To set up a new rapid method for the rapid determination of influenza virus H5N1 and H7N9 basing on the Multi-Analyte Suspension Array (MASA) technology. Sequence analysis and design of degenerate primers and specific probes were set in the comparison and analysis of H5, N1, H7 and N9 genes. In combination with MASA technology, these primers and probes were used for the determination of samples of H5N1 and H7N9 and other subtypes ( H1N1, PH1N1, H5N2, H3N2 and H9N2). We developed a rapid determination method. This method had high specificity and sensitivity that could detect H5N1 and H7N9 at one time, and could detect samples that containing 10 copies of H5N1 and H7N9. This determination method could be used for rapid determination of influenza virus H5N1 and H7N9 at one time.

Objective To analyze the impact of medical insurance reimbursement mechanism on the income of urban community health centers run by various entities. Methods The data of the baseline survey of community health centers in 28 cities made in 2007 were called into play. These data were used to compare the percentage of the annual medical insurance income from outpatient clinics of three types of community health centers in their gross annual income and their annual outpatient clinic income. Results For 929 community health centers with income from medical insurance reimbursement, the average percentage of their annual income from outpatient clinic accounts for 24% of their gross annual income and 26. 09% of their annual outpatient income; In respect of the reimbursement rates of medical insurance at outpatient clinics, the highest come from those run by enterprises, secondly those by the government, and the least those run by social organizations or individuals. Conclusion The mechanism of medical insurance reimbursement has already affected the income of the third type of community health centers. It is recommended to raise the coverage rate of medical insurance of such type of centers and their reimbursement rates of medical insurance. This may further consolidate the policies of medical insurance reimbursement.

Objective To probe into the pathogenesis of gestational diabetes mellitus (GDM) by studying expression of TLR4 acetylation in peripheral blood mononuclear cells of gravida with GDM as well as its role in TLR4 inflammatory pathway. Methods 30 normal gravidas and 30 gravidas with GDM were enrolled in the study, 15 mL peripheral blood from every participant was collected for extraction of mononuclear cells via density gradient centrifugation and culturing in vitro by LPS. The expression of TLR4 acetylation in the mononuclear cells was detected using immunoprecipitation and western blot and the levels of inflammatory cytokines, TNF-α, IL-1 and IL-10 in the supernatant were detected by ELISA. Results TLR4 acetylation appeared positive in GDM group, and negative in the control group. After the LPS intervention, the degree of TLR4 acetylation was enhanced significantly and the latter was significantly higher than those of the GDM group and the normal + Lps group (P < 0.05). The differences in the levels of inflammatory cytokines, TNF-α, IL-1, IL-10, were statistically significant (P < 0.05). Conclusion TLR4 acetylation exists in mononuclear cells of gravida with GDM, and mediates the release of inflammatory cytokines by influencing the activation of TLR4 inflammatory pathobenesis, which contributes to the promoted antiinflammatory - proinflammatory imbalance in gravidas. In this way, it is involved in the growth of GDM.
