1.Chemical constituents in carpophore of Xanthoceras sorbifolia

Objective To study the chemical constituents in ethanol extract from the carpophore of Xanthoceras sorbifolia and explore a new potential part.Methods The constituents were isolated by macroporous resin and silica gel column chromatography and their structures were elucidated by the phisi-cochemical properties and spectral evidence.Results Compounds Ⅰ-Ⅵ wre obtained and their structures were identified as 21-angeloyl-24-hydroxy-R1-barrigenol(Ⅰ),23-hydroxybetulinic acid(Ⅱ),3?,23-dihydroxy-lup-20(29) en-28-oic acid-3-caffeate(Ⅲ),22,23-dehydroxy-?-chondrillasterone(Ⅳ),3-oxotirucalla-7,24-dien-21-oic(Ⅴ),and ?-spinasterol-3-O-?-D-glucopyranoside(Ⅵ),respectively.Conclusion Compound Ⅰ is a new natural product and compounds Ⅱ-Ⅳ are isolated from the plants of Xanthoceras Bunge for the first time.

All previous literatures about Chinese herbal medicines show distinctive traditional Chinese medicine (TCM) flavors. Compendium of Materia Medica is an influential book in TCM history. The TCM flavor theory and flavor standardization principle in this book has important significance for modern TCM flavor standardization. Compendium of Materia Medica pays attention to the flavor theory, explain the relations between the flavor of medicine and its therapeutic effects by means of Neo-Confucianism of the Song and Ming Dynasties. However,the book has not reflected and further developed the systemic theory, which originated in the Jin and Yuan dynasty. In Compendium of Materia Medica , flavor are standardized just by tasting medicines, instead of deducing flavors. Therefore, medicine tasting should be adopted as the major method to standardize the flavor of medicine.

5.Control of tip apex distance with K-wire apex distance: reliability of proximal femoral nail anti-rotation in treatment of intertrochanteric fracture

[Objective]To evaluate the reliability of proximal femoral nail anti-rotation(PFNA) in treatment of intertrochanteric fracture using K-wire apex distance(KAD) intraoperatively to control tip apex distance(TAD).[Method]From January 2007 to January 2009,44 cases of intertrochanteric fractures were treated by closed reduction internal fixation with PFNA.Intra-operative KAD and post-operative TAD were measured and analyzed for their correlation.[Result]Follow-up was given to 42 patients for 6-18 months(averaged,10 months).Bone union was achieved after 11-23 weeks(averaged,13 weeks).Intra-operative KAD were 13-31 mm,with an average of 21.75 mm.Post-operative TAD were 18-35 mm,with an average of 24.61 mm.Recurrence of cerebral infarction was found in 1 case.No infection,deep vein thrombosis,intramedullary nailing rupture,screw broken or femoral fracture was found.[Conclusion]With the help of intra-operative fluoroscopy,the distance from the end of Kirschner's wire to joint surface(KAD) plays an important role in controlling TAD,and it could prevent relative complications such as helical blade cutting-out of femoral head.
6.First aid and nursing of 3 patients with cranial orbital injury

Objective To summarize first aid and nursing key points of 3 patients with cranial orbital injury. Methods Three patients with cranial orbital injury were nursed from January 2014 to March 2014, including routine nursing, nursing of eye and cerebrospinal fluid leak and prevention of infection. Results The hospital stay of the 3 patients was from 8 to 23 d, during which 1 contracted cerebrospinal fluid leakage and another rupture of left eye ball. All patients were well recovered after discharge and could be self cared in life for their own, with one patient blind on the left eye and the other 2 with facial disfigurement. Conclusions The patients with cranial orbital injury is critical. Therefore, well done first aid and nursing are key for a better prognosis.
7.Effect of sequoyitol on expression of NOX4 and eNOS induced with glucose in human umbilical vein endothelial cells.

OBJECTIVETo investigate the protective effect and mechanism of sequoyitol (Sep) on high glucose-induced human umbilical vein endothelial cells (HUVECs) injury.
METHODSHUVECs were cultured with high glucose (30 mmol/L) in the presence or absence of sequoyitol (0.1, 1 and 10 micromol/L) for 24 h. Cell proliferation was measured by BrdU marking and cell cycle was detected by flow cytometry. 2', 7'-dichlorofluorescein diacetate was used to evaluate intracellular reactive oxygen species (ROS) levels. The NO, malonydialdehyde (MDA) and H2O2 levels were determined by colorimetric method according to the manufacturer's instructions. The expression of endothelial nitric oxide synthase (eNOS) and NADPH oxidase 4 (NOX4) were measured by real-time PCR and Western blot.
RESULTSIn the present study, we found that sequoyitol pretreatment for 1 h significantly decreased cell injury, promoted cell proliferation. Meanwhile sequoyitol significantly down-regulated NOX4 expression and decreased the level of ROS, MDA and H2O2 and obviously increased NO levels and up-regulated eNOS expression.
CONCLUSIONSequoyitol alleviates high glucose-induced cell injuries in HUVECs via inhibiting oxidative stress and up-regulating eNOS expression.

8.Effect of sequoyitol on expression of NOX4 and eNOS in aortas of type 2 diabetic rats.

The aim of the present study is to investigate the effects of sequoyitol (Seq) on expression of eNOS and NOX4 in aortas of type 2 diabetic rats. Type 2 diabetic rats induced by high fat and high sugar diet and low dose of streptozotocin (STZ, 35 mg x kg(-1)) and were administered Seq (12.5, 25 and 50 mg x kg(-1) x d(-1)) for 6 weeks. The fasting blood glucose (FBG) and body weight were tested. Acetylcholine (Ach) induced endothelium-dependent relaxation and sodium nitroprusside (SNP) induced endothelium-independent relaxation were measured in aortas for estimating endothelial function. Aortic morphological change was observed with HE staining. The level of serum insulin was measured by radioimmunoassay. The total antioxidative capacity (T-AOC), malondialdehyde (MDA) and NO levels in aortas were determined according to the manufacturer's instructions. In addition, the expressions of eNOS and NOX4 in aortas were measured by immunohistochemisty, real-time PCR or Western blotting. The results showed that Seq significantly decreased FBG and insulin resistance, and improved aortic endothelium-dependent vasorelaxation function. The expressions of NOX4 and MDA content were obviously decreased, while the expression of eNOS, the levels of NO and T-AOC increased significantly in aortas of diabetic rats with Seq treatment. In conclusion, Seq protects against aortic endothelial dysfunction of type 2 diabetic rats through down-regulating expression of NOX4 and up-regulating eNOS expression.

9.Diagnosis value of shortwave autofluorescence for cystoid macular edema

Background Cystoid macular edema (CME) is a manifestation secondary to multiple fundus diseases and often leads to the decline of visual acuity.Optical coherence tomography (OCT) is a common diagnostic method for CME,but it can not reflect the metabolism information of retinal pigment epithelium (RPE).Shortwave autofluorescence (SW-AF)may find the abnormality of macula,but its clinical value remains to be confirmed.Objective This study was to investigate the value of SW-AF in the diagnosis of CME.Methods One hundred and eighty-nine eyes of 140 patients who have the relative diseases to CME or received intraocular surgery were included from May 2010 to August 2011 in Lihuili Hospital of Ningbo city.SW-AF,infrared autofluorescence (IRAF),fundus fluorescein angiography (FFA) and OCT were performed in all the eyes under the informed consent of each patient.The sensitivity,specificity and other indicators in diagnostic test of SW-AF for CME were evaluated and compared with OCT.Results SW-AF had certain accuracy in diagnosis of CME with the sensitivity 78.29%,specificity 96.67%,Youden index 0.75,positive predictive value 98.06%,negative predictive value 67.44%.The area under curve (AUC) of SW-AF for the diagnosis of CME was 0.875 with the 95% confidence interval (CI) 0.823-0.926 (P<0.001).Compared with OCT,the differences of SW-AF and IR-AF in the diagnosis for CME was statistically significant(x2 =22.53,91.35,both at P<0.001),and agreement coefficients of diagnosis result between SW-AF、IR-AF or FFA and OCT were 0.67,0.12 and 0.85.Conclusions SW-AF can be used as a new rapid,non-invasive,ancillary technique in diagnosis of CME with certain accuracy,and the diagnostic agreement of SW-AF with OCT is lower than FFA but is higher than IR-AF.
