Recently accumulating evidence has shown that innate immunity recognition of invading pathogens can be attributed mainly to the Toll-like receptor (TLR) family.TLR signaling can induce the production of proinflammatory cytokines and upregulate expression of costimulatory molecules to provoke rapid activation of innate immuniry. Activated innate immunity subsequently leads to effective adaptive immunity. It, thereby, activites not only innate but ultimately also adaptive immunity. It can mediate activation of many kinds of cells, such as immune cells, hepatocytes et al. Upon binding to its respective ligands, TLR can activate signal transduction cascades leading to activation of transcription factors and protein kinases such as NF-?B and MAPK, and expression of immune response genes. Besides,TLR family may be involved in the pathogenesis of inflammation via recognition of host products. Inhibition of TLR action may work as anti-inflammatory drugs, and possibly also as agents to treat autoimmune diseases.Thus,the increasing understanding of TLR system could be the basis for treating a variety of pathological conditions.
2.Efficacy Observation of Jiangu Tongxiao Pills in the Treatment of Osteoarthritis

OBJECTIVE:To observe therapeutic efficacy of Jiangu tongxiao pills in the treatment of osteoarthritis. METH-ODS:72 patients with osteoarthritis were randomly divided into treatment group and control group with 36 cases in each group. Treatment group was given Jiangu tongxiao pill orally,5 g/time,3 times/d;control group was treated with Glucosamine sulfate capsules orally. 0.5 g/time,3 times/d. A treatment course lasted for 7 days,and both group received 4 treatment courses. Clinical ef-ficacy,WOMAC score and VAS score were observed and compared between 2 groups. RESULTS:The total effective rate of treat-ment group(94.44%)was obviously superior to that(83.33%)of control group,with statistical significance(P<0.05). WOMAC score and VAS score of treatment group were lower than those of control group,with statistical significance(P<0.05). CONCLU-SIONS:Jiangu tongxiao pills has good therapeutic efficacy for osteoarthritis.
3.The deficit of verbal working memory in dyslexia children

Objective To explore the deficit features of verbal working memory of dyslexia children by comparing with normal children. Methods Seventeen normal children and fifteen dyslexia children finished the classic verbal n-back task which involved three memory updating difficulty levels. Results Dyslexia children ((81.7±7.0)%,(70.8±12.0)%,(60.0±7.0)%)performed worse than NC ((88. 6 ±6. 0)% , (83. 5 ± 7.0)% ,(75.6 ±9.0)%)in all memory updating levels(n = 0,1 ,2) (P<0.01). In the performances related to memory updating operating,when the difficulty of the updating rised dyslexia children(25. 9 ± 12. 0)% revealed greater decline than normal children(14.2 ± 12.0)% (P<0.01). Conclusion Dyslexia children demonstrate the deficit in the basic level of working memory, which was showed as the bad performance of n-back task in updating level zero, and also they had problems in memory updating,which was showed as the greater updating effect. Memory updating was regarded as the advanced procession of verbal working memory.
4.Guiding Rational Clinical Drug Use by Using Computer Network System

OBJECTIVE:To provide reference for clinical rational drug use.METHODS:Computer network system(CNS)of clinical rational drug use was constructed by using Hospital Information System(HIS),electronic medical records for both outpatients and inpatients and Prescription automatic screening system(PASS),which was then used for real time monitoring of clinical drug use.RESULTS & CONCLUSION:CNS has become an indispensable means for hospital management and clinical drug use in that it can provide support for the data analysis of clinical rational drug use,improve the level of clinical rational drug use,standardize the medical behavior and imrpove the treatment quality.

OBJECTIVE:A HPCE method for determination of Quercetin in acanthopanax and acanthopanax injection was developed.METHODS:Measurements were carried out in a buffer solution containing 50mmol/L boric acid solution and 50mmol/L SDS(pH 8.5),at 20kV running voltage applied to a 50cm?50?m capillary.The detection wavelength was 260nm.RESULTS:The linear range of quercetin was 0.0 125~1.0mg/ml.The recovery of acanthopanax was (98.99?0.63)%.The recovery of acanthopanax injection was (98.62?0.42)%.CONCLUSION:The method is simple,rapid,accurate and suitable for determination of quercetin in acanthopanax and acanthopanax injection.

Objective To study the variation of serum IL-1?, Hs-CRP, the monocytes infilitrating into the intima, the expression of HSP-70 in diet-induced hyperlipidemic and chronic Cpn-infected mice and the intervention of berberine. Methods Mice were inoculated intranasally with Cpn three times, every 2 weeks over a 6-week period and fed with high cholesterol diet. C57BL/6J mice were divided randomly into 6 groups:Ber1 group, Ber2 group, azithromycin group, atorvastain group, model group, and normal group. Ber1 group were given berberine at the beginning of Cpn infecting, while Ber2 group, azithromycin group and atorvastain group were given the drug after the 3rd Cpn infecting. The serum index, HE dye of the aortas and immune-histochemistry index were observed. Results The level of serum IL-1?, Hs-CRP, the monocytes infilitrating counting and the expression of HSP-70 in model group were increased. All of the index in Ber group, azithromycin group and atorvastain group declined to some extent. Conclusions Berberine has effect of decreasing inflammatory indexes of diet-induced hyperlipidemic and chronic Cpn-infected mice.
9.Study on the Effect of Anti Osteopontin Antibody in Treating Rats’Collagen Induced Rheumatoid Arthritis

Objective To investigate the anti?OPN antibody′s protective effect towards the synovial membrane of rats with rheumatoid arthritis,in?cluding the mechanism of such protective action. Methods From a total of 40 SD rats,6 were randomly selected as the normal control group while the remaining 34 were used to establish the collagen induced arthritis model. A total of 24 rats with arthritis index higher than 2 were chosen(exclud?ing the rest 10 rats),and randomly divided into model group and groups being treated with low,medium and high dose of anti?OPN anti?body,re?spectively. Weight,arthritis index and the volume of the left hind leg toe of the rats were recorded. Then on day 35,the rats were killed and their joints were taken out to make HE colored sections for observation of the pathological changes. The synovial membrane of each rat was taken out and WB method was used to test the protein expression of OPN and TNF?αin such membrane. ELISA method was used to test the change of the content of OPN and TNF?αin each rat′s blood. Results The rats in treatment group gained weight steadily from day 16 and their swollen joints were gradu?ally relieved upon medication. Comparing with the control group,the differences in weight,toe volume and arthritis index of the rats in model group were statistically significant(P<0.05). Comparing with the model group,the differences in weight,toe volume and arthritis index of the rats in the treatment groups were also statistically significant(P<0.05). Pathologic change of synovial membrane:for rats in treatment groups,synovial cell hy?perplasia was visibly reduced with a little inflammatory cell infiltration and smooth articular cartilage surfaces with no visible damage. Expression of OPN and TNF?αin synovial membrane and blood serum of rats:Judging from the OPN measured in synovial membrane and blood serum,we could see that the expression of OPN in rats of the model group increased obviously if compared with the control group. Comparing with the model group, the expression of OPN in the treatment groups was reduced(P<0.05). Judging from the TNF?αmeasured in synovial membrane and blood serum, we could see that the expression of TNF?αin rats of the model group increased obviously if compared with the control group,thus such difference was statistically significant between different groups(P<0.05). Conclusion The anti?OPN anti?body could significantly reduce the articular symp?toms of the rats with collagen induced arthritis,restrain the synovial cell hyperplasia and alleviate the inflammatory reaction. In the process of protect?ing the joints of the rats with collagen induced arthritis,anti?OPN anti?body restrained only the expression of OPN in the synovial membrane and blood serum,leaving the expression of TNF?αunaffected.
10.Comparative study of elderly-onset versus young-onset rheumatoid arthritis: clinical features and treatment

Objective To compare the clinical features and treatment between elderly onset rheumatoid arthritis(EORA)and young onset rheumatoid arthritis(YORA).Methods The EORA patients (n=60)and the YORA patients (n =90)were compared regarding sex ratio,activity of pathogenetic condition,disease severity,extra-articular manifestations,complications,laboratory indexes,and therapeutic schedules.Results The female/male ratio was 36/24(1.50 ∶ 1.0)in group EORA,and 69/21(3.3 ∶ 1.0)in group YORA,with higher female/male ratio in in group YORA.The frequency of morning stiffness,proximal interphalangeal joint involvement and metacarpophalangeal joint involvement were lower in group EORA(53.3 %,46.676% and 61.67 %) than in group YORA (72.2 %,77.8 %,81.11%) (x2 =5.521,15.385,6.960,P =0.018,0.000,0.008 respectively).Large joints involvement at onset of rheumatoid arthritis was higher in group EORA(38.33 %)than in group YORA(18.89 %)(x2=6.960,P=0.008).The joint swollen and tender counts were comparatively less (16.51 ± 7.34) and (15.92 ± 8.44) in group EORA than in group YORA(22.46 ± 7.58) and (23.8 ± 8.93) (t =5.080、5.740,all P =0.000) respectively.The accumulated disease activity score(DAS28)was higher in group EORA(5.86± 1.57)than in YORA(4.92± 1.64) (t=3.360,P =0.001).HAQ score was lower in group EORA(0.83 ± 0.85)than in group YORA (1.16±0.91) (t=2.43,P =0.02).Comorbid conditions such as osteoarthritis,osteoporosis,cardiovascular disease and chronic renal insufficiency were more frequent in group EORA (51.7 %,31.7 %,18.3 % and 15.00%)than in group YORA(27.8%,15.6%,5.6% and 4.4%)(x2 =11.722,5.445,6.168,5.067,P=0.001,0.020,0.013,0.024).The positive rate of RF were more higher in YORA(70.00%)than in group EORA (48.33%)(x2 =7.126 P=0.008).The synthetic or biologic traditional DMARDs (disease modifying antirheumatic drugs)were widely used in group YORA(78.9% and 31.1%)than in group EORA(40.0% and 10.0%)(x2 =14.940,9.153,P =0.000,0.002).The more frequently used program with glucocorticoids hormonal therapy was received in group EORA(38.3 %)than in group YORA(20.00 %)(x2 =6.092 P=0.014) Conclusions EORA patients differs from YORA patients in many of ways,including a more balanced gender distribution,atypical presentation at onset of disease,more frequent involvement of large joints,seronegativity in a higher percentage,and less frequent positivity of anti CCP-antibody,which makes diagnosis more difficult in the earlier period.Therapy of EORA with DMARDs should be instituted based on disease activity,if there is no contraindication.
