1.Nursing of patients with rheumatic valvular heart disease with atrial fibrillation treated with CoxⅢ labyrinth and simultaneous valve replacement operation

Objective To summarize the effect and experience of nursing of patients with rheumatic valvular heart disease with atrial fibrillation treated with Cox Ⅲ labyrinth and simultaneous valve replace-ment operation. Methods A retrospective analysis was carried out in the nursing of 58 patients with rheumatic valvular heart disease with atrial fibrillation treated with Cox Ⅲ labyrinth and simultaneous valve replacement operation in our hospital from April 2006 to July 2008. Results One patient died of periop-erative renal failure,one died of low cardiac output,one died of infectious endoearditis.Tbe mortality rate reached 5.2%.Other patients were discharged from hospital after completely cured.The survived patients maintained sinus rate during hospitalization days after operation and short-term follow-up after being dis-charged.Among 43 patients who completed follow-up of 6 months after operation,3 patients (7.0%) damon-strated atrial fibrillation,while others still kept sinus rate. Conclusions Attention should be paid to psy-chological nursing,respiratory tract preparation,cardiac function monitoring and nursing of anticoagulation treatment for patients treated with Cox Ⅲ labyrinth and simultaneous valve replacement operation.
2.Promotion of proliferation and migration of Müller cells by RPE cells in a co-culture system

AIM: To investigate the role of retinal pigment epithelium (RPE) in the growth of Müller cells using a co-culture system in vitro . METHODS: Müller cells were cocultured with RPE cells under both normoxic and hypoxic conditions in Transwell chamber culture system. Müller cell proliferation was evaluated by MTT assay. The number of cells which migrate through micropores and stay on the outer bottom side of insert systems were observed and counted. RESULTS: The activities of proliferation and migration of Müller cells when cocultured with RPE cells were significantly higher than those of the Müller cells when cultured alone at all time points under both normoxic and hypoxic conditions. However, for both the coculture and control groups, there is no significant difference between the measurements at 3 and 6 hours. CONCLUSION: Evidence suggests that RPE, when co-cultured with Müller cells, can stimulate migration and proliferation of Müller cells under both hypoxic and normoxic conditions in a time-dependent manner; how-ever, there is no evidence to support the synergetic interaction of RPE and Müller cells co-cultured under hypoxic conditions.
3.Clinical value of three kinds of helicobacter pylori detection methods

Objective To discuss the clinical value of three kinds of helicobacter pylori (HP) detection methods and find out the appropriate method for clinical application of the HP detection .Methods A total of 109 patients received gastroscopy ,the efficacy of RUT ,13C-urea breath test(13C-UBT) and the immunoCard STAT helicobacter pylori stool antigen (HpSA) for detection of HP were compared .Results RUT positive rate of the two pieces of gastric mucosa (the gastric antrum and the gastric body) was 34 .86% ,higher than that of single piece of gastric mucosa (gastric antrum or stomach body ) and two pieces of gastric mucosa (stomach) ,the difference was statistically significant (P<0 .05);The diagnosis of HP infection was based on 13 C-UBT ,sensitivity and accuracy of the two pieces of gastric mucosa (the gastric antrum and the gastric body) were 72 .97% and 80 .73% ,respectively , higher than that of two pieces of gastric mucosa and gastric antrum ,and lower than the four pieces of gastric mucosa (two pieces of gastric antrum + two pieces of the gastric body) .There was no statistically significant difference among RUT ,13 C-UBT and the immunoCard STAT HpSA(P>0 .05) .The diagnosis of HP infection was based on 13C-UBT ,the immunoCard STAT HpSA sensi-tivity ,specificity and accuracy were 86 .49% ,95 .83% ,92 .66% ,respectively ,which were higher than RUT .Conclusion Two pieces of gastric mucosa (the gastric antrum and the gastric body) materials is appropriate for clinical promotion RUT based solution . RUT ,13C-UBT and hpsas immune quick check card are all clinical detection of HP and reliable methods ,but hpsas immune quick check card is more suitable for clinical promotion .
4.Antitumor effect of allogeneic NK cells on human multi-drug resistant nasopharyngeal carcinoma cells subcutaneously transplanted in BALB/c nude mice

Objective:To study the inhibitory effect of allogeneic natural killer(NK) cells on subcutaneously transplanted human multi-drug resistant nasopharyngeal carcinoma cells(CNE2/DDP) in BALB/c nude mice.Methods:Human leucocyte antigen(HLA) genotypes of CNE2/DDP cells and the genotypes of inhibitory killer cell immunoglobulin-like receptor(KIR) in NK cells(isolated from 3 healthy persons by immuno-magnetic microbead technique) were analyzed by PCR-SSP.Twelve BALB/c nude mice were evenly divided into 2 groups:the control group and the treatment group.Mice in the treatment group were injected subcutaneously with 1?106 CNE2/DDP cells together with 3?107 NK cells via the tail veins;mice in the control group were injected with 1?106 CNE2/DDP cells subcutaneously.The tumor formation time,tumor formation rate and changes of tumor size were observed.Three weeks after tumor formation,all the mice were killed and human NK cells in peripheral blood were analyzed by flow cytometry;the tumors were weighed and the tumor inhibitory rates were calculated.Results:The HLA genotypes of CNE2/DDPcells were A2,24,B18,35,Cw4,and 7;the KIR genotypes of the 3 healthy persons were KIR2DL1,KIR2DL3,KIR3DL1,and KIR3DL2.There were mismatches between the KIRs expressed in NK cells and HLA class Ⅰ molecules expressed in the CNE2/DDP cells.NK cells obviously inhibited the growth of CNE2/DDP xenograft in nude mice.The tumor formation periods of control group and NK cell group were(17.17?1.17) d and(24.83?1.47) d,respectively(P
5.Effects of ulinastatin on pulmonary and hepatorenal function of elderly patients undergoing resection of esophageal cancer

Objective To investigate the effects of ulinastatin on pulmonary and hepatorenal function of elderly patients undergoing resection of esophageal cancer. Methods The clinical data of 40 elderly patients with esophageal cancer who had been admitted to Southwest Hospital from January 2005 to January 2008 were retrospectively analyzed. All the patients were randomly divided into control group (n = 20) and ulinastatin group (n = 20) according to random number table. Patients were administered with total parenteral nutrition, and patients in ulinaslatin group were additionally instilled with 4×10~5 U/d of ulinastatin. The levels of PaO_2, PaCO_2, PaO_2/FiO_2, alanine aminotransferase (ALT), aspartate aminotransferase (AST), total bilirubin (Tbil), blood urea nitrogen, creatinine and uric acid were detected preoperatively and at postoperative day 2 and day 6. All the data were analyzed by t-test and chi-square test. Results The levels of PaO_2 and PaO_2/FiO_2 were (87.3±4.2) mm Hg (1 mm Hg =0.133 kPa) and (416±20)mm Hg in ulinastatin group, which were significantly higher than (79.0±4.3)mm Hg and (376±20)mm Hg in control group (t =6.2, 6.2, P <0.05). The levels of ALT, AST and Tbil were (23±7)U/L, (38±8)U/L and (13.4±3.0) μmol/L in ulinastatin group, and (39±8) U/L, (50±9) U/L and (24.5±6.0) μmol/L in control group, with significant difference between the 2 groups (t = 7.0, 4.4, 7.6, P < 0.05). The levels of uric acid, blood urea nitrogen and creatinine were (279±84) μmol/L, (4.1±1.7) mmol/L and (66±12) μmol/L in ulinastatin group, and (386±67)μmol/L, ( 8.9±2.7) mmol/L and (95±38) μmol/L in control group, with significant difference between the 2 groups (t = 4.4, 6.4, 3.3, P < 0.05). The recurrence of complications in ulinastatin group was significantly lower than that in control group (χ~2 = 4.8, P <0.05). Conclusion Postoperative supplementation of ulinastatin and total parenteral nutrition is helpful in improving the pulmonary and hepatorenal function of elderly patients undergoing resection of esophageal cancer.
6.Research on the Application of Quality Control Circles to Improve the Turnover of the Surgery and the Satisfaction of Medical Staff

Objective:To discuss the implementation and experience of the quality control circle ( QCC) activi-ty in shortening the interval time of consecutive operation and speeding up the turnover of operating table. Method:We formed a QCC team and explored the influential factors of the interval time of consecutive operation using quali-ty control technique. We found out the real reason, put forward corresponding countermeasures, and organized the implementation. Results: Before the implementation of QCC, the average interval time of consecutive operation was 24. 6 minutes, whereas it decreased to 16. 2 minutes after the implementation of QCC with the progress rate of 64. 87%. Through the QCC activities, the problem-solving ability, responsibility, communication and coordina-tion ability, motivation, confidence, team cohesion, and the use of quality control technique of the QCC members has improved significantly. Conclusion:Through the quality management tools of QCC, the interval time of con-secutive operation has greatly shorten, the turnover of operation table has improved, and doctors′ and operating room nurses′satisfaction has improved significantly.
7.Relationship between methylation of Ras association domain family-1A and protein expression in ;endometriosis

Objective To analyze the methylation status and protein expression of Ras association domain family- 1A (RASSF1A) in endometriosis (EMS). Methods The ectopic and corresponding eutopic endometrium tissues were collected from 45 women with EMS and normal endometrium tissues of 20 women without EMS. The methylation status of RASSF1A was examined by methylation specific PCR (MSP). Immunohistochemistry was performed to measure the level of RASSF1A in endometrium tissues. Results The RASSF1A protein expression rate in ectopic endometrium, eutopic endometrium, and normal endometrium was 37.78%(17/45), 60.00%(27/45) and 85.00%(17/20), and there was significant difference (χ2 = 13.136, P = 0.001). The frequency of aberrant methylation of RASSF1A was 55.56%(25/45), 33.33%(15/45) and 0 in ectopic endometrium , eutopic endometrium, and normal endometrium, and there was significant difference (χ2 =18.770, P = 0.000). The frequency of aberrant methylation of RASSF1A had no significant differnce throughout the menstrual cycle in ectopic endometrium and eutopic endometrium: 66.67%(14/21) vs. 45.83%(11/24), 38.10%(8/21) vs. 29.17%(7/24), P>0.05. In ectopic endometrium, the frequency of aberrant methylation of RASSF1A inⅢ-Ⅲstage was significantly higher than that in Ⅰ-Ⅱstage (χ2=5.940, P=0.015). In ectopic endometrium and eutopic endometrium, the RASSF1A protein expression had negative correlation with aberrant methylation of RASSF1A (r =- 0.594、- 0.577, P<0.01). Conclusions Epigenetic inactivation of RASSF1A through aberrant promoter methylation may be strongly correlated with the formation and progression of EMS, and assessment of RASSF1A methylation status in eutopic endometrium may be a potentially useful biomarker to enhance the early detection of EMS.
9.ROC Curve of Negative Multi-detector Row CT Cholangiopancreatography in Patients with Suspected Obstructive Biliary Diseases

Objective To evaluate the value of diagnostic negative MDCT cholangiopancreatography in patients with suspected obstructive biliary diseases by receiver operating characteristic(ROC)curve. Methods Dual-phases contrast-enhanced MDCT of the abdomen was performed in 30 patients.Multiplanar reformation,minimum intensity projection and volume rendering were generated using original data in vein phase.By double-blind method,the images were readed by two radiologists.ROC curves were analysed with software SPSS11.5,the different results of reading images between two radiologists was evaluated with Kappa test.Results The Az of the area under the ROC curve of MDCT cholangiopancreatography for the two observers was 0.968 and 0.962 respectively,it was more than 0.9,which showed that the value of diagnosing the suspected obstructive biliary diseases by negative MDCT cholangiopancreatography was good.Kappa index test was satisfactory.Conclusion Negative MDCT cholangiopancreatography may be a routine choice in diagnosing the suspected obstructive biliary diseases.
10.The correlative study of detecting depths of arteries with transcranial doppler and head circumference in children

Objective To evaluate the correlation of the depths of detecting arteries vessel in transcranial dopplor(TCD) and child head circumference.Methods Five hundreds and eighty-eight health children were selected at random for the study. TCD ultrasonography was used to detect the optimal depths of arteries, middle and anterior and posterior cerebral arteries for each child, together with the head circumference.Results The data showed that the depths of detecting cerebral arteries significantly changed in different age groups, probably due to the growth of the head circumference.Conclusion There was close relationship between the depths of detecting cerebral arteries and child head circumference, which is of clinical significance.
