Objective To investigate the effect of strengthening self -management on the treatment compliance and outcome in hepatitis B patients with decompensated cirrhosis treated with entecavir. Methods A total of 120 hepatitis B patients with decompensated cirrhosis, who received antiviral therapy with entecavir in our hospital from January 2010 to January 2012,were randomly divided into control group (n=60)and intervention group (n =60).Both groups received routine nursing care;in addition, regular telephone follow -up was performed for the intervention group to provide guidance for the self -management during the comprehensive prevention and treatment of decompensated cirrhosis. The two groups were compared in terms of withdrawal rate, rehospitalization rate, hepatitis B virus (HBV)DNA negative conver-sion rate, incidence of primary liver cancer, and mortality one year later.Continuous data were analyzed by t test, and categorical data by chi -square test or Fisher′s exact test. Results The withdrawal rate and rehospitalization rate in the intervention group were 0 and 18.3%, respectively, versus 8.3% and 35% in the control group (P <0.05 for both). The HBV DNA negative conversion rate, incidence of prima-ry liver cancer, and mortality in the intervention group were 90%,3.3%,and 5.3%,versus 88.3%,5%,and 6.7% in the control group (P >0.05 for all). Conclusion Strengthening self -management,as well as regular health education, can improve treatment compliance and promote recovery in hepatitis B patients with decompensated cirrhosis treated with entecavir.

A moderately halophilic bacterium strain DF-B6 capable of degrading Tween 20 was isolated from solar saltern brine by plate method.Based on the phenotype, physiological and biochemical character-istics,and the phylogenetic analysis of 16S rDNA sequence, strain DF-B6 was identified as Idiomarina. The substrate specificity test showed that the lipolytic enzyme from strain DF-B6 was an esterase, and not a li-pase.The maximum esterase activity was observed in 8% NaCl concentration at 50?C,pH 8.0.The effect of various divalent cations was studied on the activity of esterase from Idiomarina sp.DF-B6 at the concentra-tion of 10 mmol/L,Mg2+,Ca2+ and Mn2+enhanced the esterase activity, while Zn2+,Fe3+ and Cu2+ inhibited its activity.

Objective To establish the optimal extracted method for content dete rmination of naringin in Citrus grandis. Methods RP-HPLC was used to determinat e the content of naringin extracted with the above two methods from different ye ar samples of Citrus grandis. Results The average content extracted with ultraso nic extraction was 13.53 %,and the average content extracted with soxhlet extr action was 11.98 %,there being insignificant difference between the two method s. Conclusion The content of naringin extracted with ultrasonic extraction is mo re than that with the soxhlet extraction, which be receipted in Chinese pharmeco pia. And ultrasonic extraction method is more convenient and can save time.

Objective To optimize the preparation process for Fangji Huangqi Granules. Methods With the extraction rates of tetrandrine, fanchinoline, astragaloside Ⅳand solid as the parameters, the extract conditions of Fangji Huangqi Granules were optimized by orthogonal design . Then the anti-inflammation effect of the extracts was observed on the mice and rats. Results The optimal preparation process was as follows:the mixture of medical materials was firstly refluxed twice with total 10 times of 70 %alcohol,1.5 hours for each time, and then extracted twice with total 12 times of boiling water ,1.5 hours for each time. The anti-inflammation effect of the extracts was obvious on the mice and rats. Conclusion The optimal preparation process is reasonable and with high extraction rate of active components.

BACKGROUND:With the improvement of hemodialysis technology, the long-term survival rate of maintenance hemodialysis patients has been increased continual y. Nowadays, how to improve the quality of life in maintenance hemodialysis patients has been aroused widespread concern, and gradual y become the reliable indicator for comprehensive evaluation of the effect of hemodialysis. OBJECTIVE: To evaluate the effect of individualized intervention and col ective intervention on quality of life in maintenance hemodialysis patients. METHODS:Eighty maintenance hemodialysis patients, who had accepted more than 3 months of dialysis, were randomly divided into two groups (40 cases in the experimental group and 40 patients in the control group). Al the patients in the control group received 6 weeks of individualized intervention according to the pre-established schedule based on the hemodialysis routine care. Patients in the experimental group also have a period of six weeks of col ective intervention according to the pre-established weekly schedule based on the hemodialysis routine care. The quality of life of the patients was evaluated with MOS 36-item short form health survey and kidney disease quality of life short form before and after intervention. RESULTS AND CONCLUSION:After the intervention, both of the experimental group and the control group achieved significantly greater improvement than before in MOS 36-item short form health survey and kidney disease quality of life short form (P<0.01). The physical impact, overal health, emotional state, emotional impact, social function and energy of MOS 36-item short form health survey in the patients of the experimental group were higher than those of the control group after intervention (P<0.01);and the social quality, sleep, social support and patient satisfaction of kidney disease quality of life short form in the experimental group were higher than those in the control group (P<0.01). The results show that both individualized intervention and the col ective intervention based on the individualized intervention are effective in improving quality of life in maintenance hemodialysis patients and the col ective intervention can better improve the quality of life from physical, psychological and social aspects.

Objective Through the investigation and analysis to "Chinese Medical Digest" and its relative periodicals to get a general idea of the survival and situation of these periodicals and to establish the pattern for their survival and development. Methods Such methods as telephone consultation, questionnaire survey, interview, and conference discussion were adopted to collect the relative information of medical digest periodicals and thus an analysis was made upon it. Results There were 18 kinds of "Chinese Medical Digest" periodicals originally. Of all these 18 kinds of the periodicals, 6 kinds are still publishing, 10 kinds have transferred its nature to first-hand literature periodical, and 2 kinds have ceased their publishment. Conclusion As a result of the computer, the network and the full text database development, the survival of medical digest periodicals are endangered. However, regarding to the different domain of subject, different reforming measures should be taken.

Objective To explore the effect of thrombolysis with anticoagulation treatment for early stage of posttraumatic and postoperative deep vein thrombosis of lower extremity. Methods The clinical data of 38 palients with early stage of posttraumatic and postoperative deep vein thrombosis(DVT) of lower extemity treated by thrombolysis with anticoagulation and dispersion drugs were analyzed retrospectively. Results The thrombolytic effect was significant. After the treatment, the deep veins were recanalization without regorging in 71.0% of patients. The total effective rate was 100%. Only one patient had hemorrhagic complication, two patients had symptomatic pulmonary embolism, but none of the patients died. Conclusions Thrombolysis with anticoagulation is an effective and safe method for posttraumatic and postoperative DVT in the early stage.

Different human thyroid carcinoma cell lines were treated with all-trans retinoic acid(RA). RA could inhibit cell growth, improve iodide uptake and increase some thyroid specific genes and retinoid acid receptor(RAR)mRNA expressions in FTC-133 cells. However,RA had no effect in C643,HTH74 and XTC. UC1 cells. These findings indicate that different thyroid carcinoma cells display diverse responses to RA.
