1.Clinical analysis of aspirin and ursodeoxycholic acid on treating intrahepatic cholestasis of pregnancy

Objective To investigate the clinical effect of aspirin and ursodeoxycholic acid on treating intrahepatic cholestasis of pregnancy (ICP).Methods Seventy-one ICP patients were divided by random digits table method into observation group (36 cases) and contol group (35 cases).Observation group was treated with aspirin and ursodeoxycholic acid.Control group was given unsodeoxycholic acid.The biochemical indicators,itching score and pregnancy outcomes of two groups were observed.Results Total bile acid (TBA),total bilirubin (TBIL),alanine aminotransferase(ALT),aspartate aminotransferase(AST)and itching score of two groups after treatment were all significantly lower than those before treatment (P <0.05 ).TBA,TBIL,ALT and itching score of observation group[ (31.7 ± 8.3 ) μ mol/L,(33.9 ± 5.8 ) μ mol/L,(53.1 ± 8.4) U/L,(0.48 ± 0.06) scores] were obviously lower than those of control group [(76.2 ±9.2)μ mol/L,(56.9 ± 8.2) μmol/L,(80.8 ± 10.6) U/L,(0.81 ±0.04) scores] (P <0.05).The incidences of amniotic fluid contamination,premature delivery and neonatal asphyxia of observation group were significantly lower than those of control group [38.9%(14/36) vs.71.4%(25/35),11.1%(4/36) vs.31.4%( 11/35 ),5.6% ( 2/36 ) vs.22.9% ( 8/35 ),P < 0.05 ].Conclusions The combined application of aspirin and ursodeoxycholic acid can improve the biochemical indicators,itching symptom and pregnancy outcomes.
2.Study on Reduning in the treatment of epidemic hemorrhagic fever

ObjectiveTo observe the effect and adverse reaction of Reduning injection in the treatment of epidemic hemorrhagic fever.Methods98 patients with epidemic hemorrhagic fever were randonly divided into 2 groups,the treatment group was given Reduning injection ;The control group was given ribavirin injection;The efficacy and prognosis and adverse reaction were compared between two groups.ResultsThe antipyretic effect of the treatment group was better than that of the control group ( x2 =6.41,P < 0.05 ),and the fever clearance time,oliguria time,the total duration was significantly shortened( t =2.356,2.071,5.125,all P < 0.05 ).There were no obvious adverse reaction in the two groups.ConclusionReduning injection in the treatment of epidemic hemorrhagic fever had good curative effect,safety,and it was worthy of popularization and application.
3.DIAGNOSIS AND THERAPY OF THE PANCREATIC TRAUMA CAUSED BY BLUNT ABDOMINAL LESIONS

Of the 250 cases of blunt abdominal trauma,15 cases of pancreatic lesions were complicatedwhich include 3 cases lacerated on the head of pan-creas, 5 on the body, 6 on the tail and one con-tused. The pancreatic bed drainage was performedfor one case, repair for 8 cases and pancreatic tailresection for 6 cases, In author's experiences, theroutinely inspecting the pancreas was carried outintra operatively, with blunt epigastric trauma, es-pecially the light red hemotoliquid was found bypericentesis. Operative method was classified frompancreatic bed drainage, repair and resection in ac-cordance with severity of the trauma, For prevent-ing the pancreatic juice fistula, the main pancreaticduct must try its best to be ligated. If the traumais on the head, a common bile duct and duodenumdecompression must be done.
4.Effects of dexmedetomidine on short-latency somatosensory evoked potentials

Objective To investigate the effects of two different dosages of dexmedetomidine on short-latency somatosensory evoked potentials (SLSEP)in intracranial surgery.Methods Forty pa-tients,ASA Ⅰor Ⅱ,aged 20-65 years old,selected for intracranial surgery were randomly divided in-to two groups,20 patients in each group.Bolus dose of 0.5 μg/kg dexmedetomidine was infused within first 10 min and followed by continuous infusion of 0.5 μg·kg-1·h-1 for 10 min in group A;Bolus dose of 1.0 μg/kg dexmedetomidine was infused within first 10 mins and followed by continu-ous infusion of 1.0 μg·kg-1·h-1 for 10 min in group B.SLSEP indications include N20-P25 amplitute and N20 latent period were observed.MAP,HR,N20-P25 amplitute and N20 latent period were re-corded respectively before dexmedetomidine adminstraition (T0 )and 20 mins after dexmedetomidine adminstraition (T1 ).Results Compared with T0 ,MAP and HR at T1 significantly decreased in both groups(P <0.05).N20-P25 amplitude had no statistically significant difference in both groups,while N20 latent period significantly prolonged (P <0.05).Conclusion Both doses of 0.5 μg/kg and 1.0μg/kg dexmedetomidine can significantly prolong the latent period of N20.

Objective:To study the cytotoxicity to tumor cells P815,H22 and B16 in vitro and the treatment effect of mice spleenocytes activated by lymphokine IL-2 and Tremella Polysaccharide,and their effects on main viscerals.Methods:The cytotoxicity of spleenocytes activated by lymphokine IL-2 or IL-2 and Tremella Polysaccharide to P815,H22 and B16 was detected by ?-scintillation counter after 2 hours.Subcutaneous injection of the above-mentioned spleenocytes to the local area of jepatocarcinoma of mice,one injection every two days,5 times totally.To examine tumor weight,size,histological changes and morphological changes in main viscerals.Results:The significantly cytotoxicity to tumor cells of spleenocytes activated by IL-2 and Tremella Polysaccharide was observed.To B16,the cytotoxicity of activated spleenocytes was the strongest.To H22,the cytotoxicity of activated spleenocytes was stronger and to P815,the cytotoxicity was the weakest,the cytotoxicity of TP-LAK cells was significantly stronger than that of LAK cells( P
6.Lateral ectopic thyroid:A case report and literature review

Objective:To discuss the clinical characteristics,diagnosis and treatment experience of a case of lateral ectopic thyroid.Methods:The patient with lateral ectopic thyroid was diagnosed by thyroid color ultrasound and single-photon emission computed tomography(SPECT)-CT examination.The right thyroidectomy and the thyroid tissue in cervical section Ⅳ of the patient were removed by received the operation.The pathological examination after operation was performed.The daily usage of euthyrox after operation was 50 μg.Results:The patient recovered well,who accepted 10 months follow-up with normal life and stable condition,and had no recurrence and metastasis.Conclusion:The mechanism of lateral ectopic thyroidy is not clear yet,the surgical treatment can result in good effectiveness and better prognosis.
7.Development of Multiplex Real-time PCR for Detection of Toxigenic Vibrio cholerae and Virbio parahaemolyticus

A multiplex real-time PCR was developed to detect ctxA of Vibrio cholerae, gyrB and tdh of Vibrio parahaemolyticus simultaneously. The multiplex real-time PCR were evalidated by detection for the three genes in 47 toxigenic V. cholerae O1 and O139 strains (ctxA+; O1=3, O139=44), 25 non-toxigenic V. cholerae strains (ctxA-; O1=12, O139=6, non-O1 and non-O139=7), 116 V. parahaemolyticus strains with or without tdh (73 or 43) and 9 other bacteria strains. The specificity and sensitivity of the multiplex real-time PCR in detection for the ctxA and the tdh genes in the strains tested were both 100.0%, compared to the results by routine PCRs. In the detection for V. parahaemolyticus specific gyrB using the multiplex real-time PCR, all of 116 V. parahaemolyticus strains were positive, and 9 other strains and 72 V. cholerae strains were all negative. The multiplex real-time PCR is a sensitive, specific and quick assay not only for detecting virulence genes of V. cholerae and V. parahaemolyticus but also for identifying V. parahaemolyticus at species level. In addition, two real-time PCRs for detection of V. parahaemolyticus virulence genes trh1 and trh2 were also developed.
8.Selection of surgical approach in management of cervical cord injury following ossification of the posterior longitudinal ligament

Objective To investigate the appropriate surgical approach in the management of cervical cord injury following ossification of the posterior longitudinal ligament. Methods The clinical data of 25 patients with cervical cord injury following ossification of the posterior longitudinal ligament who received surgical treatment were retrospectively analyzed. According to Frankel grades, two patients were at grade A, three at grade B, 14 at grade C and six at grade D. The surgical procedures consisted of anterior decompression (12 patients), posterior decompression (8 patients) and combined posteroanterior decompression (5 patients). Results No iatrogenic injury of great vessels, trachea, esophagus or spinal cord occurred. All the patients were followed up for 15-86 months (mean 38.3 months). All segments with anterior fixation attained solid fusion, without implants loosening or breakage. No reelosed open-door was found in patients who received posterior laminoplasty. The spinal function got improved in 21 patients, and a relief of pain or numb of the upper limb was attained in four patients whose spinal cord injury was not cured. Conclusions The surgical outcome of cervical cord injury following ossification of the posterior longitudinal ligament is satisfactory. It is important to select a suitable surgical approach according to the imaging manifestations associated with the general conditions of the patients.
9.The clinical significance of rehabilitative treatment for severe cervical spondylotic myelopathy after surgery

Objective To evaluate the effectiveness of rehabilitative treatment for severe cervical spondylotic myeiopathy after combined posterior-anterior surgery. Methods Fifty-four patients (mean age, 59.5) who had undergone combined posterlor-anterior surgery of the cervical spine due to severe cervical spondylotic myelopathy were studied with an average follow-up of 38.7 months. All the patients were allocated into either a rehabilitation treatment group (28 patients) or a control group (26 patients). Neurological function in the two groups was assessed using the Japanese Orthopedic Association (JOA) grading system. In addition, the status of axial symptoms, wound healing and post-operative complications were also evaluated. Results The bone grafts completely fused in both groups. All of the wounds in the rehabilitation treatment group healed without any infection. Three wounds were infected in the control group, but were cured by prompt treatment. Two patients in the rehabilitation group and 9 in the control group suffered laryngeal edema. There was no significant difference between the two groups before the operation with regard to their JOA scores, which significantly increased postoperation in both groups. In the rehabilitation treatment group, the postoperation JOA scores were significantly higher than in the control group. At the same time, there were significantly fewer patients with neck axial symptoms in the rehabilitation treatment group than that in the control group (25.0% versus 69.2% , P < 0.01 ). Conclusions Systematic rehabilitation treatment pro-and post-operation of patients with severe cervical spondylotie myelopathy can accelerate neurological recovery and help prevent postoperative complications and neck axial symptoms.
