1.Impact of Ticagrelor on Platelet Aggregation in Patients With Acute Coronary Syndrome After Percutaneous Coronary Intervention

Objective: To explore the impact of ticagrelor on platelet aggregation in patients with acute coronary syndrome (ACS) after percutaneous coronary intervention(PCI). Methods: A total of 98 ACS patients received PCI in our hospital from 2015-01 to 2015-12 were enrolled. The patients were randomly divided into 2 groups: Clopidogrel group, the patients received oral clopidogrel 300mg at first time and then maintained by 75mg/qd, n=48 and Ticagrelor group, the patients received oral ticagrelor 180mg at first time and then maintained by 90mg/bid, n=50. All patients were treated for 12 months.The level of vasodilator stimulated phosphoprotein (VASP) phosphorylation and platelet reactivity index (PRI) at pre-medication and 24h, 7 days and 1 month after PCI were detected; major adverse cardiovascular events (MACE) and bleeding events were recorded within 1 month after PCI, the incidence of platelet aggregation, MACE and bleeding events were compared between 2 groups.Results: The baseline information and PCI condition were similar between 2 groups, P>0.05. The overall average PRI was different between 2 groups, P<0.001 and PRI at each time point was different between 2 groups, P<0.001, different group and time point had interactive effect on PRI, P<0.001. Compared with Clopidogrel group, Ticagrelor group had the lower ratio of PRI≥50% at different time points after PCI, P<0.001. The incidence of MACE and bleeding event were similar between 2 groups within 1 month after PCI, P>0.05. Conclusion: Ticagrelor was superior toclopidogrel for anti-platelet aggregation in ACS patients after PCI, it didn't increase bleeding events.

References are provided for war,training,and construction in battlefield.
3.Protective Effect and Mechanism of Matrine Combined with Glycyrrhizic Acid in the Treatment of Chronic Liver Injury Induced by Carbon Tetrachloride

Objective: To investigate the protective effects of matrine combined with glycyrrhizic acid on chronic liver injury induced by carbon tetrachloride, and explore the protective mechanism from the points of energy metabolism and CYP enzyme.Methods: The chronic hepatic injury model of rats was induced by CCl4.The changes of activity of alanine aminotransferase (ALT) and aspartate aminotransferase (AST) in serum were measured to observe the protective effect of the two drugs and their combination.The contents of glutamate dehydrogenase (GLDH) in serum and adenine nucleoside three phosphate (ATP), adenosine diphosphate (ADP) and adenine monophosphate (AMP) in liver tissue were determined to evaluate the regulation effect on hepatic energy metabolism and mitochondrial function.The levels of CYP1A2, CYP2E1 mRNA and protein in liver tissue were detected by real-time PCR and Western Blot to evaluate the two drugs and their combination on the regulation function of liver CYP enzyme.Results: Matrine (72.8 mg×kg-1)and glycyrrhizic acid(43.4 mg·kg-1)could decrease the serum activities of ALT and ALT in chronic hepatic injury model, and the combination (matrine 36.4 mg·kg-1+glycyrrhizic acid 21.7 mg·kg-1) had the most significant protective effect (P<0.05).Matrine (72.8 mg·kg-1)and glycyrrhizic acid(43.4 mg·kg-1)could decrease GLDH in serum,and restore the content of ATP in liver (P<0.05).Matrine (72.8 mg·kg-1) had no effect on the expression of CYP1A2 and CYP2E1mRNA, and glycyrrhizin (43.4 mg·kg-1) could inhibit the expression of CYP1A2, CYP2E1mRNA and protein (P<0.05).Conclusion: Matrine combined with glycyrrhizin has obvious regulation effect on mitochondrial function and liver protective effect in chronic hepatic injury model.
4.Clinical significance of expression of PSA, hK2, PSMA in the peripheral blood of patients with prostate cancer.

OBJECTIVETo find sensitive and specific micro-metastic markers for prostate cancer.
METHODSUsing nested reverse transcription-PCR, we examined the expression of PSA, hK2 and PSMA mRNA in peripheral blood mononuclear cells of 51 patients with prostate cancer, 33 patients with benign prostate hyperplasia (BPH) and 32 normal young people.
RESULTSThe expression rates of PSA, hK2 and PSMA mRNA were 52.9%, 43.1% and 64.7%, respectively in prostate cancer group, and 6.2%, 7.7% and 4.6%, respectively in control group (BPH patients and normal young people) with statistical significance (P < 0.01). Although the expression rate of PSA and hK2 mRNA increased with cancer progression, there was no statistical significance among patients in different stages. The expression rate of PSMA mRNA was higher than that of PSA and hK2 mRNA in each clinical stage.
CONCLUSIONPSMA mRNA expression detected by nested RT-PCR is of greater value for the diagnosis, therapy choice and prognostic evaluation of prostate cancer patients.

5.Case-Control Study on the Fibroblast Growth Factor Receptor 2 Gene Polymorphisms Associated with Breast Cancer in Chinese Han Women.

PURPOSE: Genetic variation in fibroblast growth factor receptor 2 (FGFR2) is a newly described risk factor for breast cancer. This study aimed to evaluate the association of four single nucleotide polymorphisms (SNPs) in FGFR2 with breast cancer in Han Chinese women. METHODS: Two hundred three women with breast cancer and 200 breast cancer-free age-matched controls were selected. Four SNPs (rs2981579, rs1219648, rs2420946, and rs2981582) and their haplotypes were analyzed to test for their association with breast cancer susceptibility. The presence of the four FGFR2 SNPs was determined by polymerase chain reaction-restriction fragment length polymorphism analysis. RESULTS: A statistically significant difference was observed in the frequency of rs2981582 in the FGFR2 gene (p<0.05) between case and control groups. In subjects stratified by menopausal status, rs2981582 TT, rs2420946 AA, and rs1219648 CC were significantly associated with the risk of breast cancer in postmenopausal subjects, but no significant associations between these four SNPs and the risk of breast cancer were identified in premenopausal subjects. Further, there was no significant association between hormone receptor status (estrogen receptor and progesterone receptor) and breast cancer risk. Six common (> 3%) haplotypes were identified. Three of these haplotypes, CGTC (odds ratio [OR], 0.613; 95% confidence interval [CI], 0.457-0.82; p=0.001), TGTC (OR, 6.561; 95% CI, 2.064-20.854; p<0.001), and CATC (OR, 12.645; 95% CI, 1.742-91.799; p=0.001) were significantly associated with breast cancer risk. CONCLUSION: Our findings indicated that the SNP rs2981582 and haplotypes CGTC, TGTC, and CATC in FGFR2 may be associated with an increased risk of breast cancer in Han Chinese women.

6.Surgical correction of post-traumatic kyphosis of thoracolumbar spine.

OBJECTIVETo determine the effectiveness of surgical correction for post-traumatic kyphosis of thoracolumbar spine.
METHODSFrom 1996 to 2003, 33 consecutive patients with post-traumatic kyphosis of thoracolumbar spine were corrected surgically. The mean age was 40.3 years (range, 13 - 65 years). The mean time between the initial injury and surgical correction was 36.0 months (range, 6 - 220 months). The kyphotic deformity averaged 40.8 degrees (range, 20 degrees - 82 degrees ). All the patients had neurological deficits. Twelve patients had obvious back pain. Seven patients lost sphincter function completely and nineteen patients lost the function partly. Twenty-three patients had ever undergone laminectomy and/or instrumentation. The treatment procedure consisted of anterior release and posterior spinal osteotomy with instrumentation (15 patients), posterior closing wedge osteotomy with instrumentation (12 patients), anterior release and instrumentation (6 patients).
RESULTSKyphosis was corrected from an average of 40.8 degrees to an average of 5.7 degrees, the corrective rate was 86.0% (40.8 degrees - 5.7 degrees /40.8 degrees). There were no severe complications. The average follow-up period was 24.6 months (range, 6 - 84 months). There was no loss of correction at follow-up. Ten of these patients showed an improvement in neural function by one or two levels according to the classification. Sphincter function recovered partly in ten patients. Back pain was relieved significantly in all of twelve patients with back pain preoperatively. Bony fusion was achieved in thirty-two patients. One patient had nonunion and achieved bony fusion after revision.
CONCLUSIONPosterior closing wedge osteotomy was suitable to kyphosis less than 40 degrees. Anterior release and posterior spinal osteotomy was effective, especially to the patients with severe kyphosis deformity or with operation history. Patients with incomplete neurological deficits and/or severe back pain could get benefit from osteotomy of spine, even if their medical history was long.

7.Dynamic changes of renal blood perfusion in Goto-Kakizaki rats using contrast-enhanced ultrasonography

Objective To explore dynamic changes of renal cortex blood perfusion in Goto-Kakizaki rats(GK rats) by contrast-enhanced ultrasonography.The clues for early diagnosis and intervention of diabetic renal impairment may be obtained from the study.Methods Male GK rats were taken as experimental groups,and Wistar male rats were used as controls.Contrast-enhanced agent SonoVue was administered via the femoral vein cannulation in a bolus fashion.Image changes of renal cortex were observed in real time, and dynamic image changes within 0 - 5 minute after injection were observed and stored.The renal cortex blood perfusion changes were analyzed using the time-intensity curve (TIC).Results (1) The kidney was quickly enhanced after the injection of contrast agent.The renal artery, cortex, renal vein were displayed in sequence.(2)The shape of renal perfusion TIC in all the objects was a single peak,steep ascending, flat descending curve.The overall shape of TIC in GK rats was wider than controls and they were wider and wider with age increasing of the GK rats.(3) Compare to controls,the time to peak (TTP)and the half time of peak descending (HDT) in GK rats was longer (P <0.01), the slope rates of the ascending curve (S1) and the slope rates of the descending curve (S2) decreased and the area under the curve increased in 12-week-old and 20-week-old GK rats (P < 0.05 or P < 0.01).Conclusions The abnormal blood perfusion of renal cortex microcirculation was present in the early stage of diabetic in GK rats.Real-time contrast-enhanced ultrasonography and TIC could display this change effectively.
8.In vitro simulation experimental study of a fully magnetically levitated ventricular assist device based on mock circulatory system

Objective To investigate the circulatory supporting effect of the third generation fully magnetically levitated China Heart ventricular assist device (CH-VAD) under heart failure (HF) condition.Methods An in vitro mock circulatory system (MCS) was developed.This system could simulate a healthy adult under resting state and a patient with heart failure,and incorporate the CH-VAD to evaluate the assisting performance under continuous flow mode.Furthermore,CH-VAD was equipped with a pulsatile flow controller and its initial performance was accessed.The pulsatile mode was obtained by using sinusoidal velocity waveform of the pump which synchronized the CH-VAD with the ventricle simulator of the MCS.Results CH-VAD under continuous flow mode could recover the hemodynamic parameters (arterial pressure and cardiac output) under HF condition to normal range.Preliminary pulsatile test results showed that amplitude of current pulse speed had a minor influence on the hemodynamic performance.CH-VAD under continuous flow and pulsatile flow mode could obtain comparable mean arterial pressure,systolic arterial pressure,diastolic arterial pressure and mean flow.Conclusions CH-VAD can generate a certain degree of speed pulse via appropriate pulsatility control,so as to provide sufficient support on ventricular function.Further optimization on pulsatile controller of CH-VAD is required to conform to natural physiology.The developed MCS can be utilized as an effective and controllable in vitro platform for design,optimization and verification of VADs or other mechanical circulatory support devices.
9.Cefoxitin disk diffusion test in the detection of MRS heterogenic drug-resistant strains.

OBJECTIVE:
To evaluate the reliability and clinical practicability of cefoxitin disk diffusion test in the detection of methicillin-resistant staphylococcus (MRS) heterogenic drug-resistant strains.
METHODS:
Three hundred and ten strains of staphylococcus isolated from clinics were detected by the oxacillin disk diffusion test, the cefoxitin disk diffusion test as well as the oxacillin agar dilution test according to the standard operation procedures of NCCLS, and the detection of mecA gene of staphylococcus was used as a criterion. The sensitivities and specitivities of the 4 methods were compared.
RESULTS:
By the detection of mecA gene, the ratio for MRSA was 57.1%(113/198) and the ratio for MRCNS was 62.5%(70/112). Both the sensitivity and specificity of cefoxitin disk diffusion test in the detection of MRS were 100%, and those in the detection of MRCNS were 98.6% and 100%.
CONCLUSION
Cefoxitin disk diffusion test is reliable, simple and convenient, and it can be used as a conventional method for the detection of MRS in clinical laboratories.

10.Curative effect of decompressive craniectomy with reserved superficial temporal artery for treatment of severe traumatic brain injury

Objective:To investigate the curative effect of decompression with reserved superficial temporal artery for treatment of severe traumatic brain injury (sTBI).Methods:A retrospective case-control study was conducted to analyze the clinical data of 265 patients with sTBI who underwent decompressive craniectomy in Shaanxi Provincial People's Hospital from July 2016 to December 2019. Among them, 152 were males and 113 were females, with the age of 33-72 years [(50.1±12.8)years]. On admission, the Glasgow Coma Score (GCS) was 3-8 points [(5.7±1.4)points]. A total of 135 patients were treated with reserved superficial temporal artery decompression after admission (observation group) and 130 patients were treated with conventional decompression without deliberately preserving the superficial temporal artery (control group). The amount of intraoperative scalp bleeding, scalp healing time, cerebrospinal fluid incision leakage, healing degree and postoperative craniocerebral CT angiography (CTA) were compared between the two groups.Results:All patients were followed up for 7-15 days [(12.4±2.5)days]. The amount of intraoperative scalp bleeding was (15.4±4.5)ml in observation group and (65.2±4.7)ml in control group ( P<0.01). The healing time was (7.1±1.1)days in observation group and (9.5±2.3)days in control group ( P<0.01). There were 5 patients (3.7%) with cerebrospinal fluid incision leakage in observation group compared to 21 patients (16.2%) in control group ( P<0.01). There were 114 patients with Grade A healing in observation group compared to 91 patients in control group ( P<0.05). Based on the postoperative craniocerebral CTA, the superficial temporal artery was intact and the blood supply was normal in observation group, while the main trunk of the superficial temporal artery was disconnected from the zygomatic arch segment and the blood supply was incomplete in control group. Conclusions:For patients with sTBI, decompressive craniectomy with reserved superficial temporal artery can promote the healing of the scalp. Moreover, keeping the superficial temporal artery can significantly reduce the amount of bleeding during operation, reduce postoperative scalp healing time and incidence of cerebrospinal fluid incision leakage and assist cerebral blood perfusion.
