1.Three-dimensional measurement of femoral neck anteversion

Objective To investigate the evaluation of femoral neck anteversion using the digital three-dimensional model induced determination method.Methods Ten healthy volunteers,including five men and five women,aged 18-60 years (mean,43 years) were recruited.Bilateral femur CT scan was taken and processed with Mimics 17.0 software to build the three-dimensional model of the femur.Traditional Photoshop and three-dimensional reconstruction techniques were separately applied to determine the femoral neck anteversion.Anteversion angle and measurement time determined with the two methods were compared.Results Anteversion angle was (17.0 ± 4.2) ° based on the Photoshop method and was (17.0 ± 4.2) ° based on the three-dimensional method (P > 0.05).Measuring time was (1,231.5 ± 152.8) sec with the Photoshop method and (590.0 ± 37.2) sec with the three-dimensional method (P < 0.01).Conclusion Both methods provide accurate measuring of the femoral neck anteversion,but three-dimensional measurement spends far less time resulting in improved measure efficiency.
2.Glucocorticoid combined with mycophenolate mofetil versus cyclophosphamide in the treatment for adult refractory nephrotic syndrome:a Meta-analysis

Objective To systematically review the efficacy and safety of mycophenolate mofetil versus cyclophosphamide in adults with refractory nephrotic syndrome. Methods The randomized controlled trials of mycophenolate mofetil and cyclophosphamide treatment for refractory nephrotic syndrome were searched from Cochrane library, PubMed, Embase, Wanfang, VIP and CNKI till March 2014. The relevant studies were screened according to inclusion criteria and exclusion criteria. The quality of the included studies was evaluated. Meta-analyses were performed by using RevMan 5.2 software. The indexes were analyzed including the complete remission rate, efficiency, serum albumin, and adverse reaction after completing the treatment for adults with refractory nephrotic syndrome. Results There were 9 RCTs, a total of 467 patients were enrolled. The result of the meta-analysis showed that mycophenolate mofetil could significantly increase complete remission rate (RR=1.45, 95%CI=1.17~1.81, P=0.000 7) and efficiency rate (RR=1.23, 95%CI=1.11~1.36, P<0.000 1). It can also enhance the level of serum albumin (WMD=2.73, 95%CI=1.42~4.04, P<0.000 1) and decrease 24-hour urinary protein (SMD=-0.63, 95%CI=-1.16~-0.10, P=0.02) compared with cyclophosphamide in the treatment of refractory nephrotic syndrome. There was no significant difference in the serum level of cholesterol between mycophenolate mofetil group and cyclophosphamide group (SMD=0.31, 95%CI=-0.23~0.84, P=0.26 ). The incidence rates of liver dysfunction (RR=0.13,95%CI=0.06~0.28, P<0.000 01), leukopenia (RR=0.10, 95%CI=0.04~0.23, P<0.000 01), gastrointestinal reaction (RR=0.21, 95%CI=0.11~0.39, P<0.000 01) and alopecia (RR=0.08, 95%CI=0.02~0.29, P<0.000 01) were significantly lower in mycophenolate mofetil group than those of cyclophosphamide group. There were no significant differences in respiratory tract infection rate (RR=0.68, 95%CI=0.41~1.14, P=0.14) and lung infection rate (RR=0.58, 95%CI=0.31~1.08, P=0.09) between the two groups. Conclusion The safety and efficacy are better in the treatment of refractory nephrotic syndrome using mycophenolate mofetil than that of cyclophosphamide.
3.Analysis of clinical features and risk factors for adult nephrotic syndrome with pulmonary thromboembolism

Objective To investigate the clinical features and risk factors of adult nephrotic syndrome with pulmonary thromboembolism (PTE). Methods Sixty patients diagnosed with nephrotic syndrome and clinically suspected with PTE were enrolled in this retrospective study. Patients were divided into PTE group (n=32) and no-PTE group (n=28) according to the results of computed tomographic pulmonary angiography (CTPA). The single factor analysis and Logistic repres?sion analysis were used to analyse risk factors including age, gender, onset time, clinical symptoms, laboratory examination and pathological types. According to the independent risk factors, the receiver-operating characteristic curve (ROC curve) was used to determine PTE threshold value based on the evaluation index in nephrotic syndrome. Results Single factor analysis showed that there were significant differences in disease duration, hemoglobin, serum albumin, total cholesterol, low-density lipoprotein cholesterol, D-dimer and physical examination in P2 hyperthyroidism between two groups (P <0.05). Logistic regression analysis showed that D-dimer was independent risk factor of PTE. The analysis of ROC curve indi?cated that D-dimer optimal threshold was 1 015.50μg/L. Conclusion D-dimer is an independent risk factor of PTE in pa?tients with nephrotic syndrome. When D-dimer is greater than 1 015.50μg/L, should pay attention to the occurrence of PTE.
4.Application of Controlled Hypotension in Pediatric Patent Ductus Arteriosus Operations

Objective To explore the efficacy and safety of sodium nilroprusside(SNP) with remifentanil or fentanyl used for controlled hypotension during the pediatric patent ductus arteriosus (PDA) operations. Methods Sixty children undergoing operation were randomly divided into SNP with fentanyl (A group) or remifentanil (B group). Hemodynamic changes in different times and adverse reactions were observed. Results In B group, heart rate was stable and blood pressure declined quickly. The dose of SNP was fewer and hemodynamic changes and recovery from anesthesia were better in B group than in A group. Conclusion Remifentanil could potentiate the hypotensive effect of SNP and decrease adverse reaction.
5. Research progress on role of exosomes in acute kidney injury

Exosomes are extracellular vesicles with plasma membrane. They are produced intracellularly and then secreted out of the cells. Almost all types of cells in human bodies can produce and excrete exosomes. Exosomes carry a variety of signal proteins, mRNA, miRNA, lncRNA and their degradation fragments. Due to the protection of lipid membrane, these components have biological activity and get involved in the process of cell migration, apoptosis and proliferation, and can modulate the function of the recipient cells. As an important cell-cell signal carrier, exosomes are involved in the development of many kidney diseases. This article reviewed the biological characteristics of exosomes and their roles in acute kidney injury.

Objective To investigate the distribution characteristics of interleukin-27 gene rs17855750 G/T,rs40837 A/G polymorphism in Zhuang populations of Guangxi,and to compare the distribution differences of genotype and allele frequencies of interleukin-27 gene rs17855750 G/T,rs40837 A/G polymorphisms among different races.Methods The interleukin-27 gene rs17855750 G/T,rs40837 A/G polymorphisms were detected by SNaPshot SNP genotyping technique on 168 persons in Zhuang populations of Guangxi,frequencies of genotype and allele of interleukin-27 gene rs17855750 G/T,rs40837 A/G polymorphisms were analyzed in Zhuang populations,and was compared with the other four populations (HapMap-HCB,HapMap-JPT,HapMap-YRI,HapMap-TSI) from HapMap database.Results The most common genotype and allele of interleukin-27 gene rs17855750 G/T polymorphysms were TT(70.2%) and G(50.3%) in Zhuang populations of Guangxi,and the most common genotype and allele of interleukin-27 gene rs40837 G/T polymorphysms were AC(35.7 %) and C(52.1 %).There were no significant differences in the genotype and allele frequencies of interleukin-27 gene rs17855750 G/T,rs40837 A/G polymorphysms between male and female gender in Zhuang populations of Guangxi(P>0.05).The frequencies of allele and genotype distribution of IL-27 gene rs17855750 G/T polymorphisms were not significantly different when compared with HapMap-HCB(P>0.05),but were significantly different when compared with HapMap-JPT,HapMap-TSI and HapMap-YRI(P<0.01);The frequencies of allele and genotype distribution of intetleukin-27 gene rs40837 A/G polymorphisms were significantly different when compared with HapMap-HCB(P< 0.05),and were significantly different when compared with HapMap-JPT,HapMap-YRI and HapMap-TSI(P<0.01).Conclusion There are significant differences in the frequencies of allele and genotype distribution of interleukin-27 gene rs17855750 G/T,rs40837 A/G between Zhuang populations and other ethnic populations,and this variation may lead to a variety of clinical manifestation and morbidity of some diseases.
7.Analysis of kudiezi injection different dosage impact on patient's liver and kidney function based on hospital information system.

This study aims to explore the impact on patient's liver and kidney function by different dosage of Kudiezi injection. This study retrospectively analyzed 15 228 patients' records from 18 nationwide general hospital information system (HIS). All patients were treated with Kudiezi injection, 1 956 patients that were given doses of > 40 mL, which is above the recommended dose, acted as the observation group. Fifty-five patients receiving the recommended dose of < 40 mL were the control group. Data about alanine aminotransferase (ALT), aspartate aminotransferase (AST), creatinine (Cr) and blood urea nitrogen (BUN) were collected before and after using Kudiezi injection, changes after treatment were outcomes. Also recorded were: age, costs, length of hospitalization and the patients' condition on admission. Propensity score method was used to balance 71 confounding variables such as gender, age, mortality, and costs. There were no significant difference on the four indexes between the two groups. It is hard to conclude that the use of Kudiezi injection over the recommended dose could influence the four indexes of liver and kidney from this data analysis. More conclusive evidence should be collected by further prospective study.

8.Biplane-transesophageal echocardiography in measurement of aortic annulus dimension

Objective To explore the feasibility of biplane-transesophageal echocardiography (Bip-TEE) in measuring the aortic annulus dimension (AAD).Methods Totally 24 patients underwent transcatheter aortic valve implantation (TAVI) were collected,and their AAD was measured by two-dimensional transthoracic echocardiography (2D-TTE),two-dimensional transesophageal echocardiography (2D-TEE),Bip-TEE and three-dimensional transesophageal echocardiography (3D-TEE) respectively.These four methods were compared with each other.The correlation between 3D-TEE measures and other three methods were analyzed.Results The AAD measured by 2D-TTE,2DTEE,Bip-TEE and 3D-TEE were (22.02±2.21)mm,(23.34±2.34)mm,(23.89±2.37)mm,(24.21±2.78)mm,respectively.The differences among 4 groups was significant (F=3.88,P=0.01).No statistically significant differences were found between Bip-TEE and 3D-TEE,2D-TEE and 3DTEE,Bip TEE and 2D-TEE (all P>0.05).There were significant differences between 2D-TTE and 2D-TEE,2D-TTE and Bip-TEE,2D-TTE and 3D-TEE (all P<0.05).The AAD measured by 3D-TEE were positively correlated with that of 2D-TTE,2D-TEE,Bip-TE (r=0.79,0.88,0.94,all P<0.05).Conclusion Bip-TEE is a feasible method to measure the AAD rapidly and can provide reliable measurements for the prosthetic valve size in TAVI.
9.Primary Study on the Synthesis of Active Ingredients of Ophiocordyceps xuefengensis by Submerged Fer-mentation Method

OBJECTIVE:To explore basic technology for synthesis of active ingredients of Ophiocordyceps xuefengensis,and provide necessary technical support for comprehensive development of O. xuefengensis sourse. METHODS:Submerged fermenta-tion method was used to cultivate the mycelium,achieving efficient synthesis of active ingredients by controlling medium composi-tion and cultivation conditions. Using the bacteria as starting strain,the effects of different carbon sources (sucrose,glucose and soluble starch),different nitrogen sources (peptone,yeast extract powder,yeast extract,sodium nitrate,potassium nitrate and urea),different vitamin B(vitamin B1 and vitamin B complex)and different initial pH(pH was set at 4,5,6,7,8 and 9,re-spectively)on mycelial growth,extracellular and intracellular polysaccharide synthesis,cordycepin synthesis and intracellular triter-penoid synthesis were investigated to screen the optimal medium composition. RESULTS:The optimal carbon source,nitrogen source,vitamin B and initial pH were sucrose,yeast extract powder,vitamin B1 and 8,respectively. High biomass and metabolite accumulation levels can be obtained when carbon source was sucrose,nitrogen source was yeast extract powder,adding 0.1 g/L vi-tamin B1 with initial pH of 8. CONCLUSIONS:O. xuefengensis can efficiently accumulate metabolites,and achieve the optimiza-tion of strain cell growth and synthesis of active metabolite by optimizing and controlling the fermentation process.
10.Brain function of heroin addicts after withdrawal

Objective To explore what brain regions are modulated by heroin addiction and withdrawal.MethodsWe used functional magnetic resonance imaging to investigate the brain function in 15 heroin-dependent patients 3 days (acute) and 1 month (protracted) after heroin abstinence.Sixteen normal controls were included.Results The blood oxygen level-dependent signal in the orbitofrontal cortex of the brain of heroin-dependent patients was significantly elevated 3 days after the withdrawal.Hyperfunction of the orbitofrontal cortex declined 1 month after the withdrawal.Conclusion Heroin-dependent subjects at both 3 days and 1 month abstinence have persistent abnormalities in the brain function.Although some tangible beneficial effects are noted following 1month of detoxification,possible permanent damage to the brain caused by heroin use is suggested.
