1.Neurolymphomatosis, a case report

Neurolymphomatosis(NL) is characterized by lymphomatous infiltration of the peripheral nervous system. We report a case of neurolymphomatosis(NL) which was confirmed by sural nerve biopsy. Sural nerve specimen from a 49-year-old female patient with weakness of limbs was examined with routine histochemical and immunohistochemistry staining, in which the first antibodies against CD3, CD20, CD45, CD45RO and CD68 were used. Numerous T-lymphoma cells invaded in the adipose tissue of epineurium of sural nerve. The nerve biopsy showed marked axonal degeneration of myelinated fibers. The clinical and histopathologic findings confirmed the diagnosis of neurolymphomatosis.

3.Differences in distribution and drug resistance of pathogens for hospital-acquired urinary tract infections between patients in internal medicine wards and surgical wards

Objective To investigate the distribution and drug resistance of pathogens for hospital-acquired urinary tract infections between patients in internal medicine wards and in surgical wards .Methods A total of 586 midstream urine samples were collected from patients in the First Municipal Hospital of Qinhuangdao during January 2012 and December 2014.Vitek 2 Compact system was applied in bacteria identification and drug sensitivity tests .Excel and SPSS 11.5 software were applied for data analysis . Results A total of 661 strains were isolated , in which 404 strains were from internal medicine wards and 257 strains were from surgical wards .Escherichia coli (44.6%vs.33.1%) and Enterococcus (23.0%vs. 16.3%) infections were more common in the internal medicine wards (χ2 =8.620 and 4.309, P<0.05), while the occurrence of Pseudomonas aeruginosa infection (4.0%vs.24.5%) was higher in surgical wards (χ2 =63.056, P <0.01).Escherichia coli and Klebsiella pneumonia strains were highly sensitive to piperacillin/tazobactam, cefptetan, amikacin, imipenem, and meropenem, and the sensitivity rates were from 85% to 100.0%.The sensitivity rates of Escherichia coli to ampicillin/sulbactam, levofloxacin and ciprofloxacin were <30%, and strains from surgical wards had lower sensitivity rates to these drug than those isolated from internal medicine wards (χ2 =4.987, 4.575 and 5.359, P<0.05).The sensitivity rates of Klebsiella pneumonia isolated from internal medicine wards to ceftazidime , gentamicin and aztreonam were 68.8%, 60.6% and 69.7%, which were higher than those isolated from surgical wards (36.0%, 32.0%, and 40.0%), and the differences were of statistical significance (χ2 =6.068,4.661 and 5.115, P<0.05).Pseudomonas aeruginosa strains were highly sensitive to piperacillin/tazobactam and amikacin, and the susceptibilities of strains isolated from surgical wards (98.4%and 96.8%) were higher than those isolated from internal medicine wards (75.0% and 81.3%) (χ2 =11.797 and 5.221, P <0.05). Pseudomonas aeruginosa strains isolated from surgical wards were also highly sensitive to cefepime (92.1%), but the sensitive rate of strains from internal medicine wards was only 37.5%, and the difference was of statistical significance (χ2 =24.696, P<0.01).Enterococcus faecium and Enterococcus faecalis were sensitive to tigecycline , vancomycin and linezolid with the sensitivity rates over 95%.Except quinupristin/dalfopristin and tetracycline , the sensitivities of Enterococcus faecalis to other antibiotics were higher than Enterococcus faecium.Susceptibility of Enterococcus faecium from surgical wards (33.3%) to moxifloxacin was lower than those from internal medicine wards (70.8%), and the difference was of statistical significance (χ2 =4.629, P <0.05).Conclusion There are differences in distribution and antimicrobial susceptibility of pathogens isolated from internal medicine wards and from surgical wards .

5.Recent progress of potential effects and mechanisms of chlorogenic acid and its intestinal metabolites on central nervous system diseases.

Chlorogenic acid displays several important roles in the therapeutic properties of many herbs, such as antioxidant activity, antibacterial, antiviral, scavenging free radicals and exciting central nervous system. Only about one-third of chlorogenic acid was absorbed in its prototype, therefore, its gut metabolites play a more important role in the therapeutic properties of chlorogenic acid. It is necessary to consider not only the bioactivities of chlorogenic acid but also its gut metabolites. This review focuses on the potential activities and mechanisms of chlorogenic acid and its gut metabolites on central nervous system diseases.

6.A survey on the reasons of smoking and nicotine dependence in patients with schizophrenia

Objective To investigate smoking with its reasons and nicotine dependence in schizophrenic inpatients. Methods Self -designed questionnaire,russell reason for smoking questionnaire and fagerstrom test for nicotine dependence (FNTD ) were used to investigate the reasons of current smoking status in 245 schizophrenic inpatients. Results There were 43 (17.55%)current smoking among 245 schizophrenic inpatients,including 41 (95.35%)males and 2(4.65%)females;and 21(48.84%)were severely dependent on nicotine who was all male.The average score of FNTD was 4.15 ±2.18.According to the scores of russell,the total scores of smoking reasons were 22.92 ±12.89and 27.67 ±17.36 in male and female patients respectively,with no significant difference(P>0.05).The highest scores of smoking questionnaire were in addiction,activities of hands or mouth and calmness.Conclusion The schizophrenic inpatients were severely dependent on nicotine.The main reasons are addiction,activities of hands or mouth and calmness.
7.Recovery of anterior corneal biometry after discontinuation of orthokeratology

Objectivc To observe the short-term changes of anterior corneal biometry after discontinuation of orthokeratology in patients with 2-year wearing.Methods Retrospective study.Sixty myopic patients aged from 8-14 years old during October 2012 and October 2014 were wearing orthokeratology for 2 years in Tianjin Medical University Eye Hospital.According to the degree of myopia,they are divided into three groups(SE≤-2.00 D for group A,-2.00 D < SE ≤-4.00 D for group B and -4.00 D < SE ≤-6.00 D for group C).The recovery of anterior corneal curvature,including flat K(FK),steep K(SK),average K (AVEK),changes of axial length and central corneal thickness (CCT) at 1 week,2 weeks,1 month after discontinuation of orthokeratology were observed.Results There was no statistical differences in FK,SK,AVEK before and 2 weeks after wearing orthokeratology in group A (all P > 0.05).While in group B,there was no significant difference in FK,SK,AVEK before and 1 month after wearing orthokeratology;There were statistically differences in FK,SK,AVEK at 1 month after discontinuation in group C compared with the baseline (all P <0.05).As for CCT,there was no statistical differences among group A,B,C after discontinuation of orthokeratology for 2 weeks (all P > 0.05).There were statistical differences in the axial length between 1 week and 2 weeks after discontinuation of orthokeratology in group B and C (all P < 0.05);There were statistical differences in the axial length between 1 month and 2 weeks after discontinuation of orthokeratology in three groups (all P < 0.05);Compared with the state before wearing orthokeratology,the increase of axial length in group A,B,C were (0.43 ± 0.36) mm,(0.35 ± 0.21)mm and (0.36 ± 0.29) ram,respectively.Conclusion The time course of returning to the original corneal parameter varies among different degree of myopia,and the axial length has no significant growth after short-term discontinuation.
8.Mechanisms of neuroprotection of silent information regulation 2 homolog 3 against hypoxic injury in hypoxic preconditioning

Objective To investigate the mechanisms underlying neuroprotection of silent information regulation 2 homolog 3 ( SIRT3 ) against hypoxia via preconditioning.Methods PG12 cells were randomly divided into control,hypoxic preconditioning ( Hyp),Hyp with oxygen-glucose deprivation (OGD) and OGD.MTT assay and DAPI staining were used to evaluate cellular viability.MitoSOX Red was used to measure the production of mitochondrial superoxide.The protein expression of SIRT3,PGC-1α and MnSOD were assessed by Western blot.Recombinant SIRT3 was also given to further investigate its roles in hypoxic preconditioning.Results The preconditioned PC12 cells had a higher survival rate.When expressed as a percentage of the control group,MTT values following 6 h OGD were around 51.0% in the OGD group but around 74.7% in the Hyp + OGD group ( F = 56,P < 0.01).Mitochondrial ROS after Hyp was less than the OGD group.Both Hyp + OGD and OGD increased the expression of SIRT3,PGC-1α and MnSOD proteins,and these increases were greater after Hyp + OGD.Similarly,the application of recombinant SIRT3 to OGD also further increased the expression of these proteins.Conclusions Hypoxic preconditioning can protect PC12 cells against hypoxic injury.One possible mechanism of hypoxic preconditioning is via SIRT3 to upregulate PGC-la and,in turn,MnSOD to reduce generation of ROS.

10.Clinical Observation of Fu San Zhen plus Xing Nao Kai Qiao Needling for Urinary Incontinence After Cerebral Infarction

Objective To observe the clinical efficacy of Fu San Zhen (acupuncture at three abdominal acupoints) plus Xing Nao Kai Qiao (brain-awakening and orifice-opening) needling in treating urinary incontinence after cerebral infarction. Method Sixty patients with urinary incontinence after cerebral infarction were randomized into an experimental group of 20 cases and a control group of 20 cases. The experimental group was intervened by Fu San Zhen [Qihai (CV6) and bilateral Shuidao (ST28)] plus Xing Nao Kai Qiao needling by selecting Shuigou (GV26), Neiguan (PC6) and Sanyinjiao (SP6). The control group was intervened by ordinary acupuncture [Zhongji (CV3), Qihai (CV6), Shenshu (BL23) and Pangguangshu (BL28)] plus Xing Nao Kai Qiao needling method by selecting Shuigou (GV26), Neiguan (PC6) and Sanyinjiao (SP6). The maximum bladder capacity, residual urine volume and the score of International Consultation on Incontinence Questionnaire Short-form (ICIQ-SF) of the two groups were observed before and after the intervention, and the clinical efficacies were also compared. Result After the intervention, the maximum bladder capacity, residual urine volume and the score of ICIQ-SF were significantly changed in both groups (P<0.01). There were significantly statistical differences in comparing the maximum bladder capacity, residual urine volume and the score of ICIQ-SF between the two groups after the treatment (P<0.01). The total effective rate was 95.0% in the experimental group versus 75.0% in the control group, and the difference was statistically significant (P<0.01). Conclusion Fu San Zhen plus Xing Nao Kai Qiao needling can produce a more significant efficacy in treating urinary incontinence compared to ordinary acupuncture plus Xing Nao Kai Qiao needling.
