AIM:To investigate the effect of different cutting centers on the visual acuity, refractive diopter and visual quality of patients undergoing laser assisted in situ keratomileusis (LASIK).METHODS: A total of 80 patients (160 eyes) with myopia treated by elective LASIK were divided into two groups.Thirty-six cases (72 eyes) with visual axis corneal reflection point (VACRP) as the cutting center were included into the VACRP group while 44 cases (88 eyes) with pupil center (PC) as the cutting center were included into the PC group.The uncorrected visual acuity (UCVA), the best corrected visual acuity (BCVA), refractive diopter, corneal aberration [total corneal and anterior corneal surface higher-order aberrations (HOA), spherical aberration (Z40), vertical coma (Z3-1), horizontal coma (totZ31) and offset of cutting centers were determined before surgery and 1mo after surgery.RESULTS: There was no difference in the probability of UCVA ≥ 0.1, BCVA and refractive diopter between the two groups at 1mo after surgery (P>0.05).The astigmatism and cutting center deviation of VACRP group were lower than those of PC group (P<0.05).The totHOA, totZ40, totZ3-1, totZ31, froHOA, froZ3-1、froZ31 and froZ40 were lower in VACRP group than PC group at 1mo after surgery (P<0.05).CONCLUSION: The UCVA of patients treated with both cutting centers for LASIK is good but VACRP has more advantages in reducing the offset of cutting center and improving postoperative visual quality.

At present,diagnostic programs of food allergy in children mainly include medical history,skin prick test,detection of serum specific immunoglobulin E (sIgE) antibody,food challenge test,and so on.Among them,skin prick test and detection of sIgE antibody are used as primary screening tests with high diagnostic sensitivity while low specificity.Additionally,double-blind placebo-controlled food challenge test is the gold standard for the diagnosis,but it is faced with a potential risk of causing systemic allergic reactions,resulting in its rare application in clinic.Moreover,basophil activation test,as an in vitro test for allergen diagnosis,has significant value in diagnosis of food allergy in children due to its high specificity and safety.Besides,basophil activation test can also be used to evaluate the severity of food allergy in children,monitor immune therapy,detect trace allergens,and so on.

Objective To investigate the value of dynamic contrast-enhanced MRI (DCE-MRI) quantitation study with exchange model in evaluation of liver cirrhosis.Methods A total of 30 patients with liver cirrhosis confirmed to included and excluded criteria were divided into compensatory cirrhosis group (n=15) and decompensatory cirrhosis group (n=15).And 15 healthy volunteers were scrvcd as a control group.All subjects were performed liver DCE-MRI and quantitative pa rameters of three groups were obtained by exchange model,including volume transfer constant of the contrast agent (Ktrans),reverse reflux rate constant (Kep),volume fraction of EES (Ve),volume fraction of plasma (Vp),arterial perfusion index (HPI),blood volume (BV),blood flow(BF) and mean transit time (MTT).The parameters of three groups were compared by ANOVA analysis and LSD test for every two groups.The diagnostic efficacy of Ktrans,Ve,HPI,MTT for liver cirrhosis were analyzed by ROC.Results Ktrans,Ve,HPI,MTT and BF values in the control group,compensatory cirrhosis group and decompensatory cirrhosis group were (0.85 ± 0.34) min-1,(0.67 ± 0.36) min-1,(0.39±0.18)min-1;0.47±0.42,0.34±0.13,0.16±0.06;0.35±0.09,0.57±0.17,0.76±0.20;(17.38±15.38)s,(34.26111.37)s,(57.33±16.73)s;(97.22±36.29)ml/(min· 100 g),(69.30±35.22)ml/(min· 100 g),(24.64± 28.24) ml/(min · 100 g),respectively,and showing significant differences among three groups,and significant with HPI and MTT for every two groups (P＜0.05).There were no significant differences for Kep,Vp and BV among three groups (P＞0.05).According to ROC of Ktrans and Ve for decompensatory liver cirrhosis,AUC were 0.86,0.92,the sensitivity were 90％,90％ and the specificity were 80％,80％.According to ROC of HPI and MTT for compensatory liver cirrho sis,AUC were 0.79,0.76,the sensitivity were 85％,85％ and the specificity were 75％,75％.According to ROC of HPI and MTT for decompensatory liver cirrhosis,AUC were 0.91,0.88,the sensitivity were 95％,90％ and the specificity were 80％,80％.Conclusion DCE-MRI quantitative study with exchange model can obtain multiple parameters,refleet hemodynamic changes and vascular microenvironment and evaluate severity of liver cirrhosis.

Objective To evaluate the expression of serum vascular endothelial growth factor(sVEGF) in differentiated thyroid carcinoma(DTC)patients before and after thyroidectomy and when recurrence occured.To analyze the relationship between serum VEGF and serum thyroglobulin(Tg)level.Methods sVEGF levels of 30 cases of DTC (7 patients with pulmonary metastasis,23 patients with locoregional recurrence)were determined by enzyme-linked immunosorbent assay(ELISA) before and after thyroidectomy and when recurrence occured.The levels of serum Tg and thyroglobulin antibodies (TgA) were detected by chemiluminescence method.The results were compared with those of the 30 cases of normal controls.Results sVEGF levels had no significant difference between pulmonary metastasis group and locoregional recurrence group before operation (t =0.373,P ＞ 0.05),but they were significantly higher than those in the normal control group(t =8.597 and 14.14 respectively,all P ＜ 0.05).sVEGF levels in patients with pulmonary metastasis or locoregional recurrence were significantly reduced at the 3rd week after operation and the difference had statistical significance with those before operation(t =5.977,11.50,P ＜ 0.05).The difference of postoperative level of sVEGF in patients with pulmonary metastasis,locoregional recurrence and in patients of normal control group had no statistical significance (t =1.593,1.525,P ＞ 0.05).sVEGF levels in patients with pulmonary metastasis or locoregional recurrence were significantly higher after recurrence than those before recurrence (t =7.387,12.16,P ＜ 0.05).After recurrence sVEGF levels in patients with pulmonary metastasis were significantly higher than those in patients with locoregional recurrence (t =4.167,P ＜ 0.05) It showed a positive linear correlation between serum Tg level and sVEGF level in DTC patients with recurrence(r =0.786,P ＜ 0.01).Conclusion sVEGF is significantly elevated in DTC patients before operation and with postoperative recurrence,making it an important indicator to reveal the biological behaviors of DTC.

Objective To investigate the surgical method and clinical curative results of acetabulum top compression fractures with sea gull sign. Methods Data of 14 patients who had acetabulum top compression fractures with sea gull sign were retrospectively analyzed and were accepted surgical therapy and followed up. There were 5 females and 9 males, aged from 28 to 71 years (average, 49.9 years). The pre-operative time was from 4 to 14 days (average, 9 days). Letournel-Judet fracture classifica-tion:eight cases for the anterior column+posterior semi-transverse, six cases for a simple anterior column. Three patients with lum-bar compression fracture, two patients with tibial plateau fracture, one patient with multiple rib fractures. All patients were accept-ed open reduction and bone graft and internal fixation through ilioinguinal approach or ilioinguinal approach+Kocher-Langenbeck approach. The quality of fracture reduction was assessed according to the Matta reduction criterion after operation. The hip func-tion was evaluated according to Matta. Results 14 patients were followed up from 6 to 60 months (average, 36 months). All pa-tients reached bone healing;healing time was from 3 months to 4 months (average, 3.4 months). According to the Matta reset stan-dard after operation, there were 6 excellent cases, 5 good cases, 2 fair cases, 1 poor case, the excellent and good rate was 78.5%(11/14). The Matta was from 10 to 18 scores (average, 16.4 scores), 5 excellent cases, 5 good cases, 3 fair cases, 1 poor case, the ex-cellent and good rate was 71.4%(10/14). Traumatic arthritis occurred in 3 patients. Pain was serious in two patients and disap-peared after total hip joint replacement. Pain was mild for one patient and disappeared after accepted non steroidal painkiller. One patient had heterotopic ossification after operation, which was not treatmented because of no obvious symptom. Conclusion The appropriate operation time of acetabular roof compression fracture with sea gull sign was from 5 to 10 days after fracture, which was no more than two weeks had best. Through ilioinguinal approach or ilioinguinal approach+Kocher-Langenbeck approach, the articular surface could be reduced excellent and got sufficient bone graft. The clinical efficacy was satisfactory after operation.

Objective To evaluate the expression of vascular endothelial growth factor (VEGF)and endostatin (ES) in the serum of patients with differentiated thyroid cancer at the time of before and after operation and postoperative recurrence. Methods The serum samples were obtained from 7 patients with postoperative lung metastasis and 21 patients with local recurrence after operation.The serum samples from 30 normal subjects were obtained as control.The levels of serum VEGF and serum endostatin were analyzed by enzyme-linked immunosorbentassay (ELISA).Thelevelsofserumthyroglobulinwereanalyzedby Chemiluminescence method.Statistical analysis was performed with t-test using SPSS 13.0 software.Pearson correlation analysis was performed for the correlation between serum VEGF and serum ES and thyroglobulin.ResultsThe levels of serum VEGF and serum ES in patients with lung metastasis 3 weeks after operation were significantly lower than those before operation and at the time of recurrence [ VEGF:( 210.3 ± 30.4) ng/L vs (412.6 ±57.3) ng/L and(619.5 ± 126.4) ng/L,P ＜0.05 ;ES:(25.2 ±6.2)ng/L vs (34.3 ±7.6)ng/L and (38.6 ± 8.7) ng/L,P ＜ 0.05 ].And the levels of serum VEGF and serum ES in patients with local regional recurrence 3 weeks after operation were significantly lower than those before operation and at the time of recurrence [ VEGF:(209.3 ±36.7) ng/L vs (399.4 ±56.3) ng/L and (406.5 ±59.2) ng/L,P ＜0.05;ES:(25.7 ± 4.7 ) ng/L vs ( 35.2 ± 6.8 ) ng/L and ( 31.2 ± 7.6 ) ng/L,P ＜ 0.05 ].The expression of serum VEGF and serum ES in patients with pulmonary metastasis were significantly higher than those in patients with local regional postoperative recurrence ( P ＜ 0.05 ).There was a linear positive correlation between serum VEGF,serum ES and thyroglobulin levels in the patient with recurrent differentiated thyroid cancer( r =0.752,0.349,P ＜0.01 ).Conclusion The serum VEGF and serum ES level were significantly elevated in patients with differentiated thyroid cancer before operation and recurrence.The serum VEGF and serum ES were importantindicators to reveal the biological behaviors of differentiated thyroid cancer.
