1.Determination and Clinical Analysis of Blood-Lipid in Patients with Acute Cerebrovascular Disease

Objective:To discuss the relationship between blood-lipid change and acute cerebrovascular disease (ACVD) and tocompare the blood-lipid levels in the patients with cerebral hemorrhage (CH group) with those in the patients with cerebralinfarction (CI group). Methods:The blood-lipid levels in 300 patients with ACVD (ACVD group) and 100 patients withother system disease (control group) were tested with automatic biochemistry analysor and compared between the twogroups. Results:There were significant differences between ACVD group and control group as well as between CH group andCI group. Conclusion:Although there were blood-lipid metabolic disturbances in both CH and CI patients, their degrees weresignificantly different, which should be closely observed and controlled in clinic.
2.Literature review and comments on implementation outcomes of the essential medicine system in China

Objective A systematic review of implementation outcomes of the essential medicine system in China to identify scientific evidences for a better system.Methods A systematic review is made to extract data from the research papers on outcomes of the essential medicine system,followed by an analysis and description of such data.Results Of the 87 papers included,most of them focused on primary care institutions,while four of them on residents or patients,and one of them on pharmaceutical enterprises.The study found the medical institutions with rising availability of essential drugs,lowered medicine costs,rising or dropping business volume,and apparent drop of out-of-pocket expenses for patients.These have encouraged rational drug use.Evidences in hand indicate expected outcomes from the essential medicine system.Conclusion Current researches on the system focus on primary care institutions in developed areas in China,lacking rigorous design.Studies of broader scale,further depth and more rigorous designs of the implementations of the system are recommended for evaluation of the impacts and outcomes of the system on various stakeholders of the policy.
3.Evaluate the Influence of Monochrome Display on the Detection of Pulmonary Nodules Based on ROC Theory

Objective Evaluate the influence of medical monochrome liquid crystal display (LCD) with different resolutions on the detection of pulmonary nodules.Methods Ninety-three direct digital radiography (DR) chest images were selected on-line from picture archiving and communication system (PACS),including 38 positive,32 suspected and 23 normal cases.All of the images were confirmed by CT.These positive images were divided into group A and B according to the diameter of nodules.Three experienced radiologists blindly read the 93 images using three types of displays with different resolutions.Regarding the presence of nodule,five-point confidence system was used:definitely absent,probably absent,indefinite,probably present and definitely present.All observers marked their confidence levels of each image according to the presence of pulmonary nodule on different displays.Software SPSS 13.0 was employed for statistical analysis.Results In the group-A cases,the areas under ROC curves were 0.643,0.686 and 0.739,and in the group-B cases,those were 0.813,0.832 and 0.846 respectively.There was no difference in the detection efficacy among different radiodiagnostic systems.Conclusion Regardless of post-processing tools,it was equivalent to detect pulmonary nodules with different diameters of Group A or B among medical monochrome LCDs with different resolutions.
4.Therapeutic effects of smecta on multiorgans injury induced by paraquat in rats

ObjectiveTo evaluate therapeutic effect and the possible mechanism of smecta on paraqual plasma concentrations and multiorgans injury induced by paraquat intoxication in rats. Methods A total of 76 healthy adult SD rats were randomly ( random number) divided into group A (control group n =6),group B ( poisoned group n =30 ),group C (smecta-treated group n=30).Rats in groups B and C were treated intragastrically with PQ at 50 mg/kg,the rats in the group C were given with smecta at 50 mg/kg,while the rats in the other two groups were only intragastrically adminstered with saline.Live rats in groups B and C were sacrificed at 2,6,24,48,72 h after administration of PQ for the determination of paraquat plasma concentrations and for HE staining of lung,stomach and jejunum.The rats were executed at the end of trial by the same way in group A.All measurement data were expressed as means + standard deviation ((x) ±s).The data of pathological score were compared with Independent-samples T test and the data of PQ concentration compared with analysis of variance (ANOVA) followed by LSD-t multiple comparison test.P-values of less than 0.05 were considered statistically significant.ResultsThe paraquat plasma concentration ( ng/ml ) was 440.314 ± 49.776 to 4320.6150 ± 413.947.There were different pathological changes of lung,stomach and jejunum in group B. Lung injuries gradually deteriorated,congestion,edema,leukocyte infiltration,incrassated septa and lung consolidation were observed.The pathological changes were obvious such as abruption of mucosa,hyperemic gastric mucosa and leukocyte infiltration in stomach.Haemorrhage of jejunum mucosa,abruption of villus,gland damage and inflammatory cell infiltration were found. Compared with group B,all the pathological changes mentioned above were obviously alleviated in group C ( P < 0.05 ),and the concentrations reduced ( P < 0.01 ).Conclusions Smecta reduced paraquat plasma concentrations and alleviated pathologic injury of rats with PQ poisoning.
5.Optimized formulation of diammonium glycyrrhizinate sustained-release tablet by Box-Behnken design

Objective To investigate the preparation technique and optimal formulation of diammonium glycyrrhizinate sustained-release tablets and study the release mechanism of diammonium glycyrrhizi-nate release from the tablet.Methods Using HPMCk4m,Compritol 888 as skeleton material to prepare the diammonium glycyrrhizinate sustained-release tablet.To optimize the formulations by Box-Behnken design,and use Design Expert program for statistical analysis of experimental results.Release mechanism of diammonium glycyrrhizinate from sustained release tablets was established by equation fitting.ResultsThe average accumulate release rates of diammonium glycyrrhizinate sustained-release tablets in 2,4,8,and 12 h were 32.55%,49.94%,73.88%,and 97.89%.And the release of diammonium glycyrrhizinate could be controlled by diffusion associated with erosion.Conclusion Box-Behnken design could be successfully used to optimize the sustained-release tablet of diammonium glycyrrhizinate.
6.Study of P300 in senile patients with depression: a meta-analysis of domestic literatures

Objective To assess the latency and amplitude changes in P300 in senile patients with depression.Methods 13 domestic published study literatures meeting our criteria were searched from CBM,CNKI,and a meta-analysis on P300 latency and amplitude was performed with RenMan 4.1 soft ware.Results 1.The P300 latency in senile patients with depression was significant longer than that in senile normal controls in N1,P2,N2,P300 (N1:WMD=3.07,95 % CI:0.7~5.42,Z=2.55,P=0.010;P2:WMD=18.41,95%CI:15.11~21.71,Z=10.93,P< 0.01;N2:WMD=25.41,95%CI:12.80~29.02,Z=13.80,P<0.01;P3:WMD=32.14,95%CI:24.14~34.88,Z=23.04,P<0.01).2.The amplitude in senile patients with depression was significant lower than that in senile normal controls in N2,P2,P3 (P2:WMD=-0.83,95%CI:-1.07~-0.59,Z-6.78,P<0.01; N2:WMD=-0.34,95%CI:-0.59~-0.08,Z=2.57,P=0.01;P3:WMD=-2.54,95%CI:-2.75~-2.33,Z=23.99,P<0.010).Conclusions P300 longer latency and lower amplitude are the statistically characterized features for senile depressive patients.
7.Inhibitation of proliferation of human acute myelogenous leukemia cells in vitro by celastrol

Objective The current study was designed to investigate the inhibitive effect of celastrol on the proliferation of acute myelogenous leukemia (AML) cells. Methods The effect of celastrol on AML cells in vitro was examined by MTT assay and flow cytometry (FCM). Results After the AML primary cells were treated with different concentrations of celastrol, the results were analyzed by MTT. The growth of the cells were significantly inhibited compared with the control group (P <0.01). The results detected by FCM showed that the apoptosis was seen after treated with celastrol with different concentration for different times.The apoptotic rates were significantly higher than that in the control group with statistical significance (P< 0.01).Conclusion Celastrol could inhibit the proliferation of AML cells in vitro, which may be associated with inducing cell apoptosis.
8.Influence of Guishaotongluo on angiogenesis of adventitial vasa vasorum and oxidative stress in early stage of atherosclerosis

Aim To observe the effect of Guishao-tongluo ( GSTL ) on the angiogenesis of vasa vasorum and oxidative stress in the early stage of atherosclero-sis. Methods The rabbits ( n =84 ) were randomly divided into 7 groups (n=12):control group,high-fat group, adventitial injury group, GSTL high(GH)and medium ( GM ) dose group, atorvastain group ( ATO ) , and Tongxinluo group ( TXL ) . The normal group was fed with common foodstuffs, and high-fat foodstuffs for the high-fat group to establish an early model of hyper-lipidemia, and all the other groups were fed with high-fat diet combined with carotid artery cannula to build early atherosclerosis carotid artery injury rabbit mod-els. The GSTL high and medium dose was given Guishaotongluo ultrafine powder 4. 16,2. 08 g·kg-1 · d-1 respectively. The atorvastain group and Tongxinluo group were given suspension of atorvastain solution 2. 5 mg·kg-1 ·d-1 , Tongxinluo supermicro powder 0. 6 g ·kg-1 ·d-1 . All groups were treated with gastric per-fusion for 4 weeks. Biochemical method was applied to detect blood lipid change. HE staining was used to ob-serve the pathological morphology of intima-media. Aactivity of serum superoxide dismutase( SOD) ,malon-dialdehyde ( MDA ) content and the total antioxidant capacity ( T-AOC ) in artery serum were detected. NADPH subunits p22phox mRNA, gp91phox mRNA in carotid arteries were located and semi-quantitated by fluorescence in situ hybridization. The expression of VEGF, VEGFR-2 in the carotid artery adventitia was detected by Western blot. Results Compared with normal group,the contents of TC,TG and LDL-C were significantly increased, and VEGF, VEGFR-2 protein levels were remarkly increased in high-fat and adventi-tial injury group. The carotid artery injuries,the degree of angiogenesis of vasa vasorum and NADPH subunits p22phox, gp91phox mRNA in adventitia tissue of the GH,GM, ATO and TXL group were milder in varying degrees compared with those of the vasa injury group. Also the activity of SOD,T-AOC increased,while MDA content,VEGF,VEGFR-2 protein levels were remarkly decreased ( P < 0. 5 or P < 0. 01 ) . Conclusions GSTL can inbibit adventitial neovascularization in the early stage of atherosclerosis, and its mechanism might be related to the increase of total antioxidant capacity of the vascular system and adventitia tissue.

10.Pathway analysis of community doctors' motivation mechanism against the essential medicine system

Objective To identify the impact factors for community doctors with the essential medicine system in place,in support of mobilizing them and successful implementation of the essential medicine system in the community.Methods Based on the theory of ERG hierarchy of needs,the pathway analysis method was called into play to identify the three factors affecting community doctors,namely personal survival,interpersonal relations and self-development.Results Major factor of which is the interpersonal factor,with a score of 0.549,while the latter two factors do not directly affect the survival and development.The two however work on each other,and work on the interpersonal factor as well,with the impact coefficients of 0.320 and 0.402 respectively.Conclusion With the essential medicine system in place,the largest impact factor for community doctors'enthusiasm is the interpersonal one,involving the doctor-patient communications among others.This serving as the breakthrough point may improve the enthusiasm of the community doctors,paving the way for the system in the communities.
