2.Qiangguyin strengthens periprosthetic bone density after hip arthroplasty:a randomized controlled half-year evaluation

BACKGROUND:Hip arthroplasty is an effective means currently in repair of femoral neck fractures. With the increased in the amount of surgery, surgery failure rate also has increased. In particular, prosthesis loosening and fracture are very common after replacement.Qiangguyin can promote the formation of osteoblasts, resist osteoporosis, inhibit osteoclast activity, and increase bone density. Drynaria invigorates the kidney, strengthens the bone and tendon and relieves pain, and has bidirectional regulatory effects on promoting bone formation and inhibiting bone resorption.
OBJECTIVE:To investigate effects ofQiangguyin on periprosthetic bone density in patients with femoral neck fracture after hip arthroplasty.
METHODS:96 patients with femoral neck fracture were equaly divided into observation group and control group based on the random number table. Both groups were given selective joint replacement. Patients were implanted with femoral head made by US Stryker Corporation, and fixed with bone cement. After replacement, patients in the control group were treated with calcium D3 tablets and capsules alfacalcidol. On this basis, the treatment group was given theQiangguyin oral drug intervention for a month. Bone density surrounding the prosthesis was observed in both groups. Alkaline phosphatase and tartrate-resistant hydrochloric acid phosphatase activities were detected. Repair effects were compared.
RESULTS AND CONCLUSION:The surgery was successfuly completed in al patients. No infection or loosening of the prosthesis occurred. Ambulation time and healing time were significantly less in the observation group than in the control group (P < 0.05). No significant difference in bone density surrounding the prosthesis was detected between the two groups before medication, and an obvious increased trend was found after medication (P < 0.05). Bone density was significantly higher in the observation group than in the control group after medication (P < 0.05). Serum alkaline phosphatase activity was significantly higher in the two groups after medication, but tartrate-resistant hydrochloric acid phosphatase activity was significantly decreased, which showed significant differences as compared with that before medication (P < 0.05). Simultaneously, significant differences in serum alkaline phosphatase activity and tartrate-resistant hydrochloric acid phosphatase activity were detected between the observation and control groups after medication (P < 0.05). At 6 months after replacement, the excelent and good rate of joint function was significantly higher in the observation group than in the control group (94%, 79%,P < 0.05). These results indicate that Qiangguyin could resist bone resorption and suppress excessive bone turnover, improve bone density around the prosthesis, and restore joint function after hip arthroplasty.
3.Expression of ERCC-1 protein and its clinical implication in locally advanced cervical cancer

Objective To examine the protein expression of the nucleotide excision repair gene (ERCC-1) in patients with locally advanced cervical cancer and its relationship with the efficacy of radiotherapy and chemotherapy.Methods The expression of ERCC-1 protein in 88 patients with locally advanced cervical cancer treated in our hospital between 2007-2011 was measured using immunohistochemistry (IHC).The patients were divided into high-expression group (n=48) and low-expression group (n=40) based on the fluorescence intensity on the IHC staining.All patients received cisplatin (40 mg/m2 per week) during radiotherapy.The relationship between ERCC-1 protein expression and the clinicopathological factors of cervical cancer was analyzed using the chi-square test.Survival was calculated using the Kaplan-Meier method and compared by the log-rank test.Multivariate prognostic analysis was performed using the Cox model.Results The overall response rate (CR+PR) was 81%(39/48) in the high-ERCC-1 expression group and 85%(34/40) in the low-ERCC-1 expression group (P=0.641).ERCC-1 protein expression was associated with recurrence and metastasis (P=0.043,0.043).The 5-year survival rate was significantly higher in the low-ERCC-1 expression group than in the high-ERCC-1 expression group (65% vs.42%, P=0.029).Conclusions Patients with high ERCC-1 protein expression are more likely to have local recurrence and distant metastasis than those with low ERCC-1 protein expression.ERCC-1 protein expression may be a clinically significant biomarker for predicting the prognosis of cervical cancer patients.
4.Mechanism Exploration on Emodin Ameliorates Cisplatin-induced Renal Tubular Cell Injury through Activation of Autophagy

This study was aimed to observe the effect of emodin on cisplatin-induced renal tubular epithelial cells (NRK-52E) injury,in order to explore its possible molecular mechanisms.Firstly,effects of emodin on cisplatin-induced morphological changes in NRK-52E cells were observed.Secondly,the apoptosis-related protein expression of Caspase-3 and cleaved Caspase-3 were detected after the treatment of cisplatin alone or cisplatin together with emodin by western blot.Then,the expression of microtubule-associated protein 1 light chain 3 (LC3) Ⅱ/Ⅰ was detected after the treatment of emodin or rapamycin at different time points by western blot.Changes of pmRFP-LC3 fluorescent particles were observed by fluorescence microscopy.And effects of rapamycin on cellular morphological changes were observed in the environment of cisplatin.Finally,effects of emodin on the activation of AMPK and mTOR signal pathway were further observed,which is considered as the upstream of autophagy signaling pathway.The results showed that cisplatin can induce morphological changes in NRK-52E cell,which was obviously ameliorated by the intervention of emodin.Additionally,the increased protein expression of cleaved Caspase-3 induced by cisplatin was obviously reduced after the intervention of emodin.The LC3-Ⅱ/LC3-Ⅰ ratio was significantly increased after the treatment of emodin or rapamycin at different time points.Rapamycin can significantly ameliorate NRK-52E cell apoptosis induced by cisplatin.Simultaneously,the number of pmRFP-LC3 fluorescent particles increased after the treatment of emodin.As the extension of time by intervention of emodin,the protein expression of p-mTOR was significantly reduced.In contrast,the protein expression of p-AMPK was significantly increased.It was concluded that emodin can ameliorate cisplatininduced apoptosis in NRK-52E cells.Its potential mechanism may be attributed to the activation of autophagy by regulating AMPK/mTOR signaling pathway.And thus,it played a role in renal protective effects.
5.Clinical Characters of Rhegmatogenous Retinal Detachment

The clinical data of 109 patients with rhegmatogenous retinal detachment were retrospectively analyzed and the relationships among retinal detachment,refraction and the shapes of hole in different age groups discussed. The results showed that myopia was more commonly seen in the patients less than 40 years old and the occurrence of myopia was gradually decreased in the patients over 40 years old. Round hole was more commonly seen in the patients less than 40 years old and horseshoe-shaped tears were more often found in the age group of over 40 years old.
7.Impact factors of prognosis of spinal cord injury patients

ObjectiveTo explore the influence factors of prognosis of spinal cord injury patients. MethodsFollow-up data of 121 cases with spinal cord injury was analysed with retrospective cohort study.ResultsThe highest recover rate was in the patients who accepted the special rehabilitation therapy,while that of recovering two class was in the patients who accepted the magnetic stimulation.Conclusions Special recovering cure and magnetic stimulation can surely promote recovery of spinal cord injury patients.
8.Analysis of biofilm formation ability and related genes of Staphylococcus aureus isolated from urinary tract infections

Objective To study the biofilm formation ability and related gene distribution of Staphylococcus (S .) aureus iso‐lated from urinary tract infections to provide the theoretical basis for the prevention and treatment of clinical infection .Methods The minimal inhibitory concentration was detected using the agar double dilution method .The bacterial adhesion ability was deter‐mined by flat colony counting method .The biofilm formation ability was analyzed by the 96‐well crystal violet staining method .The biofilm‐associated genes were detected by PCR amplification .Results Eleven clinical strains of S .aureus were high resistant to pen‐icillin and erythromycin ,whereas were all sensitive to vancomycin and nitrofurantoin .All the isolates had a strong ability of adhe‐sion ,but the biofilm formation ability was weak .Among them ,the icaAD and icaBC genes were amplified in 10 S .aureus isolates . Conclusion The adhesion ability and biofilm formation ability of S .aureus isolated from urinary tract infections have the strain differences ,and ica is an important gene of S .aureus biofilm formation .
9.Protective action of Semen Ziziphi Spinosaeand on acute myocardial ischemia

Objective To study the protective action of Semen Ziziphi Spiosaeand on acute myocardial ischemia and its effecte on infarcted area. Methods Left anterior descending(LAD) of coronary artery was ligated to cause the big rat's acute myocardial ischemia, and electrocardiogram (ECG) was used to appraise the effect of sour jujube kernel total soap on the myocardial infarcted area. Results Compared with control group, the experimental group treated with sour jujube kernel total soap glucosidal showed significantly smaller infarcted area; and ECG showed the heart rhythm, S-T piece and T wave value were significantly decreased in the experimental group. Conclusion Sour jujube kernel total soap glucosidal can reduce the myocardial infarcted area caused by ligature of LAD, so as to protect the cardiac muscle.
10.Cerebrospinal fluid adenosine deaminase activity for the diagnosis of tuberculous meningitis

Objective To evaluate cerebrospinal fluid adenosine deaminase(CSF-ADA)activity in the diagnosis of tuberculous meningitis(TMB), and to observe its dynamic changes. Methods A total of 160 patients were included and were divided into two groups: 76 cases of TBM and 84 cases of non-TBM.Among the cases of non-TBM, there were 36 cases of bacterial meningitis, 30 cases of viral meningitis and 18 cases of cryptocoocal meningitis. All the patients were measured with their CSF-ADA activity by Enzymecoupled assay(Trinder method)and 47 patients of TBM were measured again after 2 weeks' and 6 weeks'antitubercular therapy. Results were expressed as(-x)± s. Mann-Whitney U test and paired-samples t test were used. Results CSF-ADA activity in TBM group was(12.9 ±6.4)U/L, while that in the non-TBM group was(6.0 ± 4.1)U/L, the difference was of statistical significance(U = 7.860, P < 0.05). With the cutoff value of 9 U/L, the sensitivity and specificity to differentiate TBM from non-TBM was 84.21% and 83. 33%, respectively. CSF-ADA activity decreased in TBM patients after antitubercular treatment.Conclusions CSF-ADA activity can be an effective laboratory marker for early differential diagnosis of TMB with the cut-off value of 9 U/L. Dynamic changes of CSF-ADA activity may be a indicator for the effect of antitubercular treatment.
