1.Effect of Nursing Intervention on Activities of Daily Living of Patients with Cerebral Trauma in Recovery Phase

Objective To explore the effect of nursing intervention on cerebral trauma patients in recovery phase. Methods 156 cerebral trauma patients who had more than 6 months course of disease received rehabilitation training and nursing care, and their activities of daily living (ADL) were accessed by modified Barthel index. Results The score of ADL in patients who had course of disease about 6~12 months was lower than patients who had more than 12 months course (P<0.05) before nursing care. The scores improved in both groups after nursing care (P<0.05), and there was no significant difference between 2 groups (P>0.05). Conclusion Rehabilitation training and nursing care may facilitate the recovery of ADL of cerebral trauma patients in recovery phase.
2.Expression of miRNA-148a in Bladder Carcinoma Tissues and Its Bioinformatics Analysis

Objective To explore the role of miRNA-148a in bladder tumorous development and progression.Methods Ex-pression of miRNA-148a was assessed in 35 bladder carcinoma tissues and 16 non-carcinoma tissues by fluorescence quanti-tative real time PCR,and correlation with clinical features was evaluated.Target genes and transcription factors of miRNA-148a were predicted using bioinformatic analysis,then TF-miRNA-148a-target genes network diagram was built and the tar-get genes was analyzed of gene ontology enrichment and KEGG pathway.Results Expression of miRNA-148a was lower in bladder carcinoma tissues than in non-carcinoma tissues(0.000 8±0.000 2 vs 0.002 1±0.000 5)(t=2.46,P <0.05),but its expression was no statistical significance in different groups of gender,age,pathological classification,clinical stage, lymph node metastasis (P >0.05).268 target genes of miRNA-148a were predicted by three softwares at the same time,60 transcription factors were predicted and the binding sites with combination scroes above 80 was 657.The target genes of miRNA-148a was enriched in many biological processes,such as neuron differentiation,generation of neurons,neuron projec-tion development,cytoplasmic mRNA processing body,cytoplasm(P <0.001).They also participated in p53 signaling path-way,proteoglycans in cancer-homo sapiens,pathways in cancer,prostate cancer,protein processing in endoplasmic reticulum, focal adhesion and so on(P <0.05).According to TF-miRNA-148a-target genes network diagram,miRNA-148a was regula-ted by SP1,ESR1,AP1,MYC and BRCA1,genes of IGF1,P27kip1 ,NCOA1,PTEN,SERPINE1 might be regulated by miR-NA-148a.Conclusion miRNA-148a which was significantly down-regulation in bladder carcinoma tissues may be participate in bladder tumorous development and progression,bioinformatics analysis provides some ideas for further research.
3.Total ginsenosides fought against right ventricular hypertrophy through inhibiting calcineurin signal pathway.

OBJECTIVETo observe the effect of total ginsenosides (TG) on monocrotaline (MCT) induced right ventricular hypertrophy rats, and to explore its correlation with calcineurin (CaN) pathway.
METHODSFifty male Sprague Dawley rats were randomly divided into the normal control group, the MCT model group, and the low, middle, high dose TG treatment groups, 10 in each group. All medication was performed by peritoneal injection for 18 days. Right ventricular peak systolic pressure (RVSP), right ventricular hypertrophy index (RVHI), and right ventricular weight/body weight (RVW/BW) were measured. Intracellular free calcium concentrations were measured by Ca2+ fluorescence indicator Fura2/AM. The atrial natriuretic factor (ANF) and CaN mRNA expression of the myocardial tissue were quantitatively analyzed by Real-time PCR. The protein expression of CaN was detected by Western blot.
RESULTSCompared with the MCT model group, preventive treatment of TG at the 3 doses could significantly reduce RVSP, RVHI, RVW/BW, and ANF mRNA expression, and decrease Ca2+ concentration in myocardial cells, CaN mRNA and protein expression in the myocardial tissue.
CONCLUSIONTG could obviously improve MCT-induced right ventricular hypertrophy, which was possibly achieved through suppressing MCT-activated CaN signal transduction.

4.Causes of Pulmonary Infection after Stroke during Recovery Period

Objective To explore the causes of pulmonary infection of stroke patient during recovery period. Methods 122 stroke patients combined with pulmonary infection in neural rehabilitation department from January 2008 to July 2011 were analyzed for the relative causes according to clinical nursing. Results Repeated pulmonary infection was correlated with dysphagia and tracheotomy of acute stage (P<0.05); Bilateral pulmonary infection was correlated with dysphagia, tracheotomy of acute stage and age (P<0.05). Conclusion Pulmonary infection is correlated with dysphagia, tracheotomy of acute stage and age in sequence.
5.Effect of Lep d2 from Lepidoglyphus destructor as a vaccine for specific im-munotherapy in murine with asthma

Objective To assess the effect of Lep d2 from Lepidoglyphus destructor as a vaccine for specific immunothera?py on murine model of asthma. Methods Thirty BALB/c mice(SPF)were randomly categorized into a PBS group,an asthma group,and a Lep d2 SIT group. The mice in the asthma group and Lep d2 SIT group were sensitized by intraperitoneal injection with extracts of dust mites on Days 0,7th,and 14th,while those in the PBS group were injected with PBS. From the 21st day, the asthma group and Lep d2 SIT group exposed to the extracts of dust mites were stimulated by aerosol inhalation for 7 succes?sive days. During the period of the 25th-27th Day,the mice in Lep d2 SIT group were injected intraperitoneally with Lep d2 al?lergen for SIT 30 min before nasal inhalation,whereas the PBS group and asthma group were treated with only PBS. Twenty?four hours after the final inhalation,all the mice were sacrificed,the bronchoalveolar lavage fluids(BALFs)were collected. The lev?els of IFN?γ,IL?5 and IL?13 in the BALF and the supernatant of splenocyte culture solution(SSCS)as well as the levels of spe?cific IgE(sIgE)and sIgG2a in the sera were detected by ELISA. The lung tissues of the mice in the above 3 groups were stained by haematoxylin and eosin(H&E)and observed by a microscope. Results The symptoms of acute asthma attack were observed in the mice of the asthma group and Lep d2 group,but not in the PBS group. The allergic inflammation changes in lung in the Lep d2 SIT group were significantly alleviated compared with those in the asthma group. The concentrations of IFN?γin BALFs and SSCS of the mice in the Lep d2 SIT group were significantly higher than those in the asthma group(both P<0.01),while the levels of IL?5 and IL?13 in the former group were significantly lower than those in the latter group(all P<0.01). Mean?while,the level of sIgE of mice in the Lep d2 SIT group was significantly lower than those in the asthma group(P<0.01),while the level of sIgG2a of mice in the former group was higher than those in the latter group(P<0.01). Conclusion Lep d2 allergen as a vaccine can alleviate the allergic symptoms in the lung of mice effectively after allergen specific immunotherapy.
6.Effect of Intensive Cognitive Training on Activities of Daily Living of Patients with Cognitive Disorder post Stroke

Objective To observe the effect of intensive cognitive training on the cognitive function and activities of daily living (ADL) of patients with cognitive disorder post stroke. Methods 80 patients with stroke with score of Mini-mental State Examination (MMSE) being 13~18 were divided into 2 groups. The control group (n=40) accepted routine rehabilitation nursing and the experimental group accepted intensive cognitive training in addition. They were assessed with MMSE and modified Barthel index (MBI) before and after intervention of 3 months. Results The scores of MMSE and MBI improved more in the experimental group than in the control group after intervention (P< 0.05). Conclusion Intensive cognitive training can improve the ADL of patients with cognitive disorder post stroke.
7.Effects of APRV-BIPAP ventilation On lung recruitment/open maneuvers in piglets with acute lung injury

Objective To study the effects of APBV (airway pressure release ventilation) / BIPAP(bipha-sic positive airway pressure) on lung recmitment/open maneuvers in piglets with acute lung injury. Method The model of acute lung injury (ALI) was induced by E. coll. intraperitoneal injection in piglets. Based APRV/BI-PAP model, the different pressure combinations (Phigh/Plow) of RMs increased gradually, such as RMI(30/15),RM2(35/20), RM3(40/25), RM4(45/30),RM5(50/35), RM6(55/40), RM7(60/45) cmH2O. The effects of stepwise RMs were studied by computed tomography (CT) at iaspiratory phase. Meantime the oxygen index (PaO2/FiO2), hemodynamic parameter and mean pressure of airway (Pmean) were continuously observed. The piglets were killed when RiMs finished and pulmonary pathological examination were done routinely by optical microscope. Data was analyzed by self-contrast method, using SPSS 11.5 software package. Results were expressed as mean ± standard deviation (x±s). Multiple comparisons were made with One-way ANOVA. Pearson correlative analysis was used to describe the relativity of PaO2/FiO2 and the collapsed alveolar area. Changes were considered as statistically significant if P value was less than 0.05. Results Eight piglets with ALl model were successfully made and all of them showed different degree of alveolar collapse under chest CT scan. During RMs their PaO2/FiO2 increased obviously (P<0.05) were decreased obviously (P<0.05) too, specially after RM2 finished (P<0. 05). But the alveolar over-inflatian could be found in some non-diseased area. The heart rate (HR) increased and mean artery blood pressure (MAP) decreased significantly while the pressure combinations (Phigh/Plow) of RMs were added gradual]y ( P<0.05). Meantime the Pmean and Ppeak inspiratory pressure (PIP) of airway and central venous pressure (CVP) were increased significantly ( P<0.05). But when RMs were finished,all of these indexes were hack to the levels of pre-RMs. Even there were no barowaumas happened, such as pneumothorax and pneumomedistinum, the alveolar overdistention and interruption of the alveolar separation still could be seen by pathologic examination. Conclusions RMs could be done well by APRV/BIPAP. Phigh/Plow (35/20cmH2O) would be the best pressure combination with more efficacy of RMs and less influence on hemodynamics,airway pressure indexes and others. When the effect of RMs was satisfied enough, setting Phigh/Plow to 30/15cmH2O for 20 mitt may maintain the good efficacy of RMs.
8.Effects of Creatine Phosphate Sodium on Plasma Cardiotrophin-1 and N Terminal Probrain Natriuretic in Elderly Hypertensive Patients with Heart Failure

Objective To investigate the effect of hosphocreatine therapy on the plasma cardiotrophin-1(CT-1) and N terminal probrain natriuretic (NT-proBNP) in elderly hypertensive patients with heart failure. Methods A total of 76 hy-pertensive patients with heart failure, aged 65 or over, were randomly divided into treatment group and control group (n=38 for each group). The control group received routine anti-heart failure treatment. The treatment group received conventional therapy plus creatine phosphate sodium for 2 weeks. The plasma levels of CT-1 and NT-proBNP were determined in two groups. The plasma CT-1 level was measured by a double antibody sandwich enzyme-linked immunosorbent assay (ELISA). The plasma level of NT-proBNP was tested by Rui Pu fluorescent dry quantitative analyzer. Results The plasma levels of CT-1 and NT-proBNP were significantly lower after treatment in two groups (P<0.01). The plasma levels of CT-1 and NT-proBNP were significantly decreased in treatment group than those in control group (P<0.05). The total effective rate was 89.47%in treatment group, which was significantly higher than that of control group (71.05%, P<0.05). Symptoms of heart failure improved in one week (21 cases in treatment group/9 cases in control group) and in two weeks (13 cases in treatment group/18 cases in control group). Conclusion The conventional therapy plus creatine phosphate sodium can decrease the plasma CT-1 and NT-proBNP levels in elderly hypertensive patients with heart failure, which improves symptoms of heart failure in a shorter period of time.
9.Relationship Between Myocardial Ischemia and Left Ventricular Diastolic Function by Fractional Flow Reserve Evaluation in Patients With Coronary Artery Disease

Objective: To explore the relationship between myocardial ischemia and left ventricular diastolic function (LVDF) by fractional lfow reserve (FFR) evaluation in patients with coronary artery disease (CAD).
Methods: A total of 57 patients with chest pain were studied, the diagnosis of CAD was confirmed by coronary angiography, which indicated 50%-70%of coronary stenosis. All patients received FFR examination and they were divided into 2 groups:Experimental group, the patients with FFR<0.80, n=27 and Control group, the patients with FFR≥0.80, n=30. The basic condition and risk factors affecting LVDF were compared between 2 groups. Echocardiography was conducted for evaluating left ventricular end-diastolic dimension (LVEDD), left atrial dimension (LAD), left ventricular ejection fraction (LVEF) and E/e ' value in both groups.
Results: The patients’ gender, age, history of hypertension, diabetes, blood levels of cholesterol, TG, LDL-C, HDL-C and glucose were similar between 2 groups, P>0.05. Compared with Control group, the Experimental group had the increased LVEDD, LAD and E/e ' value and decreased LVEF, all P<0.05.
Conclusion: The impact of CAD on early diastolic function depends on functional myocardial ischemia in relevant patients.
10.Application of the improved lateral decubitus position in the operation

Objective To explore the application effect of the improved lateral decubitus position in the operation.Methods 160 cases were randomly divided into the modified lateral surgical group and traditional group two groups (n =80 for each group).Fourth edition of the traditional group was “routine medical care and technical operations” approach placed.Modified group:before anesthesia,a small pad under the chest wall soft pillow,according to lesion location after anesthesia choose left or right lateral position,his hands in front of music was set to sleep like the two arms of a small soft pillowbetween the pad; Front legs apart,on the tuiqu Xi,the next leg straight; head elevated after the first lap put the gap at the ear; Chest and abdomen and the back of each fixed a cylindrical yam bags,and finally tied with fixed hip,knee.Placed in two groups were observed an average completion time position,the incidence of intra-operative movement position,pressure and skin after postoperative follow-up physical fatigue,nerve damage,etc.Results Placed in modified group method average completion time is shorter than the traditional group,skin pressure,body fatigue,nerve injury rate of postoperative complications was significantly lower than the traditional group,intra-operative position more stable than the traditional group [ ( 13.97 ± 1.63 ) vs (6.71 ± 12.9 ),t =31.238,P < 0.01 ; x2 =6.826 7,8.901 1,1 1.756 6,P < 0.01 ).Conclusions Placed by the improved method can shorten the display side of time.For BP,HR,SpO2,respiration rate change had no effect,and complications of place due to improper posture can be significantly reduced and further enhance the quality of surgical care.
