1.Comparison of anterior segment parameters with Lenstar 900 and Sirius system

AlM: To compare the anterior segment measurements derived from optical low coherence reflectometer ( Lenstar LS900 ) and combined Scheimpflug - Placido disk topographer ( Sirius) .
METHODS: ln this study, we enrolled healthy myopic subjects 54 ( 87 eyes ) . The central corneal thickness (CCT), anterior chamber depth (ACD), flat keratometry ( FK) readings , steep keratometry ( SK ) readings and white to white ( WTW ) were measured by LS900 and Sirius. Evaluation and analysis were performed using paired t tests, the Pearson correlation, and Bland-Altman analyses.
RESULTS: The CCT and ACD measurements were significantly lower whereas FK, SK and WTW measurements were higher with LS900 (P<0. 001). Mean differences were -6. 11 ± 6. 32μm,-0. 09 ± 0. 07mm, 0. 18 ± 0. 25D, 0. 21 ± 0. 36D and 0. 25 ± 0. 39mm with statistical significant (P<0. 01). Pearson correlation analysis showed high correlation between the 2 devices for all measurements (P<0. 001). On Bland-Altman analysis, 95% limits of agreement for all measurements were 6. 26 to -18. 49μm; 0. 04 to 0. 22mm; 0. 68 to 0. 32D; 0. 92 to 0. 50D and 1. 00 to 0. 5mm.
CONCLUSlON: Anterior segment parameters evaluated with LS900 and Sirius systemare correlated well and achieve good agreement. However, there are significantly statistical differences which may be caused by the different measurement modes, so they may not be interchangeable use under certain clinical circumstances.

4.The choice of anterior approach for acetabular fractures: a systematic review

Objective To evaluate the efficacy and safety in the treatment of acetabular fractures via anterior approaches,including ilioinguinal,stoppa,pararectus approach.Methods Systematic literature retrieval was carried out to obtain two-arm and one-arm researches on the treatment of acetabular fractures via any one of the three approaches before May 2017,from pubmed,embase,cochrane library Databases.Data extraction and quality evaluation of studies were performed by 3 investigators independently.A meta-analysis was performed by RevMan 5.3 and Meta-Analyst beta 3.13.Results Totally 22 low to moderate quality studies,including 7 two-arm and 15 one-arm were included.There were 980 patients,including 581 in ilioinguinal approach group,264 in stoppa approach group and 135 in pararectus approach group.Meta-analysis showed the rate of anatomic reduction in stoppa approach was higher than that in ilioinguinal approach (OR=0.58,95%CI:0.36-0.94,P=0.03),which in pararectus approach was higher than that in ilioinguinal approach (0R=2.95,95%CI:1.22-7.10,P=0.02).Compared to the ilioinguinal approach,the operation time in the Stoppa approach was shorter (MD=48.01,95%CI:17.08-78.95,P=0.002),there was no statistically significant difference between pararectus approach and ilioinguinal approach.In addition,there were no statistically significant differences among three approaches in intra-operative blood loss,postoperative functional outcomes and complications.Conclusion The available limited evidence suggests that the rate of anatomical reduction in stoppa and pararectus approach may be higher than ilioinguinal approach.Compared to the ilioinguinal approach,the operation time in the stoppa approach may be shorter.In this field,further rigorous design,baseline parallel,direct comparison controlled studies on this topic are still needed.
5.Effect of homeopathic bidirectional-traction reduction device and traction table in surgical treatment of femoral intertrochanteric fractures in the elderly

Objective To compare the outcomes of homeopathic bidirectional-traction reduction device and traction table in surgical treatment of femoral intertrochanteric fractures.Methods A retrospective case control study was made on 94 cases of femoral intertrochanteric fractures treated from July 2015 to December 2015.There were 26 males and 68 females,aged 60-75 years.According to the Evans classification,the fractures were type Ⅱ in 24 cases,type Ⅲ in 32 and type Ⅳ in 38.Fifty-seven cases sustained chronic diseases.According to the random number table,the subjects were assigned to receive homeopathic bidirectional-traction reduction (homeopathic reduction group,48 cases) and traction table reduction (control group,46 cases).All fractures were fixed with proximal femoral nail antirotation.Operation time,reduction time,tluoroscopy time,blood loss and rate of closed reduced cases were recorded.Fracture union and rotation of the affected femur to the tibia were detected after operation.Functional outcome was evaluated using the Harris score at the final follow-up.Results All fractures were reduced closely in homeopathic reduction group,while 15 fractures in control group were reduced via a small-incision anterior approach.Operation time,reduction time and fluoroscopy time in homeopathic reduction group were (62.9 ± 12.1) min,(6.8 ± 1.5) min and (11.3 ± 5.6) s respectively,significantly less than the corresponding data in control group (all P < 0.05).One month after operation,rotation of the affected femur to the tibia was (2.8 ± 1.2) ° in homeopathic reduction group,significantly less than that in control group [(11.5 ± 4.7) °] (P < 0.05).Bone union was observed in all cases.At the final follow-up,Harris hip score was (92.6 ± 7.6)points in homeopathic reduction group,significantly higher than that in control group [(87.3 ± 6.5) points] (P < 0.05).Conclusion For the elderly patients with intertrochanteric fractures,homeopathic bidirectional-traction device has advantages of high rate of closed reduction,shorter operation time,less radiological exposure and satisfactory function recovery of the affected hip joint.
6.Isolating method of rat pancreatic islets and affect factors for its activity

Objective To assess the factors which affect the activity of isolated islets,and to construct a steady and effective isolation method of rat pancreatic islets.Methods Pancreatic islets were isolated by collagenase method.The glucose-stimulation of insulin secretion(GSIS) in different conditions was performed by batch incubation method and measured by RIA.Results GSIS was improved by BSA;There was a evident decrease of insulin secretion in cultured islets of 7 days,however,no difference was observed between the freshly isolated islets group and the 1~5days cultured islets groups.Compared to the groups of(5.5 mmol/L) and(25 mmol/L) glucose in the culture medium,(11.1 mmol/L) glucose group stimulated higher insulin release.Conclusion BSA,glucose concentration of RPMI1640 medium and the culture period are related to the activity of isolated islets.
7.Inhibitory effect of 131 I-labeled epidermal growth factor receptor-targeted liposome nanoparticle on EGFR-overexpressing cancer cells in vitro

Objective To construct 131 I labeled anti?EGFR immunoliposome nanoparticle ( 131 I?Cetuaximab ( C225)?BSA?PCL) , and investigate its inhibitory effect on EGFR?overexpressing cancer cells in vitro. Methods Anti?EGFR liposome nanoparticle C225?BSA?PCL and non?targeted liposomes BSA?PCL were constructed. The products were observed with transmission electron microscopy and dynamic light scat?tering. The EGFR?targeted binding and cellular uptake in EGFR?overexpressing cancer cells were observed with flow cytometry and confocal microscopy. Anti?EGFR and non?targeted liposomes were labeled with 131 I using the chloramine?T method. The targeted cell killing effects of 131 I labeled liposomes were analyzed using MTT assay. The time?dependent cellular uptake analysis was used to evaluate the slow?release effects of the 131 I labeled liposomes. The independent?samples t test was used for data analysis. Results The EG?FR?targeted liposome C225?BSA?PCL and non?targeted liposome BSA?PCL were successfully constructed, and the effective diameters were approximately 130-180 nm. Flow cytometry and confocal microscopy re?vealed significant uptake of C225?BSA?PCL in EGFR?overexpressing tumor cells. BSA?PCL could also bind to cells with minimal and weak tumor retention. The EGFR?targeted radioactive liposome 131I?C225?BSA?PCL showed greater targeted cell killing effect than non?targeted liposome 131I?BSA?PCL,the IC50 values of 131I?C225?BSA?PCL and 131 I?BSA?PCL were 0. 03-1. 32 and 0. 25-12. 19, respectively. The uptakes of 131 I?C225?BSA?PCL was higher than that of 131 I?BSA?PCL ( t=3.03-16.86, all P<0.05) and reached the maxi?mal level at 4 h after incubation. Conclusions The EGFR?targeted liposome C225?BSA?PCL demonstrated superior cellular binding and uptake on EGFR?overexpressing cancer cells compared with BSA?PCL. The EGFR?targeted radioactive liposome 131 I?C225?BSA?PCL had favorable intracellular retention and excellent targeted cell killing effect, and could effectively suppress the growth of EGFR?overexpressing cancer cells.
8.Evaluation of the internal therapeutic effectiveness of 131I-antiEGFR-BSA-PCL in nude mice with colorectal cancer

Objective To investigate the biological effects of internal radiation and therapeutic effectiveness of 131I-labeled anti-epidermal growth factor receptor (EGFR) in colorectal cancer of model mice.Methods Nano-liposome characterized for EGFR-targeting was constructed.The efficacy of cellular binding and uptake of the liposome was evaluated by the analysis of confocal microscopy observation and the iodide uptake assay.After intra-tumor injections of 74 MBq (740 MBq/ml) 131 I-antiEGFR-BSA-PCL,131 I-BSA-PCL,131 I or an equivalent volume of normal saline.The biological effects of internal irradiation and therapeutic efficacy of the liposomes on colorectal cancer modeled in a male BALB/c mouse were evaluated by means of tumor size,body weight,histopathology,and SPECT imaging.Results The confocal fluorescence images showed that the antiEGFR-BSA-PCL was successfully internalized into LS180 cells.The 131I uptake efficacy of 131I-antiEGFR-BSA-PCL was significantly higher than that of 131I-BSA-PCL in LS180 cells (t =2.77-5.40,P < 0.01).Tumor size measurement showed that tumor growth was inhibited by the treatment with 131 I-EGFR-BSA-PCL and 131I-BSA-PCL,but had no significant differences between these two groups (P >0.05).It was found that the 131I-antiEGFR-BSA-PCL was markedly taken up by the tumor and reac hed its uptake value of (21.61 ± 1.0 1) and (20.58 ± 0.65)% ID/g at 72 h following drug injection,which was higher than the uptake value of 131 I (t =9.36,8.69,P < 0.01).SPECT imaging assay showed that,after being injected into mouse tumor,the 131 I-EGFR-BSA-PCL and 131I-BSA-PCL were uniformly distributed inside the tumor.Conclusions 131 I-antiEGFR-BSA-PCL obviously suppresses the development of colorectal cancer in mice.
9.Methylation status in the promoter region of Dickkopf-3 gene in patients with myelodysplastic syndromes

Objective To investigate the methylation status in the promoter region of Dickkopf-3 (Dkk3) gene in patients with myelodysplastic syndromes (MDS),and to initially explore the relationship between the methylation of this gene and survival time.Methods Methylation-specific PCR (MSP) was applied to measure the promoter methylation of Dkk3 gene in 43 bone marrow or peripheral blood samples of MDS patients.As controls,70 normal peripheral blood samples from general outpatients were examined.Results In 43 patients with MDS,7 patients (16.3 %) showed Dkk3 gene methylation.And 5 of them were semi-methylation status,2 of them were exhaustive methylation status.In 70 controls,1 showed Dkk3 gene semi-methylation.The frequency of methylation in MDS patients was significantly higher than that of controls (x2 =8.93,P =0.005).In the Dkk3 methylation group,2/7 were from bone marrow and 5/7 were from peripheral blood.Meanwhile,2 patients were RA,1 patient was RCMD,4 patients were RAEB.There was no significant difference between the different sample source (bone marrow or peripheral blood) for the results of the methylation status (x2 =0.051,P =0.821).Either between the different sex,age,type,chromosome and WPSS score (P > 0.05).The progress of disease didn't influence the methylation frequency (P > 0.05).The smvival analysis showed no relationship between the methylation of this gene and smvival time.Conclusions In this MDS group,there is high level of methyl-modification in Dkk3 gene.The methylation of Dkk3 might be one of the molecular mechanisms that contribute to the progress of patients with MDS.The peripheral blood sample maybe a better substitute in detective of Dkk3 with MDS.
10.Changes in substance P in spinal cord and dorsal root ganglion before and after peripheral arterial occlusive disease

BACKGROUND: Peripheral arterial occlusive disease is a kind of commonly seen peripheral arterial disease. Its pathogenesy and process of pathological change are still unclear. Most of its clinical symptoms are very severe and therapeutic effect is not very good.OBJECTIVE: To observe the effect of limb negative pressure on substance P (SP)-positive-immunoreactive nerve fibers in the spinal cord and dorsal root ganglion of dogs with peripheral arterial occlusive disease.DESIGN: A randomized and controlled experimental trial.SETTING: Department of Anesthesiology, Shaanxi Provincial Corps Hospital, Chinese People's Armed Police Forces; Third Department of General Surgery, Xijing Hospital, Fourth Military Medical University of Chinese PLA.MATERIALS: The experiment was carried out in the animal laboratory,Xijing Hospital, Fourth Military Medical University of Chinese PLA between April 2003 and May 2004. Totally 17 healthy hybrid dogs, of clean degree, weighing 12 to 18 kg, were enrolled in the experiment. After being numbered, they were randomly divided into 3 groups: treated group (n=10),non-treated group (n=5) and normal control group (n=2).METHODS: Left hindlimb ischemia models were created on the animals in the treated group. Fourteen days after modeling, the affected limbs were treated by negative pressure, once a day, 15 minutes once. Perfusion was given 10 days after negative pressure treatment. Segment L1-L5 spinal cord and dorsal root ganglion were dissected respectively. Immunohistochemical staining was conducted to detect SP-positive- immunoreactive nerve fibers;In the non-treated group, treatment and detection of affected limbs were performed as those in the treated group, but negative pressure treatment was omitted. Ischemia model preparation and negative pressure treatment were not conducted in the normal control group, and only immunohistochemical staining was given.MAIN OUTCOME MEASURES: Changes in SP of sensory nerve fiber of spinal cord and dorsal root ganglion of dogs among 3 groups RESULTS: The dyeing of SP-positive-immunoactive nerve fibers of spinal cord and dorsal root ganglion at affected sides in non-treated group (mean gray scale value: 70.20 ±3.70,67.00 ±3.30 respectively )was significantly stronger than that in normal control group (20.00 ±2.80,35.50 ±6.40),while that in treated group (48.50±6.60,51.70±4.00) was weaker than that in non-treated group, but was still stronger than that in normal control group (all P < 0.01).CONCLUSION: The number of SP-positive-immunoactive nerve fibers of spinal cord and dorsal root ganglion following peripheral arterial occlusive disease is increased significantly, which might be a kind of self-protective mechanism of organism. Limb negative pressure can reduce the importation of nocuous stimulation and release limb pain following arterial occlusive disease of limbs.
