1.Treatment of Shoulder and Neck Pain in Patients with and without Upper Cervical Degeneration

Objective To compare the results of clinical treatment of shoulder and neck pain in patients with and without upper cervical degeneration. Methods Fifty two patients with diagnosed shoulder and neck muscular fasciitis(with high tension and tenderness on the trapezius) were divided into 3 groups: group A,with upper cervical degeneration(16 cases);group B,with upper cervical degeneration(15 cases),and group C,without cervical degeneration(21 cases).There were no significant difference in gender,age and course of disease among the 3 groups.Patients in 3 groups were regularly treated by naprapathy and electric therapy(intermediate frequency).Cervical traction and manipulation was added to the patients in group A.Treatment for 3 groups lasted for 2 weeks.The Visual Analog Scale(VAS) scores and Odom criteria before and after treatment were used for determining the results of clinical treatment. Results The VAS scores after treatment were significantly improved in group A and group C as compared with that in group B(P=0.000).Odom criteria of group A(93.8%) and group C(95.2%) were excellent and good without significant difference(P=0.692),but they were significantly higher than that of group C(P=0.001). Conclusions Patients with shoulder and neck pain accompanied by upper cervical degeneration(narrowing of intervertebral foramen) should be treated with cervical traction and manipulation beside the routine therapy.
2.Cloning and characterization of the group 5 allergens from Dermatophagoides farinae

Objective To obtain the gene coding for the group 5 allergens from Dermatophagoides farinae ( Derf5 ) and predict its molecular characteristics. Methods The total RNA of D. farinae were extracted, and the gene Derf5 was amplified by RT-PCR with the primers designed according to previous sequence published in GenBank. The target gene was linked into pMD19-T Simple plasmid, sequenced and analyzed by bioinformatics software. Results The sequence homology reached to 97.8% between our sequenced result with one complete open reading fragment (ORF) and the reference. The gene encode an extracellular hydrophobic protein with 132 amino acid resides, one signal peptide from 1 to 19 position and one transmembrane domain from 1 to 19 position. The secondary structure was composed of extended strand (1. 52% ), random coil (7.58%) and alpha helix (90.91%). The encoded protein was deduced to have two Casein kinase Ⅱ phosphorylation sites. The similarity of the amino acid sequence of the group 5 allergens were 78% between D. farinae and D. pteronyssinus. Conclusion The gene Derf5 was cloned successfully, and its characteristics was primarily predicted.

Proposals in the paper include building the price parameters fitting local conditions,and setting prices using the standardized price model;forming a comprehensive price adjustment scheme with reference to international price comparison relations and domestic pricing level; upgrading the fine pricing management level of medical service pricing and improving charging management dimensions;enhancing interactions and assessment of price-related departments to guide public hospitals to enhance.

Objective To investigate the expression and significance of serum soluble fms-like tyrosine kinase 1 ( sFlt-1 ) in preeclampsia placenta. Methods Immunohistochemistry and semi-quantitative RT-PCR were used to investigate the expression of sFlt-1 and sFlt-1 mRNA in placenta of 30 women with preeclampsia (including mild and moderate preeclampsia 11 cases, severe preeclampsia 19 cases) and 45 normal pregnant women (including first trimester pregnancy 18 cases, 2nd trimester pregnancy 12 cases, and 3rd trimester pregnancy 15 cases). Objective To investigate the expression and significance of Rho-associated protein kinaseⅡ(Rock Ⅱ)in preeclamptic placenta and umbilical artery.Methods Semiquanfitative RT-PCR and Western blot were used to investigate the expression of RockⅡmRNA and RockⅡprotein in placenta and umbilical artery from 35 women with moderate preeclampsia(MPE group)、38 women with severepreeclampsia(SPE group)and 45 normal third trimester pregnant women(control group),the S/D value of umbilical artery was examined by ultrasound.Results (1)The expression of Rock Ⅱ mRNA of Dlacenta in MPE group(0.82±0.14)and SPE group(0.93±0.13)were signifieantly higher than that in control group (0.70 ±0.12,P<0.01).The expression of Rock Ⅱ protein of placenta in MPE group(0.79±0.15)and SPE group(0.92±0.12)were significantly higher compared with control group(0.68±0.11,P<0.01).The expression of Rock Ⅱ mRNA and protein of placenta in SPE group were higher compared with MPE group(P<0.01).(2)The expression of Rock Ⅱ mRNA of umbilical artery in MPE group(0.69±0.13)and SPE group(0.55±0.12)were significantly lower than that in control gmup(0.76±0.10,P<0.01).The expression of RockⅡ protein of umbilical artery in MPE group(0.68±0.10)and SPE group(0.51±0.12)were lower compared with control group(0.75±0.13,P<0.01).The expression of RockⅡ mRNA and protein of umbilical artery in SPE group were significantly lower compared with MPE group(P<0.01).(3)There were no correlations between the expression of RockⅡ mRNA and protein in placenta and umbilical artery and the S/D value and birth weight(P>0.05).Conclusion The upregulated expression of Hock Ⅱ in placentas and downregulated expression in umbilical artery may be a compensation in preeclampsia.
7.The effects of simulated family nursing on the cognitive function and activity of daily living in patients with Alzheimer's disease

Objective To evaluate the effects of simulated family nursing on the cognitive function and activity of daily living in patients with Alzheimer's disease.Methods Sixty-eight patients with Alzheimer's disease were assigned to routine nursing condition and simulated family nursing condition.The patients were assessed with Mini-Mental State Examination (MMSE) and Activity of Daily Living Scale (ADL) before intervention,and 3 months and 6 months after intervention.Results Compared with the control group,the patients' cognitive function (t=2.31,P=0.026) and activity of daily living (t=2.59,P=0.012) were improved significantly in the experimental group.Conclusion The simulated family nursing can improve the cognitive function and activity of daily living in patients with Alzheimer's disease.
8.Predictive value of serum soluble fms-like tyrosine kinase 1 concentration in preeclampsia at second trimester

Objective To explore the predictive value of serum soluble fms-like tyrosine kinase-1 (sFlt-1) and vascular endothelia growth factor (VEGF) levels in preeclampsia at second trimester. Methods Serum sFlt-1,VEGF concentrations were determined in 172 initial normal pregnant women at 26-28 gestation week.The outcomes of pregnancies were followed.In a cohort of 172 pregnant women, 16 cases of preeclampsia were developed (preeclampsia group),and 156 cases were with no complication (control group).Results The serum levels of sFlt-1 in preeclampsia group (11.4?6.2)?g/L were significantly higher than that in control group(4.5?2.1)?g/L(P<0.01).The serum levels of sFlt-1 in precelampsia women with the onset before 32 gestation week and fetal growth retardation,(14.0?6.8)?g/L,(14.4?6.7)?g/L were significantly higher than that in women with the onset after 32 gestation week and with no fetal growth retardation (9.0?4.1)?g/L,(8.9?4.0)?g/L,respectively (P<0.05).There was no significant difference in the serum levels of VEGF between preeclampsia group and control group.A sFlt-1 cutoff value of 8.75?g/L at 26-28 gestation week yielded a sensitivity of 87.5%,specificity of 97.4%,positive predictive value of 80.0%,negative predictive value of 88.5%,respectively,for subsequent onset of preeclampsia.Conclusion Maternal serum sFlt-1 concentration at second trimester can be used as an early predictive marker of preeclampsia.

Objective To study the relationship between tyrosine phosphorylation(TP)and protein expression of insulin receptor substrate-1(IRS-1)and insulin resistance in patients with gestational diabetes mellitus(GDM).Methods IRS-1 expression and TP in skeleton muscle tissue were determined by Western blot and immunoprecipitation in women with GDM(GDM group,n=22),normal pregnant women(normal pregnancy group,n=22)and normal nonpregnant women(normal nonpregnant group,n=13).Fasting plasma glucose(FPG)and fasting insulin(FINS)were measured by oxidase assay and immunoradioassay. 10.Effects of MicroRNA-154 on Apoptosis of Hepatic Stellate Cells

Objective:To investigate the effect of miR-154 on the apoptosis of hepatic stellate cells (HSC-T6). Methods:Hepat-ic stellate cells (HSC-T6) were transfected with miR-154 mimics and miR-154 inhibitor, and the cells were trandfected with mimics NC and inhibitor NC as the negative control. The effects of miR-154 on the proliferation of HSC-T6 were detected at different time points by CCK-8. A flow cytometry with double staining of Annexin and PI was used to detect the cell cycle and apoptosis rate of HSC-T6. Results:The proliferation ability of the cells was increased,the apoptosis rate was decreased significantly, and the proportion of cells in G0/G1 phase were decreased, and those in G2/M phase were increased significantly after transfected with miR-154 mimics. The proliferation ability of the cells was decreased,the apoptosis rate was increased significantly, and the proportion of cells in G0/G1 phase were increased, and those in G2/M phase were decreased significantly after transfected with miR-154 inhibitor. Conclusion:MiR-154 can promote the proliferation of HSC-T6 and inhibit the apoptosis of HSC-T6.
