2.Construction of the training mode of emergency medical personnel in traditional Chinese Medicine Colleges

Establishing emergency medicine training mode of integrated traditional Chinese and Western medicine in traditional Chinese Medicine College is of great significance.Through the construction of clinical skills teaching materials,the construction of training base and standard operating procedures (SOP) management,first aid training and related drama contests,and improving teachers' comprehensive quality,we set up emergency medicine training mode of traditional Chinese medicine and Western medicine,which helps students make comprehensive use of first aid knowledge and practical skills of traditional Chinese medicine and Western medicine for syndrome differentiation.

Objective:To study the significance of standard operating procedures of construction emergency medical simulation teaching-based. Methods:Emergency medical simulation teaching base on the construction of standard operating procedures mode. Results:SOP model was used in emergency medicine in practice teaching and management, it improved the connotation of practical teaching system construction;and was helpful to improve the practice teaching content construction. Evaluation is beneficial to improve the practice teaching system. Evaluation is beneficial to improve the practice teaching system. To strengthen the emergency medical simulation base radiation ability, promote the standardization of practice teaching and management level, this itself is a kind of innovation, but still need to constantly improve and improve. Conclusion:Explore with Chinese characteristics of Chinese and western medicine combined with emergency medicine education mode, cultivate high-quality talents in the field of traditional Chinese and western medicine emergency.
4.Establishment and evaluation of a rat model of arthritis-hypertension disease

Objective To establish and evaluate a rat model presenting symptoms of arthritis-hypertension disease (AHD).Methods A total of forty healthy 5-6 week-old male SD rats were used in this study.Hypertension was induced by constriction of the left renal artery by two kidney one clip (2K1C) with a 0.25 mm silver clamp, and AHD model was established by injecting 0.1 mL complete Freund adjuvant to the left hind paw.Tail artery pressure was measured with a non-invasive blood pressure measurement system.The degree of swelling in the non-inflammatory joint of rats was measured with a paw volume measuring instrument, the arthritis index and incidence of inflammation were evaluated.The rats were sacrificed on the 35th day.The thoracic aorta, ankle joint and spleen tissues were examined by pathology using HE staining.Results The joint of AHD model rat was significantly swollen, extensive synovial cell hyperplasia, inflammatory cell infiltration, vascular pannus formation, and bone and cartilage destruction.The number of germinal centers in spleen was increased, and a large number of lymphocyte infiltration, diffuse proliferation of white pulp, and red pulp infiltration were present.The arthritis index, incidence of inflammation and histopathological scores of the joint and spleen were significantly higher than adjuvant arthritis (AA) rats;meanwhile, the blood pressure of AHD model rat was significantly increased, the thickness and cross-sectional area of thoracic aorta were significantly increased, while the lumen diameter was significantly reduced.The blood pressure and vascular injury were significantly increased or aggravated compared with the HT rats.Conclusions A rat model of arthritis-hypertension disease is successfully established by using complete Freund adjuvant intradermal injection to the footpad and surgery to narrow the left renal artery.

6.Efficacy and safety of intravenous recombinant human brain natriuretic peptide in patients with severe heart failure: a prospective multicenter clinical study

Objective To explore the efficacy and safety of recombinant human brain natriuretic peptide (rhBNP) in the patients with severe heart failure (HF). Methods A prospective multicenter study was conducted. Patients whose age > 18 years old, and with the New York Heart Association (NYHA) cardiac function grade overⅢ - Ⅳ , acute cardiac insufficiency and the acute exacerbation of chronic cardiac insufficiency admitted to intensive care unit/cardiovascular care unit (ICU/CCU) of 58 Hospitals in China were enrolled. On the basis of the conventional treatment, all patients would be given rhBNP (neo adjuvant) with a loading dose of 1.5 μg/kg for 3-5 minutes, and followed by a maintenance dose of 0.010-0.015 μg·kg-1·min-1 for 3-7 days. Before the treatment and 1, 3, 7 days after treatment, researchers detected indexes of cardiac and renal function, the levels of N-terminal B-type natriuretic peptide (NT-proBNP), left ventricular ejection fraction (LVEF), cardiac output (CO), pulmonary capillary wedge pressure (PCWP), central venous pressure (CVP), urea nitrogen (BUN), serum creatinine (SCr), and urine output; the renal function index was re-evaluated at 30 days after administration, and the time entering ICU again, re-admission, cardiovascular events were recorded. Results 408 patients were enrolled, with 241 males and 167 females. Age range was 28-95 years, the average age was (63.0±15.8) years, and 50-70 years old accounted for 46.8%. Compared with the data before treatment, NT-proBNP, PCWP and CVP significantly decreased at 6 hours after treatment [NT-proBNP (μg·kg-1·min-1): 4378.58±4082.29 vs. 6403.41±5759.48, PCWP (mmHg, 1 mmHg = 0.133 kPa):12.41±2.21 vs. 14.26±2.85 , CVP (mmHg): 10.63±2.62 vs. 11.45±3.45, all P < 0.05], and with the prolongation of injection, NT-proBNP, PCWP and CVP were gradually declined; CO 1 day after treatment (mL: 4.89±0.81 vs. 4.40±0.92) and LVEF 3 days after treatment (0.465±0.100 vs. 0.431±0.107) were significantly increased (both P < 0.05), and with the prolongation of injection, CO and LVEF were gradually increased. There were no obvious changes in BUN and SCr during the treatment, but 30 days after treatment, SCr was significantly lower than that pre-treatment (μmol/L: 110.98±47.40 vs. 132.62±75.60, P < 0.01). Compared with the data pre-treatment, urine output per hour was significantly increased at 3 hours after treatment (mL: 129.59±82.16 vs. 89.60±53.49, P =0.000); urine output every 24 hours was significantly increased at day 1 and day 2 after administration (mL: 2676.54± 1006.83, 2678.74±975.97 vs. 2150.36±283.76, both P < 0.01). In 7 days, the re-entry ICU rate was 2.7%, and the re-hospitalization rate was 2.88% within 30 days, re-cardiac failure rate was 1.43% in 30 days, and the overall fatality rate was 9.55% in 30 days. Conclusions The rhBNP can significantly improve heart function in patients with HF. And, it has a certain effect on renal function. The rhBNP is effective and safe for the treatment of cardiac insufficiency.
7.Hand hygiene compliance and cost in a hospital

Objective To evaluate the effect of controlling cost by introducing hand hygiene products with lower price on promoting hand hygiene compliance.Methods The application status and cost of hand hygiene products in 2012 was as pre-intervention group,2013 was as post-intervention group.Effective and lower price hand hygiene products were introduced in 2013,consumption and cost of hand hygiene products before and after the intervention was compared.Results Consumption of hand hygiene products per patient-day before and after the intervention was significantly different ([10.56±16.46]mL vs [13.79 ± 16.93 ]mL,Z=4.14,P <0.01 );cost of hand hygiene products per patient-day before and after the intervention was not significantly different ([1 .23 ±2.07]yuan vs [0.92±0.59]yuan,P >0.05).Conclusion Introduction of hand hygiene products with lower price in this hospital can improve hand hygiene compliance to certain degree without increasing the cost of hand hygiene.
8.Multi-level identification and analysis about infrared spectroscopy of lophatheri herba.

Based on the infrared spectra of Lophatheri Herba and Commelinae Herba, one-dimensional infrared spectra, second derivative spectra and two-dimensional correlated spectra were used to find out the differences between Lophatheri Herba and its imitations, respectively. The common peak ratio and variant peak ratio dual-indexes sequential were calculated and established according to infrared spectra of eleven batches of herbs. Infrared spectral data of Lophatheri Herba cluster analysis was applied to explore the similarity between each sample. The grouping results trend of sequential analysis of dual-indexes and cluster analysis was accordant. The results showed that the differences could be found by multi-level identification, and the source and the quality of the herbs could be effectively distinguished by the two analysis methods. Infrared spectroscopy, used in the present work exhibited some advantages on quick procedures, less sample required, and reliable results, which could provide a new method for the identification of traditional Chinese medicine with the imitations and adulterants, and the control of quality and origin.

10.Analysis for inpatients with infectious diseases of central nervous system in Shijiazhuang,China

Objective To investigate the clinical characteristics of infectious diseases of CNS.Methods Inpatients treated in the second hospital of Hebei Medical University from 1996 to 2005 were subjected to retrospective study.Results During 10 years,the hospitalization number of infectious diseases of CNS was 3929.The proportion of inpatients with viral meningitis(encephalitis),cerebral cysticercosis,tuberculous meningitis(TBM),purulent meningitis(PM)and cryptococcal meningitis(CM) was 63.8%,17.4%,7.7%,0.7% respectively.The hospitalization number of viral meningitis (encephalitis),TBM,PM and CM in the 2001—2005 5-year period was 53.5%,21.3%,35.3%,and 77.3% higher than that of the 1996—2000 period.Whereas the hospitalization number of cerebral cysticercosis decreased by 32.4% than that of the last 5 years.The peak months of viral meningitis (encephalitis)in a year were July,August and September.The peak months of TBM in a year were April, May and June.The case fatality of viral meningitis(encephalitis),cerebral cysticercosis,TBM,PM and CM was 8.1%,6.9%,22.4%,12.9%,33.3% respectively.Conclusions In the past l0 years,the incidence of common infectious diseases of CNS has increased in Shijiazhuang except cerebral cysticercoids. Our vigilance should be improved and some effective measures should be adopted to keep away infectious diseases of CNS.
