ObjectiveTo determine the frequency of cerebral microembolism in patients with asymptomatic or symptomatic extracranial and intracranial arterial stenosis and to assess its relationship to the onset and course of ischemic stroke.MethodsTCD was used to monitor patients with extracranial and intracranial cerebral arterial stenosis. Double channel four-gated and power M-Mode were used to detect microembolic signals (MES). The recording time was 60 min and the number of MES was counted. Patients were divided into 2 groups as extracranial internal carotid artery (ICA) stenosis and middle cerebral artery (MCA) stenosis. Each group was divided into 3 subgroups as asymptomatic, acute ischemic stroke (<30 d) and old ischemic stroke (≥30 d).ResultsThe total number of monitored artery was 74 in 63 patients. In ICA stenosis with asymptom, old ischemic stroke and acute ischemic stroke, the frequency of MES was 0(0/10), 0(0/7), 33%(6/18) and the number of MES in acute ischemic stroke was 3, 9, 8, 10, 1, 40 (mean=11.8). In MCA stenosis with asymptom, old ischemic stroke and acute ischemic stroke, the frequency of MES was 14%(2/14), 14%(1/7), 39%(7/18) and the number of MES was 4 and 1 in asymptom, 1 in old ischemic stroke, 4, 1, 4, 15, 16, 1 and 29(mean=10) in acute ischemic stroke. In ICA stenosis, the difference between acute ischemic stroke and asymptom (P=0.013), between acute and old ischemic stroke (P=0.031) reached statistic significance. In MCA stenosis, there was no significant difference between acute ischemic stroke and asymptom (P=0.115), so as between acute and old ischemic stroke (P=0.214).ConclusionEmbolism was important in the mechanism of ischemic stroke due to extracranial and intracranial arterial stenosis. The closer to ischemic stroke onset, the higher the frequency and the number of MES. TCD monitor was helpful to study the pathogenesis of ischemic stroke due to extracranial and intracranial arterial stenosis and determine the treatment.

At present, the pathophysiological mechanisms of progressive stroke remain unclear. It is difficult to accurately predict which patients will have deterioration of neurological function. 1he factors of blood pressure, C-reactive protein concentration, cerebral artery stenosis, hyperglycemia, and infarct location in early stroke are closely associated with progressive stroke. It has some predictive values for its occurrence.

Chinese Journal of Primary Medicine and Pharmacy 2009;16(z2):17-19
Objective To analyze the hemodynamic characteristics in patients with iron deficiency anemia(IDA) using transcranial Doppler ultrasonography(TCD),and to discuss the diagnostic value of it.Methods There were 46 patients with IDA,among them 28 patients had moderate or severe anemia, and 18 patients had mild anemia.Hemodynamic changes in patients were observed with TCD,and compared with 40 case healthy controls. Results In addition to 3 case results of TCD were normal, the increase of velocity were found in the others patients. The cerenbral blood flow velocity of the moderate or severe anemia group were significantly higher than that of the mild anemia group(P<0.05),and these were significantly higher than that of the control group(P<0.01).The pulsatility index of the moderate or severe anemia group were significantly lower than that of the control group(P<0.01).2 case artery stenosis in 46 patients were found by TCD,there were 5 case with syncope in 46 patients. Conclusion The severness of anemia can be observed by the increased cerenbral blood flow velocities.TCD may be evaluat Effestiveness of blood flow, and TCD may be possible to raise the discover rate of artery stenosis in anemia.

SUMMARY Here we report a case of cervical spondylosis misdiagnosed as cerebral infarction .The pa-tient was a 55-year-old man with a one-day history of weakness in his right extremities .Brain magnetic resonance imaging ( MRI) showed no acute abnormality , cerevical MRI showed that cervical spondylisis , C4/5 , C5/6 disc herniation , spinal canal stenosis and compression of the spinal cord .Then the patient was transferred to the Department of Orthopaedics and underwent surgical treatment of cervical spondylo -sis.Followed-up for six months , the weakness of his right extremities returned to normal .

Objective To investigate the regulation of electroacupuncture(EA)on gastric mucosal blood flow(GMBF) and its relation to the contents of plasma and gastric mucosal calcitonin gene-related peptide(CGRP) in dogs. Methods Twenty dogs were randomly divided into four groups:blank control group,no meridian point group,Shangjuxu group and Zusanli point group.By using laser doppler flowmeter(LDF)and radioimmunoassay method,GMBF,contents of CGRP were simultaneously measured, with or without EA stimulation at different acupoints in expermental dogs. Results In the Zusanli group, after EA stimulation, an increase was observed in plasma CGRP content(P

Objective To investigate left ventricular(LV) systolic dyssynchrony in heart failure of different etiology which caused respectively by ischemic heart disease and dilated cardiomyopathy and analyse the correlation between the systolic dyssynchrony index(SDI) and the LV ejection fraction(LVEF). Methods Forty-three subjects were divided into two groups. Group A consisted of 17 heart failure patients which caused by ischemic heart disease and group B included 26 heart failure patients which caused by dilated cardiomyopathy. Three-dimensional datum of left ventricle were obtained using real-time three-dimensional echocardiography (RT-3DE) in full volume mode. Post-processing software Qlab was used for advanced analysis. The end-diastolic volume (EDV), end-systolic volume (ESV), LVEF and bull eye graph of seventeen segments were obtained. Standard deviation of time-to-minimal-systolic-volume of 16-LV segments corrected by R-R interval was calculated as SDI of LV. Results There was no statistical difference in age, heart rate, LVEF and EDV between the two groups (P >0.05). SDI of group A was lower than that of group B,but there was no statistical difference between them (P>0.05). Bull eye graph of seventeen segments revealed that the distribution of delayed systolic segments were different between the two groups. SDI of two groups appeared negative correlation with LVEF(r = -0.83, r = -0.71, P-< 0.01). Conclusions LV systolic dyssynchrony existed in heart failure which caused by ischemic heart disease and dilated cardiomyopathy, but the distribution of delayed systolic segments were different between them. SDI increased with worsening LV systolic function.
