1.Analysis of the Utilization of Antiemetics in Cancer Patients of 87 Hospitals from 2012 to 2014

OBJECTIVE:To provide reference for rational use of antiemetics in cancer patients. METHODS:The utilization of antiemetics in cancer outpatients and inpatients from 87 hospitals involved inHospital Prescription Analysisproject during 2012-2014 was analyzed statistically by time or by hospital category. RESULTS:14 antiemetics were used in 87 hospitals during 2012-2014. The consumption sum and person number of antiemetics in cancer outpatients were 5763000 yuan and 34000 persons [22000 yuan/(year·hospital),133.1 person/(year·hospital) in average];those of cancer inpatients were 61711000 yuan and 515000 persons [256000 yuan/(year·hospital),2137.9 person/(year·hospital) in average];those of outpatient were lower than those of inpatient. The ratio of consumption sum of antiemetics in cancer outpatients and inpatients were 5.2‰-34.0‰;the ratio of person number ranged 2.9%-10.9%. Those of inpatient were higher than those of outpatient;those of special hospital were higher than those of comprehensive hospital. Main drugs included ondansetron,tropisetron,metoclopramide and palonosetron. CONCLU-SIONS:Antiemetics are frequently used in cancer patients,especially for 5-HT3 receptor antagonists and metoclopramide.
2.The clinical significance of HLA-B27 antigen detection in the diagnosis of ankylosing spondylitis

Objective To investigate the correlation between the expression of human leukocyte antigen B27(HLA-B27)antigen and ankylosing spondylitis(AS),and its implications for the diagnosis and differential diagnosis of AS.Methods Flow cytometry was used to detect HLA-B27.354 suspected AS patients,35 patients final diagnosed with AS and 30 patients diagnosed with rheu-matoid arthritis(RA)were enrolled in the study.Results The positive rate of HLA-B27 in 354 patients with suspected AS was 22.0%,in patients diagnosed with AS was 85.7%,which was higher than the RA group(P <0.05).In AS patients,Males accoun-ted for 77.1%,females accounted for 22.9%,and the patients′age mainly concentrated in youth.The sensitivity and specificity of HLA-B27 test was 85.7% and 100.0% respectively in the diagnosis of AS.Conclusion HLA-B27 antigen detection is important for early diagnosis and differential diagnosis of AS patients.
3.Determination of frequencies of Th17 cells in human peripheral blood by flow cytometry

Objective To investigate the effect of different stimulating culture conditions and detecting methods on the frequency of Th17 cells and to set up and optimize the experimental method for detecting the frequencies of human Th 17 cells in peripheral blood by flow cytometry (FCM ) .Methods The peripheral blood in the healthy people was performed the stimulating culture by phorbol myristate acetate (PMA) at different final concentrations(50 ,100 ,150 ,200 ,250 ,300 ng/mL) and ionomycin 1 μg/mL .The frequencies of Th17 cells were assessed by FCM after different stimulating time points (4 ,6 ,8 ,10 ,12 h) .The mean fluorescence in-tensity(MFI) and percentage of the ratio of CD4 positive cells on the cellular surface and inside cell membrane were measured be-fore and after PMA stimulation .Results The frequency of Th17 cells was highest with the good stimulation effect under the condi-tion of PMA 150 ng/mL and ionomycin1μg/mL stimulation for 6 h (P<0 .05) .MFI of CD4 inside cellular membrane after stimula-tion was lower than that in the cells with stimulation after labeling membrane surface CD 4(P<0 .05) ,and the Th17 cell frequencies and the ratio of CD4 positive cells were higher than those in the latter ,but the differences were not statistically significant (P>0 .05) .Conclusion After stimulating whole blood with the final concentration of PMA 150ng/mL and ionomycin1μg/mL for 6 h , measuring intracytoplasm CD4 and IL-17 levels is the simple and effective method for detecting the frequency of human Th 17 cells .
4.Optimization of Monochromatic Imaging in Reducing Iodinated Contrast Dose in CT Pulmonary Angiography

Purpose To explore the feasibility of optimizing monochromatic images in reducing iodinated contrast dose in CT pulmonary angiography (CTPA). Materials and Methods Sixty-eight patients undergoing CTPA were randomly divided into two groups, with half in research group and half in conventional group to evaluate image quality. Research group underwent spectral CT imaging with the injection of 30 ml Omnipaque (300 mgI/ml) and optimal monochromatic images were postprocessed using the software of GSI viewer. Conventional group underwent conventional CTPA with the injection of 80 ml Ultravist (370 mgI/ml). The CT values were measured respectively in the main pulmonary artery, left pulmonary trunk, right pulmonary trunk, left lobe artery and right lobe artery. The contrast noise ratio (CNR) in two groups were calculated, and image quality were subjectively assessed. Results The total iodine intake in research group (9000 mg) was significantly lower than that of conventional group (29 600 mg). CNR in the main pulmonary artery, left pulmonary trunk, right pulmonary trunk, left lobe artery and right lobe artery in research group was significantly higher than that of conventional group (t=2.07-2.71, P<0.05). Subjective image quality scores in research group were also higher than that of conventional group but had no statistical difference (Z=-0.944, P>0.05), while there was a good agreement between the two readers (Kappa=0.8, P<0.05). Conclusion It is possible to reduce iodinated contrast dose using spectral CT imaging and improve the image quality of CTPA.
6.Effect of Continuous Blood Purification on Inflammatory State and Immune Response in Patients with Multiple Injury and Sepsis

Objective:To study the effect of continuous blood purification on inflammatory state and immune response in patients with multiple injury and sepsis.Methods:88 patients with multiple injury and sepsis who were treated in our hospital from January 2015 to May 2016 were selected as the research object,they were divided into the control group (n=44) and observation group (n=44) according to random number table method.The control group was given conventional treatment,and the observation group was given continuous blood purification treatment.The acute physiology and chronic health conditions Ⅱ (APACHE Ⅱ)score was observed in two groups Ⅱ at 3 d after treatment.The serum content of Interleukin-2(IL-2),Interleukin-4(IL-4),Interleukin-10(IL-10) and interferon-γ (IFN-γ) were detected by ELISA method.The levels of CD3 +,CD4 +,CD8 + and NK cell were analyzed by flow cytometry.The urea nitrogen (BUN),serum creatinine (Scr),prothrombin time (PT),thrombin time (TT),activated partial blood coagulation time (APTT) were tested by automatic biochemical analyzer.Results:The APACHE Ⅱ score,BUN and Scr levels in observation group were lower than the control group (P<0.05).The content ofIL-2,IL-4,IL-10 and IFN-γ in observation group were lower than the control group (P<0.05).The levels of CD3+,CD4+,CD8+ and NK in observation group were higher than the control group (P<0.05).The levels ofPT,TT and APTT in observation group were lower than the control group (P<0.05).Conclusion:Continuous blood purification in patients with multiple injury and sepsis had better clinical curative effect,can reduce inflammation,improve immune function and the function of blood coagulation.
7.Comparison of Ocular Rinse Effect of Povidone Iodine Solution with Sodium Chloride Solution

OBJECTIVE To observe and compare the effects of conjunctival sac rinse of povidone iodine solution with sodium chloride solution.METHODS Eighty patients with senile cataract were randomly divided into 2 groups(group A and group B).Swab samples were taken for bacterial cultures from each patients of both groups before and 24 hours after application of 0.3% ofloxacin eye drops.Povidone iodine solution was used to rinse the conjunctival sac before cataract surgery in group A while 0.9% sodium chloride solution was used in group B,and swab samples were also taken for bacterial cultures.RESULTS Positive rates of the bacterial cultures before and after topical ofloxacin application were 58.8% and 27.5%,respectively.Positive rates of the bacterial cultures after conjunctival sac rinse with povidone iodine and sodium chloride were 5.0% and 20.0%,respectively.CONCLUSIONS Conjunctival sac rinse with povidone iodine solution should be taken as a routine procedure before intraocular surgery to decrease the risk of postoperative infections.
8.Research advances in biomarkers for early prediction of preeclampsia

Preeclampsia is a leading cause of maternal and infant morbidity and mortality. It is very important to explore the biomarkers for the early prediction of preeclampsia. Some peptides released from placenta, such as soluble Flt-1 and placenta growth factor (PlGF), have been revealed for definite prospects of application. Meanwhile, the recent advances in proteomics, metabolomics and microRNA shed light on searching of new biomarkers for preeclampsia prediction.
9.Effects of dexamethasone on expression of nuclear factor kappa B/inhibitor kappa B alpha and apoptosis of lens epithelial cells

Background Researches demonstrated that the long-term application of glucocorticoids can induce cataract. However, its molecular mechanism is unclear. Objective Present study was to investigate the effects of dexamethasone on the regulation of nuclear factor kappa B( NF-κB)/ inhibitor kappa B alpha( IκBα) line on human lens epithelial cells (LECs) and the LECs apoptosis. Methods Human LECs line(HLE2B3) were cultured and passaged in DMEM containing 20% fetal bovine serum and treated by different concentrations of dexamethasone(0. 01,0. 1,1,10,100 μmol/L) for 24,36 and 48 hours respectively. The LECs cultured in free-serum DMEM without dexamethasone were as blank control group. The expressions of IκBo: in the LECs were examined by reverse transcription polymerase chain reaction ( RT-PCR) and Western blot, and the expressions of NF-κB neucleoprotein in LECs were detected by Western blot after exposure to dexamethasone. The apoptosis rate of LECs was determined by flow cytometer. Results Agarose gel electrophoresis showed that the amplified gene fragment was coincident to designed one. The expressing level of NF-κB neucleoprotein in LECs was significantly lowed with the increase of dexamethasone concentration ( F = 36. 077 , P = 0. 004 ) , and that of IkBo: was evidently ascended ( F = 35. 741 ,P = 0. 002). In the same concentration of dexamethasone group,the expression of NF-κB in LECs showed the considerable alteration in different duration after treated of dexamethasone with the lowest expressing level in 36 hours, and significant differences were found in the expressing level between 24 hours and 36 hours ( P = 0. 002) and between 24 hours and 48 hours (P = 0. 01). The differences of expression of IκBá in LECs appeared the same pattern to NF-κB neucleoprotein. Flow cytometry showed that the apoptosis rate of LECs was obviously enhanced after action of dexamethasone in a dose-dependent manner, showing a significant difference among different groups ( F = 73. 261, P = 0.001). Conclusion It is implied that dexamethasone results in the pathogenesis and development of glucocorticoid cataract by up-regulating the expression of IκBα in LECs and suppressing the activity of NF-κB and herein induce the apoptosis of LECs at concentration-and time-dependent manner. This might be one of cellular and biological mechanisms of glucocorticoid cataract formation.
10.Survey of influential factors of female stress urinary incontinence in a county in Chengdu

Objective To investigate the incidence and related risk factors of female stress urinary incontinence(SUI) in a county of Chengdu. Methods The women were interviewed with questionnaires,which were designed to the modified standard of the Advisory Committee of Urinary Incontinence of Asia-pacific Region. Results The total number of the women investigated was 563,while the number of the valid questionnaires was 512,with the efficient rate 98.5%.Of these women,46.1%(236/512) had urinary incontinence,21.9%(112/512) had simplex SUI,4.7%(24/512) had simplex urgent urinary incontinence and 19.5%(100/512) had complex urinary incontinence.The incidence of SUI(simplex SUI plus complex urinary incontinence) was 41.4%(212/512).The incidence of SUI was associated with factors such as age,pregnancies,drinking and education level,and the severity was associated with age,body mass index and education level.Conclusion The incidence of SUI in this county of Chengdu is relatively higher.Heath education is very important for the female health.
