1.Simultaneous Determination of Aygdalin, Aucubin, Harpagide, Peimisine, Peimine and Peiminine in Keling Capsules by HPLC-ELSD

Objective: To develop an HPLC-ELSD method for the determination of amygdalin, aucubin, harpagide, peimisine, peimine and peiminine in Keling capsules simultaneously.Methods: An Ultimate XB C 18 (250 mm× 4.6 mm , 5 μm) chromatographic column was adopted with an ELSD (the drift tube temperature was 105℃, the flow rate of nitrogen was 2.0 L·min-1).The mobile phase was acetonitrile-methanol (1∶1) and 0.4% acetic acid solution with gradient elution at a flow rate of 0.7 ml·min-1 , and the column temperature was set at 35 ℃.Results: Amygdalin, aucubin, harpagide, peimisine, peimine and peiminine was linear within the range of 13.56-271.20 μg·ml-1 (r=0.999 2), 8.48-169.60 μg·ml-1 (r=0.999 9), 4.89-97.80 μg·ml-1 (r=0.999 7), 2.66-53.20 μg·ml-1 (r=0.999 4), 1.82-36.40 μg·ml-1 (r=0.999 8) and 2.04-40.80 μg·ml-1 (r=0.999 6), respectively.The average recovery and the corresponding RSD were 97.90% (1.20%), 99.21% (1.62%), 97.68% (0.75%), 98.36% (1.38%), 99.70% (0.79%) and 97.95% (1.56%)(n =6), respectively.Conclusion: The method is simple and specific, and the results are accurate and repeatable.The method is helpful to the quality control of Keling capsules.
2.Establishing an intervertebral disc degeneration model using annulus puncture

BACKGROUND:Annulus puncture method is commonly reported in the establishment of intervertebral disc degeneration (IDD) models, but there is a lack of comparative studies concerning complete MRI, pathologyical and extracellular matrix changes. Thereafter, a further exploration is imperative. OBJECTIVE:To investigate the feasibility of establishing IDD models using annulus puncture. METHODS:Thirty New Zealand white rabbits were randomly divided into two groups (n=15 per group). In experimental group, through extraperitoneal approach, the L1-2, L2-3, and L3-4 intervertebral space were exposed, and then stabbed by 16-gauge hypodermic needles;while rabbits in sham operated group were only subjected to making and incision but not to puncture. Five rabbits in each group were randomly selected for MRI examination at 4, 8 and 16 weeks after modeling, and were sacrificed through air embolism to obtain the nucleus pulposus. Histological changes were observed by hematoxylin-eosin staining, and the level of proteoglycan was detected by Safranin O staining. RESULTS AND CONCLUSION:In the experimental group, T2-weighted signal in the disc was decreased in a time-dependent manner;an obvious reduction was found at 4 weeks after modeling, and reached the peak at 16 weeks. In contrast, the sham operated group showed no significant changes at each time point. Hematoxylin-eosin staining showed that in the experimental group, cartilage-like cells decreased, while fibroblast-like cells in the disc increased in number and proportion, and extracellular fibers were in disorder;all above changes became obvious at 4 weeks after surgery and the most obvious at 16 weeks, but there were no significant changes in the sham operated group. Safranin O staining revealed that the color of the tissues under puncture became lighter, and this variation became overt at 4 weeks after surgery and the most obvious at 16 weeks. In conclusion, annulus puncture is a reliable method to make IDD models.
3.A research for accuracy of infrared computer navigation in arthroscopic anterior cruciate ligament reconstruction bone tunnel positioning of assisted

Objective To explore the accuracy and feasibility of infrared computer navigation for femur,tibia tunnel positioning in arthroscopic anterior cruciate ligament (ACL) reconstruction.Methods Data of patients who had undergone ACL reconstruction using infrared computer navigation from January 2012 to June 2013 were retrospectively analyzed.There were 26 males and 9 females,with an average age of 26.8 years old.35 patients were with concomitant meniscal injury 21 cases,and 3 cases with medial collateral ligament injury.All patients were treated with gracilis and semitendinosus knit stitch as grafts,and bone ends were made with hydroxyapatite extrusion screws,door-shaped nails and porous plate.Results The mean follow-up time was 16 months.The rate of postoperative femoral tunnel site measurements was 63.8%±6.6%,and tibial tunnel site measurements was 45.7%±4.2%.1 patient appeared heterotopic ossification in tibia tracker pinhole 3 months after surgery,and the joint activities was normal.Lachman test:there were 1 case with Ⅱ degree,30 with Ⅲ degree,and 4 with Ⅳ degree before operation,and 33 with Ⅰ degree,1 with Ⅱ degree,1 with Ⅲ degree after operation.The KT-1000 test was from 6.47±0.75 mm preoperatively to 1.01±1.14 mm postoperatively.Preoperative knee laxity differences was more than 5 mm in 32 cases,3-5 mm in 3,and after operation they were 0-3 mm in 31,3-5 mm in 2,and less that 0 mm in 2 cases.Lysholm score and Tegner scores were from preoperative 52.74± 10.91 points and 4.64± 1.34 points to postoperative 93.97±4.13 and 6.56±0.81 points.ROM was from 85.6°± 11.2° to 118.3°±5.7°;33 cases were without flexion and extension limited; two cases had knee flexion,extension limitation less than 10°.After straight leg raising training and proprioception functional training for 6 months,the knee ROM returned to normal.Conclusion Infrared computer assisted navigation system for arthroscopic ACL reconstruction is to navigate through the use of anatomical landmarks and kinematic data,kinematic data by detecting and recording the knee,selected a secure tunnel site,viable surgical method,you can make femur,tibia tunnel position more accurately,good curative effect evaluation.
4.Service provision by township hospitals in Beijing

Objective To probe into the current services and outcomes of township hospitals following their transformation, and their future positioning of functionality. Methods Pinpointing townships A, B and C as samples from a district in Beijing, in which field surveys were made in the township hospitals to learn their services, service provision methods and outcomes. Results Among the current services provided by these three hospitals, those of public health and primary healthcare nature in hospital A account for 50% each; those in hospital B account for 70% and 30% respectively; those in hospital C account for 60% and 40% respectively. Recommendable services provided in these hospitals include universal coverage of medical files, management of infectious diseases, public health emergency management, and immunization. Conclusion Township hospitals as the center of the 3-tier rural health service network, deserve a repositioning of their functionality to fit the health needs of residents at large.
5.Gegenqinlian decoction decreases serum TNF-αlevel in rats with chronic periodontitis and atherosclerosis

Objective:To investigate the effect of gegenqinlian on serum TNF-αlevel in rats with chronic periodontitis and athero-sclerotic.Methods:40 healthy male Wistar rats were randomly divided into control group(N,n =10)and model group(D,n =30).After model establishment and identfication the rats in model group were divided into D1,D2,D3,D4 and D5 groups(n =5) and were administered with saline,metronidazole,atorvastatin,metronidazole combined with atorvastatin and gegenqinlian respec-tively for 4 weeks.Periodontal indexes(PD and AL)were measured,the maximum intima-media thickness of abdominal aorta(IMT) was pathologically examined,TNF-αlevel was detected by ELISA.Results:After treatment PD,AL and serum TNF-αlevels in D2,D3,D4 and D5 groups were significantly lower than those in D1 group(P <0.05),There was no difference of lipid and TNF-αlevels between D4 and D5 groups(P >0.05).Conclusion:Gegenqinlian can reduce serum level of TNF-αin rats,suggesting that the drug is effective in the treatment of periodontitis with atherosclerosis.
6.Analysis on predictive value of neutrophil to lymphocyte ratio in rheumatoid arthritis

Objective To investigate the predictive value of neutrophil to lymphocyte ratio (NLR) in rheumatoid arthritis (RA) by using the retrospectively analysis method .Methods The blood cell differential counting in 150 patients with RA ,101 patients with non‐RA diseases and 151 individuals undergoing the healthy physical examination were selected in a hospital from January 2014 and February 2015 .Blood cells classification and count were performed .Then the NLR difference was compared among these three groups .The clinical and laboratory data in the patients with RA were collected and the association between NLR and other in‐flammatory indicators was statistically analyzed .Moreover ,the diagnostic and predictive value of NLR for RA was also analyzed by using the receiver operating characteristic (ROC) curve .Results The NLR level in the RA patients was significantly higher than that in the non‐RA patients with autoimmune disease and the healthy controls .The diagnostic sensitivity of NLR and neutrophil count(NC) for RA was equivalent(62 .4% vs .67 .5% ) ,but NLR had higher diagnostic specificity than NC (85 .5% vs .68 .7% ) . The correlation analysis indicated that NLR revealed a weak but significant correlation with ESR (r=0 .210 ,P=0 .043) and CRP (r=0 .149 ,P=0 .043) respectively .Conclusion NLR has more predictive value for RA than those traditional inflammatory mark‐ers including CRP ,NC and lymphocyte count ,which is expected to serve as a new biomarker for independently predicting the RA in‐flammation severity .
7.Clinical observation on nursing interventions for improving the quality of life after radiotherapy in nasopharyngeal cancer

Objective To investigate the effect of nursing interventions on the quality of life after radiotherapy in nasopharyngeal cancer. Methods 56 patients with nasopharyngeal cancer who received radiotherapy were randomly divided into control group and intervention group. All the patients were given conventional nursing interventions, and those in the intervention group were given accurate evaluation, enhanced psychological guide, reasonable dietary guide, and special nursing for preventing and treating complications. All the patients were evaluated by using quality of life questionnaire core 30 items(QOL-C30). Results The scores of the dimensions of somatic function,psychological function, social function and material life after interventions in the intervention group were significantly higher than those in the control group (P<0.05 ). Conclusion Nursing interventions can improve the quality of life, and the therapy can be completed smoothly.

9.Clinical efficacy of preoperative radiotherapy and chemotherapy in 39 patients with locally advanced cervical cancer

Objective To analyze the clinical efficacy of preoperative concurrent radiotherapy and chemo-therapy and radical operation in the treatment of cervical cancer patients with locally advanced cervical cancer (IB -IIB).Methods 80 patients who were diagnosed locally advanced (stage IB -IIB)cervical cancer,were divided into observation group (preoperative radiotherapy and chemotherapy group)and control group (radical radio-therapy group).The short -term efficacy,complications and follow -up data were compared between the two groups. Results The short -term efficacy of the observation group was 87.2%,which of the control group was 95.1%,and there was no significant difference (χ2 =1.579,P =0.209).As to the early complications of the two groups,there were no significant differences in gastrointestinal tract reaction(χ2 =0.000,P =0.987),rectal reaction(χ2 =2.709, P =0.100),bladder reaction (χ2 =1.343,P =0.247)and bone marrow suppression(χ2 =1.898,P =0.168).Late complications:in the observation group,the radiation proctitis[4 cases(10.3%)vs.14 cases(34.1%)]and the radiation cystitis[3 cases(7.7%)vs.13 cases(31.7%)]were less than those of the control group,the differences were statistically significant(χ2 =6.542,P =0.011&χ2 =7.205,P =0.007).The average follow -up time was 38.6 months in the observation group,which was 44.8 months in the control group.There was no significant difference in survival rate between the two groups(χ2 =0.075,P =0.784 & χ2 =0.032,P =0.858).Conclusion Locally advanced cervical cancer (IB -IIB)patients received preoperative concurrent radiotherapy and chemotherapy has important clinical value.The treatment makes the local giant block tumor shrink significantly,which is conducive to the implementation of surgery,and reduce the incidence of late complications compared with radical radiotherapy.
10.The effect of intravitreal injection with ciproflaxacin on retina

Objective To evaluate the security of intravitreal injection with ciproflaxacin to retina. Methods Tweenty-four rabbits were randomly divided into 4 groups with 6 rabbits in each group. 0.1 ml ciproflaxacin in doses of 2 500, 5 000, and 10 000 ?g was intravitreally injected into the rabbits eyes, retrospectively. And 0.1 ml saline solution was injected into the vitreous body of the rats in the control group. Indirect microscope, light microscope and electroretinogram (ERG) were used to observe the changes of ocular fundus. Results Normal results of light microscopy and ultrastructure were found in 250 ?g and 500 ?g groups; irregularly arranged outer and inner nuclear layers, dropsical or even lost ganglion cells, and ultrastructural changes were in 1 000 ?g group. There was no apparent difference of ERG′s a and b amplitudes before and after intravitreal injection with ciproflaxacin in each group. Conclusion Intravitreal injection with ciproflaxacin is safe, and 500 ?g or less is the secure dosage in rabbits' eyes.
