1.Clinical observation on Yiyi Fuzi Baijiang powder for treatment of patients with ulcerative colitis accompanied by yang-deficiency of spleen and kidney: a report of 80 cases

Objective To observe the clinical effects of Yiyi Fuzi Baijiang powder for treatment of patients with ulcerative colitis (UC) accompanied by yang-deficiency of spleen and kidney.Methods Eighty patients with UC admitted to the First Affiliated Hospital of Tianjin University of Traditional Chinese Medicine (TCM) from January 2012 to December 2015 were enrolled, and they were divided into an integrated TCM and western medicine group (combined group) and a control group by random number table method, 40 cases in each group. The control group was treated by normal saline 100 mL with dexamethasone 10 mg enema, whilethe combined group was given Yiyi Fuzi Baijiang powder 100 mL (coix seed 30 g, aconitum carmichaeli debx 6 g, herba patriniae 15 g, radix paeoniae alba 15 g, frankincense 6 g, myrrh 6 g, glycyrrhiza uralensis 6 g) combined with dexamethasone 10 mg enema, one therapeutic course was 10 days, and after 3 courses, the clinical efficacy was observed in the two groups. The differences of erythrocyte sedimentation rate (ESR), the number of red blood cell (RBC) and white blood cell (WBC) positive patients in stool routine were compared before and after treatment, and the clinical therapeutic effects were observed in the two groups.Results After treatment, the ESR was reduced in both groups compared to that before treatment, the numbers of RBC and WBC positive patients in stool routine were decreased in both group compared to those before treatment, and the degrees of changes were more significant in combined group than those in the control group [ESR (mm/1 h): 15.00±3.07 vs. 20.55±3.87, the number of RBC positive patients: 7 cases vs. 21 cases, the number of WBC positive patients: 14 cases vs. 21 cases, allP < 0.05]. The total effective rate of combined group was significantly higher than that of the control group [92.5% (37/40) vs. 75.0% (30/40),P < 0.05].Conclusion The clinical effect of Yiyi Fuzi Baijiang powder combined with dexamethasone enema was superior to that simply using hormone enema therapy for treatment of patients with UC accompanied by yang deficiency of spleen and kidney, this combined method is simple, and its curative effect is definite.
2.An observation on clinical efficacy of paeoniae decoction add and subtract combined with mesalazine for treatment of ulcerative colitis

Objective To investigate the clinical efficacy of paeoniae decoction (PD) add and subtract or modified PD combined with mesalazine for treatment of patients with ulcerative colitis (UC).Methods A prospective study was conducted, 98 patients with UC admitted to the First Affiliated Hospital of Tianjin University of Traditional Chinese Medicine from August 2015 to August 2016 were enrolled, and they were divided into an observation group and a control group by random number table, 49 cases in each group. The control group was treated with mesalazine 1 g, 3 times daily; the observation group was treated with paeoniae decoction (ingredients: paeonia 30 g, coptis chinensis 15 g, Chinese angelica 15 g, betel nut 6 g, Chinese rhubarb 6 g, baikal skullcap 6 g, cinnamon 6 g, licorice 6 g) add and subtract, on the basis of treatment of the control group ,1 dose daily, each dose was decocted 2 times and warm decoction was taken once in the morning and once in the evening; the therapeutic course in both group was consecutive 6 weeks. The contents of different inflammatory factors, interleukin (IL-1, IL-8), tumor necrosis factor-α (TNF-α), C-reactive protein (CRP)in serum were compared before and after treatment between the two groups, and the clinical curative effects and adverse reactions were observed in two groups.Results After treatment, the levels of serum IL-1, IL-8, TNF-α, CRP in two groups were significantly lower than those before treatment, and the degrees of decrease were more significant in the observation group than those in the control group [IL-1 (ng/L): 8.48±3.05 vs. 9.38±3.37, IL-8 (ng/L): 10.15±2.23 vs. 11.94±2.30, TNF-α (μg/L): 122.13±6.40 vs. 137.02±7.35, CRP (mg/L): 7.16±1.93 vs. 8.02±2.63, allP < 0.05]. After treatment, the total effective rate in observation group was significantly higher than that in control group [91.84% (45/49) vs. 77.55% (38/49),P < 0.05]; the adverse reactions of observation group and control group had no statistical significant difference [4.08% (2/49) vs. 6.12% (3/49),P > 0.05].Conclusion The clinical efficacy of add and subtract of paeoniae decoction combined with mesalazine for treatment of ulcerative colitis is prominent, and no increase of adverse reaction occurrence was seen.
3.Influence of Prenatal Diagnosis and Immediate Surgery Intervention on the Prognosis of Congenital Hydronephrosis in Children

Objective To explore the influence of prenatal diagnosis,active follow up and immediate surgery intervention on the prognosis of congenital hydronephrosis in children.Methods The clinical data in 85 cases were analyzed retrospectively.They were divided into prenatal diagnosed group and postnatal diagnosed group.Prognostic parameters such as preoperative statuse,pathology during surgery,operation process,status before discharging and complications were collected and statistically analyzed.Results The numbers of grade 4 hydronephrosis,secondary renal calculus,opposite kidney hydronephrosis,preoperative infectioin,perinephric adhesion and postoperative complication in prenatal diagnosed group were significantly lower than those in postnatal diagnosed group(Pa0.05).Conclusions 1.Ultrasonagraphy examination and follow-up are indispensable both before and after delivery which will help doctors to diagnose the disease at early stage.2.Surgical operation on congenital hydronephrosis at early stage is reliable and will achieve the same effect with the children undergo surgical operation at older age.3.Immediate surgical operation is of great clinical significance on preventing serious complication and irreversible kidney damage.
4.Prevention and therapy progress in allograft rejection of high - risk corneal transplantation

As a result, application of immunosuppressive agent in high - risk corneal transplantation rejection reaction occupies an irreplaceable position. However, many local and systemic adverse reactions appeared in the process of application. In order to improve the achievement ratio of the corneal transplantation, experts and scholars at home and abroad have obtained remarkable achievements in dosage form, route of administration and drug combination;meanwhile, a considerable progress was also made in genetic engineering. This paper is a review of prevention and treatment on high - risk corneal graft rejection reaction.
5.Protective effect of different concentrations of propofol postconditioning against glutamate neurotoxicity to brain slices of neonatal rats

Objective To explore the effects of different concentrations of propofol postconditioning against glutamate neurotoxicity to brain slices of neonatal rats.Methods The brain slices of neonatal rats were prepared and cultured in complete medium.They were randomly divided into five groups:the normal control group,glutamate injury group(RI group),1 mg/L propofol postconditioning group(PL1+RI group),3 mg/L propofol postconditioning group(PL3+RI group),5 mg/L propofol postconditioning group(PL5+RI group),12 cases in each group.The RI,PL1+RI,PL3+RI,PL5+RI groups were cultured for 6 days,then the brain slices were moved into the culture medium containing glutamate(1 mmol/L) and incubated for 30 minutes.And then,respectively,the brain slices of RI group were put into another complete culture medium,the PL1+RI group,PL3+RI group and PL5+RI group were put into the medium containing corresponding concentrations of propofol medium and long chain fat emulsion injection.All of the above were cultured for 24 hours in order to establish the injury model.The numbers of the Nissl body,the LDH release rates and the brain tissue damage rates of each brain slice were detected to evaluate the effects of propofol postconditioning on the reperfusion injury in the glutamate-damaged brain slices of neonatal rats.Results Compared with the RI group,the numbers of the Nissl body of the PL1+RI group,PL3+RI group and PL5+RI were higher,the LDH release rates and the brain tissue damage rates of the PL1+RI group,PL3+RI group and PL5+RI were lower,the diferences were significant(P<0.05).Among the three PL+RI groups,the LDH release rates and the brain tissue damage rates of the PL3+RI group were lower than those of the other two groups,the diferences were significant(P<0.05),at the same time,the numbers of Nissl body were more than the other two groups,the diferences were significant(P<0.05).Conclusion Propofol postconditioning has protective effects on the reperfusion injury in the glutamate-damaged brain slices of neonatal rats.However,the protective effects are not dose-dependent,and 3 μg/mL is the best dose of propofol to keep the glutamate-damaged brain slices from reperfusion injury in this research.

7.Nosocomial Infection among Burn Patients:Analysis and Prevention

OBJECTIVE To discuss the risk factors of nosocomial infection in burn patients in order to raise their treatment level.METHODS Picked out the nosocomial infection and the risk factors in burn patients during recent three years,we then made the review analysis.RESULTS From 234 cases 46 patients were with nosocomial infection,the infection rate was 19.66%,the most common location was wound infection,accounted for 63.04%,followed by respiratory tract for 23.91%.The main risk factors were age(≥ 60 years),severe degree burns,a long stay in hospital,and the large-scale application of antibiotics.CONCLUSIONS As for the high incidence of nosocomial infection among burn patients,we must pay close attention to and strengthen prevention.
8.Expressions of chemokine CXCL12 and its receptor CXCR4 in human sperm.

OBJECTIVETo explore the expression patterns of the chemokine CXCL12 and its receptor CXCR4 in human sperm.
METHODSWe collected semen samples from 10 fertile men, performed density gradient centrifugation, and then determined the expressions of both CXCL12 and CXCR4 in the sperm by RT-PCR and immunofluorescence staining.
RESULTSRT-PCR revealed the mRNA expressions of CXCL12 (0.641 +/- 0.180) and CXCR4 (0.464 +/- 0.100) in the sperm. However, only CXCR4 rather than CXCL12 was expressed at the protein level, and the positive staining for CXCR4 was observed mainly in the posterior part of the acrosome.
CONCLUSIONCXCL12 and CXCR4 are involved as important molecules in regulating the function of human sperm.

10.Evaluation of Regional Wall Motion in Patients with Suspected Coronary Artery Disease by Real-Time Three-Dimensional Echocardiography and Two-Dimensional Speckle Tracking Imaging

Objective To evaluate the clinical value of the regional wall motion in patients with suspected coronary artery disease by real-time three-dimensional echocardiography (RT-3DE) and two-dimensional speckle tracking imaging (2D-STI). Methods A total of 143 hospitaized patients with suspected coronary artery disease and normal wall motion de?tected by two-dimensional echocardiography (2DE) were enrolled and devided into group A (coronary stenosis ≥75%) 73 cases and group B (coronary stenosis<75%) that include 70 cases according to coronary angiography results. Left ventricu?lar segmental ejection fraction (sEF) was measured by RT-3DE and peak systolic longitudial strain (SL) was measured by 2D-STI. ROC curves of the two measurements were generated and compared. Differences in sEF and SL between group A and group B were respectively analyzed. Results (1)Both sEF of RT-3DE and SL of 2D-STI in group A were significantly low?er than those in group B (P<0.05);(2)sEF was positively correlated with SL(r=0.689,P<0.05);(3)Compared with SL, sEF had larger area under ROC curve in some segments of left ventricle where both parameters are abnormal for the diagnosis of the coronary stenosis ≥75% (0.922 vs 0.874). Conclusion Regional wall motion of left ventricular can be measured by both RT-3DE and 2D-STI which can be both used to estimate the extend of stenosis of coronary artery. And RT-3DE is su?perior over 2D-STI.
