Objective To discuss the effect of mesenchymal stem cells (MSCs) in modulating immune responses in a rat renal transplantation model.Methods An in vivo trial of cytology was performed in one centre from March to December in 2008.Wistar rat donors and Lewis rat recipients in a renal transplantation model were randomly divided into 4 groups:MSCs (low dose,1 × 106 )therapy,MSCs (high dose,1 × 107) therapy,CsA monotherapy,and no therapy.Biochemistry methods were used to detect the levels of creatinine in serum.The survival time,renal grafting function and pathological changes of transplanted renal tissues were observed.Results Animal survival was significantly prolonged by MSCs (high dose) therapy and CsA monotherapy as compared with the no therapy group (both P＜0.01).Animal survival in the MSCs (low dose) therapy group was prolonged as compared with no therapy group (P＜0.01),but shortened as compared with MSCs (high dose) therapy group (P＜0.05) and CsA monotherapy group (P＜0.05).The MSCs (high dose) therapy and CsA therapy groups showed no special changes in histology,hut the control group showed acute rejection.Conclusion MSCs down-regulated immune responses,reduced production of some inflammatory mediators,preserved graft function in the initial stage after transplantation,and prolonged animal survival,and these effects were the same as those of CsA therapy with 1 × 107/day.

Objective To discussi the feasibility and indication of selective primary suture after choledo-chotomy. Methods The selected patients were intra-operatively randomized to have either primary suture or T tube drainage after choledochotomy. Comparison of the clinical effects in different groups was done. Re-sults Totally 161 cases were randomized into two groups: the primary sutures group 85 cases and the T tube drainage group 76 cases. All patient had no serious or long-term complications. The average hospital stay in primary suture group was significantly shorter (8.5±3.2) d than T-tube group(16.3±6.5) d. Conclusion Under strict indications, the primary suture after choledochotomy is feasible, and has advantages compared with the traditional method.

Objective To investigate the effect of individual nostalgia therapy on positive feelings and sadness of the spouses of postoperative patients with esophageal cancer. Method From June 2014 to June 2015, 60 hospitalized patients and their 60 spouses were set as control group. Routine health education and guidance were conducted once or two times a week for 7 weeks. Another 60 cases of hospitalized patients and their 60 spouses during July 2015 and June 2016 were set up as observation group. The patients were treated with the same nursing intervention as in the control group, and their spouses were subjected to individual nostalgia treatment once a week for 7 weeks. Before the intervention (postoperative day 2) and 7 weeks after the intervention, the positive aspects of caregiving (PAC) and the Margit-Meuser caregiver grief ineventary short form (MM-CGI-SF) were used to investigate their positive feelings and sadness. Result The score on spouse's positive feelings in the observation group after intervention was significantly higher than those of the control group and that of the observation before intervention (P < 0.01). Conclusion Individual nostalgia can effectively improve the feelings of the spouses of esophageal cancer patients and reduce their grief.

OBJECTIVE: To identify the main chemical constituents in the extract of deep fried strychnas by UPLC-Q-TOF-MS. METHODS: Agilent SB-C18(2.1 mm×100 mm, 1.8 μm) column was used. Gradient elution was conducted with mobile phase consisting of 0.1% formic acid solution (A) and acetonitrile (B)at a flow rate of 0.2 mL•min-1. The column temperature was maitained at 35 ℃. MS analysis was based on information associated mode (IDA) that positive and negative ions were respectively collected. RESULTS: A total of 31 compounds were identified in deep fried strychnas, of which eight had not been reported as Semen Strychni,and four new compounds were found in the positive and negative ion mode. The main chemical constituents included alkaloids, organic acids, glycosides, etc. CONCLUSION: The method is accurate, reliable, and efficient, and is suitable for rapid identification of the ingredients in deep fried strychnas, which provides a reference for the development and utilization of processed products of Semen Strychni and clarification of its efficacy and material basis.

Objective: To establish UPLC-Q/TOF method for simultaneous qualitative and quantitative analysis of main chemical components in Sinodielsia yunnanensis. Methods: High performance liquid chromatography tandem flight time mass spectrometry was used. Chromatographic column was Agilent shell 120 Hilic, and the flow phase was acetonitrile-0.1% acid aqueous solution (gradient elution) at a flow rate of 0.3 mL/min; Qualitative and quantitative analysis adopted Q-TOF positive anion full scanning + IDA (information correlation acquisition) mode. Results: Using the established screening database, the target compounds with a mass error of less than 5×10-6, a correct isotope distribution and fragment ion are used as the target compounds. Combined with software Formula Finder, Mass Calculators and other functions, online database (Human Metabolome Database, PubChem, Chemical Book, etc.), and two stage fragmentation rule, 23 compounds were identified by qualitative identification. Ferulic acid was determined as a quantitative indicator component, and the quantitative and qualitative ions were 178 and 149, respectively; The linear range was 1.14-1 140 ng/mL (r = 1.000), and the limits of detection and quantitation respectively was 0.87 ng/mL and 2.91 ng/mL; RSDs of precision, stability, and reproducibility tests were lower than 2%; The recovery rate was 98.13%-101.25% (RSD = 1.37%, n = 5). Conclusion: This method has high sensitivity, good reproducibility, and its analysis is fast, accurate, and reliable, and it can be used for simultaneous qualitative and quantitative determination of main chemical components in the medicinal materials of S. yunnanensis; The content of ferulic acid in Yunnan is higher than that in Tibet.

Objective: To study the chemical differences between natural and synthetic of Bambusae Concretio Silicea by ultra-high performance liquid chromatography-quadrupole tandem time-of-flight mass spectrometry (UPLC-Q-TOF-MS). Methods: The qualitative analysis of chemical composition was made combined online database and secondary fragmentation cleavage rules, and difference analysis was carried out by the Enhance Peak Find function of PeakView software. Then the converted data was imported into SIMCA-P software to establish an OPLS-DA statistical model and differential secondary metabolites were analyzed. Results: The constituents of Bambusae Concretio Silicea mainly included amino acids, organic acids, alkaloids, glycosides, etc. Among them, 11 components were known, while other 43 compounds were identified for the first time. A total of 14 distinct components, including 2,5-dimethyl-1,3-oxazole-4-carboxylic acid and sucrose, and 12 biomarkers, including 4-(heptyloxy)phenyl-4-(hexyloxy)benzoate, and N-lauryldiethanolamine were identified. Conclusion: This study reveals that the chemical differences between the two herbs are obvious, while betaine and sucrose can be used as the distinguishing indicators. And it provides new ideas and data references for the quality control and clarification of medicinal substances.

OBJECTIVE: To evaluate the influence of fewer courses and prolonged intervals of chemotherapy on survival rate of advanced non small cell lung cancer (NSCLC) patients treated by sequential chemo-radiation therapy combined with traditional Chinese medicine (TCM). METHODS: From Jan. 2000 to Dec. 2001, 54 untreated advanced NSCLC patients (2 stage IIIa, 18 stage IIIb, 34 stage IV) were treated by sequential chemo-radiation therapy combined with TCM. The courses of chemotherapy were reduced and the intervals of chemotherapy were longer than that of the standard regimen. The efficacy and survival rate were documented and the prognostic factors were analyzed. RESULTS: Complete remission (CR) was observed in 1 case and partial remission (PR) in 20 cases. The overall objective response rate was 40.4%. Median survival was 15.3 months, 1-, 2- and 3-year survival rate were 53.7%, 28.9% and 9.6% respectively. The median survival of stage III and IV were 21.8 months and 12.5 months respectively. The 1-, 2-, and 3-year survival rates of stage III were 65.0%, 49.5%, 24.7% and that of stage IV were 47.0%, 23.3%, 0%, respectively. The quality of life was improved in most of the patients. Cox's proportional hazards regression showed that improved quality of life and treatment of TCM were the significant prognostic factors of overall survival. CONCLUSION: Chemotherapy and radiotherapy combined with TCM is beneficial to extending the interval of chemotherapy, improving the quality of life, and increasing the survival rate of advanced NSCLC patients.

Objective To investigate the possibility of immune tole rance induced by bone marrow-derived mesenchymal stem cells in allogeneic organ transplantation. Methods Allogeneic bone marrow-derived mesenchymal stem cells and syngeneic bone marrow cells were cotransplanted to lethally irradiated female C57BL/6 recipient mice. FACS was used to analyze the chimerism 150 days later. Mixed lymphocyte reaction and ConA induced proliferation test were performed to evaluate proliferative activity of mice spleen cells in cell-transplanted group. Skin transplantation test was done to observe immune response of cell-transplanted group mice against organ graft from donor mice. Results About 5 97% donor T cells were detected in splenocytes of cell-transplanted group mice. MLR showed that mean SI of cell-transplanted group mice was 1.79, and that of untreated group mice was 7 28. ConA induced proliferation test showed that mean SI of cell-transplanted group mice was 31 92; and that of untreated group mice was 34 99. Mean survival time of donor-derived skin graft in cell-transplanted group mice was more than 90 days; and that in untreated group mice was 8 days. Conclusion Our results showed for the first time that induction of stable mixed hematopoietic chimerism after allogeneic bone marrow derived mesenchymal stem cells transplantation lead to stable donor-specific tolerance in allogeneic host and skin graft survival from donor mice.

Objective To analyze the relationship between the clinical features of hepatoblastoma and serum alpha-fetoprotein level.Methods We did a retrospective study in 74 cases into the relationship of clinical stages,treatment,prognosis,and AFP value in hepatoblastoma.Results In Stage Ⅰ and Ⅱ patients,the average AFP was (36 333 ± 13 782) ng/ml and in Stage Ⅲ and Ⅳ,the average AFP was (78 346 ± 27 956) ng/ml,P ＜ 0.05.68 patients received preoperative chemotherapy with alpha-fetoprotein determinations before and after chemotherapy respectively.In 44 cases AFP decreased ＞ 50％ after chemotherapy,and in 20 cases AFP declined ＞90％,while in 13 cases AFP increased after chemotherapy.15 cases had serum AFP measured on the first postoperative day,among them 10 cases had a ＞ 50％ AFP decline,3 cases had a ＜ 50％ AFP decline,while in 2 cases AFP value increased.52 cases were postoperatively followed-up,with overall 3 year-survival rate of 86.5％ ; 7 cases recurred,4 died.The average preop AFP level in 7 recurred cases was (27 060 ± 3 569) ng/ml,while the average preop AFP level in those of 3-year recurrence-free was (29 865 ± 5 867) ng/ml,P ＞ 0.05.Only 57％ tumor recurred cases had back to normal AFP level within one month postop,while 89％ long term survivals reported normal range of AFP during that period,P ＜ 0.05.Conclusions Serum AFP level in patients with hepatoblastoma is related to the course of disease.It can be used to estimate the effect of clinical chemotherapy.The speed of alpha-fetoprotein decline postop can be used as an indicator of prognosis.The postoperative normal AFP level within 4 weeks predicts a favorable prognosis.

BACKGROUND: Some studies in vitro have reported that there are CD30 positive T cells in immunological response of allogenic transplantation.OBJECTIVE: To detect the relationship between the level of serum CD30 (sCD30) and clinical rejection in the patients with or without kidney transplantation, and analyze the importance of sCD30 in the estimation of immune state, monitor of acute rejection, and judgment of prognosis. DESIGN, TIME AND SETTING: Clinical case analysis study was performed at Jiangsu People's Hospital between April 2004 and March 2007. PARTICIPANTS: 153 kidney transplantation cases comprising 103 males and 50 females, averagely aged 37 years. METHODS: 3 mL peripheral blood was obtained from recipients before transplantation (without immunosuppressive agent) and at 0, 1, 3, 5, 7, 14, 21, and 28 days. Serum was isolated from obtained blood and placed at -20 ℃. Soluble CD30 levels were detected using CD30 cytokine ELISA kit supplied by BenderMedSystems. MAIN OUTCOME MEASURE: The relation between the soluble CD30 levels and rejection prior to and following transplantation.RESULTS: There was a significant relation in the sCD30 level between the patients with (n=17) and without acute rejection (n=136). The CD30 levels were 113.2 U/mL in the rejection group and 83.2 U/mL in the non-injection group (P < 0.01). No significant difference was determined between both groups in 5 days following surgery (P > 0.05). Significant difference were detected between both groups from 5 days following surgery (P < 0.01). There was no relation between the soluble CD30 level and the time of rejection and release after kidney transplantation (P > 0.05). Receiver operating characteristic (ROC) curve demonstrated that soluble CD30 levels on day 5 post-transplantation could predict acute rejection (area under ROC curve: 0.850). Meanwhile, 100 U/mL was the optimal operational cut-off level to predict rejection (specificity: 85.0%; sensitivity: 83.6%). The patients with positive of soluble CD30 level showed a lower survival rate than those with negative CD30 level (P < 0.01). CONCLUSION: The soluble CD30 levels contributed to predictive the acute rejection and prognosis of kidney transplantation.
