1.Comparative Study on Two Extraction Methods of Glycyrrhizic Acid

Objective To compare diffenrent amounts of the glycyrrhizic acid extracted by two methods-decoct method and Ammonia-water method.Methods The best extraction conditions including amount of affusion water and solvent,extraction time,boiling time,crash time,vibrating time and concentration of ammonia were choosed by orthogonal design.Amounts of the glycyrrhizic acid were determined by ultraviolet spectrophotometry at 252 nm wavelengh.Result The best extraction conditions from decoct method were that amount of affusion wate was 200 mL,extract 2 times,boiling time was 5 min,crash time was 20 s.The best extraction conditions from Ammonia-water method were that amount of 0.4% ammonia was 200 mL,vibrating time was 2 h,extract 2 times.The extraction rate of Ammonia-water method was 32%,and the extraction rate of decoct method was 26%.Conclusion For glycyrrhizic acid extraction and purity,Ammonia-water has superior efficient than decoct method at the best conditions.
2.Effect of Chinese Medicine Steam Therapy on Activity of Daily Life of Patients with Knee Osteoarthritis

Objective To observe the effect of the Chinese medicine steam therapy on activity of daily life(ADL)of subjects with knee osteoarthritis.Methods68 subjects with knee osteoarthritis were divided into two groups:34 subjects in Chinese medicine steam group(treated with Chinese medicine steam and physical therapy),and 34 subjects in physical therapy(with microwave and magnetism with caloric)group.The clinical feature and ADL of all the subjects were evaluated with Lequesne index of severity and activity for osteoarthritis before and after the treatment.After 0.5 year,they were followed up,the recurrence rate were compared.ResultsAll the subjects improved in clinical feature and ADL after the treatment(P<0.05),but the Chinese medicine steam group improved greater in the ADL than the other group(P<0.05).After 0.5 year,the recurrence rate of the Chinese medicine steam group is lower than that of the other group(P<0.05).ConclusionIn addition with the Chinese medicine steam is more effective on improving ADL and recurrence.
3.The clinical recent effects of two neoadjuvant chemotherapies in the treatment of advanced gastric cancer

Objective To observe the recent effects and toxicity in advanced gastric carcinoma by two kinds of neoadjuvant chemotherapies ( PF and FOLFOX).Methods A total of 60 patients with advanced gastric cancer ( Ⅲ ~ Ⅳ ),they were randomly divided into 2 groups; PF group ( 30 cases) and FOLFOX group ( 30 cases).All patients were given 3 cycles of docetaxel or L-OHP chemotherapy preoperatively,to observe the recent effects and toxicity in advanced gastric carcinoma.Results The total effective rate in PF group was 77.67%,while it was 50.00% in FOLFOX group,it had significant difference between in two groups ( x2 =4.59,P < 0.05 ).There is no difference in the incidence of neutropenia,transaminase elevation,nausea and diarrhea two groups.The incidence of hair loss in PF group is higher than in FOLFOX group,but there is no difference between two groups; The incidence of peripheral nerve paresthesia in FOLFOX group is higher than that in PF group( x2 =5.08,P <0.05).Conclusion PF as neoadjuvant chemotherapy is a very effective and well-tolerated regimen in patients with advanced gastric cancer.
4.Anti-cicatricial effect of tetrandrine drug delivery system in glaucoma filtration surgery in rabbit

Background Scarring of the filtering bleb is a main cause of filtering surgical failure in glaucoma.It has been reposed that tetrandrine could suppress the proliferation of cultured human fibroblast of Tenons capsule in vitro and thus has the potential effect to prevent scarring after the filtering surgery. Objective Present study was to investigate the anti-cicatricial effect of tetrandrine drug delivery system(Tet DDS)during filtration surgery. Methods Filtration surgery was performed in bilateral eyes of 18 New Zealand white rabbits.The Tet DDS with 0.3 mg Tet,0.2 mg Tet or free-Tet were implanted subcunjunctially during the surgery.The filtering blebs were scored in 1 day,4,7,10,14 days after referring to the corneal thickness and bleb range under the slit-lamp biomicroscopy.The morphology of filtering bleb was assessed by in vivo confocal microscopy in 7 and 14 days after operation.The filtering bleb specimen was prepared in 7 and 14 days for the histopathological examination. Results The filtering bleb scores in Tet DDS implantation groups were significantly higher than those in free-Tet DDS group from 4 days through 14 days after trabeculectomy(P<0.01),and the scores showed a considerably increase in 0.3 mg Tet DDS group compared with 0.2 mg Tet DDS group from 7 days through 14 days after trabeculectomy(P<0.05).The filtering blebs of Tet DDS implantation groups were found with distinct subepithelial cystic spaces under the light microscopy and in vivo confocal microscopy on the 7th day and 14th day after surgery.Compared with free-Tet DDS group,the numbers of subepithelial mierocysts were much more(P<0.01)and the area of microcysts was larger(P<0.01)in Tet DDS group.The filtering tissue presented with more subepithelial microcysts and larger microcysts range in 0.3 mg Tet DDS group than 0.2 mg Tet DDS group in 7 and 14 days after operation(P<0.05).The inflammatory cell infiltration wag milder in 0.3 mg Tet DDS group in comparison with 0.2 mg Tet DDS group and free-Ted DDS group.Conclusion Tet DDS has strong inhibitory effects on inflammatory cells activity and fibroblagt activity the early stage after filtering surgery and therefore improve the surgery success rate.
5.Transglutaminase 1 gene mutation in a family with lamellar ichthyosis

Objective To report a consanguineous family with lamellar ichthyosis and to detect the mutations in transglutaminase 1 (TGM1) gene in this family. Methods Genomic DNA was extracted from the blood samples of a 19-year-old male patient with lamellar ichthyosis, his family members and 100 normal human controls. PCR was carried out to amplify all the encoding sequences (15 exons) and adjacent flanking sequences of TGM1 gene followed by bidirectional sequencing. Results A C1666T mutation in the 11th exon in TGM1 gene, which resulted in the substitution of ACA (threonine) by ATA (isoleucine) at codon 529, was detected in the proband, while both his parents carried the C1666T mutation in heterozygous form, and his sister was a C/C homozygote. None of the 100 normal control individuals carried the mutation in TGMlgene. Conclusions The de novo mutation from ACA (threonine) to ATA (isoleucine) at codon 529, may contribute to the development of lamellar ichthyosis. Consanguineous marriage can increase the risk for lamellar ichthyosis by raising the probability of homozygosis of C 1666T mutation in TGM 1 gene.
6.Expression of SCC-Ag and CEA and its clinical significance in patients with lung cancer

Objective To investigate the ex pr ession of squamous cell carcinoma antigen (SCC-Ag) and carcinoembryanic antigen (CEA) and its clinical significance in patients with lung cancer. Meth ods The pre-treatment serum from 134 patients with lung cancer from 1998 to 2002 were analyzed for the SCC-Ag level and CEA level by IMX, and the correlation of the SCC-Ag level and CEA level with the clinicopathologic charac teristics were also detected. Results Significant correlations were found between the pre -treatment SCC-Ag level, and CEA level and the pathologic classification, clin ical stage, the positive rate of diagnosing lung cancer was raised by combined a ssay with serum SCC-Ag and CEA. Conclusion Pre-treatment and after-treatment measurement of serum SCC-Ag level and CEA level can be used for diagnosis, prediction of pr ognosis and monitor of after-treatment recurrence in patients with lung cancer.

Objective To investigate the effect of atorvastatin intensive therapy on cardiac function and lipoprotein phospho‐lipase A2 (Lp‐PLA2) in patients with hypertension complicated with ST segment elevation acute myocardial infarction .Methods Totally 78 patients with hypertension complicated with ST segment elevation acute myocardial infarction from June 2014 to April 2016 in our hospital patients ,according to the random number table method divided into observation group (atorvastatin therapy ) and control group (routine treatment) .The changes of blood pressure ,blood lipid ,Lp‐PLA2 ,cardiac function of two groups were compared before and after treatment .Results Before treatment ,there was no significant difference in blood pressure and blood lipid between the two groups (P>0 .05) .After treatment ,the two groups of patients with SBP ,DBP ,LDL‐C significantly decreased , HDL‐C significantly increased ,and compared with the control group ,the SBP ,HDL‐C ,LDL‐C in the observation group were signif‐icantly different (P<0 .05) .Before treatment ,there was no significant difference in Lp‐PLA2 between the two groups(P>0 .05) . After treatment ,the two groups of patients with Lp‐PLA2 were significantly reduced ,and the observation group was significantly greater (P<0 .05) .Before treatment ,there was no significant difference in cardiac function between the two groups (P>0 .05) .Af‐ter treatment ,the observation group of patients with LVESD ,LVEDD significantly decreased ,LVEF ,CO ,CI significantly in‐creased ,compared with the control group ,the difference was statistically significant (P<0 .05) .Conclusion Atorvastatin intensive therapy can significantly reduce high blood pressure acute ST segment elevated blood pressure and Lp‐PLA2 levels in patients with acute myocardial infarction and improve cardiac function ,worthy of promotion .
9.Study on efflux pump MexAB-OprM in carbapenem-resistant Pseudomonas aeruginosa

Objective To study the relationship between efflux pump MexAB‐OprM and carbapenem resistance of Pseudomonas aerginosa strains .Methods The minimum inhibitory concentrations of imipenem and meropenem were determined by agar dilution method for 75 strains of P .aerginosa in the absence or presence of MC207110 to screen the phenotypes of active efflux pump .Reverse transcriptase‐polymerase chain reaction (RT‐PCR) method was used to determine the mRNA expression level of mexA which encodes the membrane fusion protein in active efflux pump MexAB‐OprM and the reference (housekeeping) gene rpoD .PCR method was used to amplify the regulatory genes mexR ,nalC ,and nalD of active efflux pump MexAB‐OprM in the strains overexpressing the efflux pump . The PCR products were subject to DNA sequencing and BLAST analysis . Results Of the 75 P .aeruginosa strains ,13 (17 .3% ) were positive for efflux pump MexAB‐OprM .Overexpression of the efflux pump was identified in 10 of the 13 strains and associated with positive regulatory genes mexR ,nalC and nalD .A Gly71→Glu mutation in nalC was found in 9 strains ,and a Ser209→Arg mutation in nalC was identified in 8 strains .Only one strain had a Thr158→Ile mutation in nalD .Eight strains had mutation in mexR .Conclusions Overexpression of multidrug efflux pump MexAB‐OprM plays an important role in carbapenem resistance of P .aeruginosa .High level expression of MexAB‐OprM is related to the mutations of its regulatory genes .
10.The clinical application of objective hearing monitoring technology in cochlear implants.

OBJECTIVE:
Compare the clinical characteristics and application value between elcetrically evoked stapedius responses and neural response telemetry.
METHOD:
Select number 1, 7, 11, 17, 22 electrodes from 21 cochlear implanting nucleus 24R cochlear implants, using NRT3.0 software provided by Cochlear corporation to test the threshold of elcetrically evoked stapedius responses and neural response telemetry during operation and subjective psychological physical test after operation repectively named elcetrically evoked stapedius responses threshold, electrically evokded compound aciton potentials threshold, and T-levels. Respectively study the correlations of ESRT, ECAP threshold, and T-levels.
RESULT:
The detection rates of ESRT, NRT, and T-levels were 96.19%, 93.33%, 100%. The mean thresholds of ESRT were 206.61 +/- 10. 74, 208.48 +/- 13.64, 205.52 +/- 14. 63, 203.76 +/- 12.97, 199.50 +/- 11.19; The mean thresholds of NRT were 184.11 +/- 6.35, 188.55 +/- 11.70, 187.00 +/- 12.29, 181.85 +/- 13.22, 179.00 +/- 10.50; The mean thresholds of T levels were 145.48 +/- 18.66, 148.62 +/- 8.22, 146.62 +/- 18.08, 142.52 +/- 13.11, 140.33 +/- 13.68. The mean thresholds of ESRT in each electrode were rather greater than that of T-levels. There is a high degree of consistency in the mean thresholds of ES RT, NRT and T-levels. The test time, test means, unilaterally detection rate during operation, ESR is better than NRT. All the electrodes were set as observation objects, the correlation of ESRT and NRT is 0.69, the correlation of NRT and T is 0.62, and the correlation of ESRT and T is 0.39.
CONCLUSION
The application of elcetrically evoked stapedius responses and neural response telemetry during operation can rapidly estimate wether the implant is in good condition and the pathway is normal. Rather than neural response telemetry, elcetrically evoked stapedi us responses is more flexible, convienient, accurate and visual, while it is more objective superiority after operation especially for young children for the former detector.
