Objective We sought to compare the ability of MEWS score,APACHE Ⅱ score and combination of the two scoring systems to predict the prognosis of patients in emergency medical department. Methods During January to March 2014,640 patients in emergency medical department who met the criteria were set as the research object. The patients admission was the starting point for clinic observation. The relevant data were collected for carrying on the MEWS and APACHE Ⅱ ratings, tracking the patients' prognosis. The corresponding predictors for prognosis of patients such as sensitivity, specificity, positive predictive value, negative predictive value and ROC curve by MEWS score,APACHE Ⅱ score and combination of the two scoring systems were compared. Results The area under the receiver operating characteristic curve was 0.93,0.79 and 0.93 with MEWS score,APACHE Ⅱ score and combination of the two scoring systems. The comparison of either of the two scoring systems showed significant difference. When death was named as the prediction factor, the sensitivity, specificity, positive predictive value and negative predictive value were 76.92%,91.70%,51.02%,97.23% for MEWS score; 83.08%,62.80%,20.15% and 97.04% for APACHE Ⅱ score; 92.31%,86.43%,43.48% and 97.58% for combination of the two scoring systems. Conclusions Combination of the MEWS and APACHE Ⅱ scoring systems can be used to predict the prognosis of patients in emergency medical department. It posesses high sensitivity, specificity, positive predictive value and negative predictive value, which indicating a high predictive capability.

The immunoregulation effect to polysaccharides from Bifidobacterium spp. was investigated on the base of functional assessment standards of health food. Effects of the EPS on immunity were investigated by promoted the proliferation of spleen lymph cells, delayed type hypersensitivity reaction, the HC50 value and macrophage function assay in mice. Data showed that the EPS could obviously increase the ratio of swallowed chicken red blood cell by macrophage and the HC50 value in mice. However, no significant effect was found on the delayed type hypersensitive induced by sheep red blood cell, for only the low dose of 100 mg/(kg?d) EPS promoted the proliferation of spleen lymph cells. Bifidobacterium spp. EPS can certain immunomodulating function.

Using colon adenocarcinoma cell line HT-29 as testing material, adhesion test was carried out for 24 strains of Bifidobacteria isolated from dejecta of longevous people aged one hundred years. The results showed that all strains could adhere to colon adenocarcinoma cells HT-29, among which the strains of TTF, Z2, TZ5 and (J-1) showed higher adhesive capability. Thus these four strains of Bifidobacteria were further tested for their endurance to stomach acid and bile acid. The production of vitamin B by these bacterial were also tested. All these tests indicated that the strain of TTF has high ability to produce Vitamin B_(1), B_(2), B_(6), B_(12), The survival rate after the treatment under pH 3.0 for 120 min was 93.11%, at the same time, the strain TTF can survive after being treated with 2% bile acid for 24h.

Objective To evaluate the efficacy and safety of olopatadine hydrochloride for the treatment of chronic idiopathic urticaria (CIU). Methods A multicentre, double-blind, randomized, parallel-group, controlled clinical trial was conducted. A total of 144 patients with CIU from 3 research centers were enrolled into this study, and randomly and equally divided into a test group and a control group. The test group administrated olopatadine hydrochloride 5 mg twice a day for 28 consecutive days, while the control group administrated levocetirizine hydrochloride 5 mg in the forenoon and a placebo tablet of olopatadine hydrochloride 5 mg in the afternoon for 28 consecutive days. The symptom score reducing index(SSRI)served as the primary outcome, and global assessment score for efficacy and total response rates as the secondary outcome. Results Totally, 137 patients completed the trial, including 70 in the test group and 67 in the control group. As intention-to-treat analysis showed, there were no significant differences in the total response rate between the test group and control group on day 7 (64.29% (45/70)vs. 56.72%(38/67), P > 0.05), 14(82.86%(58/70)vs. 74.63%(50/67), P > 0.05), or 28(87.14%(61/70)vs. 77.61%(52/67), P >0.05)after start of treatment. The SSRI was significantly higher in the test group than in the control group after 4 weeks of treatment(82.67% ± 22.70% vs. 70.51% ± 32.07%, P < 0.05). In addition, no significant difference was observed in the incidence of adverse reactions between the test group and control group(33.80%(24/71)vs. 27.94%(19/68), P > 0.05), and adverse reactions mainly included lethargy, dry mouth, fatigue, etc. Conclusion Olopatadine hydrochloride is effective and safe for the treatment of CIU.

ObjectiveTo observe the effect of warm needle moxibustion and exercise therapy on scapulohumeral periarthrits.Methods90 scapulohumeral periarthrits cases were randomly divided into the synthetic treatment group and exercise therapy group with 45 cases in each group. Patients of the synthetic treatment group were treated with warm needle moxibustion and exercise therapy, but that of the exercise therapy group only with exercise therapy. Some local acupoints, for example Point LI15 (Jian Yu), Piont SJ14 (Jian Liao), Piont SI9 (Jian Zhen) and so on, were selected when warm needle moxibustion performed. Eexercise therapy includes joint mobilization and initiative function training. One course was 15 days. The effect was evaluated after two courses.ResultsApparent efficiency of synthetic treatment groups was 71% and 42% for exercise therapy groups. There was a significant difference between two groups (χ2=22.815,P<0.01). There was also a obvious difference between two groups in improving range of shoulder joint movement and alleviating extent of shoulder pain (P<0.05).ConclusionWarm needle moxibustion cooperated with exercise therapy can increase cure rate and reduce recrudescence of scapulohumeral periarthrits.

Acta Academiae Medicinae Sinicae 2002;24(5):539-541
T cell can only recognize specific antigenic peptide-MHC complex on antigen-presenting cell. This is MHC restriction in antigen recognition of T cell. This phenomenon was discovered by an Austrian scientist Peter. C. Doherty and a Swedish scientist Rolf. M. Zinkernagel by chance. Then, in order to explain the phenomenon, they proposed two hypotheses: dual receptor and modified self. In the during following 20 years numbers of scientists spent great amount of time in the study of the phenomenon. The process of cell-mediated immune response becomes clear, which greatly promotes the advancing of immunology and many related disciplines.

OBJECTIVETo explore biomechanical properties and stress-strain of mucosa scars after cleft palate surgery.
METHODSAfter the model of mucosa scars was made, the mucosa scars and normal mucosa were excised and examined immediately by tensionometry.
RESULTSThe mucosa scars after cleft palate surgery were compared with normal mucosa. The Poisson's ratio of mucosa scars and normal mucosa was 0.5 and 0.49, respectively, showing no significant difference between the two groups. The ultimate Young's modulus of mucosa scars was about 24.22 MPa, however, it declined to 3.32 Mpa in normal mucosa.
CONCLUSIONSThe mucosa scars after cleft palate surgery are biomechanically weaker than normal mucosa. It can be used for further research, such as maxillary orthognathic surgery, distraction osteogenesis, and orthodontic treatment.
