Terapeutic options used to repair tendon and ligament injury consist of autografis,allografts and synthetic prostheses.Tendon and ligament of tissue engineering have their unique advantages as alternative therapy.Choosing the proper scaffold materials is important for successfully constructing tissue engineering tendon and ligament,so in this review we focus on natural and artificial scaffold materials used in tissue engineering of tendon and ligament in recent years.
2.Comparison of transesophageal arterial blood oxygen saturation monitoring and finger pulse oximeter

Objective To investigate the sensitivity and accuracy of monitoring oxygen saturation of arterial blood flowing through descending aorta with a probe placed in the lower segment of esophagus ( SeO2). Methods This study was approved by the ethics committee of our hospital and written consent was obtained. Forty-five ASA Ⅰ-Ⅲ patients (34 males, 11 females) aged 14-79, weighing 40-90 kg undergoing elective (36 patients) or emergency (9 patients) surgery were studied. After induction of anesthesia a probe we made (patent number ZL 200320115080.2) was placed in the lower segment of esophagus and connected to a monitor (Philips-150A) . The depth of insertion and the direction of the probe were adjusted until the same waveform as SpO2 was obtained. Once the best waveform was obtained, 10-15 min stabilization was allowed. The ventilator was then disconnected. When SeO, decreased from 100% to 90% mechanical ventilation was started again. The following parameters were recorded:(1) the onset time (the time from disconnection of ventilator to the time when SeO2 and SpO2 started to decrease), (2) the time needed for SeO2 and SpO2 to decrease from 100% to 95% (T95) and 90% (T90), and (3) the recovery time (the time needed for SeO2 and SpDO2 to return to 100% after mechanical ventilation was started again. Arterial blood samples were taken for blood gas analysis when Se02 and SpO2 were 90% , 95% and 100% . Results SeO2 , SpO, and SaO2 were 100% when the patients were being ventilated with 100% oxygen. When the ventilator was disconnected SeO, dropped much faster than SpO2. It took (286?15)s for SeO2 to drop from 100% to 95% while for SpO2 it took (398 ?28)s. The most readings of SeO2 were in the range of 2 standard deviation from SaO2. SeO2 correlated well with SaO2 and SpO2. r was 0.9754 and 0.9656 respectively(P
3.The anti-rejection effect of blocking BT/CD28 co-stimulatory pathway by RNA interference in mice heart transplantation

Objective To explore the influence of blocking B7/CD28 co-stimulatory pathway by RNA interference on the rejection response in mice heterogeneous heart transplantation and its mecha- nism.Methods siRNA of which sequence specified to CD80 and CD86mRNA was synthesized in vitro respectively and transfected into donor derived myeloid dendritic cells (DCs).The expression levels of CD80 and CD86 mRNA and surface antigen CD80,CD86 were assayed by semi-quantitative reverse transcription polymerase chain reaction and flow cytometry before and after CD80siRNA and CD86siRNA transfection.Seven days prior to heterogeneous heart transplantation in mice,DCs modi- fied by siRNA were transfused into recipients intraveneously (DC interference group).At the same time,group of allograft transplantation,cyclosporine A (CsA)-treated (CsA injected subcutaneously postoperatively,5 mg.kg~(-1)?d~(-1)) group,group of isograft transplantation,and non-interference DC group (transfusion of non-interfered DCs pre-transplanting) were assigned.The graft survivals were individually recorded and the graft rejection grading was pathologically evaluated.Interleukin 2 (IL- 2),interferon?(IFN-?),and IL-10 mRNA expression levels in grafts tissue were determined.Results After siRNA transfected into DC,the expression levels of CD80 and CD86 mRNA were down- regulated significantly and the the antigen CD80~+ and CD86~+ reduced from 84%,67% to 35% and 30% respectively.As compared with groups of allograft and non-interference DC,survival of the grafts was significantly longer in DC interference group (P<0.01),pathological grade of rejection significantly lower (P<0.01),IL-2 and IFN-?mRNA expression levels lower,and IL-10 mRNA ex- pression levels higher in grafts tissue (P<0.01).Conclusion Knocking down the molecule B7 expres- sion level in donor-derived myeloid DCs through RNAi,which could block B7/CD28 co-stimulatory pathway,could exhibit inhibitive effect on rejection response in mice heart transplantation.The mechanism might be due to induction of T lymphocyte anergy and Th cell differentiation deviating to T_H2.
5.Clinical study of azithromycin on patients with idiopathic pulmonary fibrosis

ObjectiveTo estimate the effect of azithromycin on lung function and transforming growth factor β1(TGF- β1) in patients with idiopathic pulmonary fibrosis (IPF). MethodsOne hundred and ten patients with IPF were divided into two groups by random digits table. Patients in azithromycin group(60cases) were treated by azithromycin tablets and thoee in placebo group ( 50 cases) were treated by placebo tablets, and they were observed for 12 weeks. Lung function was detected at beginning and after 6,12 weeks' treatment. TGF- β1 was tested by ELISA, meanwhile patient's condition was observed. ResultsAfter 6 and 12 weeks' treatment, the forced expiratory volume in 1 second (FEV1), forced vital capacity (FVC), carbonmonoxide diffusion capacity (DLCO) in the two groups had no significant differences compared with those at beginning of treatment (P > 0.05). After 12 weeks' treatment, the level of TGF- β1 in azithromycin group was significantly decreased compared with that at beginning and after 6 weeks' treatment (P < 0.05) ,but there was no significant difference in placebo group (P > 0.05). The ratio of acute exacerbation admission and infection was 8.3%(5/60) and 60.0%(3/5) in azithromycin group, 22.0%( 11/50) and 81.8%(9/11 ) in placebo group,and there were significant differences between the two groups(P <0.05 or <0.01). Conclusion Azithromycin may reduce the ratio of acute exacerbation admission and infection, decrease TGF- β 1 expression in patients with IPF by taking orally for a long time, but it may have no obvious effect on lung function.
6.Effect of tinnitus masking on auditory brainstem response wave amplitude in patients with tinnitus

BACKGROUND: Research shows that the auditory brainstem response (ABR) wave amplitude of tinnitus patients often act as big Ⅲ small Ⅴ.Tinnitus masking is a therapy of tinnitus, then how dose the ABR change in the process of tinnitus masking?OBJECTIVE: To observe the changing characteristics of ABR in patients with tinnitus as well as the changing rules of ABR after tinnitus masking.DESIGN: Case-control and self-control.SETTING: Department of Otorhinolaryngology, First Hospital of Jilin University.PARTICIPANTS: Continuous tinnitus group: 63 cases (68 ears) were selected from the Clinic of First Hospital of Jilin University from March 1999to October 2001. Control group: 10 cases (20 ears) of normal hearing youth (20-23 years old). All subjects received the examination and treatment voluntarily.METHODS: The ABR was firstly detected in tinnitus patients (foreheadmastoid conducting method was adopted), and examination of tinnitus as well as tinnitus masking test (masking for 5 minutes) were respectively conducted, immediately after that, the ABR was detected to do control-observation with the control group.MAIN OUTCOME MEASURES: ①The characteristics of ABR in patients with tinnitus. ②Effects of tinnitus masking on the amplitude wave of ABR, which was divided into positive group in residual inhibition test and negative group in residual inhibition test for analyzing.RESULTS: A total of 83 subjects were involved in the analysis of results.①The amplitude wave of ABR in youth of normal hearing were small Ⅲ big Ⅴ, whereas that in 22 ears (32.4%) out of 63 subjects (68 ears) were big Ⅲ smallⅤ, in which 17 ears were of patients positive residual inhibition test, and 5 were of negative.②After tinnitus masking therapy, 10 ears out of 17 ears of patients positive in residual inhibition test turned from big Ⅲ small Ⅴ to small Ⅲ big Ⅴ, and the wave amplitude of Ⅲ wasre duced in 3 ears, while the ABR in 2 ears out of 5 ears of negative patients turned to small Ⅲ bigⅤ, and there were no changes in other 3 ears.CONCLUSION: Tinnitus masking therapy can convert the abnormal ABR wave amplitude in patients with tinnitus to normal, which will be taken as the objective index in judging whether it is positive in residual inhibition test.
7.Preventive effects of sodium butyrate on gastric epithelial cells damaged by N-methyl-N’-nitro-N-nitrosoguanidine

Objective To investigate the effects of sodium butyrate (SB) on fetal gastric epithelial cells (GECs) in vitro damaged by N-methyl-N’-nitro-N-nitrosoguanidine (MNNG). Methods The fetal gastric epithelial cells were isolated, cultured and identified. Then the cells of the second generation were cocultured with 10 -5 mol/L, 10 -6 mol/L, 10 -7 mol/L SB for 4 h, then added with 10 -5 mol/L MNNG culturing for another 24 h. After the treated cells were finally cultured in normal culture medium for 40 d, the unscheduled DNA synthesis (UDS), lipid peroxidation (LPO) and ras p21 were detected. The cells were only cultured with 10 -5 mol/L MNNG served as negative control and without MNNG or SB as normal control. Results The level of UDS, the contents of lipid peroxidation products and the concentrations of ras p21 in GECs treated with 10 -5 mol/L and 10 -6 mol/L SB decreased obviously, and had significant differences with that of GECs treated without SB. Conclusion SB can effectively prevent the damage of GECs induced from MNNG.

10.Hypoplastic Vertebral Artery:Diagnosis of Spiral CT Angiography

Objective To evaluate the diagnosis of of hypoplastic vertebral artery by spiral CT angiography(SCTA).Methods 180patients with clinical diagnosis of vertebrobasilar insufficiency were undergone spiral CT angiography. For SCTA, 80~100 ml contrastmedium (omnipaque 350 mg/100 ml) was injected into the foot vein at the rate of 3 ml/s. The delay time was selected through a time-density curve made by Smart prep. Collimation thickness was 3mm and the table speed was 4.5mm/s. Reconstruction was done with 1.5mm increments. The SCTA images were postprossed with multiplanar volume reconstructions (MPVR) and shaded surface display (SSD). Original axial images were used to measure the diameter of vertebral artery. DSA was performed for those cases with vertebral artery stenosis within one week. Results Seventy eight (21.7%) unilateral vertebral arteries were diagnosed as hypoplastic vertebral artery,the left in 24 and the right in 54,the average diameter was 1.3 mm(1.0~1.5 mm),among them,opposite artery coexisted with local calcifications in13 cases, with twist in 20 and with compression by luschka joint hyperplasia in 11. The results were corresponding to that of DSA.Conclusion Hypoplastic vertebral artery is one of the popular reasons of vertebrobasilar artery insufficiency , it could be diagnosed by SCTA and should be used generally in clinic to substitute the more invasive DSA inspection.
