1.Evaluation of radiofrequency ablation in treatment of primary liver cancer and its risk factors

Objective To evaluate the radiofrequency ablation in treatment of primary and recurrent liver cancer and to explore the risk factors affecting prognosis.Methods 1 64 patients with primary and recurrent liver cancer underwent radiofrequency ablation treatment.The clinical data including liver function before and after treatment,alpha-fetoprotein (AFP),the rate of relapse and survivalwere retrospectively analyzed.The univariate Logistic regression analysis was used to analyzed the risk factors associated with the prognosis.Then the risk prognostic factors were analyzed using COX risk model.Results After treatment,AFP and liver function were significantly improved in patients (P<0.05);Complete tumor ablation rate was 87.50%.After cancer treatment 1,2,3,4 years,the recurrence rate was 17.9%,29.9%, 45.9% and 55.0% respectively.Conclusion Radiofrequency ablation is an effective treatment for primary liver cancer treatment,the risk factors affecting prognosis include tumor differentiation,tumor recurrence,TNM stage,AFP,intraoperative blood transfusion, surgical,cancer and surgical site before surgery transaminase levels.
2.Relationship between concentrations of antigen-specific IgG, nitric oxcide in CSF and intracranial hypertension and epilepsy in patients with neurocysticercosis

Objective To evaluate the relationship between concentrations of nitric oxide (NO) and antigen-specific immunoglobulin G (IgG) in CSF and intracranial hypertension and epilepsy in patients with neurocysticercosis.Methods The NO concentration and the IgG level were measured in 40 cases of neurocysticercosisk, and the results were compared according to the patients whether had intracranial pressure or epilepsy. 23 patients with varicosis of great saphenous vein required subarachnoid anesthesia were served as control group.Results Compared with control group, the NO concentration in CSF significantly increased in patient group (P0.05) in the patients with epilepsy as compared with the cases without epilepsy.Conclusions The CSF NO concentrations are higher in the patients with neurocysticercosis who had intracranial hypertension or epilepsy. The IgG levels are also increased in the patients who had intracranial hypertension.
3.The clinical features of myasthenia gravis associated with thyroid abnormalities

Objective To analyze the clinical characteristics of myasthenia gravis(MG) associated with thyroid abnormalities.Methods A total of 300 MG patients admitted to the department of neurology from July 2008 to September 2009 were reviewed retrospectively.Based on the thyroid function and abnormality of thyroid related antibodies (thyroglobulin antibody and peroxidase antibody), the patients with MG were divided into two groups (260 cases without thyroid disease and 40 cases with thyroid abnormalities).The different clinical features, the relationship between the antithyroid antibodies and antiacetylcholine receptor antibody (AChRAb) and the distribution of T cell subsets between the two groups were compared.Results ( 1 ) Among the 300 MG patients, 13.3% of them was accompanied with thyroid disease and the most common abnormality was positive thyroid antibody.(2) Between the groups of MG with and without thyroid abnormalities, no significant differences on the sex percentage, age of onset, duration of disease, clinical classification and thymic abnormalities were found ( P > 0.05 ).( 3 ) The blood levels of AChRAb in postsynaptic membrane in the thyroid antibody-positive patients were 1.15 ± 0.11, being much higher than those in the antibody-negative patients ( 1.01 ±0.11 ).(4) The percentage of CD8+ T cells was significantly decreased [(21.63 ±5.17)% vs (24.28 ±5.79)%] and the ratio of CD4+/CD8+ T cells was increased (2.10 ± 0.67 vs 1.81 ± 0.61, P < 0.05 ) in the group of MG with thyroid abnormality as compared with the group of MG with normal thyroid.Conclusion In MG patients the incidence positive thyroid related antibodies was much higher than that of other thyroid abnormalities.It is suggested that there are higher levels of AChRAb in MG coexisting with positive thyroid antibodies and more abnormalities of T lymphocyte subset distribution in MG patients with thyroid disease.

5.Clinical effect of loop electrosurgical excision procedure (LEEP) in diagnosis and treatment of cervical diseases

Objective To study the clinical effect of cervical loop electrosurgical excision procedure (LEEP) in diagnosis and treatment of cervical diseases (including cervical intraepithelial neoplasia of different grade, cervical erosion of moderate and severe degree, cervical with human papillomavims (HPV) infection and cervi-cal polyp) . Method The data of 136 cases with cervical disease treated by LEEP were analyzed retrospectively. Re-sult The postoperative diagnosis were upgraded in 18 cases (13.2%) ,and downgraded in 38cases (27.9 %) . The preoperative diagnosis of 80 cases (58.8 %) were in accordance with the postoperative ones. The difference was sig-nificant(P<0.05). The mean operative time was 11minutes, the mean bleeding volume was 15 ml. Postoperative bleeding of LEEP occurred in 2.2 % (3/136) patients. Postoperative cervical conglutination of LEEP occurred in 1. 5 % (2/136) patients. The cure rate of CIN was 96 % after LEEP. The residual CIN occurred in 4 patient (4 %). Recurrence CIN occurred in 2 patient (2 %). At 12th months after LEEP,the negative rate of HPV detection was 86. 7% and the persistent positive rate was 13.3%. The HPV clearance rate was increased gradually after LEEP. Conclu-sion LEEP was safe and effective for the treatment of the cervical disease and was a kind of good method to clear HPV infection in cervical.
6.The clinical randomized trials of celecoxib in combination with octreotide on the growth of human gastric cancer

Objective To investigate the inhibitory effect of cyclooxygenase-2 (COX-2) inhibitor celecoxib alone and in combination with somatostatin (SST) analog octreotide on human gastric cancer. Methods Fifty one gastric cancer patients were randomly divided into 3 groups. The patients in control group(n=15)took no medicine before gastric cancer resection. Celecoxib group(n=18)patients took celecoxib orally 0.2 g/d for 7 days before surgery. Combination group(n=18) patients took celecoxib orally 0.2 g/d and were injected octreotide 100 ?g/d subcutaneously for 7 days before surgery. The resected specimens were examined histologically, including status of tumor necrosis, inflammatory cells infiltration and fibrous tissue proliferation. The microvessel density (MVD) and the expression of COX-2 in gastric cancer were evaluated by immunohistochemical methods. The apoptosis of tumor cells was measured by terminal deoxynucleotidyl transferse-mediated-dUTP nick end labeling. The expression of somatostatin receptor (SSTR) -1,SSTR-2 and SSTR-3 mRNA in gastric cancer tissue obtained by biopsy in all patients were detected by using real time fluorescence quantitative RT-PCR. 7.Effects of serum from burned rats on the gene expression in rat bone mesenchymal stem cells

Objective To investigate the effects of serum from burned rats on the gene expression in cultured rat mesenchymal stem cells (MSCs). Methods Bone marrow was extracted from sacrificed Wistar rats, and MSCs were then incubated for 24h in F-12 medium in the presence of normal rat serum (group N) or serum obtained from burned rats 3 days after injury (group B). Total RNA was extracted from both groups.The mRNA was isolated.An Oligo microarray containing 5705 genes was used to compare the differences of gene expression between two groups. Results There were four genes which expressed differently in two groups.In comparison with group N, the expression of steroid sensitive gene 1 was decreased, but that of fibroblast growth factor-4, dihydroxyacetonephosphate acyltransferase and a EST, which was moderately similar to Bmp2-inducible kinase, were increased in group B. Conclusion Serum from burned rats was able to change the gene expression of MSCs, and it might play the key role in wound repair.
8.Ventilatory Treatment of Flail Chest with Pulmonary Contusion

Objective To evaluate the early therapeutic effect of flail chest with pulmonary contusion by using different mode of mechanical ventilation. Methods Twenty-nine patients of flail chest with pulmonary contusion were analysed retrospectively. All the patients were treated with the ventilator Bear1000. Two groups were established: invasion group was treated with SIMV+PEEP(8 cases) and CPAP+PSV (7 cases), noninvasion group was treated with NIPPV(14 cases). Results There was no death in all the patients. 9.Effect of occlusal contacts on stress distribution in the dentin and peridentium after post-core crown restoration

BACKGROUND:The post-core crown system is an effective method for dental defect repair, but its repair outcomes are influenced by various factors;thereby, a further study is needed to optimize this method.OBJECTIVE:To investigate the stress distribution in different degrees of dental defect after post-core crown restoration under different occlusal contacts. METHODS:Three-dimensional models of the mandibular first molar were established by CT images, and models of complete natural teeth, six different defected teeth (total maxil ofacial slight and severe defects, distal middle maxil ofacial slight and severe defects, proximal middle maxil ofacial slight and severe defects) and two different post-core crowns (fiber post and zirconia post) were created by software. These models were imported into ANSYS software to analyze the stress distribution in dentin and periodontium under stable occlusal contact (ABC) and unstable occlusal contact (AC), respectively. RESULTS AND CONCLUSION:The stress in dentin and the periodontium after post-core crown restoration under AC was obviously higher than that under ABC, and AC affected the restored teeth more markedly. The post-core crown material with higher elastic modulus could induce higher stresses, and severe defected teeth showed higher stress distribution. Different defected types made no overt effects on the stress distribution in the periodontium. These results suggest that the B area contact is critical in regulating occlusal contacts of restoration.
10.Evaluation of reliability and validity of Diabetes Diet-Related Quality-of-Life Scale

Objective To evaluate the reliability and validity of Diabetes Diet-Related Quality-of-Life Scale (DDR-QOL) in Chinese version,in order to provide an efficient instrument for assessing the quality of life of diabetes in diet therapy.Methods Translating and edit the DDR-QOL scale initially,275 eligible subjects were invited to the study.DDR-QOL scale and 36 item Short Form Health Survey (SF-36) were used to collect data which were conducted for evaluating the internal consistency,reproducibility,construct validity,convergent and discriminant validity of DDR-QOL.Results The total Cronbach's α was 0.86.The factor analysis suggested good construct validity of DDR-QOL.Significant correlations were found between DDR-QOL and SF-36.The quality of life of diabetes who is on the job or not can be identified by DDR-QOL.Conclusions The Chinese version of DDR-QOL is a valid and reliable tool to assess the quality of life of diabetes who are in diet therapy.
