2.Observation of clinical effects of surgery for trailing edge fracture of tibial plateau medial condyle and posterior cruciate ligament avulsion fracture.

OBJECTIVETo investigate the clinical effects of enblock distal radius T-plate fixation in treating trailing edge fracture of tibial plateau medial condyle and posterior cruciate ligament (PCL) avulsion fracture.
METHODSFrom September 2008 to November 2011, 5 patients with trailing edge fracture of tibial plateau medial condyle and PCL avulsion fracture caused by knee joint hyperextension,were treated with surgery. There were 3 males and 2 females with an average age of 35 years old (25 to 42). Left knee injury was in 3 cases and right knee had in 2 cases; 2 cases caused by sports injury and 3 cases caused by road accident. All patients were undergone emergency treatment. Fractures were anatomically reduced and fixed with T-plate through poples approach. Complications were observed after operation. Bone healing and clinical effects were respectively evaluated by X-rays and HSS system.
RESULTSFive patients were followed up for 1 to 4 years with a mean of 2 years. All incisions obtained healing of I stage. All knee joints recovered well and were stable with inflexion more than 120 degrees, no complications such as knee joint pain, injuries of nerve and blood vessel, infection, internal fixation failure were found. The mean score of HSS system was 94.40 +/- 6.09 and all patients got excellent result.
CONCLUSIONFor the treatment of trailing edge fracture of tibial plateau medial condyle and PCL avulsion fracture caused by knee joint hyperextension, enblock distal radius T-plate fixation had advantages of good stability,higher success rate, and knee joint function can recover well.

3.Clinical effects of steel plate fixation for the failure of arthrodesis of ankle by screw fixation.

OBJECTIVETo observe the therapeutic effects of steel plate fixation after the failure of arthrodesis of ankle by screw fixation.
METHODSFrom August 2001 to October 2011, 15 patients were with steel plate fixation after failure of arthrodesis of ankle by screw fixation. Among patients, 9 cases were males and 6 cases were females,ranging age from 40 to 65 years old with the average of 56 years old. Ten cases were in left and 5 cases were in right. Screws were removed and steel plate was fixed intraoperatively, and plaster external fixation for postoperation. Clincal effect were evaluated according to AOFAS scoring system from pain, waliking ability and aligment before and after operation, and X-ray was used to evaluate joint fusion after operation.
RESULTSAll patients were followed up, and the duration ranged from 4 months to 4 years with an average of 2 years. The incison were healed in stage I. No ankle pain,injury of blood vessel and nerve,infection and farilure of internal fixation occuerred. The AOFAS score increased from 36.86 +/- 8.32 preoperatively to 85.09 +/- 4.65 (t = -26.366, P = 0.000).
CONCLUSIONSteel plate fixation after the failure of ankle arthrodesis of screw fixation has the advantages of rigid stability, simple manipulation and high success rate, less pain, perfect recovery.

6.Correlation between Toll-like receptor 4 in peripheral blood monocytes and asthma predictive index in wheezing children under 5 years of age

Objective To study the expression of Toll-like receptor 4(TLR4) in peripheral blood mononuclear cell (PBMCs) and serum interleukin-6(IL-6) in wheezing children under 5 years of age.Methods A total of 224 wheezing children under 5 years of age were divided into API(asthma predictive index)-positive (n=116) and API-negative groups (n=108).Serum level of IL-6 and TLR4 expression on CD14+ monocytes were measured after wheezing was stable for one month.TLR4 expression on CD14+ monoeytes was quantified via flow-cytometry.Serum level of IL-6 was detected by ELISA.Results Serum level of IL-6 and TLR4 expression on CD14+ monocytes of API-positive group were higher than API-negative group [LR4(%):34.9±10.0 vs.30.2± 8.8;IL-6(ng/L):46.4±15.1 vs.40.5±13.6].There was a significant positive correlation between the expression of TLR4 and the content of serum IL-6 in two groups of wheezing children(P<0.05).Conclusion TLR4 may play a role in the pathogenesis of asthma through promoting the expression of IL-6.TLR4 may be a index to predicting asthma in wheezing children.
7.Perioperative rehabilitative care for rupture of flexor tendon pollicis longus at ZoneⅡ

Objective To discuss perioperative rehabilitative care for flexor tendon injuries by examples of successful treatment of 30 cases of rupture of flexor tendon pollicis longus at ZoneⅡ.Methods From March 2001 to March 2006,30 cases of rupture of flexor tendon pollicis longus at ZoneⅡwere treated in our department by suture with double loops and crossing six strands before external fixation.Patients were encouraged to do rehabilitation exer- cises for functional recovery under the guidance and supervision of orthopaedists.Results All the patients were followed up for 6 to 16 weeks (mean,8 weeks).According to TAM (Total Active Motion) criteria,12 cases were rated as excellent,10 cases as good,6 cases as fair,and 2 cases as poor.The excellent to good rate was 73.3%.The 22 excellent and good cases could perform daily-life activities with affected fingers,such as writing,holding chopsticks and doing and undoing buttons.No patient experienced rerupture.Conclusion In perioperative rehabilitative care for rupture of flexor tendon pollicis longus at ZoneⅡ,it is important to have correct diagnosis,noninvasive and secure suture,safe external fixation and functional exercises under the guidance of orthopaedists.
8.Relationship Between CT Perfusion Parameters and Vascular Endothelial Growth Factor Expression in Rabbits Portal Vein VX2 Implanting Tumor Emboli

Purpose To establish rabbit VX2 tumor model and to explore the relation between perfusion parameters and the expression of the VEGF in the portal vein VX2 implanting tumor emboli. Materials and Methods VX2 tumor was implanted in the portal vein of eight experimental rabbits. Multi-slice CT (MSCT) perfusion scan was performed after tumor formation to measure and compare portal vein tumor thrombus, hepatic blood lfow (HBF) near tumor foci and far away from tumor foci, hepatic blood volume (HBV), probability of surface area product (PS) and mean transit time (MTT). The VX2 tumor emboli were then resected to analyze the relationship between the liver perfusion parameters and VEGF expression using immunohistochemical method. Results MSCT liver perfusion parameters were not statistically signiifcant between foci close to or far away from the tumor (P>0.05). The HBF, HBV and PS within the tumor emboli were higher than that in hepatic parenchyma (P<0.05) and the MTT was higher (P<0.05). There was positive correlation (r=0.711, 0.646 and 0.626, P<0.05) between the HBF, HBV and PS of portal vein VX2 tumor emboli and VEGF expression, and there was negative correlation between MTT and VEGF expression (r=-0.565, P<0.05). Conclusion MSCT perfusion parameters in the portal vein VX2 implanting tumor emboli and the expression of VEGF are positively related. MSCT can evaluate the angiogenesis of portal vein VX2 implanting tumor emboli.

Guizhi Fuling capsule is a traditional Chinese medicine composed of five kinds of medicinal plants, Cinnamomi Ramulus, Poria, Moutan Cortex, Persicae Semen, and Paeoniae Radix Alba. Pharmacology studies have shown that Guizhi Fuling capsule has many activities: anti-inflammatory, analgesic, anti-tumor, regulating smooth muscle, endocrine regulation and enhancing immunity. It achieved obvious effects in the treatment of uterine fibroids, pelvic inflammatory disease, dysmenorrheal, endometriosis, ovarian cysts, breast hyperplasia and other gynecological diseases. This paper reviewed the main progress on studies of pharmacological activities and clinical applications of Guizhi Fuling capsule in recent years.

10.The influence of three different methods for the treatment of osteoporotic vertebral compression fracture on bone density

Objective To observe the effect of three different methods for the treatment of osteoporotic vertebral compressive fractures on bone density.Methods Fifty-three patients with osteoporotic vertebral compressive fractures were retrospectively analyzed.These patients were divided into conservative treatment group,vertebroplasty(PVP)group and balloon kyphoplasty(PKP)group.Quantitative computed tomography measurement of bone density(L2-4)was performed before treatment,after treatment for 3 months,half a year,1 year,1 and half a year.Results Before and after treatment,no significant changes was found in bone density during follow-up in PVP group and PKP group.Bone density was decreased after treatment for 3 months compared with that before treatment,from(86.12 ± 8.21)mg/cm3 to (85.23 ± 8.31)mg/cm3 in PVP group,from(86.32 ± 8.38)mg/cm3 to(84.98 ± 8.26)mg/cm3 in PKP group,but there was no significant difference(P > 0.05),bone density returned to pre-injury level after treatment for half a year.Bone loss was found significantly after treatment for 3 months and half a year follow-up in conservative treatment group,bone density decreased significantly[(74.42 ± 8.36),(76.10 ± 8.31)mg/cm3 vs.(86.87 ±8.27)mg/cm3],and there was significant difference(P <0.05),and there was no significant difference between after treatment for 1 year and before treatment(P> 0.05).Bone density after treatment for 3 months and half a year in PVP group and PKP group was higher than that in conservative treatment group [(85.23 ±8.31),(84.98 ± 8.26)mg/cm3 vs.(74.42 ± 8.36)mg/cm3 and(86.23 ± 8.05),(86.41 ± 8.17)mg/cm3 vs.(76.10 ± 8.31)mg/cm3],and there was significant difference(P< 0.05).There was no significant difference among three groups after treatment for 1 year and 1 and half a year(P > 0.05).Conclusions PVP and PKP are positive treatment of osteoporotic vertebral compression fractures,which could reduce the loss of bone mass and do function exercise early.It could prevent brittle fracture and vertebral compression fracture further aggravated,which are a better clinical treatment methods.
