1.Effect of lead aVR on recent prognosis in patients with non -ST -elevation acutecoronary syndrome

Objective To investigate the effect of lead aVR on recent prognosis in patients with non -ST -elevation acutecoronary syndrome(NSTE -ACS).Methods 195 patients with NSTE -ACS were divided into ST segment elevation in lead aVR group(n =54 cases)and non -ST segment elevation in lead aVR group(n =141 ) according to ST segment elevation in lead on admission;clinical data,laboratory index and coronary artery lesion were recorded,and major adverse cardiac (MACE)in hospitalization time were also recorded.Results Compared with non -ST segment elevation in the lead aVR group,the heart rate of ST segment elevation in the lead aVR group was faster[(85.4 ±12.0)beats per minute vs.(79.3 ±13.2)beats per minute,t =2.959,P <0.05],systolic blood pressure and diastolic blood pressure were higher[(145.0 ±20.3)mmHg vs.(136.0 ±19.4)mmHg,t =2.874,P <0.05];NT -proBNP[(743.9 ±125.8)pg/mL vs.(416.4 ±95.3)pg/mL,t =19.625,P <0.05],cTnI[(3.4 ± 1.0)ng/mL vs.(1.2 ±0.4)ng/mL,t =21.994,P <0.05],CK -MB[(31.5 ±9.8)ng/mL vs.(19.8 ±6.5)ng/mL, t =9.682,P <0.05]were higher (P <0.05);LVEF were lower[(50.1 ±5.2)% vs.(53.8 ±5.7)%,t =4.153, P <0.05];the rate and amplitude of low ST segment in outside lead aVR also was higher[(9.5 ±3.8)mm vs. (7.4 ±2.8)mm,t =4.224,P <0.05];the rate of left main and three -vessel disease in ST segment elevation in lead aVR group were higher (P <0.05).Compared with non -MECE group,proportion of ST segment elevation in lead AVR on admission was higher (P <0.05).Logistic regression analysis showed that heart rate(OR =1.472),NT -proBNP(OR =1.732),cTnI(OR =1.893),left main coronary artery(OR =2.478)and three -vessel disease (OR =2.310)were risk for ST segment elevation in lead AVR(P <0.05).Logistic regression analysis also showed that ST segment elevation in lead AVR were risk for MACE(OR =1.474).Conclusion ST segment elevation in lead AVR has close relationship with large area of myocardial ischemia,and patients prognosis is poorer,which may be independ-ent risk factor for the recent outcomes of patients with NSTE -ACS.Early record of the change in the case of aVR lead ECG ST segment NSTE -ACS patients is of important value in evaluation of the condition and prognosis.
2.Introduction and analysis on acupuncture education and qualification examination system in U.S.A.

Based on research of the Accreditation Commission for Acupuncture and Oriental Medicine (ACAOM), the National Certification Commission for Acupuncture and Oriental Medicine (NCCAOM) and the related public websites of acupuncture educational institutions, the first hand information was studied, summarized and sorted on the status quo of the educational qualification or the relative authentication of education programs of ACAOM as well as the status of professional qualification and examination of NCCAOM. The education curriculum of the Master degree of Acupuncture, Master degree of Oriental Medicine Science as well as the Doctor degree of Acupuncture and Oriental Medicine were also introduced in this article including student enrollment and graduation in recent years. The developing tendency of the federal unified acupuncture education, management, professional qualification and examination system of U. S. A. were discussed and analyzed as well.

3.Comparative Study on Two Extraction Methods of Glycyrrhizic Acid

Objective To compare diffenrent amounts of the glycyrrhizic acid extracted by two methods-decoct method and Ammonia-water method.Methods The best extraction conditions including amount of affusion water and solvent,extraction time,boiling time,crash time,vibrating time and concentration of ammonia were choosed by orthogonal design.Amounts of the glycyrrhizic acid were determined by ultraviolet spectrophotometry at 252 nm wavelengh.Result The best extraction conditions from decoct method were that amount of affusion wate was 200 mL,extract 2 times,boiling time was 5 min,crash time was 20 s.The best extraction conditions from Ammonia-water method were that amount of 0.4% ammonia was 200 mL,vibrating time was 2 h,extract 2 times.The extraction rate of Ammonia-water method was 32%,and the extraction rate of decoct method was 26%.Conclusion For glycyrrhizic acid extraction and purity,Ammonia-water has superior efficient than decoct method at the best conditions.
4.The Protective Effect of Rhodobryum giganteum (Schwaegr.) Par. on Umbilical Vascular Endothelial Cells and the Influence on Secretion of NO and NOS

Objective: To study the effect and mechanism of Rhodobryum giganteum (Schwaegr.) Par. 's anti-atherosclerotic effect. Methods:Vascular endothelial ceils were cultivated and the H2O2 were used to induce the oxidative stress injury of human umbilical vascular endothelial cells (HUVEC). Water extract of Rhodobryum giganteum (Schwaegr.) Par. Was added to cultivated HUVEC, and the activity of the cells was carefully determined by OD value with MTF method. The Griess Reagent was used to detect the NO concentration of different groups. At the same time, the NO fluorescence analysis probe, DAF-FM DA. (3-amino, 4-aminomethyl-2', 7'-difluorescein, diacetate), was used to determine the activity of NO synthase. Results:The most suitable stimulating concentration of H2O2 on HUVEC is 12.5 mmol· L-1. Water extract of Rhodobryum giganteum (Schwaegr.) Par. Was co-cultured with HUVEC damaged byH2O2 .The OD values indicate that 3.33,2.50 mg· mL-1 water extract groups increase activity of the cells significantly compared with the model group (P <0.05) ,and 3.33 mg·mL-1 is much better than 2.50 mg·mL-1 dose group. To determine the content of NO and NOS, the 2.50 mg·mL-1 dose has significant effect (P < 0.05). Conclusions: The H2O2 concentration of 12.5 mmol· L-1 could be used to establish the injured model of endothelial cell successfully. 2.50 and 3.33 mg·mL-1 water extract of Rhodobryum giganteum (Schwaegr.) Par. Have protective effect on ECV304 injured by H2O2. 2.50 mg· mL-1 water extract of Rhodobryurn giganteum (Schw aegr.) Par could increase the activity of NOS and promote the synthesis and secretion of NO.
5.Comparison of set-up errors detected by EPID and CBCT using two different immobilization ;techniques for patients with nasopharygeal carcinoma

Background and purpose: With the development of therapy equipments and technology, the treatment for nasopharyngeal carcinoma(NPC) has entered into the era of precision radiotherapy, and setup errors have become a very important factor affecting treatment effects. The purpose of this study was to analyze the set-up errors detected by the kilovoltage cone beam CT(EPID) and the megavoltage electronic portal imaging device(CBCT) using 2 kinds of different immobilization techniques (pillow+head neck shoulder mask and vacuum bag+head neck shoulder mask) for NPC patients. Methods:A total number of 40 NPC patients were randomly assigned into 2 groups (pillow+head neck shoulder mask group and vacuum bag+neck shoulder mask group). Then each group was further divided into CBCT scan group and EPID group for veriifcation before treatment delivery. We matched the EPID images with the DRRs and acquired the set-up errors in x, y, z axis. Setup errors of CBCT were calculated according to its matched and planned CT images in left-right (x), superior-inferior (y) and anterior-posterior (z) directions. Paired t-test was used to evaluate the differences. Results:In the pillow+head neck shoulder mask group, the set-up errors of CBCT in the x, y, z axis were x (0.67±2.01)mm, y (0.51±1.71)mm and z (0.57±2.04)mm, respectively. The errors of EPID were x (0.69±2.19)mm, y (0.54±2.03)mm and z (0.61±2.11)mm. In the vacuum bag+head neck shoulder mask group, the set-up errors of CBCT in the x, y, z axis were x (0.42±1.81)mm, y (0.33±1.55)mm and z (0.50±1.75)mm, respectively. The errors of EPID were x (0.44±1.87)mm, y (0.43±1.70)mm and z (0.54±1.77)mm. The vacuum bag+head neck shoulder mask ifxed technique was more accurate when compared to the pillow + head neck shoulder mask ifxation method (P<0.05). Conclusion:CBCT and EPID were similar in detecting set-up errors for the NPC patients. However, the vacuum bag+neck shoulder mask ifxed technique was more accurate when compared to the pillow+head neck shoulder mask ifxation method.

Sirtuin 1 (SIRT1),the Ⅲ class deacetylation enzyme,is a kind of NAD + dependent histone deacetylation enzyme.The role of SIRT1 in tumor has the duality.It can inhibit inflammation,tumor angiogenesis and interact with tumor related gene to inhibit tumor development.However,it can also regulate tumor related genes,and epithelial-mesenchymal transition,promote tumor cell proliferation and tolerance of radiotherapy and chemotherapy,and maintain the stemness of cancer stem cells to promote tumor proliferation,invasion and metastasis.
7.Novel small-molecule CDK2-cyclinA2 inhibitors: design, synthesis, and biological evaluation

Objective · To design and synthesize a series of benzenesulfonamide derivatives, test their inhibitory activity to CDK2-cyclinA2 kinase, and investigate the structure-activity relationship. Methods · Virtual screening was executed via computer-aided drug design according to the ATP binding site in CDK2-cyclinA2 protein crystal. A series of benzenesulfonamide derivatives were designed and synthesized on the basis of the interaction modes between the lead compound and the CDK2-cyclinA2. The biological evaluation of compounds was made through the CDK2-cyclinA2 in-vitro kinase activity detection system. Results · Twenty-nine new benzenesulfonamide compounds were prepared, and their inhibitory activity to CDK2-cyclinA2 was elicited. WZ-026 had the highest inhibitory parameter, which half maximal inhibitory concentration (IC50) was 3.81 μmol/L. Conclusion · By multipurpose utilization of virtual screening, chemical synthesis, and biological activity test, a benzenesulfonamide compound WZ-026 was found, which has great inhibitory activity towards CDK2-cyclinA2. Preliminary structure-activity relationship of compounds was obtained.
8.Peptide Labeling with Improved ~(18)O Incorporation Method

In order to optimize the ~(18)O labeling method, two key aspects, peptide dispersion and trypsin deac tivation were discussed o The addition of Rapigest SF in H_2~('8)O and microwave heating enhanced labeling efficiency of α-casein digested peptides(~(18)O/~(16)O) ratio >99%).Chemical modification with tris(2-carboxyeth yl) phosphine (TCEP) and iodoacetamide (IAA) resulted in trypsin deactivated completely.No significant back-exchange from ~(18)O to ~(16)O was observed after labeling in 6 days.The experiment result with peptide mixture from showed that the improved method could be effectively used to label protein and peptide.

10.Identification of pelvic dose-volumetric parameters that predict acute bone marrow suppression in concurrent chemoradiotherapy for cervical cancer

Objective To identify the dose-volumetric parameters associated with acute bone marrow suppression in concurrent chemoradiotherapy for cervical cancer,and provide the reference standard for the radiotherapy planning.Methods In total,155 patients concurrently receiving chemoradiotherapy for cervical cancer were enrolled in this study.The pelvis was contoured for each patient in radiotherapy treatment planning system and divided into three subsites:lumbosacral spine,ilium,and lower pelvis.The pelvic dose volume parameters were analyzed using univariate analysis (Chi-Square and t test),and multivariate analysis (Logistic regression model).Dose volume parameters include V5,V10,V15,V20,V25,V30,V35,V40,V45,V50 and the average dose (D).Results The percentage of patients that developed acute bone marrow suppression (≥ grade 2) was 87.7% (136/155).The univariate analysis revealed that pelvic V5,V10,V15,V,0,ilium V15,lower pelvis V15 of patients with acute bone marrow suppression (≥ grade 2) were significantly higher than those of < grade 2 patients (t =-2.277,-2.142,-3.475,-2.018,-2.963,-2.741,P <0.05).Multiple regression analysis indicated that pelvic V15 was associated with acute bone marrow suppression (OR =1.728.P < 0.05).The threshold of pelvic V15 was 88% as determined by receiver operating curve (ROC).Conclusions The results show that Pelvic V15 is associated with acute bone marrow suppression in concurrent chemoradiotherapy for cervical cancer and is thus an independent risk factor.To better predict and control acute bone marrow suppression,pelvic V15 should be carefully controlled below 88% in treatment planning to reduce the incidence of acute bone marrow suppression.
