2.Preliminary Study of Cerebrovascular Capacity with MR Perfusion-Weighted Imaging after Administration of Acetazolamide

Objective To study the value of MR perfusion-weighted imaging(PWI)after administration of acetazolamide(ACZ)in evaluating the cerebrovascular capacity.Methods MR perfusion-weighted imaging were performed before and after ACZ administeredorally in 7 cases with cerebral infarction.The values of rCBV and rMTT were analysed and the cerebrovascular capacity were evaluated.Results The prolonged rMTT was found in rMTT map before and after ACZ in all cases.Only 5 subjects were abnormal at rCBV map visually.rMTT was increased at the abnormal region on rMTT map.The abnormal regions at rCBV maps included both rCBV increased or decreased.The abnormal region was larger at rMTT map than that at rCBV map.Conclusion MR perfusion-weighted imaging with ACZ test has significant value in assessing the cerebrovascular capacity.
3.Comparison of Motor Unit Number Estimates and Conventional Electromyogram in the Detection of Neuromuscular Diseases

: ObjectiveTo compare the clinical significance of motor unit number estimates (MUNE) and conventional electromyogram (EMG) in the evaluation of neuromuscular disorders. MethodsUnder the model of MUNE or quantity of motor unit potential (QMUP), 53 patients with various neurogenic disorders were tested with EMG at extensor digitorum brevis, thenar, or hypothenar eminence. ResultsFor 8 patients with amyotrophic lateralizing sclerosis, large and long-duration polyphasic potentials were detected in needle electrode EMG tests, and the motor unit numbers reduced. For 45 patients with peripheral neuropathies, few distinctive features could not be detected by needle electrode EMG, but motor unit numbers reduced in 2 patients; spontaneous activities were the only abnormality in the other 2 patients, and there were no obviously abnormal changes in the configuration and size of motor unit potentials and in motor unit numbers; EMG tests revealed neurogenic features and motor unit numbers significantly reduced in the remaining 41 patients. ConclusionConventional EMG and MUNE can work for the diagnosis of neuromuscular disorders, and do better in combination.
4.Analysis on clinical features and treatment of herpes zoster patients hospitalized in real world.

From the hospital information system (HIS) of 20 national grade III-A general hospitals, 2 960 cases of herpes zoster as the research object, analyzes the relations between the general information, syndrome of traditional Chinese medicine (TCM), western medicine combined diseases, the relationship between the solar term and the incidence of herpes zoster, and the combined use of Chinese and western medicine. Among the patients with 46-65 year old has the highest percentage of diseased; admission to general outpatient clinic is the most; the most common medical payment is medicare; combined disease such as hypertension, diabetes and coronary heart disease is more common; early treatment effect of herpes zoster is better than the sequelae; summer and autumn solar term patients is hospitalized more, TCM syndrome is damp heat of liver fire; about drugs, western medicine is the most commonly used vitamin B1 and mecobalamin, traditional Chinese medicine is the most frequently used Danhong injection, combination therapy with promoting blood circulation drugs and neurotrophic drugs. Thus, herpes zoster, more common in elderly patients, with no obvious relationship between solar term, should be early diagnosis and early treatment, often with combination of Chinese traditional and western medicine treatment.

5.Clinical efficacy of Yindanxinnaotong soft capsules combined with zopiclone in treatment of elderly patients with chronic insomnia

Objective To observe the therapeutic effect of Yindanxinnaotong soft capsules combined with zopiclone on elderly patients with chronic insomnia. Methods Ninety-two elderly patients with chronic insomnia, admitted to our hospital from January 2010 to August 2011, were randomly assigned into Yindanxinnaotong soft capsule treatment group (n=30),zopiclone treatment group (n=31) and combined treatment group (n=31); these patients received treatment for 4 weeks. With pre-treatment and post-treatment self-control statistical method,Pittsburgh sleep quality index (PSQI) was used to evaluate the sleep quality, and polysomnogram was used to detect the changes in sleep architecture. Results All of 3 treatment groups enjoyed considerably improved PSQI, scores especially after 4 weeks of treatment; the improvement in the combined treatment group was significantly more obvious than that in the Yindanxinnaotong soft capsule treatment group and zopiclone treatment group (P<0.05); the number of patients enjoyed S3 and S4 during relatively non-rapid eye movement sleep (REM sleep) was obviously increased 2 weeks after treatment; the number was further increased 4 weeks after treatment with combined treatment group enjoying the best results; most of the patients could recover the sleep continuity.All of 3 treatments considerably prolonged total sleep time,slow wave sleep (S3+S4) time and relatively rapid eye movement sleep (REM sleep) time,which markedly repaired the disruption in sleep architecture and improved sleep quality. The sleep quality of the elderly from combined treatment group could be more effectively and rapidly improved as compared with that of the other 2 groups; Yindanxinnaotong soft capsules used alone could receive the same result,but it need a longer time. Conclusion The combination of Yindanxinnaotong soft capsules and zopiclone could be an alternative method in the treatment of patients with chronic insomnia.
6.Continuous hemofiltration for prevention of contrast induced nephropathy in patients with advanced chronic kidney disease after percutaneous coronary intervention

Objective To assess the value of hemofiltration in the prevention of contrast induced nephropathy(CIN) post percutaneous coronary intervention(PCI) in severe chronic kidney disease stages(CKD) patients. Methods We evaluated 30 CKD patients who underwent PCI followed by bedside hemofiltration. We measured serum creatinine levels before PCI, 24 hours, 72 hours, and one week after PCI, and calculated creatinine clearance(CrCl) according to Cockcroft and Gault equation.We observed the incidence of CIN, and the short term clinical efficacy of hemofiltralion. Results The average age of the 30 patients was (72.87 ± 8.71) years old, with 21 (70%) male patients. The stages of CKD among the patients included CKD 3 (3 patients, 10%), CKD 4 (20 patients, 66.7%) and CKD 5 (7 patients, 23.3%). The average duration of hemofiltration was (7.5±4.1) hours. Serum creatinine before PCI, 24 hours, 72 hours and 1 week after PCI was (498.7±143.7)μmol/L, (353.2±128.0)μmol/L, (450.0±132.2)μmol/L, (488.0±145.7)μmol/L respectively,and CrCl was (20.3±10.2) ml/min, (36.5±14.3) ml/min, (28.3±10.4) ml/min, (21.0±10.3) ml/min respectively. There was no CIN. Mean follow-up was (3.1 ± 2.6) months. There was no new cardiovascular events, and no new patients need to rely on long term hemodialysis. Conclusions For patients with severe renal insufficiency, hemofiltration may reduce the incidence of CIN, It is an alternative preventive measures to prevent CIN.
7.Expression of 15-Lipoxygenase isoenzymes in the pulmonary arteries during hypoxia

Aim The purpose of this study was to compare the differential expression of 15-lipoxygenase isoenzymes in the pulmonary arteries between normoxia and hypoxia and to explore their roles in the formation of hypoxic pulmomary vasoconstriction. 8.Isolation and 16S rRNA Gene Sequence Analysis of a Thermotolerant Halophilic Bacterium

A strain of thermotolerant halophilic bacteria YJ0238 was isolated from the salt well at 47℃ in the Kangning countryside where was located at the side of Lancang River in the Tibet Autonomous Region. Some physiological and biochemical properties were characterized.16S rRNA gene sequence of YJ0238 was amplified by PCR,and its nucleotide sequence was determined. Based on it’s physiological and biochemical properties,homology and phylogenetic analysis of 16S rRNA gene sequence,strain YJ0238 was identified as a subspecies of the species Idiomarina zobellii. The GenBank accession number of the 16S rRNA gene sequence of strain YJ0238 is EF693953. Until now,there were few reports on the study of high-temperature and high-salt microbial in domestics. The results of this study will provide research material and information for further studies in this area.
9.Studies on pharmacokinetics features of characteristic active ingredients of daidai flavone extract in different physiological status.

In order to explore the clinical hypolipidemic features of Daidai flavone extract, the pharmacokinetics features of characteristic active ingredients of Daidai flavone extract in normal and hyperlipemia rats were studied and compared. The study established the quantitative determination method of naringin and neohesperidin in plasma by UPLC-MS. Study compared the pharmacokinetics differences of naringin and noehesperidin in normal and hyperlipemia rats on the basis of establishment of hyperlipemia model. Results indicated that the pharmacokinetics features of characteristic active ingredients of Daidai flavone extract in normal and hyperlipemia rats showed significant differences. The C(max) of naringin and neohesperidin in hyperlipemia rats plasma after oral administration of Daidai flavone extract increased obviously, while t1/2, MRT and AUC0-24 h decreased, compared to normal rats. But t(max) showed no differences to that of normal rats. The results further proved Daidai flavone extract would have better hypolipidemic effect in the hyperlipemia pathological status. And the characteristic active ingredients naringin and noehesperidin were the material base of Daidai flavone extract to express the hypolipidemic effect.

10.Protective effect and mechanism of astragaloside IV on oxidative stress injury of mesangial cells.

OBJECTIVETo study the protective effect of astragaloside IV (AS IV) on H2O2 induced human mesangial cells (HMC), and further explore its molecular mechanism.
METHODThe cultured mesangial cells were divided into 5 groups: the normal control group, the H2O2 model group, the AS IV (12.5, 100 nmol x L(-1)) group and the Tempol (1 x 10(5) nmol x L(-1)) group. The MTT method was used to observe cell viability. Hoechst 33258 staining was used to observe the HMC apoptosis and DHE staining was used to detect the generation of reactive oxygen species (ROS). The flow cytometry was used to detect the changes in cell cycle. Western blot was used to detect the expression of Cyclin D1, CyclinA, p38, and T-p38.
RESULTH2O2 (1 x 10(5), 2 x 10(5), 3 x 10(5), and 4 x 10(5) nmol x L(-1)) could induce HMC oxidative stress injury, with significant decrease in the cell survival rate. AS IV (100 nmol x L(-1)) could significantly inhibit HMC oxidative stress injury induced by H2O2 (3 x 10(5) nmol x L(-1)), increase the survival rate of HMC cells, inhibit cell apoptosis, and decrease intracellular ROS production. AS IV could also increase the expression of Cyclin D1, recover normal cell proliferation, and decrease the expression of p38.
CONCLUSIONAS IV can protect H2O2 induced oxidative stress injury in mesangial cells. Its mechanisms may be related to inhibiting the p38/MAPK signaling pathway, increasing the expression of Cyclin D1 and decreasing the intracellular ROS oxidative stress injury.
