1.Clinical value of three kinds of helicobacter pylori detection methods

Objective To discuss the clinical value of three kinds of helicobacter pylori (HP) detection methods and find out the appropriate method for clinical application of the HP detection .Methods A total of 109 patients received gastroscopy ,the efficacy of RUT ,13C-urea breath test(13C-UBT) and the immunoCard STAT helicobacter pylori stool antigen (HpSA) for detection of HP were compared .Results RUT positive rate of the two pieces of gastric mucosa (the gastric antrum and the gastric body) was 34 .86% ,higher than that of single piece of gastric mucosa (gastric antrum or stomach body ) and two pieces of gastric mucosa (stomach) ,the difference was statistically significant (P<0 .05);The diagnosis of HP infection was based on 13 C-UBT ,sensitivity and accuracy of the two pieces of gastric mucosa (the gastric antrum and the gastric body) were 72 .97% and 80 .73% ,respectively , higher than that of two pieces of gastric mucosa and gastric antrum ,and lower than the four pieces of gastric mucosa (two pieces of gastric antrum + two pieces of the gastric body) .There was no statistically significant difference among RUT ,13 C-UBT and the immunoCard STAT HpSA(P>0 .05) .The diagnosis of HP infection was based on 13C-UBT ,the immunoCard STAT HpSA sensi-tivity ,specificity and accuracy were 86 .49% ,95 .83% ,92 .66% ,respectively ,which were higher than RUT .Conclusion Two pieces of gastric mucosa (the gastric antrum and the gastric body) materials is appropriate for clinical promotion RUT based solution . RUT ,13C-UBT and hpsas immune quick check card are all clinical detection of HP and reliable methods ,but hpsas immune quick check card is more suitable for clinical promotion .

4.Expression changes of Toll-like receptor 4 and its down-stream cytokines in incisional tissues from rats with postoperative pain

Objective To evaluate the changes in mRNA expressions of Toll-like receptor 4 (TLR4) and its down-stream cytokines IL-1β,IL-6 and TNF-a in incisional tissues from a rat with postoperative pain.Methods Incisional pain was induced in 74 male adult SD rats weighing 200-250 g.Paw mechanical withdrawal threshold (PMWT) around the wound on the operated and nonoperated sides was measured at 1 day before operation and at 0.5,1,2,6 and 12 hours as well as 1,2,3,5 and 7 days after operation.Skin incisional tissues were removed for determination of mRNA expressions of TLR4,IL-1β,IL-6 and TNF-a using real-time quantitative PCR at 1 day before operation and at 2 and 8 hours as well as 1,2,3,5 and 7 days after operation.Results Compared with the baseline value before operation,PMWT on the operated side was significantly decreased at 0.5 hours-5 days after operation,mRNA expression of TLR4 around the wounds on the operated side was down-regulated at 2 hours after operation followed by a gradual increase,mRNA expressions of IL-1β,IL-6 and TNF-α on the operative side were up-regulated at 2 and 8 hours as well as 1,2,3 and 5 days after operation (P < 0.05),but no significant changes were found in PMWT and mRNA expressions of TLR4,IL-1β,IL-6 and TNF-α on the non-operated side(P > 0.05).PMWT on the operated side was lowest at 6 hours after operation followed by the gradual increase,mRNA expression of TLR4 on the operated side peaked at 2 days after operation,and mRNA expressions of IL-1β,IL-6 and TNF-a respectively peaked at 2 hours,1 day and 3 days after operation (P < 0.05).mRNA expressions of TLR4,IL-1β,IL-6 and TNF-a were negatively correlated with PMWT on the operative side (r =-0.501,-0.743,-0.893,-0.657,P < 0.05),and mRNA expressions of IL-1 β,IL-6 and TNF-a were positively correlated with the level of TLR4 mRNA(r=0.764,0.283,0.667,P<0.05).Conclusion mRNA expressions of TLR4 and its down-stream cytokines IL-1 β,IL-6 and TNF-a in skin incisional tissues are up-regulated,which may be involved in the development and maintenance of postoperative pain.
5.The diagnostic and prognostic value of CT scans in patients with acute pancreatitis complications

Objective To analyze the diagnosis and prognosis value of abdominal CT scans in patients with acute pancreatitis complications.Methods 151 cases with acute pancreatitis were selected.The relationship between abdominal CT performances and the common complications and death in patients was analyzed.The prognosis of patients with different Balthazar CT severity index (CTSI) grade was compared.Results The complication rate of patients with CT signs of fatty liver, pleural effusion, liver gap effusion, adrenal gland involvement (AGI), penirenal space involvement (PSI) and gastric bare area involvement (GBAI) was significantly higher than that of patients with negative CT findings above (P<0.05).With CTSI grading increasing, the patient''s fasting time, heating time, hospital stay, recovery time of blood amylase were extended, and the incidence of pseudo cyst, transit surgery, organ failure and death was gradually increasing (P<0.05).Conclusion Severe fatty liver, AGI, GBAI, PSI and liver gap effusion are risk factors for acute complications and death in patients with pancreatitis.
6.Determination content of baicalin in Yinhuang granules from different pharmaceutical factories by near-infrared spectroscopy.

OBJECTIVETo determine the content of baicalin in Yinhuang granules fastly from different pharmaceutical factories with near-infrared spectroscopy.
METHODNear-infrared diffuse reflectance spectroscopy technology was used to collect NIR spectra of Yinhuang granules. With HPLC anilysis values as reference, the fast determination method of baicalin was established with partial least squares (PLS).
RESULTThe correlation coefficients (R2), the root-mean-square error of cross-validation and the root-mean-square error of calibration (RMSEC) of the calibration model for baicalin were 0.9982, 0.1899 and 0.0514, respectively; the root-mean-square error of prediction (RMSEP) and the average rate of recovery were 0.0802 and 104.27%.
CONCLUSIONThe method is accurate, fast and simple, and can be applied for the fast determination of large quantities of numbers of samples.

7.Occlusal contacts in intercuspal position after orthodontic treatment.

OBJECTIVETo observe the occlusal contacts in intercuspal position(ICP) in adolescent patients after orthodontic treatment.
METHODSFourteen adolescent patients after orthodontic treatment were selected as experimental group and 17 healthy adolescents and young adults with normal occlusion were selected as control group. The occlusal contact in ICP in the two groups were recorded with T-Scan II system and compared.
RESULTSWhen the total of force (TOF) were 1/10 TOFmax, 1/4 TOFmax, 1/2 TOFmax, 3/4 TOFmax and TOFmax, the number of occlusal contacts in the anterior segment in the experimental group were 1.5(1.0), 3.0(1.3), 4.5(3.3), 5.5(3.0), 5.5(3.3), which were significantly larger than that in control group [0.0(0.3), 0.0(0.6), 0.3(0.5), 0.3(1.0), 0.0(1.8), P < 0.05]. There were no significant difference in the area of occlusal contacts in the posterior segment between experimental group and control group (P > 0.05). The center of force (COF) in the experimental group located significantly more mesially than that in control group. The value of ICP contact time in experimental group was 0.192(0.141) s and no significant difference was found between experimental group and control group [0.163(0.200) s].
CONCLUSIONSAlthough the patients achieved normal occlusion after orthodontic treatment, their anterior teeth received more occlusal force than those in healthy adolescents and young adults with normal occlusion.

8.Three-dimensional analysis of craniofacial growth in adolescents with normal occlusion in Beijing.

OBJECTIVETo analyze craniofacial growth three-dimensionally for adolescents with normal occlusion in Beijing.
METHODSOne hundred and twenty-six adolescents with normal occlusion were selected according to the criteria. The sample was divided into four age groups (53 within 4 years, 30 within 7 years, 27 within 10 years and 16 within 13 years). Information of growth was collected. Three-dimensional cephalometric system based on cone-bean CT was established.
RESULTSFrom 4 to 13 years Co-A increased (14.55 ± 1.15) mm on average on the left and (13.66 ± 1.14) mm on the right, and Co-Gn increased (22.89 ± 1.40) mm on the left and (22.82 ± 1.38) mm on the right; and U1-NA increased (2.20 ± 0.44) mm on the left and (1.60 ± 0.46) mm on the right; and CoL-CoR and GoL-GoR increased (13.31 ± 1.21) mm and (18.59 ± 1.40) mm, and N-Me increased (18.03 ± 1.32) mm.SN-PP and SN-MPL basically remained unchanged.
CONCLUSIONSAdolescents with normal occlusion in Beijing grew obviously in three-dimensions and developed harmoniously.

9.Determination of forsythoside A in fructus forsythiae by NIR.

OBJECTIVEDetermine the content of forsythoside A in fructus forsythiae by combining near-infrared spectroscopy with the chemometrics method.
METHODHPLC was used as a reference method to determine the content of forsythoside A in fructus forsythiae. Multivariate calibration model based on PLS algorithm was developed to correlate the spectra and the corresponding values determined by the reference method.
RESULTRMSECV (root-mean-square error of cross-validation) of the model for forsythoside A was 0.221 and the correlation coefficients of the calibration model was 0.969. External validation with external validation samples proved that the relative coefficient of the predicted value and the HPLC value was 0.961 and the RMSEP was 0.18.
CONCLUSIONNIRS can be used in the determination of forsythoside A, which sets up the foundation of product-line control of fructus forsythiae.

10.Efficacy and cost analysis of microsurgical clipping and vascular embolization in treatment of patients with cerebral aneurysms

[Objective]To investigate the efficacy and cost analysis ofmicrosurgical clipping and vascular embolization in the treatment of patients with cerebral aneurysms.[Methods]Two hundred and fitly-one patients with cerebral ameurysms,admitted to our hospital from January 2007 to December 2010,were collected in our study;170 patients (174 aneurysms) received microsurgical surgical clipping (clipping group) and other 181 patients (185 aneurysms) were performed vascular embolization (embolization group).Duration of hospitalization,vascular occlusion rate,intraoperative rupture rate,hospital mortality and happening of complications were compared between the 2 groups;and the efficacy were evaluated by modified Rankin scale (mRS) and cost analysis of the 2 treatments were compared 3 months after the surgeries.[Results] The length of hospital stays and cerebral aneurysm rupture rate in patients of the embolization group were significantly shorter/lower than those in the clipping group (P< 0.05);the scores of mRS showed no significant differences between the 2 groups (P>0.05).The cost and sensitivity analysis indicated that the ratio of average cost/good prognosis rate in the clipping group was smaller as compared with that in the embolization group.[Conclusion] The efficacies of microsurgical clipping and embolization in the treatment of cerebral aneurysms are considerable,and both of them have their advantages and disadvantages respectively;however,the microsurgical clipping is cost-effective and economical.
