1.Efficacy assessment of early enteral nutrition intervention in patients with alimentary tract carcinoma after surgery

Objective To study the difference in occurrence of complications and health economics between early enteral nutrition (EN) and total parenteral nutrition (TPN) in patients with alimentary tract carcinoma after surgical operation.Methods Totally,164 patients with alimentary tract carcinoma hospitalized at the Department of General Surgery in China-Japan Friendship Itospital,Beijing during September 2003 to December 2005 with nutritional intervention,84 cases with early EN and 80 with TPN within 24 to 48 hours after surgical operation,were reviewed.Nutrition index,complications,recovery of gastrointestinal movement,length of hospital stay after operation and total cost of the patients were analyzed. 2.Role of Retinoic Acid Receptor Antagonist on Immunoglobulin M Synthesis by Cord Blood Lymphocytes

Objective Retinoic acid receptor(RAR?) selective antagonist,Ro41-5253(Ro) was used in the counteract test,to confirm the role of RAR? in enhancement of IgM synthesis in cord blood lymphocytes(CBLs) by retinoic acid(RA).Methods CBLs were cultured in vitro and stimulated with or without RAR? agonist RA and(or) antagonist Ro.The cells were harvested at 24 hours and 48 hours of culture time to measure the levels of gene expression of RARa,IL-4 and IL-6.ELISA was used to measure the concentration of IgM in the supernatant of 5 days culture.Results Ro could inhibit the promotion of RA on IgM synthesis in CBLs.RA could up-(regulate) RAR? gene expression,which could be restrained by Ro.Ro also could counteract the up-regulation of RA on IL-4 and IL-6 gene expression.Conclusion The effect of RA on IgM synthesis in CBLs is modulated by RAR?.
4.Three-dimensional diagnostics and surgical correction of late deformities of orbital blowout fractures

Objective To investigate the mechanisms of enophthalmos and deformities of orbital walls in patients with orbital blowout fracture and to evaluate the reconstructive surgical methods. Methods The features of orbital deformities, the change of orbital volume and the mechanisms of enophthalmos were measured and analyzed by means of 3-D CT technique and 3-D diagnostic computer-assisted system. The reconstructive operations were employed on these 11 cases with orbital blowout fractures to correct deformities and restore orbital volume by applying autogenous bone grafts or hydroxyapatite bone substitutes. Results There were no serious complications such as infection of bone graft, exposure of hydroxypatite implant in all 11 cases, 10 cases got satisfactory appearance of eyelids and orbit, of which thediplopia and enophthalmos were corrected with protrution of eyeball restoring normally. 1 case remained diplopia after surgical correction. Conclusions The enophthalmos is mainly caused by orbital volume enlarging and orbital content herniating into paranasal sinuses. Implanting autogenous grafts and hydroxyapatite substitutes to restore orbital volume are proved to be effective.
5.The distribution of tislevator ani muscle and motor endplate distribute in embryonic female

Objective To investigate the distribution characteristics of motor endplates in musculuslevatorani of dif-ferent gestational ages. Methods 20~24 weeks female fetuses were categorized as Group A, and 24~28 weeks female fetuses as group B. Karnovsky-Roots iron cyanide copper staining ( hematoxylin) was used to study the aver-age weight, the morphology, distribution and changes in number of motor end plate. Results The motor endplates morphology and distribution of consistent oval of 20 ~28 weeks female fetuses showed a curve distribution in the musculuslevatorani fibers foreign 1/3 . The weight and numbers of motor endplates in Group A were less than those of group B. Conclusion 20 ~28 weeks female fetuses' musculuslevatorani are visible on the motor end plates, and no significant difference is found in morphology and distribution of the curve along the muscle fibers. The num-ber of motor endplates increases with the increase of anal muscle weight mentioned.
6.Clinical effect of loop electrosurgical excision procedure (LEEP) in diagnosis and treatment of cervical diseases

Objective To study the clinical effect of cervical loop electrosurgical excision procedure (LEEP) in diagnosis and treatment of cervical diseases (including cervical intraepithelial neoplasia of different grade, cervical erosion of moderate and severe degree, cervical with human papillomavims (HPV) infection and cervi-cal polyp) . Method The data of 136 cases with cervical disease treated by LEEP were analyzed retrospectively. Re-sult The postoperative diagnosis were upgraded in 18 cases (13.2%) ,and downgraded in 38cases (27.9 %) . The preoperative diagnosis of 80 cases (58.8 %) were in accordance with the postoperative ones. The difference was sig-nificant(P<0.05). The mean operative time was 11minutes, the mean bleeding volume was 15 ml. Postoperative bleeding of LEEP occurred in 2.2 % (3/136) patients. Postoperative cervical conglutination of LEEP occurred in 1. 5 % (2/136) patients. The cure rate of CIN was 96 % after LEEP. The residual CIN occurred in 4 patient (4 %). Recurrence CIN occurred in 2 patient (2 %). At 12th months after LEEP,the negative rate of HPV detection was 86. 7% and the persistent positive rate was 13.3%. The HPV clearance rate was increased gradually after LEEP. Conclu-sion LEEP was safe and effective for the treatment of the cervical disease and was a kind of good method to clear HPV infection in cervical.

8.New Research of Nerve Growth Factor on Fracture Healing.

Fracture healing has long been a major concern for orthopedists.Currently,about 10% of fracture patients still have poor fracture healing or bone nonunion.In recent years,research has found that nerve growth factor(NGF)can promote fracture healing.This article reviews the mechanism and research progress of NGF in promoting fracture healing.NGF can promote vascular regeneration and nerve growth at callus and plays an important role in the proliferation and migration of bone cells.New animal experiments and clinical trials have confirmed the role of NGF in promoting fracture healing and further revealed its possible mechanism of action.Further research on NGF can provide new ideas for promoting fracture healing,especially for treating nonunion.

9.Clinical analysis of chronic hepatitis B virus carriers and chronic hepatitis B diagnosed through pathology

Objective To compare the clinical data of patients with chronic hepatitis B virus (HBV) carriers and chronic hepatitis B so that to provide pathological evidence for management of chronic HBV carriers with different alanine aminotransferase (ALT) levels.MethodsLiver biopsies were performed in totally of 292 cases of chronic HBV infection.The subjects were divided into HBV carrier group (G0-G1 and S0-S1) and hepatitis group (G> 1 and/or S> 1) according to the pathological diagnosis. The relationships between different age subgroups, different ALT level subgroups and pathological diagnosis were analyzed. Meanwhile,other clinical,biochemical,and iconographic indexes which were possibly related to the pathology diagnosis were compared. The multivariate analysis was done by Logistic regression equation (withdrawal method, maximum likelihood method) to definite the independent influencing factors of pathologically diagnosed with chronic HBV carrier.ResultsAmong the 292 patients,140 (47.9%) were pathologically diagnosed with chronic HBV carries and 152 (52.1%)were chronic hepatitis B. There were statistical differences between ≤35 years group and 36-40 years,>40 years group (x2 =3.936 and 8.534,respectively; P =0.047 and 0.003,respectively). There were statistical difference among patients with ALT<0.5×upper limit normal (ULN),(0.5-1.0) ×ULN,(1.1-1.5) ×ULN,(1.6-2.0) ×ULN and >2.0 × ULN (x2 =55.314,P<0.01),while there was no significant difference between (1.1-1.5) × ULN and >2.0 × ULN (x2 =3.810,P=0.051). Multivariate analysis indicated that course of disease,alcohol consumption,ALT,HBV DNA level and the surface of liver (smooth or not smooth)detected by ultrasonography were independent influencing factors of pathological diagnosis of chronic HBV carriers (OR =0.995,0.224,0.516,1.308 and 0.270,respectively; P=0.005,0.007,0.000,0.025 and 0.001,respectively).ConclusionLiver biopsy is much meaningful in patients with age >35 years old and ALT (1-2)× ULN.
10.Distribution of Cd,P, S, K, Ca,Cu and Zn Elements in Indian Mustard by LA-ICP-MS

Imaging of trace metal distribution in the cadmium ( Cd) hyperaccumulator Indian mustard by laser ablation inductively coupled plasma-mass spectrometry ( LA-ICP-MS ) was typically performed using spatial resolutions of 25 μm. Indian Mustard was submitted to 50 mol/L Cd for 14 days exposure and analysed using Nd:YAG laser (213 nm) . Intensities of 13 C, 34 S, 39 K, 44 Ca, 66 Zn, 111 Cd, 65 Cu and 31 P were measured by ICP-MS to study elemental distribution. Preferential Cd accumulation in vascular bundles was observed in stem tissue, whereas Cd was mainly localized to the mesophyll and vascular cells. The high relationship between Ca and Cd distribution indicated that the two elements had a very similar pathway. In vivo analytical method developed in this work was useful to study spatial element distribution across stem samples and had great potential for applications in other areas of plant pathology research.
