMed,EMbase and PubMed from Jan.2000 to May 2017.Review Manager 5.3 was used for data analysis to get the odds ratio (OR) as final effect value.Publication bias of the literatures and the sensitivity of the study were also analyzed with the software.Results A total of 12 articles involving 86 849 patients were included,i.e.,8 random controlled trials,2 case control studies and 2 cohort studies.Quality assessment with Cochrane handbook for systematic reviews shows that,most studies gave low risks in 7 bias aspects.Jadad score assessment was employed in 8 random controlled trials,with 4 studies getting 3 points,3 getting 4 points and 1 getting 5 points,implying the significant quality of the included studies.Meta-analysis showed that compared with clopidogrel,significantly lower cardiovascular mortality (OR=0.80,95%CI:0.72-0.89,P<0.01) and incidence of myocardial infarction (OR=0.78,95%CI:0.61-0.99,P<0.05)were with ticagrelor.Conclusion Compared to clopidogrel,ticagrelor may lead to lower cardiovascular mortality and incidence of myocardial infarction in treatment of ACS.
10.Endothelial function evaluation in salt-sensitive normotensive and mild hypertensive subjects and effects of potassium supplement.

OBJECTIVESalt-sensitivity plays an important role in essential hypertension and is associated with more severe target organ injury and higher mortality in patients with essential hypertension. However, the pathologic mechanism of salt-sensitivity is poorly understood and endothelial dysfunction might be involved in salt-sensitive hypertension. We, therefore, observed the endothelial function changes by measuring plasma and urine nitric oxide (NO) concentrations in salt-sensitive (SS) normotensive and mild hypertensive subjects underwent various salt loading protocols and the effects of potassium supplement.
METHODSThirty-nine normotensive and mild hypertensive subjects (< 160/100 mm Hg), aged 16-60, were enrolled and the study protocol is as follows: 3 days baseline investigation, 1 week low-salt loading (3 g/day), 1 week. high-salt loading (18 g/day) and 1 week high-salt loading plus potassium chloride (4.5 g/day).
RESULTSPlasma and urine NO levels were significantly lower in SS (n = 8) subjects at baseline, low-salt and high-salt loading phases compared with salt-resistant subjects (SR, n = 31) and oral potassium supplement to SS subjects with high salt loading significantly increased plasma and urine NO levels.
CONCLUSIONEndothelial function is impaired in normotensive and mild hypertensive SS subjects. Oral potassium supplement could improve endothelial function in normotensive and mild hypertensive SS subjects.
